BACKGROUND: The association between Wilms' tumor (WT) and genitourinary (GU) anomalies has long been appreciated; however, associated GU anomalies have been described almost exclusively in males.
METHODS: To investigate whether females with WT also have an increased prevalence of GU anomalies, the authors evaluated the uterine anatomy of 24 WT survivors using magnetic resonance imaging and ultrasonography.
RESULTS: Two of 24 female survivors (8%) had anomalies. One had a septate uterus, and a limited molecular analysis of her constitutional DNA revealed no mutations or deletions of the tumor

Study of genotype/phenotype relationships involving the Wilms' tumor (WT) gene, WT1, in WT patients has provided insights into the function of the WT1 protein, a transcriptional regulator, and has suggested possible mutational mechanisms important in the etiology of WT. For example, the identification of deletion/insertion mutations in the first exon implicates a deletion hotspot consensus sequence in the etiology of these mutations. The disproportionate number of WT/aniridia patients with such mutations further suggest that this genetic mechanism may be enhanced by the hemizygous state. WT1 m

We reviewed 56 cases in which the preoperative diagnosis was Wilms' tumor at the Children's Hospital of Philadelphia (CHOP) during the three calendar years 1983 through 1985. Fifty-one had Wilms' tumor (38 operated on at CHOP; 13 were referrals for further treatment or consultation), three had renal cell carcinoma (RCC), and one each with metastatic mesoblastic nephroma and clear cell sarcoma. We reviewed these cases to determine the significance of the urographic finding of nonopacification of kidney with suspected Wilms' tumor, and what additional diagnostic studies are needed. Five Wilms' t

A high percentage of children with Wilms' tumor are cured with multimodal treatment. A small percentage of these children will develop second tumors, perhaps related to a genetic predisposition to neoplasia or possibly secondary to the treatment utilized for Wilms' tumor. Malignant mesothelioma has been associated with contact with asbestos but has also been reported after radiation exposure. Two patients are reported who developed malignant mesothelioma of the pleura after treatment for Wilms' tumor in childhood. Both received orthovoltage radiation; one patient also received triethylenemelam

The case history of a 8 year old boy is described who presented with haematuria without other signs or symptoms. No abdominal mass could be palpated. Excretory urography revealed a tumor in the right pyelum . Histological examination confirmed the diagnosis. The tumor was mainly located in the submucosal area of the right pyelum and proximal ureter.

Recent advances in the diagnosis and treatment of Wilms' tumor are reviewed. Our study disclosed that principal clinical and pathological features of Wilms' tumor are as follows: (1) It occurs in young infants and children under 5 years of age; (2) It is frequently associated with other congenital anomalies; (3) Deletion of a portion of short arm of chromosome 11 can be closely related to the tumor genesis of nephroblastoma; (4) The occurrence of distant metastases is rare at diagnosis; (5) Differential diagnosis between Wilms' tumor and neuroblastoma is occasionally quite difficult; (6) There

Five children with intrathoracic metastases from Wilms tumor are presented. In two patients pneumothoraces developed, and in one patient pneumonitis developed secondary to a primary intrabronchial metastasis. Two other patients had paraspinal widening from metastases to paraaortic or posterior mediastinal lymph nodes. These are extremely rare complications of metastatic Wilms tumor.

PURPOSE: According to the guidelines of International Society of Pediatric Oncology (SIOP) and National Wilms Tumor Study (NWTS), Wilms tumor with preoperative rupture should be classified as at least stage III. Few clinical reports can be found about preoperative Wilms tumor rupture. The purpose of this study was to investigate our experience on the diagnosis, treatment and prognosis of preoperative Wilms tumor rupture.
METHODS: Patients with Wilms tumor who underwent treatment according to the NWTS or SIOP protocol from January 2008 to September 2017 in Beijing Children's Hospital were revi

