Pathophysiology of flow impairment during carotid artery stenting with an embolus protection filter.

OBJECTIVE: Carotid artery stenting (CAS) is a well-accepted treatment for atherosclerotic stenosis of carotid arteries. Since the occurrence of distal embolization with CAS is still a major concern embolus protection devices (EPD) are usually employed during the procedure. We examined two types of embolus protection filters (Angioguard XP (AG); Filterwire EZ (FW)) and evaluated the function. Thus, the filter was examined postoperatively and the cause of intraoperative flow impairment was evaluated. MATERIALS AND METHODS: CAS was performed for 54 patients with carotid artery stenosis (55 lesio

Feasibility of simultaneous pre- and postfilter transcranial Doppler monitoring during carotid artery stenting.

OBJECTIVE: Carotid artery stenting (CAS) is emerging as an acceptable treatment alternative to surgery for patients with carotid artery stenosis. The major risk of CAS is cerebral embolization of plaque and thrombus causing stroke or asymptomatic brain infarction. Use of embolic protection devices (EPD) to trap emboli before they reach the brain is now standard practice in CAS. The pore size of the currently available filters is >100 microns and emboli smaller than the EPD pores can still reach the brain. While the use of EPD is widespread, little evidence exists of their in vivo efficacy in p

Immediate and 30-day clinical outcome of patients treated with the TwinOne cerebral protection system: multicenter experience in 217 cases.

There are several cerebral protection devices used for carotid artery stenting (CAS). All of them require manipulation and exchange of two separate materials (cerebral protection device ? dilatation balloon), making the procedure longer and posing possible complications such as vasospasm and embolism. The new system described here is an evolution of the cerebral protection concept and allows temporary occlusion of the distal internal carotid during CAS procedures with both materials in one device. We present our experience using this new device. Between January 2007 and March 2008, consecutive

Snare-assisted transcatheter aortic valve implantation: balancing success with risk-a case series.

Snare-assisted transcatheter aortic valve implantation (TAVI) has emerged as a bailout technique to facilitate valve delivery in patients with hostile anatomies when standard maneuvers fail. However, while it can increase procedural success, this strategy may also carry significant risks. We present two cases of transfemoral TAVI in patients with severely calcified and tortuous aortic anatomies. In the first case, an 88-year-old woman with severe aortic stenosis and highly tortuous aortic arch underwent successful valve implantation after snare-assisted advancement of the delivery system, achi

Cerebral-Protected Percutaneous Thrombectomy for Near-Occlusive LVAD Outflow Graft Thrombosis.

BACKGROUND: Late low-flow alarms in left ventricular assist devices (LVADs) most commonly result from volume depletion or right-sided failure, but mechanical outflow graft obstruction is an infrequent yet life-threatening cause. CASE SUMMARY: A 70-year-old man with a HeartMate 3 LVAD presented with persistent low-flow (0 L/min) alarms after defecation but felt entirely well. Laboratory testing showed no overt hemolysis, and transthoracic echocardiography excluded right-sided failure. Computed tomographic angiography revealed >90% thrombotic narrowing of the distal outflow graft. The patient u

[Advances in stroke after transcatheter aortic valve replacement].

With the clinical generalization and popularization of transcatheter aortic valve replacement (TAVR), cerebrovascular events related to TAVR occur more frequently, which significantly impairs neurocognitive function, increases mortality, and seriously affects prognosis and quality of life in these patients. However, the reported incidence rates of TAVR-related stroke differ in literature due to inconsistent diagnostic criteria. According to the onset time, TAVR-related stroke can be divided into acute (≤24 h), subacute (24 h-30 d), early (31 d-1 year) and late (>1 year) types, and the cause of

Types of carotid stents and their associated strengths and limitations.

Carotid artery stenting (CAS) is now established as a less invasive alternative to carotid endarterectomy for patients with carotid artery stenosis. Over the past two decades, stent technologies have undergone significant refinements to improve navigability, conformability, and plaque coverage while aiming to reduce periprocedural complications and enhance long-term durability. Current devices are broadly categorized as open-cell, closed-cell, and dual-layer micromesh stents. Open-cell designs provide excellent flexibility in tortuous anatomy but may be limited by reduced plaque scaffolding. C

Carotid artery stenting using the proximal or dual protection method for near occlusion of the cervical internal carotid artery.

The treatment for patients with near occlusion of the cervical internal carotid artery (ICA) is controversial. The aim of this study was to examine the results of carotid artery stenting (CAS) as a surgical treatment for ICA near occlusion. Between April 2008 and September 2012, 14 patients (all men; mean age, 75.4 years) with ICA near occlusion were treated with CAS. This represents 5.2% of a total of 267 patients treated with CAS during the study period. All patients were treated with CAS using an embolic protection device. The proximal balloon protection method was performed in five patient

Early Clinical Impact of Cerebral Embolic Protection in Patients Undergoing Transcatheter Aortic Valve Replacement.

