Improvement in the neutron beam collimation for application in boron neutron capture therapy of the head and neck region.

In June 2020, the Japanese government approved boron neutron capture therapy for the treatment of head and neck cancer. The treatment is usually performed in a single fraction, with the neutron irradiation time being approximately 30-60 min. As neutrons scatter in air and loses its intensity, it is preferable to bring the patient as close to the beam port as possible to shorten the irradiation time. However, this can be a challenge, especially for patients with head and neck cancer, as the shoulders are an obstacle to a clean positioning. In this study, a novel neutron collimation system for a

Boron encapsulated in a liposome can be used for combinational neutron capture therapy.

Boron neutron capture therapy (BNCT) is an attractive approach to treat invasive malignant tumours due to binary heavy-particle irradiation, but its clinical applications have been hindered by boron delivery agents with low in vivo stability, poor biocompatibility, and limited application of combinational modalities. Here, we report boronsome, a carboranyl-phosphatidylcholine based liposome for combinational BNCT and chemotherapy. Theoretical simulations and experimental approaches illustrate high stability of boronsome. Then positron emission tomography (PET) imaging with Cu-64 labelled boron

A realistic appraisal of boron neutron capture therapy as a cancer treatment modality.

Boron neutron capture therapy (BNCT) is a binary therapeutic modality based on the nuclear capture and fission reactions that occur when the stable isotope boron-10 is irradiated with neutrons to produce high-energy alpha particles and recoiling lithium-7 nuclei. In this Commentary we will focus on a number of papers that were presented at a Symposium entitled "Current Clinical Status of Boron Neutron Capture Therapy and Paths to the Future", which was held in September 2017 at the China National Convention Center in Beijing. Results were presented by clinicians from Japan, Finland, the United

A study on out-of-field leakage of an accelerator-based neutron beam for boron neutron capture therapy.

More and more accelerator-based boron neutron capture therapy (AB-BNCT) facilities are under the construction or commissioning stage, and the neutron beam characteristic measurements at each facility will start soon. In addition to the in-field neutron beam properties, the leakage of neutron beam is also important, which is related to the side effects of the patient. In the Virtual Technical Meeting on Advances in Boron Neutron Capture Therapy held by International Atomic Energy Agency (IAEA) in July 2020, the issue of the out-of-field leakage in BNCT was addressed. Development and dosimetric evaluation of a freely deformable 6Li-based neutron shield for boron neutron capture therapy.

BACKGROUND: Boron neutron capture therapy (BNCT) enables selective tumor irradiation by exploiting the high-linear energy transfer particles generated from neutron interactions with 10B atoms. BNCT has been approved as an insurance-covered medical treatment for recurrent head and neck cancer in Japan. Unlike photon radiotherapy, neutrons that come out of the collimator have an angular distribution. Therefore, it is necessary to keep the distance between the collimator and the patient as short as possible. However, for head and neck cancer treatments, patient anatomy often limits proximity to t

Optimization of target materials for a 3 MeV proton-driven accelerator neutron source for boron neutron capture therapy.

Choosing the right target material for a 3 MeV (10 mA, 30 kW beam power) proton-driven accelerator-based boron neutron capture therapy (AB-BNCT) system is far from straightforward a material that produces the most neutrons under proton bombardment does not necessarily deliver the best therapeutic beam. This paper addresses that gap through a systematic two-phase PHITS (version 3.28) study of four candidate targets: lithium (Li), beryllium (Be), beryllium oxide (BeO), and iron (Fe), simulated over target thicknesses of 0.01-1.4 cm. In Phase 1 (proton-driven), lithium produced the highest raw th

Biological evaluation of a glucose-based boron carrier as a potential agent for boron neutron capture therapy.

Boron neutron capture therapy (BNCT) is an innovative radiation oncology approach that targets tumors selectively, minimizing damage to healthy tissues through high-linear-energy-transfer particles released during the boron neutron capture reaction. Current boron carriers like sodium mercaptoundecahydrododecaborate (BSH) and L-p-boronophenylalanine (BPA) face limitations in specificity and solubility. Our recently developed 6-O-(o-carboranylmethyl)-d-glucopyranose (B-Glc) shows promise as an alternative, demonstrating strong interactions with glucose transporters in human head and neck squamou

On-Demand Biodegradable Boron Nitride Nanoparticles for Treating Triple Negative Breast Cancer with Boron Neutron Capture Therapy.

Compared with photon-induced binary cancer therapy, such as photothermal therapy (PTT) and photodynamic therapy (PDT), boron neutron capture therapy (BNCT) emerges as an alternative noninvasive treatment strategy that could overcome the shallow penetration of light. One key factor in performing successful BNCT is to accumulate a sufficient amount of B-10 (>20 ppm) within tumor cells, which has been a long-standing challenge for small-molecule-based boron drugs. Boron nitride nanoparticles (BNNPs) are promising boron carriers due to their high boron content and good biocompatibility, as certain

Compassionate boron neutron capture therapy for locally recurrent nasopharyngeal cancer: A retrospective study.

