Congenital anterior glottic webs with subglottic stenosis: surgery using perichondrial keels.

UNLABELLED: Four children with severe congenital anterior glottic webs required surgical reconstruction of their laryngeal airway to either avoid a tracheotomy or allow tracheotomy decannulation. The technique of re-establishing a glottic inlet allows both normal respiration, good cough and a satisfactory voice outcome. It utilizes an autogenous graft with perichondrium acting as an overlay keel to minimize glottic web reformation. All children were found to have Shprintzen syndrome. OBJECTIVE: The objective of this case series was to document the method of surgically using this perichondrial

Jet ventilation in upper airway obstruction: description and model lung testing of a new jetting device.

UNLABELLED: Patients with critical upper airway stenosis require a tracheotomy for corrective surgery. We describe a new transtracheal device that permits safe ventilation of these patients without tracheotomy. It is based on a coaxial bicannular design that allows "push-pull" ventilation by jetting gas through the inner cannula and applying suction through the outer cannula. It further allows monitoring of airway pressure, tidal volume, and end-tidal CO2. The device was placed in the "trachea" of an artificial lung, and the preparation was made airtight by sealing the proximal end of the trac

Posterior wall laceration of the thoracic trachea: the transcervical-transtracheal approach.

A large membranous wall laceration of the thoracic trachea was surgically treated. The surgical approach consists on a low collar incision followed by a longitudinal tracheotomy. The membranous tear was repaired with a running suture and tracheotomy sutured with interrupted crossed stitches. The procedure was effective and endoscopic follow-up showed a perfect healing process with no signs of tracheal stenosis. This new technique proved to be a reliable, quick and safe procedure, which allows to repair membranous lacerations as far as the carina, avoiding thoracotomy.

Risk factors for delayed oral dietary intake in patients with deep neck infections including descending necrotizing mediastinitis.

Although clinical outcomes of descending necrotizing mediastinitis (DNM) and/or deep neck infection (DNI) have been extensively reported, no study has addressed delay in recovering oral ingestion after surgical interventions other than sporadic case reports. We herein compared clinical features of DNM and DNI cases over the same period, and clarified precipitating factors of delay in recovering oral ingestion by logistic regression analysis. We reviewed records of patients with DNI and DNM at our institution from August 2005 to July 2015. We extracted data on patient age, sex, complication wit

[Armand Trousseau, laryngologist].

Trousseau's contribution to laryngology was considerable, but it was eclipsed by his reputation in other fields. His work in the field was not limited to the diffusion of tracheotomy for the treatment of croup, despite opposition from certain surgeons. He totally transformed the intervention, which up until then had been undertaken without any specific methodology, into a controlled procedure with principles that still apply today. He undertook the study of chronic diseases of the larynx and wrote the first work dedicated to this pathology, before the arrival of laryngoscopy. He reported the f

Complications of Respiratory Surgery in the Horse.

Respiratory surgery and procedures typically focus on conditions of the upper airway that restrict airflow at rest or exercise. This article focuses on respiratory anatomy and common surgeries of the trachea and larynx in horses. The trachea serves as the airway conduit from the larynx to the bronchi. A variety of upper airway disorders can lead to obstruction and in these instances, tracheotomy is typically utilized to bypass these obstructions and maintain airway patency. Emergency and permanent tracheotomy procedures are discussed. Other common upper airway operations, such as laryngotomy,

Bilateral vocal cord paralysis in children.

Bilateral vocal cord paralysis in children with its many causes presents a challenging problem to the pediatric otolaryngologist. Traditionally, management of bilateral vocal cord paralysis includes securing the airway with a tracheotomy and waiting for spontaneous recovery. Surgeons have tried a variety of surgical procedures in lieu of or in addition to tracheotomy, but none are perfect solutions to the problem. This article reviews the current surgical procedures for bilateral vocal cord paralysis in the pediatric population with a particular focus on the senior author's experience with the

District nurses' attitudes toward patient consent: the case of mechanical ventilation on amyotrophic lateral sclerosis patients: results from a French national survey.

PURPOSE: In France, a recent law emphasizes patient rights and prohibits unwanted therapies in end-of-life care. As end-of-life home care is increasing, we aimed to assess French district nurses' attitudes toward terminally ill patients' autonomy. MATERIALS AND METHODS: We used data from a nationwide telephone survey conducted in 2005 among a random sample of French district nurses (n = 602). Nurses' attitudes were assessed with a clinical case describing a patient (randomly defined as a male or a female aged 50 years) having amyotrophic lateral sclerosis (ALS) who urgently needs mechanical v

[Acute obstructive laryngitis in children (author's transl)].

Three types of acute obstructive laryngitis in children are distinguished: 1. Acute supraglottic laryngitis (Epiglottitis acuta): The usual causative organism is Hemophilus influenzae, type B. Characteristic findings include a swollen red epiglottis. Treatment is based on Ampicillin, Solucortef (hydrocortisone-sodium succinate) i.m., air humidification, intravenous infusions and airway protection (tracheotomy or intubation). From 1958-1967, 68 children were treated, of whom boys were twice as commonly affected as girls. In about 80% of cases, tracheotomy was still found necessary. 2. Acute sub

Two babies, one heart, and no airway.

The medical literature on conjoined twins is replete with descriptions of the complexity of surgical separations. However, airway management, although quite challenging, is neglected in literature. We present experience with conjoined twins united anteriorly at the thorax and abdomen sharing one six-chambered heart. Laryngoscopy and intubation recommendations, bronchoscopy techniques, and tracheotomy strategies are outlined for the face-to-face twins. Difficult, unique case report at tertiary care medical center. Conjoined thoraco-omphalopagus twins were successfully intubated at birth using t

Advanced and rapidly progressing head and neck cancer: good palliation following intralesional bleomycin.

The authors herein report the case of a 61-year-old man undergoing adjuvant therapy for locally advanced laryngeal cancer, who developed parastomal recurrence in his radiation field around his tracheotomy site, while he was undergoing radiation therapy, and compromised the secure placement of his tracheotomy tube and maintenance of his upper airway. MRI restaging and biopsy confirmed recurrence and progressive disease in his mediastinum. He underwent local therapy with intralesional bleomycin with good palliation, and ability to maintain the patency of his upper airway.

Hemosiderosis of lower cranial nerves: A pitfall in difficult-to-wean tracheotomized patients - A case report.

Cerebral hemosiderosis is a condition resulting from recurrent or extensive intracranial hemorrhages, leading to hemosiderin deposition in central nervous system structures and affecting cranial nerves. The classic clinical triad includes sensorineural hearing loss, cerebellar ataxia, and pyramidal signs, with less frequent involvement of other cranial nerves. In our case however, we report a unique presentation of multiple lower cranial nerve involvement following a right-myelencephalic subarachnoid hemorrhage secondary to hemorrhagic ventricular effraction in a 58-year-old male patient.
