The retrosigmoid approach (RS) approach is the workhorse of the posterolateral neurosurgical techniques to access various posterior fossa structures and even extends into the middle fossa. Many studies have detailed two-dimensional (2D) descriptions of the RS technique from either the lateral or posterior view. This study is the first to provide a comprehensive analysis of the RS technique, soft tissue, extracranial landmarks, and intracranial structures of the posterolateral region using interactive three-dimensional (3D) volumetric models (VMs). The visuospatial understanding of the neuroana

Defects in the structure of the oropharynx can result in significant functional deficit and lead to nasal regurgitation and rhinolalia aperta. Many techniques have been described to reconstruct surgical oropharyngeal defects that are created during resection of squamous cell carcinoma, including the use of local advancement flaps and free-tissue transfers. We describe our experience with using a myomucosal uvular transposition flap for oropharyngeal reconstruction in a series of 11 patients. Unlike flap techniques that involve the use of palatal or uvular tissue, our technique does not require

BACKGROUND: Repair of the rare median cleft lip occurring with agenesis of the columella is a challenge as there is a major deficiency of skin and underlying structures.
MATERIAL AND METHODS: Over a 23- year period, five children underwent construction of a columella. A new surgical technique was designed for this: an internal dorsal nasal flap was used to create a cutaneous-cartilaginous flap inside the nasal dome, and was then sutured inferiorly against a superior triangle of the repaired median cleft lip.
RESULTS: Projective nasal growth could be seen in three of the five surgically const

A substantial number of patients with coronal synostosis who undergo frontoorbital advancement still require additional surgical treatment to correct increased intracranial pressure or unsatisfactory craniofacial structure. However, frontoorbital advancement currently requires elevation of the frontal as well as the orbital bone, which can result in a fragile dura mater and partial resorption of the advanced bone. Thus the dura is easily torn by dissection and the advanced bone is further resorbed and deformed during repeated craniofacial operations. To avoid these drawbacks and to create an e

OBJECTIVE: The goal was to develop an interactive three-dimensional (3-D) computerized anatomic model of the skull base for teaching microneurosurgical anatomy and for operative planning.
METHODS: The 3-D model was constructed using commercially available software (Maya 6.0 Unlimited; Alias Systems Corp., Delaware, MD), a personal computer, four cranial specimens, and six dry bones. Photographs from at least two angles of the superior and lateral views were imported to the 3-D software. Many photographs were needed to produce the model in anatomically complex areas. Careful dissection was nee

Degenerative constrictions of the spinal canal with compression of neural elements arise as a result of bony, disk, capsular, or ligament structures. The most frequent causes are disk herniations and spinal stenoses. The lumbar and cervical spine is the most prominent cause. After conservative treatments have been exhausted, surgical intervention may be necessary. Today, microsurgical, microscopically assisted decompression is regarded as the standard procedure for disk herniation and spinal stenosis in the lumbar region, while in the cervical spine microsurgical, microscopically assisted ante

OBJECTIVE: Different fast track programs for patients undergoing radical cystectomy (RC) can be found in the current literature. The aim of this work was to develop a new enhanced recovery protocol (ERP).
PATIENTS AND METHODS: The ERP was designed after a structured literature review focusing on reduced bowel preparation, standardized feeding, postoperative nausea, vomiting and pain control. In order to test the ERP, a pilot observational prospective cohort study was planned, enrolling all patients consecutively undergoing RC and Vescica Ileale Padovana (VIP) neobladder. These patients were c

Elbow synovectomy has traditionally been performed using direct visual observation. However, recent advancements in elbow arthroscopy have increased the popularity of arthroscopic synovectomies. This minimally invasive procedure has garnered attention because of its efficacy in halting joint deterioration while minimizing patient discomfort and invasiveness. Conversely, the technical demands of arthroscopic elbow synovectomy pose challenges, often resulting in notable complications, such as nerve damage. Considering the utility of real-time nerve visualization for injury prevention, we suggest

BACKGROUND: Transformational techniques empower others to create change while transactional techniques utilize rewards and punishments to modify behavior. We sought to understand the use of these leadership techniques by surgical department chairs.
METHODS: Semi-structured interviews were conducted with 17 Chairs of Surgery and analyzed using reflexive thematic analysis.
RESULTS: Transformational techniques are effective and require an aligned vision that is associated with change, growth, and empowerment. Transactional techniques are necessary but can damage intrinsic motivation and the sen

