Beyond treatment: Ethical decision-making and palliative care in critically ill patients with cancer.

Over recent decades, advances in cancer diagnosis and treatment have improved survival and quality of life, leading to a growing number of cancer patients being admitted to Intensive Care Units (ICUs). In this setting, clinical decision-making is challenged by prognostic uncertainty, intensive technology use, and therapeutic interventions whose proportionality may be uncertain, generating relevant ethical conflicts in clinical practice. The application of bioethics to critical oncology provides a deliberative framework based on the principles of autonomy, beneficence, nonmaleficence, and justi

The Sword of King Solomon.

Conjoined twin pregnancies are one of the greatest dilemmas we face in healthcare practice. Thanks to scientific knowledge and evolution, technology and the higher level of wealth in our society, conjoined twins have a chance to survive, albeit with the risk of major consequences on their lifespan and quality of life. Particularly, in the case of newborns with extreme prematurity or congenital malformations, special care must be taken in the use of treatments that offer little to no benefit. This is especially the case with procedures and techniques of unproven efficacy that could create unfou

Discoursive Humanity as a Transcendental Basis for Cognitive (Dis)Ability Ethics and Policies.

This article explicates two approaches to the basis of moral worth and status: Eva Kittay's relational view and Jeff McMahan's psychological personhood view. It is argued that these theories alone do not provide adequate support for the conclusions Kittay and McMahan want to draw concerning individuals whose entitlement to fundamental protections can be challenged-infants with severe cognitive disabilities and infants without the support of their families and social environments. The real justification can in each case be found in deeply held convictions regarding entities that must and entiti

The morality of assisted reproduction and genetic manipulation.

The author analyzes the pros and cons of various forms of assisted reproduction, including the use of so-called 'genetic manipulation'. He shows how in ethics the only arguments with any chance of reaching a consensus (or at least an agreement) are those of the rational type, based on universally acceptable ethical principles or corroborated by empirical facts and real life experience (as the starting point for identifying problems requiring analysis). After an analysis in which he identifies the incoherence and inconsistency of arguments against assisted reproduction, the author defends the r

An affordable implantable vagus nerve stimulator system for use in animal research.

In this research, a vagus nerve stimulator has been developed and miniaturized for use in epilepsy research. The board contains all the components necessary for its operation during the standard duration of the experiments, being possible to control it once implanted and even being able to reuse it. The VNS system has been designed for rodents since the VNS devices available for human are not only too large for laboratory animals, but also too expensive. With this solution the expenditure on materials made by laboratories is greatly reduced and bioethical considerations were kept in mind. The

Medical progress, psychological factors and global care of the patient: lessons from the treatment of childhood leukemia.

The history of treatment of childhood leukemia is a meaningful model of ethical, bioethical and organizational repercussions of medical progress. Specifically, it has provided precious indications and very useful tools to cope with several of the more important problems of modern medicine: the value of controlled randomized studies; the risks of intense medicalization impairing the quality of care; the importance of a valid doctor-patient relationship; the psycho-emotive involvement of the pediatric staff; and last but not least, the need of an unrelenting effort of humanization of the procedu

In the wake of Guatemala: the case for voluntary compensation and remediation.

Recently unearthed records reveal that between 1946 and 1948, researchers with the US Public Health Service engaged in nonconsensual inoculation of vulnerable Guatemalan populations with syphilis, gonorrhea, or chancroid. The US government has issued formal public apologies to the Guatemalan government and its people, and the Presidential Commission for the Study of Bioethical Issues has been tasked with reviewing the historical record and the adequacy of protection of human research participants. We argue that the US response is insufficient and call for a restitution program directed at the

Trisomy 18 in monozygotic twins with discordant phenotypes.

The incidence of trisomy 18 in monozygotic twins is approximately 1 per million. We report a pair of liveborn monozygotic twins with trisomy 18. Both twins had esophageal atresia with tracheoesophageal fistula (type C) and intrauterine growth retardation. Twin A had cleft lip, choanal atresia and perimembranous ventricular septal defect. Twin B had hypoplastic left heart syndrome. The twins died without aggressive intervention at the age of 2 months and 52 hours, respectively. These 2 babies had significantly discordant phenotypes, which suggests an epigenetic or environmental effect. Bioethic

Ethical and Practical Challenges of the Communication and Behavioral Manifestations of Primary Progressive Aphasia.

The communication and behavioral manifestations of primary progressive aphasia (PPA) present ethical and practical challenges for individuals with this clinical syndrome as well as for individuals who are involved closely in their care. In this article, cases representing all three PPA variants (logopenic variant, nonfluent agrammatic, semantic variant) are presented to illustrate commonly encountered situations in which self-determination is at risk in decisions about housing, driving, social interactions, finances, and treatment interventions. Potential approaches, including patient/family e

A report on the Fetal Alcohol Spectrum Disorders Study Group meeting of 2012, theme title, "Biomarkers for FASD".

