Randomized trial of parent-child interaction therapy improves child-welfare parents' behavior, self-regulation, and self-perceptions.

OBJECTIVE: We conducted a large (N = 204) randomized, clinical trial to test the efficacy of parent-child interaction therapy (PCIT) on observed parenting, two key drivers of maladaptive parenting-self-regulation and social cognitions, and child behavior outcomes in a sample of child welfare-involved families. METHOD: Participants were randomly assigned to standard PCIT (n = 120) or services-as-usual (SAU; n = 84). The sample was characterized by low household income, significant exposures to adverse childhood experiences, and substance abuse. Intention-to-treat analyses were conducted on mul

The intersection of infant substance exposure, foster care placement & child welfare policy.

OBJECT: The author studied the specific issue of ventral tethering in cases of type II split cord malformation (SCM). He also discussed the issue of ventral tethering in patients with type I SCM in whom a bony septum and dural sleeve have been resected. METHODS: Eleven (21%) of 52 patients with type II SCM were found to have a ventral fibrous septum tethering the hemicords to the ventral dura. Four categories of ventral septa were noted: 1) pure ventral intradural septa (three patients); 2) complete dorsoventral intradural septa (three patients); 3) dorsoventral septa continuous with a dermal

Early childhood maltreatment and profiles of resilience among child welfare-involved children.

Given the high burden of child maltreatment, there is an urgent need to know more about resilient functioning among those who have experienced maltreatment. The aims of the study were to: 1) identify distinct profiles of resilience across cognitive, emotional, behavioral, and social domains in young children involved in the child welfare system; and 2) examine maltreatment characteristics and family protective factors in relation to the identified resilience profiles. A secondary analysis was conducted using data from the National Survey of Child and Adolescent Well-Being (NSCAW-II). Latent pr

Caregiver depression and trajectories of behavioral health among child welfare involved youth.

Child welfare involvement has been linked to child behavioral health issues, including increased likelihood of internalizing mental health problems such as depression and anxiety, and externalizing behavioral problems such as oppositional behaviors and substance use problems. One predictor of child behavioral health is caregiver mental health. Although, there remains a specific gap associated with identification of factors among caregivers that are associated with longitudinal child behavioral health trajectories. Using LONGSCAN, we explore the effects of caregiver depression on the behavioral

Pregnancy and reproductive healthcare among adolescent girls in the child welfare system.

OBJECTIVE: This study examines the prevalence and predictors of first pregnancy and the use of long-acting reversible contraceptives (LARCs) among adolescent girls aged 12-17 involved in the child welfare system. METHODS: The study leverages linked longitudinal administrative data from the child welfare, juvenile legal, and Medicaid claims systems of Pennsylvania. The sample comprised 28,016 girls born between 2000 and 2005 who received Medicaid and were involved in the child welfare system during adolescence. Survival analysis was used to predict onset of first pregnancy and uptake of LARC.

Resilience in Adolescent Girls in Child Welfare: Reliability and Validity of the RS-14.

PURPOSE: Up to one-third of patients treated with primary radiotherapy (RT) for prostate cancer (PCa) experience biochemical failure (BCF). In selected cases, re-irradiation represents a salvage option. PSMA PET/CT is a valuable tool for detecting intraprostatic recurrence and guiding re-treatment planning. Although visual scoring systems such as the PRIMARY score (PS) have been proposed, none are currently validated for this setting. This pilot study investigates the role of a visual classification system in optimizing salvage re-irradiation strategies. METHODS: We retrospectively analysed p

Institutional outbreak of varicella in a child welfare institute in Chandigarh, North India.

INTRODUCTION: Varicella outbreaks are known to occur in developing nations as vaccine coverage is still low. MATERIAL AND METHODS: In the present study, an institutional outbreak from Chandigarh, India, is reported wherein the utility of non-invasive samples such as saliva and urine was studied for the molecular diagnosis of varicella by conventional polymerase chain reaction (PCR), real-time PCR and real-time loop-mediated isothermal amplification (real-time LAMP). RESULTS: The results of the present study showed that saliva and urine samples can be used for outbreak investigation of varice

Housing and Child Welfare: Emerging Evidence and Implications for Scaling up Services.

Inadequate housing threatens family stability in communities across the United States. This study reviews emerging evidence on housing interventions in the context of scale-up for the child welfare system. In child welfare, scale-up refers to the extent to which fully implemented interventions sustainably alleviate family separations associated with housing instability. It incorporates multiple aspects beyond traditional measures of effectiveness including costs, potential reach, local capacities for implementation, and fit within broader social services. The framework further encompasses ever

Relationship of risk assessment to placement characteristics in a statewide child welfare population.

