Reconstruction of a segmental bone defect using vascularized tissue-engineered bone substitutes: An animal study.

BACKGROUND: Repairing large segmental bone defects remains a major challenge in orthopedics, with conventional autografting and allografting limited by donor shortages and high complication rates. Tissue-engineered bone substitutes have emerged as a potential solution. This study aimed to assess the vascular regeneration of dual-vascularized tissue-engineered bone substitutes in the reconstruction of large segmental bone defects in rabbits. METHOD: Demineralized bone matrix (DBM) was used as a scaffold, with endothelial progenitor cells (EPCs) seeded onto it. A vascular channel was created wi

Ceramic Bone Substitutes in Orthopaedic Surgery: Standards, Trends and Future Perspectives.

In contrast to other tissues and organs, bone and its marrow have been successfully transplanted to promote bone healing for more than 100 years. Nevertheless, the amount of autologous bone grafts is limited and associated with relevant donor site morbidity. Therefore, intensive efforts have been made to develop adequate bone substitute materials. In combination with regenerative strategies including the application of bone marrow /mononuclear stroma cells, growth factors, cytokines (VEGF, PDGF, BMP2 etc.) as well as extracellular vesicles (EVs), bone substitutes (BS) may augment, supplement o

Strategies for advanced particulate bone substitutes regulating the osteo-immune microenvironment.

The usage of bone substitute granule materials has improved the clinical results of alveolar bone deficiencies treatment and thus broadened applications in implant dentistry. However, because of the complicated mechanisms controlling the foreign body response, no perfect solution can avoid the fibrotic encapsulation of materials till now, which may impair the results of bone regeneration, even cause the implant materials rejection. Recently, the concept of 'osteoimmunology' has been stressed. The outcomes of bone regeneration are proved to be related to the bio-physicochemical properties of bi

Controlling the biological function of calcium phosphate bone substitutes with drugs.

There is a growing interest in bone tissue engineering for bone repair after traumatic, surgical or pathological injury, such as osteolytic tumor or osteoporosis. In this regard, calcium phosphate (CaP) bone substitutes have been used extensively as bone-targeting drug-delivery systems. This localized approach improves the osteogenic potential of bone substitutes by delivering bone growth factors, thus extending their biofunctionality to any pathological context, including infection, irradiation, tumor and osteoporosis. This review briefly describes the physical and chemical processes implicat

A meta-analysis of alveolar bone grafting using bone substitutes in cleft lip and palate patients.

In individuals with cleft lip and palate (CLP), an alveolar bone graft (ABG) is carried out for alveolar cleft closure. Several sources for ABG include autologous bone, xenologous bone, and alloplastic substitutes. Autologous bone has been the preferred source for ABG. Alloplastic substitutes might serve as an alternative. This study aimed to compare the outcomes between autologous and alloplastic as sources for ABG. This study made use of eight web databases. Randomized control trials (RCTs) and non-RCTs were included. CLP patients with alveolar cleft with imaging studies, computed tomography

The role of orthobiologics in foot and ankle surgery: Allogenic bone grafts and bone graft substitutes.

Orthobiologics are biological substances that are used therapeutically for their positive effects on healing skeletal and soft-tissue injuries. The array of orthobiological products currently available to the foot and ankle surgeon is wide, and includes bone allografts, bone substitutes, growth factors, and chondral scaffolds. Nonetheless, despite the surge in interest and usage of orthobiologics, there remains a relative paucity of research addressing their specific applications in foot and ankle surgery. In this review, we attempt to provide an overview of the literature on commonly availabl

Fantastic plastic? Experimental evaluation of polyurethane bone substitutes as proxies for human bone in trauma simulations.

Recent years have seen steady improvements in the recognition and interpretation of violence related injuries in human skeletal remains. Such work has at times benefited from the involvement of biological anthropologists in forensic casework and has often relied upon comparison of documented examples with trauma observed in skeletal remains. In cases where no such example exists investigators must turn to experimentation. The selection of experimental samples is problematic as animal proxies may be too dissimilar to humans and human cadavers may be undesirable for a raft of reasons. The curren

Fusion in degenerative spondylolisthesis: comparison of osteoconductive and osteoinductive bone graft substitutes.

