The Spanish Version of the Fear of Kidney Failure Questionnaire: Validity, Reliability, and Characterization of Living Donors With the Highest Fear of Kidney Failure.

BACKGROUND: Measures of fear of progression or recurrence of illnesses have been criticized for neglecting cross-cultural validity. Therefore, we assessed the psychometric properties of the Spanish version of the Fear of Kidney Failure Questionnaire (FKFQ), to determine whether postdonation fear of kidney failure (FKF) influenced the donors' psychosocial status, and define variables that characterized donors with high FKFQ scores. METHODS: We included 492 participants (211 donors) in a multicenter, 11-year, retrospective, cross-sectional study. Donors were classified with a Latent Class Analy

A legacy of fear: Physiological evidence for intergenerational effects of trauma exposure on fear and safety signal learning among African Americans.

OBJECTIVE: To determine the influence of maternal trauma and posttraumatic stress disorder (PTSD) symptoms on children's physiological response to threat and safety signals during a fear conditioning task in trauma-exposed mothers and children. METHOD: Participants were African American mother-child dyads (N = 137; children aged 8-13 years). Mothers' trauma history and PTSD symptoms were assessed; Latent Class Analysis (LCA) was conducted from these measures to identify distinct classes. Children reported violence exposure and completed a differential fear conditioning task using fear-potenti

Oxytocin Reduces Subjective Fear in Naturalistic Social Contexts via Enhancing Top-Down Middle Cingulate Amygdala Regulation and Brain-Wide Fear Representations.

Accumulating evidence from animal and human studies suggests a fear-regulating potential of the neuropeptide oxytocin (OT), yet the clinical translation into novel interventions for pathological fear requires a behavioral and neurofunctional characterization under close-to-real-life conditions. Here, we combined a naturalistic fMRI-design that elicited strong and immersive fear experience in social and non-social contexts with a preregistered between-subjects randomized double-blind placebo-controlled intranasal OT trial (24 IU, n  =  67 healthy men). OT selectively reduced subjective fear in

Optogenetic manipulation of the prelimbic cortex during fear memory reconsolidation alters fear extinction in a preclinical model of comorbid PTSD/AUD.

RATIONALE AND OBJECTIVE: Post-traumatic stress disorder (PTSD) and alcohol use disorder (AUD) are disorders of learning and memory that often occur comorbidly. Exposure to trauma-related cues can increase alcohol intake in PTSD patients that are using alcohol to self-medicate. The recurrence of anxiety symptoms with subsequent alcohol use may initiate a destructive cycle where stress and alcohol exposure impair the function of the prefrontal cortex (PFC). While the incidence of these disorders has steadily increased, current therapies and treatments often lack efficacy. Thus, investigation int

Fear-EEG-AI: A Conceptual and Operational Framework for Designing EEG-AI Systems for the Inference of Fear-Compatible Neurophysiological States.

Electroencephalography (EEG) and artificial intelligence (AI) are central tools in affective computing, yet fear-related inference remains constrained by a gap between experimental performance and deployable evidence: high accuracy on controlled datasets guarantees neither neurobiological specificity, nor robustness across users, nor traceable inference. EEG does not measure fear directly; it captures cortical dynamics compatible with threat processing, arousal or vigilance. This paper introduces the Fear-EEG-AIFramework, a technology-independent architecture for designing, validating and depl

The role of fear learning in the development of psychosis: an EEG study utilizing a differential fear conditioning paradigm in people with psychotic vulnerability.

By studying how individuals in an "at-risk" state of psychosis learn about threat and safety cues - specifically, how they develop and unlearn fear responses to neutral cues - we might better understand the mechanisms leading to heightened arousal and fear that are characteristic of acute psychotic episodes. At-risk individuals (N = 88; of which 28 fulfilled ultra-high-risk criteria on the Comprehensive Assessment of At-Risk Mental States interview and 60 scored above a predefined threshold on the Community Assessment of Psychic Experiences) and healthy controls (N = 44) underwent a standardiz

Psychological Factors Associated with Fear of Falling and Fear of Falling Avoidance Behavior in Older Adults: Results from a National Sample.

OBJECTIVES: This study aimed to identify psychological factors and characteristics associated with fear of falling (FOF) and fear of falling avoidance behavior (FFAB) among older adults. METHODS: This cross-sectional study used data from the National Health and Aging Trends Study (Wave 9, n = 4,977). RESULTS: We found that increased fall history, more frequent depression and anxiety, and poorer perceived overall health were significantly higher among older adults with FFAB compared to FOF (ps < .001). Perceived overall health, depression, and anxiety explained a significant amount of varianc

Testing the Bivalent Fear of Evaluation Model of Social Anxiety: The Relationship between Fear of Positive Evaluation, Social Anxiety, and Perfectionism.

The Bivalent Fear of Evaluation (BFOE) model of social anxiety proposes that fear of negative evaluation (FNE) and fear of positive evaluation (FPE) play distinct roles in social anxiety. Research is however lacking in terms of how FPE is related to perfectionism and how these constructs interact to predict social anxiety. Participants were 382 individuals from the general community and included an oversampling of individuals with social anxiety. Measures of FPE, FNE, perfectionism, and social anxiety were administered. Results were mostly consistent with the predictions made by the BFOE model

Fear-induced hyperalgesia in quiescent inflammatory bowel disease.

