The Association Between Rapid Opioid Dose Tapering and Polydrug Overdose Deaths Among High-Dose, Long-Term Opioid Therapy Recipients.

BACKGROUND: Patients receiving high-dose, long-term opioid therapy (HDLTOT) who are subjected to rapid dose tapering may be at elevated risk of opioid withdrawal and illicit drug use that can lead to polydrug overdose. OBJECTIVE: We aim to estimate the association between rapid tapering of HDLTOT and polydrug overdose death (POD) in a commercially insured cohort of North Carolina residents between 2006 and 2018. DESIGN: Retrospective cohort study. PARTICIPANTS: In total, 21,478 commercially insured North Carolina residents aged 18-64 who received HDLTOT (≥ 90 morphine milligram equivalents

Sustaining treatment response in alopecia areata through tofacitinib dose tapering: A single-center retrospective study.

BACKGROUND: The advent of Janus kinase inhibitors has revolutionized the management of alopecia areata (AA). However, evidence remains scarce regarding the maintenance of disease control following an individual-based taper strategy. OBJECTIVE: To assess the outcome of a patient-centered tofacitinib dosing regimen in patients with AA who were in complete remission. METHODS: This retrospective observational cohort study included 46 patients with AA who achieved complete hair regrowth on tofacitinib and underwent structured dose tapering between February 2021 and July 2025. Clinical outcomes, c

Immediate versus wait-and-gradual discontinuation in antipsychotic switching: A meta-analysis.

INTRODUCTION: In two previous meta-analyses of randomized controlled trials (RCTs) examining antipsychotic switching strategies in patients with schizophrenia, we showed no significant differences in any clinical outcomes between immediate versus gradual and gradual versus wait-and-gradual discontinuation of the pre-switch antipsychotic. In this report, we compared immediate versus wait-and-gradual antipsychotic discontinuation. METHODS: We identified five RCTs examining immediate versus wait-and-gradual discontinuation of the pre-switch antipsychotic in antipsychotic switching involving pati

Pharmacological principles for safe benzodiazepine and Z-drug dose reduction.

Many guidelines recommend deprescribing benzodiazepines and z-drugs in most long-term users. Due to physical dependence in those using medication as prescribed (distinct from addiction), discontinuation can be difficult and is often unsuccessful, as withdrawal symptoms can be severe and long-lasting, especially after long-term use. There is a lack of clarity on how to taper patients in a tolerable manner that minimizes discomfort. Current guidelines vary with some suggesting linear reductions (e.g. 1 mg every 1-4 weeks) and some proportionate reductions (e.g. 5%-10% of the most recent dose per

A single-centre, randomised study on platelet reactivity after abrupt or gradual discontinuation of long-term clopidogrel therapy in patients after percutaneous coronary intervention.

BACKGROUND: Clinical studies have suggested increased risk of thrombotic events after planned cessation of clopidogrel therapy, due to increased platelet reactivity (platelet rebound); however, in many studies platelet function was not assessed before introducing clopidogrel. Patients who are scheduled to stop clopidogrel therapy, do it abruptly, so a gradual drug cessation might provide a beneficial treatment strategy. AIM: To determine whether a clopidogrel discontinuation results in platelet rebound hyperaggregability with increased activity compared to pre-treatment values and to assess w

What I have learnt from helping thousands of people taper off antidepressants and other psychotropic medications.

Although psychiatric drug withdrawal syndromes have been recognized since the 1950s - recent studies confirm antidepressant withdrawal syndrome incidence upwards of 40% - medical information about how to safely go off the drugs has been lacking. To fill this gap, over the last 25 years, patients have developed a robust Internet-based subculture of peer support for tapering off psychiatric drugs and recovering from withdrawal syndrome. This account from the founder of such an online community covers lessons learned from thousands of patients regarding common experiences with medical providers,

Tapering and Discontinuation of Biologics in Patients with Psoriatic Arthritis with Low Disease Activity.

The introduction of biologic disease modifying anti-rheumatic drugs (bMDARDs) have revolutionised the treatment of psoriatic arthritis (PsA). This combined with a 'treat-to-target' approach, means that achieving remission is increasingly possible. In patients with well-controlled PsA, there is little consensus on whether bDMARDs should be continued, tapered or discontinued altogether. Tapering or discontinuation of bDMARDs could offer significant financial savings and minimise patient burden and unwanted drug-related side effects. However, there is a risk of loss of remission. The primary focu

Rates of glucocorticoid taper in the management of polymyalgia rheumatica: the science behind the "art".