Risk assessments allow child and youth services to identify children who are at risk for maltreatment (e.g., abuse, neglect) and help determine the restrictiveness of placements or need for services among youth entering a child welfare system. Despite the use of instruments by many agencies within the U.S. to determine the appropriate placements for youth, research has shown that placement decisions are often influenced by factors such as gender, age, and severity of social-emotional and behavior problems. This study examined ratings of risk across multiple domains using a structured assessmen

Addressing the impact of trauma before diagnosing mental illness in child welfare.

Congress set requirements for child welfare agencies to respond to emotional trauma associated with child maltreatment and removal. In meeting these requirements, agencies should develop policies that address child trauma. To assist in policy development, this study analyzes more than 14,000 clinical assessments from child welfare in Illinois. Based on the analysis, the study recommends child welfare agencies adopt policies requiring that (1) mental health screenings and assessments of all youth in child welfare include measures of traumatic events and trauma-related symptoms; (2) evidence-bas

The Canadian child welfare system response to exposure to domestic violence investigations.

OBJECTIVE: While child welfare policy and legislation reflects that children who are exposed to domestic violence are in need of protection because they are at risk of emotional and physical harm, little is known about the profile of families and children identified to the child welfare system and the system's response. The objective of this study was to examine the child welfare system's response to child maltreatment investigations substantiated for exposure to domestic violence (EDV). METHODS: This study is based on a secondary analysis of data collected in the 2003 Canadian Incidence Stud

Trajectories of change in caregiver social supports: Associations with child maltreatment among families with child welfare involvement.

BACKGROUND: Evidence suggests that families with child welfare contact experience substantially limited access to social supports, a key protective factor against child maltreatment. OBJECTIVE: This study examined dynamic trajectories of caregivers' functional social supports (i.e., confidant, instrumental, and affective support) over time in relation to risk for maltreatment. METHODS: Participants were 707 caregivers of infants and toddlers with child welfare contact. Repeated-measures latent class analysis was used to profile trajectories of caregiver social supports across a five-year per

Risk of Child Welfare System Involvement Among Mothers With Substance Use Disorder.

INTRODUCTION: This paper aimed to examine the factors associated with child welfare system involvement among mothers receiving public substance use treatment services. METHODS: This was a cohort study using 2015-2023 linked data from Allegheny County, Pennsylvania, on residents' interactions across public service systems. Independent variables, measured within 3 months of study inclusion, included family characteristics, public benefit receipt, jail release, and substance use disorder diagnoses. The data, captured over the subsequent 21 months, included any child welfare interaction, active c

Delinquency, depression, and substance use disorder among child welfare-involved adolescent females.

Although adolescents with delinquency are known to have higher-than-average rates of depression or substance use disorder (SUD), research on the topic is inconsistent. It remains unclear weather depression or SUD leads to delinquency, whether delinquency leads to depression or SUD, or whether there is bi-directionality. Utilizing the National Survey of Child and Adolescent Well-Being (Wave I: 2008-2009; Wave II: 18 months later: N=5872), we used logistic regression to predict depression from delinquency (and vice versa), and SUD from delinquency (and vice versa). Reunifying families, cutting costs: housing-child welfare partnerships for permanent supportive housing.

In the absence of an adequate supply of affordable, quality housing, child welfare agencies are placed in the unenviable position of separating families to protect children from the debilitating effects of homelessness. This article presents recommendations for costeffective housing-child welfare partnerships that will shift the burden of providing adequate housing back to housing agencies. These partnerships have the potential to move child welfare agencies closer to achieving permanence and well-being for all children.

Child maltreatment characteristics as predictors of heterogeneity in internalizing symptom trajectories among children in the child welfare system.

This study investigated heterogeneity in the developmental trajectories of internalizing symptoms among 541 children who were involved with the child welfare system and examined child maltreatment characteristics, including types, level of harm, and timing, as predictors of internalizing trajectory patterns. Secondary longitudinal research was conducted using data from the National Survey of Child and Adolescent Well-Being-I, collected from 1999 to 2007 in the United States. Three distinct trajectory groups were identified: high-decreasing; low-increasing; and low-stable Sexual abuse, emotiona

Declining Investigations and Shifting Profiles: A 20-Year Analysis of Child Sexual Abuse Cases in Canadian Child Welfare.

This study examines 20-year trends in child sexual abuse (CSA) investigations within Canada's child welfare system, using data from the Canadian Incidence Study of Reported Child Abuse and Neglect (CIS) between 1998 and 2019. Data in the current study represent all Canadian provinces/territories except for Quebec. Findings indicate a substantial decline in both investigated and substantiated CSA cases, particularly among children aged 0-7 and cases involving non-parental perpetrators. Substantiated investigations among the youngest children fell by more than 80%, highlighting a marked shift in
