To treat or not to treat: a clinical series of retinal arterial macroaneurysms: A single-center retrospective study.

Retinal arterial macroaneurysms (RAMs) develop as outpouchings of the arterial wall that is weakened by arteriosclerosis. The traditional treatment of RAMs comprises observation, focal laser photocoagulation, or surgery. Recently, intravitreal injection of anti-vascular endothelial growth factor (VEGF) drugs has been announced as an effective therapy for fovea-threatening RAMs and quickly improve visual acuity and central retinal thickness (CRT).In the retrospective series, medical charts and ocular images of 24 patients diagnosed as having RAM between May 2011 and November 2018 in our facilit

Appearance of retinal arterial macroaneurysms in patients using swept-source optical coherence tomographic angiography.

BACKGROUND: Retinal arterial macroaneurysm (RAM) is a common clinical disease leading to vision loss in elderly individuals. The appropriate interpretation of swept-source optical coherence tomographic angiography (SS-OCTA), a noninvasive examination, is easy and convenient for detecting the status of RAMs and guiding treatment. METHODS: The objectives of this study were to describe the morphologic characteristics of RAMs using SS-OCTA and to observe whether there are differences in the morphologies of RAMs between SS-OCTA and fundus fluorescein angiography (FFA), before and after treatment.

Macular Hole Formation Six Months after Hemorrhage Displacement for Submacular and Henle Fiber Layer Hemorrhage due to Retinal Arterial Macroaneurysm Rupture.

A 78-year-old woman presented with sudden vision loss and central scotoma. Visual acuity in the right eye was 20/222, with submacular hemorrhage (SMH) and Henle fiber layer hemorrhage (HFLh) due to retinal arterial macroaneurysm (RAM) rupture. She underwent SMH displacement, including cataract surgery, vitrectomy, intravitreal injection of tissue-plasminogen activator, and air tamponade. Three months postoperatively the SMH and HFLh had disappeared and visual acuity had improved to 20/200. Six months postoperatively, a macular hole had developed. We performed an inverted internal limiting memb

Multifocal intracranial aneurysms and a clinical diagnosis of ruptured retinal arterial macroaneurysm in presumed hepatitis B-associated polyarteritis nodosa: A case report.

Polyarteritis nodosa (PAN) is a necrotizing medium-artery vasculitis; intracranial aneurysms are rare. A woman in her early thirties with hypertension and hepatitis B surface antigen (HBsAg) positivity presented with thunderclap headache, vomiting, and blurred vision. Available records described intracranial hemorrhage associated with a right posterior cerebral artery (PCA) aneurysm, but the index computed tomography/angiography images and full report were unavailable to classify the hemorrhage. The aneurysm was coil embolized with intentional parent-vessel sacrifice. Subsequent magnetic reson

Isolated retinal venous macroaneurysm.

BACKGROUND: Unlike retinal arterial macroaneurysms, retinal venous macroaneurysms are rare. Retrospective studies have previously described venous macroaneurysms predominantly in association with branch retinal vein obstruction. Only 2 cases of isolated retinal venous macroaneurysm without venous occlusion have been described in the literature. The patient being described in this case report has the third isolated nonocclusive venous macroaneurysm to be reported. CASE: A healthy 43-year-old white woman presented for a routine eye examination with no history of visual disturbance other than lo

Multimodal imaging in pediatric arterial macroaneurysm: A case report.

Our aim is to report a case of asymptomatic retinal arterial macroaneurysm in a 9-year-old female patient. She was referred to our Ophthalmology clinic for a routine ophthalmologic examination and, after the detection of a juxtafoveal saccular vascular enlargement in indirect ophthalmoscopy, underwent a multimodal imaging assessment. Optical coherence tomography-angiography and fluorescein angiography were important to determine the nature of the lesion, identified as a congenital retinal arterial macroaneurysm. Retinal vascular abnormalities represent a rare finding in pediatric patients and

Multimodal imaging of congenital retinal macrovessel with secondary foveal hypoplasia.

Congenital retinal macrovessel (CRM) is defined as a large aberrant blood vessel that traverses through the central macula. It can have large tributaries extending on both sides of the horizontal raphe. The condition is typically asymptomatic. However, visual loss may be reported in case of associated branch retinal artery occlusion, cavernous haemangioma, retinal artery macroaneurysm, retinal deep capillary ischaemia, retinal detachment and vitreous haemorrhage. However, association of CRM and foveal hypoplasia has rarely been reported. We report multimodal imaging in a patient who was present

[Hypertensive retinopathy].

Retinal blood vessels differ from most other vessels in the body (with the exception of those in the brain) in two important respects. The first is the presence of blood-retinal barriers, best illustrated by fluorescein angiography. The second important difference is that retinal vessels do not have sympathetic innervation and blood flow is modulated by autoregulation mechanisms. In 1939 Keith, Wagener and Barker proposed a classification system for hypertensive retinopathy which was innovative and of prognostic importance at that time. However, the different features of hypertensive and arter

Under pressure: Foveal mountain peak sign and macular hole after ruptured RAM.

To describe a case of a macular hole (MH) secondary to a submacular hemorrhage caused by a ruptured retinal arterial macroaneursym (RAM). An individual case and its observations were examined. Initial examination revealed no visible MH due to the hemorrhagic clot. Two weeks later, a full-thickness MH measuring 340 μm was detected. Pneumatic displacement with sulfur hexafluoride (SF6) was performed to reduce hemorrhagic toxicity and potentially promote spontaneous closure. However, after three months, the MH persisted with no evidence of closure, and visual acuity remained reduced (20/150). The

Neodymium:YAG laser treatment for hemorrhages under the internal limiting membrane and posterior hyaloid face in the macula.

BACKGROUND: A dense premacular hemorrhage may occur from proliferative diabetic retinopathy, a ruptured retinal artery macroaneurysm, or Valsalva retinopathy. Spontaneous clearing of the hemorrhage in Valsalva retinopathy usually occurs, taking several months. In diabetic retinopathy, a traction macular detachment may result as early as 5 weeks after the hemorrhage. In diabetic retinopathy or a macroaneurysm, only a fibrotic epiretinal membrane overlying the macula may develop. Observation or vitrectomy is the current way of managing a dense premacular hemorrhage. This study was undertaken to

Pathophysiology and management of subretinal hemorrhage.

Subretinal hemorrhage can arise from the retinal and/or choroidal circulation. Significant subretinal hemorrhage occurs in several conditions, but most commonly is associated with age-related macular degeneration, presumed ocular histoplasmosis, high myopia, retinal arterial macroaneurysm, and trauma. Released toxins, outer retinal shear forces, and a diffusion barrier created by subretinal hemorrhage all contribute to photoreceptor damage and visual loss. The use of tissue plasminogen activator and improvements in surgical instrumentation have facilitated surgical drainage and have made it a

[Hypertension-related eye disorders].

Hypertension affects a significant proportion of the population, however, it is often diagnosed with a delay. The aim of this article is to review the well known and less known eye abnormalities related to hypertension, and place them in the context of population based studies. Hypertension affects various parts of the eye. The originally classified hypertensive retinopathy (retinal microvascular changes) is still relevant, but new features are visible in cases of controlled hypertension. Signs of mild hypertensive retinopathy are more common than expected occurring in nearly 10-15% of the adu