INTRODUCTION: The emergent widespread options of bone graft substitutes for spinal fusion procedures vary in their osteobiologic activity. A majority of current literature focuses on the comparison of osteoinductive (OI) or osteoconductive (OC) bone graft substitutes individually against ICBG. These studies have demonstrated the legitimacy of bone graft substitutes, but despite the widespread use in spinal fusion procedures there is a dearth in the current literature in the direct comparison of OC and OI substitutes. This retrospective comparative analysis compares the efficacy of OI vs. solel

Beta-TCP bone graft substitutes in a bilateral rabbit tibial defect model.

The use of artificial bone graft substitutes has increased as the surgical applications widen and the availability of allograft bone decreases. The ideal graft substitute should reabsorb with time to allow and encourage new bone formation whilst maintaining its properties as an osteoconductive scaffold until it is no longer required. A potential disadvantage of some synthetic substitutes is their long dissolution time. Beta-tricalcium phosphates (beta-TCPs) have some advantages when compared to hydroxyapatite (HA), when used as a filler, in that it is more rapidly reabsorbed. Three commerciall

Bone graft substitutes and allografts for reconstruction of the foot and ankle.

In reconstructive foot and ankle surgery, the use of bone graft is common. Whether for trauma, acquired or congenital deformities, arthrodeses, joint replacement, bone loss from infection, or bone tumor resection, the foot and ankle surgeon must be knowledgeable about current bone grafting options to make informed decisions. Innovation and technologic advances have produced an impressive and exciting array of options, advancing us closer to mimicking the gold standard: autograft. However, the sheer volume of available products makes it challenging for the foot and ankle surgeon to stay abreast

Synthetic Bone Graft Substitute for Treatment of Unicameral Bone Cysts.

BACKGROUND: Treatments for unicameral bone cysts (UBCs) have high documented failure rates (27% to 63%) because of recurrence or persistence of the cyst, similar to nonoperative management. Recent evidence suggests that filling of the defect with a synthetic bone graft substitute (SBGS) supports the weakened cortex and promotes new bone growth. A calcium sulfate, brushite, calcium phosphate, composite graft material (PRODENSE, Wright Medical, Memphis, TN) has been evaluated as a substitute for autogenous or allogenous graft in animal and human studies. The purpose of this study was to compare

Treatment of benign bone lesions with an injectable biphasic bone substitute.

BACKGROUND: Injectable biphasic ceramic bone substitutes (BCBSs) represent a modern alternative to conventional options for bone defect filling, as they further open the possibilities for percutaneous cavity reconstruction. Although recent studies have shown good surgical outcomes after treatment with BCBSs, mid-term follow-up data are still missing. PATIENTS AND METHODS: Between 2013 and 2017, 18 patients were [1] treated with BCBS [2] for benign bone lesions and [3] had a complete set of retrospective information, including surgical protocols, imaging, patient dismission letters and outpati

FDA-approved bone grafts and bone graft substitute devices in bone regeneration.

To induce bone regeneration there is a complex cascade of growth factors. Growth factors such as recombinant BMP-2, BMP-7, and PDGF are FDA-approved therapies in bone regeneration. Although, BMP shows promising results as being an alternative to autograft, it also has its own downfalls. BMP-2 has many adverse effects such as inflammatory complications such as massive soft-tissue swelling that can compromise a patient's airway, ectopic bone formation, and tumor formation. BMP-2 may also be advantageous for patients not willing to give up smoking as it shows bone regeneration success with smoker

Autografts, Bone Substitutes, and Combined Approaches for Secondary Alveolar Bone Grafting: A Systematic Review and Meta-Analysis.

BACKGROUND: Alveolar clefts require bone grafting to restore function and esthetics. Although autogenous iliac crest grafts remain gold standard, their morbidity motivates exploration of alternative materials. PURPOSE: This systematic review and meta-analysis evaluated 3 approaches to alveolar cleft repair: 1) autogenous bone grafts combined with alternative bone substitute materials (group A), 2) alternative bone substitutes alone (group B), and 3) autogenous iliac crest grafts as the reference standard (group C).