Background We aimed to compare the rate of neurological events in patients with or without cerebral embolic protection (CEP) during transcatheter aortic valve replacement (TAVR). Methods and Results Data on clinical end points including neurological events ≤30 days post-TAVR were collected for all patients who underwent transfemoral TAVR in 2 academic tertiary care institutions. Patients were matched through propensity scoring, which resulted in 333 pairs of patients with versus without CEP out of a total of 831 consecutive patients. The median age was 81 (76-85) years, and the median logistic

Proximal Protection Devices for Carotid Artery Stent Placement: A Benchtop Assessment of Flow Reversal Performance.

BACKGROUND AND PURPOSE: Proximal protection devices, such as TransCarotid Artery Revascularization (TCAR), aim to yield better outcomes in carotid artery stent placement (CAS) than distal protection devices by preventing plaque embolization to the brain. However, transfemoral catheters may not fully reverse flow from the external carotid artery (ECA) to the ICA. We assess a new balloon-sheath device, Femoral Flow Reversal Access for Carotid Artery Stent placement (FFRACAS), for this purpose. MATERIALS AND METHODS: The FFRACAS prototype (inner diameter [ID] = 0.117 inches; L = 80 cm) was compa

Endovascular carotid artery revascularization utilizing multiple distal embolic protection strategies: active and passive flow reversal in combination with balloon assisted reperfusion.

BACKGROUND: Carotid artery stenosis is a common cause of stroke. Carotid artery stenting (CAS) includes multiple approaches to endovascular carotid revascularization, including transfemoral/transradial CAS (TF-CAS/TR-CAS) and transcarotid artery revascularization (TCAR). Although CAS has declined in frequency since the development of TCAR, distal embolic protection (DEP) strategies differ between the two procedures, with studies reporting a lower risk of stroke with TCAR. TCAR uses open surgical common carotid artery clamping combined with passive flow reversal (PFR), whereas traditional TF-CA

Carotid artery stenting versus endarterectomy: a systematic review.

For about 2 decades, investigators have been comparing carotid endarterectomy with carotid artery stenting in regard to their effectiveness and safety in treating carotid artery stenosis. We conducted a systematic review to summarize and appraise the available evidence provided by randomized trials, meta-analyses, and registries comparing the clinical outcomes of the 2 procedures. We searched the MEDLINE, SciVerse Scopus, and Cochrane databases and the bibliographies of pertinent textbooks and articles to identify these studies. The results of clinical trials and, consequently, the meta-analys

Carotid artery stenting with dual-layer stent implantation in a patient with complex anatomy.

Carotid atherosclerotic disease associated with hemodynamically significant stenosis is a serious condition that requires accurate diagnosis and effective treatment to prevent cardiovascular events such as stroke. Treatment strategies, including carotid angioplasty and carotid endarterectomy, have evolved considerably, providing safer and less invasive therapeutic options. This report describes the case of a male patient with dyslipidemia, hypertension, and coronary artery disease who presented with near-occlusion of the left internal carotid artery caused by a plaque with features of instabil

"Seat belt and air bag" technique for cerebral protection during carotid stenting.

PURPOSE: To present a 2-device technique for cerebral protection during carotid stenting of the internal carotid artery (ICA) in patients with high-grade lesions, contralateral occlusion, and/or an aberrant or nonfunctioning circle of Willis. TECHNIQUE: A reverse flow system (Parodi Anti-Embolism System [PAES]) is first placed with a 260-cm exchange wire in the common carotid artery 3 cm below the carotid bifurcation. Flow reversal is obtained by inflating the balloons in the external and common carotid arteries. Via an external connector, a guidewire and E-Trap filter are delivered to the di

Mechanical protection of cardiac microcirculation during percutaneous coronary intervention of saphenous vein grafts.

Saphenous vein bypass grafts permeability is one of the most important limitations of open heart surgery. The risks associated with surgical re-intervention are greater than those associated with the initial procedure. While native coronary arteries usually have fixed, fibrotic or calcified atherosclerotic plaques, the disease in the vein grafts contains soft material. When this material is compressed during percutaneous angioplasty, there is an unfavorable immediate outcome due to distal embolization of thrombus and plaque debris. In addition, the risk of post-procedure adverse events are hig

Imaging and Clinical Outcomes with Sentinel Cerebral Embolic Protection During TAVR: A Meta-Analysis of Randomized Trials with Trial Sequential Analysis.

Background: Stroke and subclinical cerebral ischemia remain important neurological complications of transcatheter aortic valve replacement (TAVR). The Sentinel cerebral embolic protection (CEP) device is designed to capture embolic debris during TAVR, but its impact on clinical and imaging outcomes remains incompletely characterized. Methods: PubMed, Embase, and Cochrane databases were systematically searched for randomized controlled trials (RCTs) comparing Sentinel CEP versus no protection when TAVR was performed. Outcomes of interest included all stroke, disabling stroke, infarct volume by