Recurring pain is a debilitating symptom in inflammatory bowel disease (IBD), often persisting beyond acute gut inflammation with unclear underlying mechanisms. Altered emotional reactivity to pain has been proposed as a key contributor to pain persistence, but experimental evidence is scarce. This study investigated whether pain-related fear learning shapes the perception of acute experimental pain in quiescent IBD. Implementing a 2-day differential fear conditioning paradigm, we assessed the acquisition and extinction of conditioned fear in response to nociceptive (thermal pain) and non-noci

Reconceptualizing human fear memory through the defense cascade.

Human fear memory and its associated pathologies are predominantly studied within fear conditioning frameworks. While this reductionist approach has provided valuable insights into the neural and behavioral mechanisms of fear memory, it inadequately captures the diverse dysfunctions in defense responses observed in post-traumatic disorders. These dysfunctions include maladaptive immobility and aberrant fight-or-flight reactions, contributing to substantial individual variability. Traditional paradigms in human studies typically present conditioned stimuli without accounting for the imminence o

Should We Fear Wipe-Out in Glaucoma Surgery?

Wipe-out is defined as a sudden, unexplained, and irreversible loss of residual central vision following glaucoma surgery, typically in eyes with advanced visual field damage and severely compromised optic nerves. The purpose of this review is to critically assess the current incidence, risk factors, pathophysiological mechanisms, and clinical relevance of "wipe-out", a rare but devastating complication of glaucoma surgery characterized by sudden, unexplained central vision loss postoperatively. A comprehensive literature review was conducted, analyzing key peer-reviewed studies from electroni

Molecular mechanisms underpinning deconditioning-update in fear memory.

Traumatic experiences are closely associated with some psychiatric conditions such as post-traumatic stress disorder. Deconditioning-update promotes robust and long-lasting attenuation of aversive memories. The deconditioning protocol consists of applying weak/neutral footshocks during reactivations, so that the original tone-shock association is replaced by an innocuous stimulus that does not produce significant fear response. Here, we present the molecular bases that can support this mechanism. To this end, we used pharmacological tools to inhibit the activity of ionotropic glutamate recepto

Mussel memory: can bivalves learn to fear parasites?

Fear plays a crucial role in predator-prey interactions and can have cascading impacts on the structure of whole ecosystems. Comparable fear effects have recently been described for hosts and their parasites but our understanding of the underlying mechanisms remains limited by the lack of empirical examples. Here, we experimentally tested if bivalves Mytilus edulis can potentially 'learn to fear' the infective transmission stages (cercariae) of the trematode Himasthla elongata, and if experienced mussels change their parasite-avoidance behaviour accordingly. Our results show that previous expe

Fear modulates parental orienting during childhood and adolescence.

Adults quickly orient toward sources of danger and deploy fight-or-flight tactics to manage threatening situations. In contrast, infants who cannot implement the safety strategies available to adults and depend heavily on caregivers for survival are more likely to turn toward familiar adults, such as their parents, to help them navigate threatening circumstances. However, work has yet to investigate how readily children and adolescents orient toward their parents in threatening or fearful contexts. The current work addressed this question using a visual search paradigm that included arrays of

Chronic gut inflammation impairs contextual control of fear.

Chronic inflammatory diseases are highly comorbid with anxiety in humans. The extent to which chronic inflammation is responsible for this relationship remains to be determined. We therefore tested the hypothesis that prolonged, but not brief, gut inflammation is sufficient to evoke anxiety-related behaviours in mice. We used the discriminative fear to context conditioning paradigm to assess fear generalization, which is a prominent feature of anxiety disorders. Gut inflammation was induced by exposure to dextran sodium sulfate (DSS) in the drinking water, a well-established rodent model of ul

Priming with skin-problems increases fear of clusters.

Trypophobia is induced by viewing multiple clustered objects. To date, several studies have investigated why certain people experience discomfort when looking at clustered patterns. Recently, "involuntary protection against dermatosis" (IPAD) hypothesis was proposed to explain the causes of trypophobia. The IPAD hypothesis suggests that involuntary aversive responses to skin diseases cause discomfort in response to image clusters. However, this idea has not been fully investigated empirically. Therefore, the present study used a modified version of the priming procedure and tested whether the

Low Perceived Self-Efficacy Impedes Discriminative Fear Learning.

Perceived self-efficacy refers to a subject's expectation about the outcomes his/her behavior will have in a challenging situation. Low self-efficacy has been implicated in the origins and maintenance of phobic behavior. Correlational studies suggest an association between perceived self-efficacy and learning. The experimental manipulation of perceived self-efficacy offers an interesting approach to examine the impact of self-efficacy beliefs on cognitive and emotional functions. Recently, a positive effect of an experimentally induced increased self-efficacy on associative learning has been d

Generalization of fear in post-traumatic stress disorder.

Overgeneralization (i.e., the transfer of fear to stimuli not related to an aversive event) is part of alterations in associative fear learning in mental disorders. In the present experimental study, we investigated whether this holds true for post-traumatic stress disorder (PTSD) related to childhood abuse. We expected that fear generalization under experimental conditions reflects generalization of aversive stimuli to different social domains in real life. Sixty-four women with PTSD after childhood abuse and 30 healthy participants (HC) underwent a differential fear conditioning and generali

[ARFID: food restriction without fear of weight gain].

Avoidant/restrictive food avoidance disorder, or ARFID, is characterized by restrictive eating or avoidance of food in the absence of the cognitive restraint and weight phobia typically seen in anorexia nervosa. It is often based on a general disinterest in eating, selective eating due to sensory preferences, and/or fear of adverse consequences such as choking, although the diagnostic criteria allow for a number of other clinical presentations. Patients with ARFID tend to be younger, more often male, and have a longer duration of illness compared to patients with other eating disorders. Delimi