The 2012 meeting of the Fetal Alcohol Spectrum Disorders Study Group (FASDSG) focused on the development and ethics of biomarkers for fetal alcohol exposure. This one-day international conference brought students and trainees together with clinicians and researchers to discuss the latest research on FASD. One keynote speaker discussed the value of profiling epigenetic modifications in readily available fetal tissues to diagnose fetal exposure to environmental agents, while the second speaker discussed the ethics of biomarker development within the context of core principles of justice, autonom

Ethical considerations when prescribing biologics in dermatology.

There are no formal publications describing and discussing the various ethical dilemmas encountered when treating dermatologic disease with biologic therapy. Multiple issues such as safety, efficacy, drug accessibility, and cost need to be considered when prescribing biologics. This contribution uses a case-based approach to evaluate the ethical principles involved with biologic therapy.

Ethics, transplantation, and the changing role of anatomists.

Anatomists are regarded as custodians of cadaveric material donated to science. Almost every facet of medical science has experienced explosive advances. This has impacted directly on anatomists and their role. Increasingly, anatomists are raising concerns with regard to the treatment of human tissue (Jones,2002, Clin. Anat. 15:436-440). The Korperwelten (Bodyworlds) of Gunther von Hagens et al. (1987, Anat. Embryol. 175:411-421) has evoked considerable debate about the treatment of human cadavers. Thus far clinical anatomists have had little role to play in policy formulation, legislation, an

The Role of Step Therapy in the Treatment of Migraine.

PURPOSE OF REVIEW: This review examines recent evidence and applies bioethical principles to evaluate the benefits and risks of using step therapy in the treatment of migraine. RECENT FINDINGS: With the CGRP mAbs, gepants, and lasmiditan now on the market for up to 5 years, new research, including network meta-analyses and long-term use studies, can evaluate the comparative efficacy, tolerability, and adherence of these medications relative to older acute and preventive medications for the treatment of migraine. Deciding how medications are chosen for patients requires accounting for many fac

Chest compressions and epinephrine during resuscitation of infants born at the border of viability: Yes, no or maybe?

Neonatology, in large part due to its population of babies born at the edge of viability, is rife with bioethical issues. This unique population is at high risk of mortality and considerable neurodevelopmental morbidity. One contentious, ongoing debate concerns whether these extremely low birth weight infants born at the border of viability should, if required by the Neonatal Resuscitation Program guidelines, receive chest compressions and epinephrine as part of their delivery room resuscitation. The present article, through a case presentation and discussion based on the ethical framework of

Exploring the gene of "alcohol flush"--in-depth exploration and application of genetic experimental cases.

Genetic related technologies are increasingly influencing human health and life. From theory to practice, teaching through typical cases of disciplinary applications has become a popular new model in the field of genetics education. This experiment takes the phenotype case of "alcohol flush" as an example, organically integrating genetic knowledge from multiple perspectives and levels such as phenotypic analysis, population genetic analysis and pedigree analysis with experimental cases. In teaching practice, the "alcohol flush" case study can greatly stimulate students' interest in active lear

Review of Current Care Models for Transgender Youth and Application to the Development of a Multidisciplinary Clinic - The Seattle Children's Hospital Experience.

Care of transgender and gender diverse youth is complex and requires a multidisciplinary approach. Many transgender patients and providers feel the limited availability of affirming, knowledgeable professionals is a barrier to obtaining care. Such care can be provided through a clinic with providers from different disciplines who are trained in the unique care of transgender youth. In this paper, we discuss the care guidelines for transgender youth and the unresolved challenges that need to be addressed during the development of a transgender clinic. We describe our experience at Seattle Child

Julius Wagner-Jauregg and the legacy of malarial therapy for the treatment of general paresis of the insane.

Julius Wagner-Jauregg, a preeminent Austrian psychiatrist was awarded the Nobel Prize in Medicine in 1927 for the development of malaria therapy for the treatment of neurosyphilis, or general paresis of the insane. Despite being only one of three psychiatrists to win a Nobel Prize, he has faded from public consciousness and his name recognition pales in comparison to his contemporary and fellow Austrian, Sigmund Freud. This paper explores his contributions to the field of biological psychiatry and also touches upon reasons, such as the growing bioethics movement, his controversial affiliation

Conflicts in Best Interest; Infants with severe neuromuscular disorders presenting to UK Courts.

Infants with severe neuromuscular disorders, presenting a birth, are often unable to move, communicate, breathe, and feed. Unlike infants with severe birth asphyxia and cerebral palsy, these neuromuscular conditions are often associated with presumed normal cognition. Supporting these very severely affected infants and maintaining life through medical intervention or re-orientating care to palliation is often debated, and decision-making can be extremely challenging. Although guidelines exist to help navigate this process (Nuffield Council of Bioethics and Royal Collage of Paediatrics guidelin

The principle of stewardship truthful communication for the restoration of trust in public health officials.

In the wake of widespread public skepticism toward health authorities, this paper introduces a stewardship model of communication to guide public health officials in restoring trust. In critically examining the broader ethical concept of stewardship as developed by the World Health Organization and the Nuffield Council on Bioethics, the paper argues that public health officials-as stewards of the population's health and well-being-have an ethical duty to convey scientific knowledge truthfully. Rather than promoting excessive transparency or manipulating public perception through noble lies, th