BACKGROUND: Boron neutron capture therapy (BNCT) is a targeted form of particle radiotherapy (RT) that better spares normal tissues than does conventional photon RT. We describe our experience with compassionate-use BNCT for locally recurrent nasopharyngeal cancer (rNPC) after prior RT. METHODS: Data from patients with rNPC who received BNCT outside of clinical trials at the Tsing-Hua Open-Pool Reactor between 2020 and 2024 were retrospectively analyzed. RESULTS: Ten patients (eight men and two women) with a median age of 54 years and recurrent stage T2-T4N0-N1 disease were included. The med

Salvage boron neutron capture therapy for pediatric patients with recurrent diffuse midline glioma.

PURPOSE: Pediatric diffuse malignant glioma located in the brainstem was officially named "diffuse midline glioma" (DMG) by the World Health Organization in 2016. For this disease, radical surgery is not beneficial, and the only major treatment strategy is radiotherapy. However, the dose limitations to brainstem tissue mean that treatment by radiotherapy can only control and not eradicate the tumors, and there is no effective treatment for recurrence, resulting in short overall survival of 6-12 months. This paper reports our experience with boron neutron capture therapy (BNCT), a new treatment

A dose calculation algorithm for boron neutron capture therapy using convolution/superposition method.

The convolution/superposition (C/S) method originally designed for photon dose calculation was first applied for developing a treatment planning system for boron neutron capture therapy. The original concept of TEGMA (total energy generated per unit mass) was proposed to represent distinctive dose components from neutron reactions with the elements in the patient's tissue. First, neutron fluence distributions in a homogeneous brain phantom irradiated with an energy-groupwise pencil beam of 2.5 × 2.5 mm2 were calculated using the MCNP6.2 code. Then, a library of energy-groupwise TEGMA and KERMA

A case of radiation-induced osteosarcoma treated effectively by boron neutron capture therapy.

We treated a 54-year-old Japanese female with a recurrent radiation-induced osteosarcoma arising from left occipital skull, by reactor-based boron neutron capture therapy (BNCT). Her tumor grew rapidly with subcutaneous and epidural extension. She eventually could not walk because of cerebellar ataxia. The tumor was inoperable and radioresistant. BNCT showed a marked initial therapeutic effect: the subcutaneous/epidural tumor reduced without radiation damage of the scalp except hair loss and the patient could walk again only 3 weeks after BNCT. BNCT seems to be a safe and very effective modali

10Boron-doped carbon nanoparticles as delivery platforms for boron neutron capture therapy and photothermal therapy.

The current clinical application of the boron drug boronophenylalanine (BPA) in BNCT faces various issues owing to its low boron loading (approximately 5%), which limits its therapeutic efficacy. Therefore, the development of boron drugs with higher boron contents is essential. Enhancing the boron content in boron drug materials is the focus of this study. Two-dimensional (2D) boron nitride-doped nano graphene (BNNG) with a high boron content of 24.97% ± 1.14% w/w was synthesized via the chemical vapor deposition (CVD) method. The size of BNNG was controlled through gradient density centrifuga

Incorporating boron distribution variations in microdosimetric kinetic model-based relative biological effectiveness calculations for boron neutron capture therapy.

This study introduces the MKM_B model, an approach derived from the MKM model, designed to evaluate the biological effectiveness of Boron Neutron Capture Therapy (BNCT) in the face of challenges from varying microscopic boron distributions. The model introduces a boron compensation factor, allowing for the assessment of compound Biological Effectiveness (CBE) values for different boron distributions. Utilizing the TOPAS simulation platform, the lineal energy spectrum of particles in BNCT was simulated, and the sensitivity of the MKM_B model to parameter variations and the influence of cell siz

A Retinoid X Receptor Ligand-Based Borocaptate Conjugate with Enhanced Intracellular Boron Retention for Boron Neutron Capture Therapy.

Boron neutron capture therapy requires sustained intracellular 10B accumulation, yet the clinical standard, L-boronophenylalanine (BPA, 1), rapidly effluxes from cells. We explored an intracellular retention strategy by converting borocaptate (BSH, 2) into a nuclear receptor ligand-based conjugate to improve intracellular retention of boron delivery agents. Retinoid X receptor (RXR) ligand scaffolds were selected as a proof-of-concept platform because RXRs are broadly expressed and predominantly nuclear. The lead conjugate CBTF-BSH (3), designed from fluorescent RXR antagonist 5, showed greate

Bone Marrow-Derived Macrophages Loaded with Boron Carbide Nanoparticles Targeting the Glioblastoma Microenvironment for Boron Neutron Capture Therapy.

INTRODUCTION: Boron neutron capture therapy (BNCT) is a targeted radiotherapy that represents a promising treatment for glioblastoma multiforme. BNCT efficacy depends on selective boron carriers capable of crossing the blood-brain barrier (BBB). Therefore, we propose an original strategy for boron delivery in BNCT, based on the cellular carriers. This study assessed the ability of bone marrow-derived macrophages (BMDMs) to accumulate boron carbide (B4C) nanoparticles, transport them across an in vitro BBB model toward glioblastoma cell-conditioned medium, and interact with tumor cells.
