Varicocele repair in the era of modern assisted reproductive techniques.

PURPOSE OF REVIEW: This review looks at the literature on varicocele repair and its effect on assisted reproductive techniques. The effects of varicocele correction on improved sperm production have been well documented with semen analysis data. What remain to be elucidated are the causes of the varicocele effect and how correction of the resultant pathophysiology may affect the outcomes of modern assisted reproductive technology. RECENT FINDINGS: Basic science research shows us that varicoceles exert deleterious effects on Leydig cells, Sertoli cells, and germ cells via very different mechan

Five cases of dizygotic triplet pregnancy following assisted reproductive techniques.

Of 816 patients who became clinically pregnant by assisted reproductive techniques between September 2000 and August 2004, we experienced 10 cases (1.2%) of monozygotic twinning, and in five of these 10 cases, implantation of another embryo resulted in dizygotic triplets. Here, we report these five cases of dizygotic triplets. Fresh embryo transfer was performed in all five cases. Intracytoplasmic sperm injection or assisted hatching was not carried out in these cases. Blastocyst transfer was performed in three cases. Three embryos were transferred in case 1 (40-year-old female). While only tw

Assisted Reproductive Techniques in Multiple Sclerosis: Recommendations from an Expert Panel.

INTRODUCTION: Multiple sclerosis (MS) is mainly diagnosed in women of reproductive age. However, there is a paucity of guidelines jointly prepared by neurologists and gynaecologists on managing women with MS and the desire for motherhood. Therefore, in this review we propose recommendations for such cases, with an particular focus on those requiring assisted reproductive techniques (ART). METHODS: A group of seven MS experts (4 neurologists and 3 gynaecologists) came together for three discussion sessions to achieve consensus. RESULTS: The recommendations reported here focus on the importanc

Could assisted reproductive techniques affect equine fetal membranes and neonatal outcome?

Embryo transfer (ET) and intracytoplasmic sperm injection (ICSI) are widely used in equine species, but their effects on fetal adnexa and neonates have not been investigated yet. The aim of this study was to retrospectively evaluate whether pregnancies obtained by ET or ICSI could be associated with the presence of macroscopic alterations of fetal membranes (FM) and umbilical cord (UC) and if the use of these techniques could influence neonatal outcome. Sixty-six light breed mares hospitalized at the Veterinary Teaching Hospital, University of Bologna, for attending delivery were included in t

Outcome of twin pregnancies after assisted reproductive techniques--a comparative study.

OBJECTIVE: To compare obstetrical and perinatal outcome of twin pregnancies after assisted reproductive techniques (ART) with that of twins conceived spontaneously. STUDY DESIGN: Hospital based retrospective study. RESULTS: There were 132 twin deliveries of which 36 were conceived after ART. Patients of the ART group were mostly nulliparous and slightly older. There was no statistically significant difference in the frequency of preterm delivery or mean gestational age at delivery. Elective Caesarean delivery was more frequent in twin pregnancies conceived after ART, and there were no other

Reproductive Health After Kidney Transplantation: Insights Into Infertility and Assisted Techniques.

Fertility challenges are significantly more prevalent among individuals with chronic kidney disease compared with healthy populations. Kidney transplant often restores reproductive potential and makes parenthood attainable for many recipients; however, a considerable proportion remains infertile. This infertility is multifactorial, arising from sexual dysfunction, hormonal disturbances, and uremia-induced gonadal toxicity. Patients should be counseled on potential adverse effects of medications for fertility, pregnancy progression, and fetal health. Interdisciplinary care is particularly impor

[Assessment of genital microbiota: An emerging approach in assisted reproductive techniques].

The genital microbiota actively participates in women's reproductive health. Indeed, a genital dysbiosis (microbial imbalance associated with adverse effects on host health) can lead to vaginal infections (such as mycoses or bacterial vaginosis). Recent data reported that genital dysbiosis (e.g. vaginal or endometrial) was associated with fewer chances of live births in assisted reproductive technologies (ART), via decreased pregnancy rates and an increased risk of miscarriages. The presence or diversity of certain bacterial strains (in particular Gardenellavaginalis, Proteobacteria, Lactobaci

[Assisted reproductive techniques in single women: Which proposals for which demands?].

The French bio-ethic law concerning ART is more restricted than in other countries. Techniques can only be applied in heterosexual couples presenting a documented infertility. Nevertheless, concerns about fertility planning are numerous in young women, leading to a growing demand of reproductive medicine consultations. Two situations can be distinguished: firstly, single patients wishing sperm donation and, secondly, single patients who wish to preserve their fertility for future parenting project. This latter situation can be discussed in the French legislative context while the other will re

Reproductive support of a valuable canine colony with assisted reproductive technique - Short communication.

For many years, artificial insemination (AI) has been regarded as one of the most significant assisted reproductive techniques. The objective of our manuscript is to summarise the preliminary results of the program in which AI with fresh and frozen semen was being used to support the reproduction of a foxhound colony with valuable genetic properties. In this manuscript, three AI cases are presented, one with fresh and two with frozen semen.Artificial insemination with fresh semen was conducted two days after the LH peak, when the progesterone (P4) concentration exceeded 5 ng*mL-1. Assisted reproductive technology in South Africa: first results generated from the South African Register of Assisted Reproductive Techniques.

OBJECTIVE: We present the first report from the South African Register of Assisted Reproductive Techniques. METHODS: All assisted reproductive technology (ART) centres in South Africa were invited to join the register. Participant centres voluntarily submitted information from 2009 on the number of ART cycles, embryo transfers, clinical pregnancies, age of female partners or egg donors, and use of fertilisation techniques. Data were anonymised, pooled and analysed. RESULTS: The 12 participating units conducted a total of 4 512 oocyte aspirations and 3 872 embryo transfers in 2009, resulting

Advanced sperm selection techniques for assisted reproduction.

RATIONALE: Assisted reproductive technologies (ART), including in vitro fertilisation (IVF) and intracytoplasmic sperm injection (ICSI), combine an egg and sperm outside the body to enhance the probability of fertilisation and pregnancy. Advanced sperm selection techniques are increasingly employed in ART, most commonly in cycles utilising ICSI. Advanced sperm selection techniques are hypothesised to improve the likelihood that structurally intact and mature sperm with high DNA integrity are selected. Strategies include selection according to surface charge; sperm apoptosis; sperm birefringenc

Advanced Sperm Selection Techniques for Assisted Reproduction.

Male infertility accounts for approximately 40% of infertility cases. There are many causes of male infertility, including environmental factors, age, lifestyle, infections, varicocele, and cancerous pathologies. Severe oligozoospermia, cryptozoospermia, and azoospermia (obstructive and non-obstructive) are identified as severe male factor infertility, once considered conditions of sterility. Today, in vitro fertilization (IVF) techniques are the only treatment strategy in cases of male factor infertility for which new methodologies have been developed in the manipulation of spermatozoa to ach

[Assisted reproductive technique for an opiates dependant couple].

The aim of our network "Motherhood and Addiction Alsace" is to take care of opiates addicted pregnant women in order to permit the birth of a healthy child, raised by stabilized parents both on drug use and psychosocial status. A couple, both on methadone treatment, with chronic hepatitis for the husband had access to an in vitro fertilization program thanks to the network care. The difficulties on the path to parenthood, the adequate use of painkillers, neonatal care and the use of a coordinated action of all health professionals are discussed.