We present a case of a 17-year-old pregnant girl, with hematuria. Magnetic resonance imaging showed a solid left renal mass. The patient had a radical nephrectomy during the 20th week gestation and the diagnosis was nephroblastoma. She gave birth to a healthy child at term and refused adjuvant treatment. She is disease-free after 24 months of follow-up. Based on the excellent results achieved while treating children with Wilms' tumor using surgery, chemotherapy and radiotherapy, adults with the same disease should be treated similarly. This treatment must be instituted early, even in pregnant

Hirschsprung disease (HD) has been associated with multiple renal malformations. We describe a child with HD who also had Wilms tumor (WT). Because the RET protooncogene, the major susceptibility gene for HD, also plays a role in normal kidney development and in the multiple endocrine neoplasia type 2 cancer syndrome, we analyzed our patient's constitutional DNA for mutations in RET commonly found in multiple endocrine neoplasia type 2A. Although this limited evaluation failed to show abnormalities, we speculate that comprehensive analysis of the RET gene may identify mutations to account for

Urticaria pigmentosa is the most common manifestation of mastocytosis, with the majority of cases undergoing spontaneous resolution, especially in children. Several reports have documented hematologic malignancies developing in patients with urticaria pigmentosa. We present a 4.5-year-old boy with urticaria pigmentosa who developed Wilms tumor. To our knowledge, coexisting urticaria pigmentosa and Wilms tumor have not previously been described.

Wilms tumor or nephroblastoma is a pediatric kidney cancer arising from pluripotent embryonic renal precursors. Multiple genetic loci have been linked to Wilms tumorigenesis; positional cloning strategies have led to the identification of the WT1 tumor suppressor gene at chromosome 11p13. WT1 encodes a zinc finger transcription factor that is inactivated in the germline of children with genetic predisposition to Wilms tumor and in a subset of sporadic cancers. When present in the germline, specific heterozygous dominant-negative mutations are associated with severe abnormalities of renal and s

A 2-year-old boy presented with an atypical renal mass with extension into the pyelocalyceal system and distal ureter which was diagnosed initially as xanthogranulomatous pyelonephritis (XGP). The final diagnosis was intrapelvic Wilms tumor extending into the ureter. In the literature, only three patients have been described with Wilms tumor extending into the ureter, none of whom had a continuous extension into the distal ureter.

We have evaluated the clinicopathologic characteristics, survival probabilities, patterns of treatment failure, and late toxicities in a group of 51 Wilms' tumor patients with pulmonary metastases evaluated at our center between 1968 and 1988 and treated intensively with surgery, multiagent chemotherapy, and radiotherapy. Twenty-one patients had pulmonary metastases at diagnosis (Stage IV-p) and 30 had pulmonary relapse after initial treatment for nonmetastatic disease. With a median follow-up time of 83 months, actuarial 5- and 10-year survival probability for the group as a whole is 59%. For

A case of Wilms' tumor in a 67 year-old female is presented. The tumor totally replaced the left kidney with extension to Gerota's fascia, the adrenal gland and ureter, with tumor thrombi in the left renal vein. Multiple bony metastases to the skull, shoulder, rib, ilium, acetabulum and femur were noted also. Microscopically, the tumor was composed predominantly of blastemal tissue with diffuse anaplasia with areas of tubular differentiation and chondroid elements. Clear cell nests were found in the proximal ureter with gradual maturation downward.

Heritability of the unilateral-sporadic (non-familial) form of Wilms tumor was examined in the offspring of 96 long-term survivors of the neoplasm. No Wilms tumor has developed in any of the 179 offspring of these patients. The maximum likelihood estimate of a hereditary Wilms tumor in our patients is zero and the corresponding 95% upper confidence limit ranges between 0.06 and 0.11, depending on penetrance. Among their offspring, the 95% upper bound of the risk of Wilms tumor is 0.02. These figures can be applied in genetic counseling of other survivors of unilateral-sporadic Wilms tumor.

We report two children with mediastinal metastasis of Wilm's tumor. One case presented with a right hilar mass and the other with a posterior mediastina mass. In each instance, no additional radiologic evidence of metastasis was found. Metastasis of Wilms' tumor to the mediastinum is uncommon, usually occurring with radiologic evidence of either additional metastases or direct cephalad growth of the primary tumor.