A quadricuspid mitral valve is an uncommon congenital anomaly whose imaging features are inconsistently reported. Prior case reports frequently describe this finding in patients with hypertrophic cardiomyopathy (HCM), complicating characterization of this anomaly as an isolated finding. We report a 64-year-old male with severe mitral regurgitation of incompletely defined origin and atrial fibrillation presenting for robotic thoracoscopic mitral valve repair and a Cox-Maze III procedure. Intraoperative transesophageal echocardiography demonstrated a thickened, myxomatous mitral valve with deep

With the increasing use of elbow arthroscopy, there is a growing concern about the risk of injury to neural structures, particularly the median nerve, when the anteromedial portal is created. This risk, which was previously underestimated, underscores the need for a guide to safe access that can significantly impact surgical practices and patient outcomes. We retrospectively evaluated 83 computer tomographies of the elbow without a higher grade of osteoarthritis and surgical treatment. In a 3D reconstruction, the median nerve was located, and a circle was set around it with a radius of 1.5 cm.

Advanced ovarian serous carcinoma presents significant surgical challenges due to tumor size, deep location, and complex anatomical relationships with surrounding structures. This case report highlights the use of 3D printing technology to improve surgical planning and outcomes in such complex scenarios. A 48-year-old female presented with bilateral ovarian tumors, confirmed as serous cystadenocarcinoma. Preoperative 3D modeling was employed to create a detailed anatomical model based on imaging data. This model provided precise visualization of tumor size, vascular supply, and relationships w

BACKGROUND: Cor triatriatum sinistrum (CTS) is a rare congenital structural heart defect conventionally managed surgically early in life. However, at times it is discovered much later.
CASE SUMMARY: We present the first in-human case of a transcatheter CTS membranoplasty with mitral transcatheter edge-to-edge repair (mTEER) in an elderly patient with CTS. Ultimately, the patient experienced reduction in mitral regurgitation (MR), translating to significant symptomatic improvement.
DISCUSSION: Although surgical resection is the standard treatment for CTS, this approach may not be suitable for

The foramen magnum meningioma (FMM) is one of most challenging tumors among all the meningiomas because of its distinctive location, clinical course, insidious onset, and the relative large size of the tumor during presentation. These tumors are slow-growing and clinical symptoms vary according to involvement of different structures including the spinal cord, vertebral artery, lower cranial nerves, and medulla oblongata. Controversies regarding appropriate surgical approach for resection of tumor in this unique location continue. Tumors in the posterior or posterolateral region of the foramen

PURPOSE OF REVIEW: There has been a need for an acceptable common minimum data set in the scientific literature as regards the surgical treatment of female stress urinary incontinence (SUI). Such a data set, if widely adopted, would improve the quality of the literature and allow objective comparisons between and across interventions.
RECENT FINDINGS: The surgical treatment of female stress urinary incontinence has evolved considerably over the past few decades. The corresponding body of literature has grown exponentially describing the outcomes of hundreds of studies of these novel intervent

Mandibular reconstruction following oral and maxillofacial tumor resection is essential for restoring occlusion and facial appearance. Conventional methods require intraoperative bending of plates, increasing surgical time and risking metal fatigue and plate fracture. Patient-specific implants (PSIs), created via preoperative computed tomography (CT) data and three-dimensional (3D) metal printing, have improved surgical accuracy and reduced operation times. In Japan, Cosmofix® is the only PSI system available, but it lacks osteotomy guides, complicating precise bone cuts and screw insertion an

PURPOSE: En bloc spondylectomy for spine tumors involves extensive removal of posterior vertebral elements, which are difficult to reconstruct. Often, the dura is left exposed, leading to scarring and adhesions which complicate revision surgeries. Synthetic barriers can be used to reduce adhesions, but they offer no structural support. Here, we present a novel technique using cadaveric femoral struts for posterior column reconstruction, creating a lamina-like structure spanning across the defect.
METHODS: We conducted a retrospective institutional review of patients who underwent en bloc spon

BACKGROUND: Patients who undergo laparoscopic right hemicolectomy often have vascular anomalies, creating challenges for surgeons. Preoperative identification of vascular anomalies and intraoperative precise navigation can enhance surgical safety and reduce the difficulty of the procedure. Accordingly, this study aimed to explore and evaluate the application of three-dimensional (3D) reconstruction and printing technology in laparoscopic right hemicolectomy and its assistance in preoperative planning and intraoperative navigation.

BACKGROUND: Large tumors arising from the middle scalene region can displace the middle scalene muscle and distort regional anatomy, placing nerves at risk. Understanding the surgical anatomy of these nerves is key to approaching pathology of the middle scalene muscle and avoiding damage to the dorsal scapular, long thoracic, and spinal accessory nerves, each of which can cause scapular winging and associated morbidity if injured.
METHODS: IRB approval was obtained for this study, allowing cases with relevant pathology to be reviewed and presented to highlight the relevant surgical technique.