Polymyalgia rheumatica (PMR) is a common and debilitating disease for which glucocorticoids remain the therapeutic mainstay. Guideline recommendations on tapering regimens have been largely based on expert consensus. This exploratory narrative review provides a discussion on the available evidence for the rates of steroid tapering in PMR, as well as relevant pharmacology of corticosteroids. Key studies related to rates of steroid tapering are reviewed. Results favor a slow tapering regimen from a low initial steroid dose (between 10 and 20 mg) to minimize risk of relapse. This should be balanc

Slow tapering from methadone maintenance in a program encouraging indefinite maintenance.

Longitudinal studies have indicated that most opioid agonist-using patients are not able to successfully complete tapering attempts. Little is known, however, about tapering within a treatment environment that is supportive of indefinite agonist treatment and medication tapering. In this study, all records of patients beginning a slow methadone taper were reviewed (N = 30). No patient successfully completed methadone tapering. Four patients (13.3%) successfully switched to buprenorphine/naloxone, one of whom tapered off buprenorphine/naloxone. Three patients (10%) were continuing their taper a

Impact of Tracer Dose Reduction in [18 F]-Labelled Fluorodeoxyglucose-Positron Emission Tomography ([18 F]-FDG)-PET) on Texture Features and Histogram Indices: A Study in Homogeneous Tissues of Phantom and Patient.

BACKGROUND: Histogram indices (HIs) and texture features (TFs) are considered to play an important role in future oncologic PET-imaging and it is unknown how these indices are affected by changes of tracer doses. A randomized undersampling of PET list mode data enables a simulation of tracer dose reduction. We performed a phantom study to compare HIs/TFs of simulated and measured tracer dose reductions and evaluated changes of HIs/TFs in the liver of patients with PETs from simulated reduced tracer doses. Overall, 42 HIs/TFs were evaluated in a NEMA phantom at measured and simulated doses (ste

[Sumatriptan 3 mg subcutaneous : Clinical relevance of acute treatment of migraine despite dose reduction].

BACKGROUND: Triptans are a highly effective substance class in the acute treatment of migraine attacks. They contribute to a substantial improvement in the quality of life and help to reduce the socioeconomic burden of the disease. RESULTS: Sumatriptan is the only triptan that is available for subcutaneous administration. It is primarily indicated in patients with the need for rapid relief or insufficient enteral resorption due to nausea and vomiting. In the treatment of migraine attacks with 6 mg subcutaneous sumatriptan the number needed to treat (NNT) is 2.3 for freedom from pain within 2

New approach for detoxification of patients dependent on benzodiazepines and Z-drugs for reduction of psychogenic complications.

Benzodiazepines (BZDs) and Z-drugs are strongly addictive substances, acting on identical GABA receptors. Detoxification should be long-term and gradual, usually by tapering a long-acting BZD (diazepam) but no suitable commercial pharmaceutic product exists with the necessary low drug content. This review describes the specific pharmacological aspects and comparisons of individual BZDs in relation to their effects and addictiveness. The success of the treatment relates to the patients comfort during this process. Patients are typically afraid of switching to a more suitable long-acting BZD (di

Recent tapering from long-term opioid therapy and odds of opioid-related hospital use.

PURPOSE: The number of patients tapered from long-term opioid therapy (LTOT) has increased in recent years in the United States. Some patients tapered from LTOT report improved quality of life, while others face increased risks of opioid-related hospital use. Research has not yet established how the risk of opioid-related hospital use changes across LTOT dose and subsequent tapering. Our objective was to examine associations between recent tapering from LTOT with odds of opioid-related hospital use. METHODS: Case-crossover design using 2014-2018 health information exchange data from Indiana.

Hypnotic Discontinuation Using a Blinded (Masked) Tapering Approach: A Case Series.

Chronic use of hypnotic medications such as benzodiazepines is associated with adverse consequences including increased risk of falls. Efforts to help patients discontinue these medications have had varying levels of success. We developed a blinded (masked) tapering protocol to help patients taper off hypnotics. In this blinded protocol, patients consented to a drug taper but agreed to forego knowledge about the specific tapering schedule or the actual dose each night until the end of the taper. Blinded tapering aims to reduce negative expectancies for withdrawal effects that may impair a pati

Outcomes and Predictors of Sustained Response After Dose Tapering of Targeted Therapies in Moderate-to-Severe Atopic Dermatitis: A Systematic Review.

BACKGROUND: Moderate-to-severe atopic dermatitis (AD) often requires prolonged systemic therapy. As disease control is achieved, optimizing dose tapering and identifying factors associated with sustained response are increasingly relevant. OBJECTIVE: The aim was to identify factors associated with maintained disease control following dose tapering of biologics and small-molecule inhibitors used for AD. METHODS: A systematic review was conducted in accordance with Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines. MEDLINE, Embase, and the Cochrane Library

Supporting clinical guidelines for opioid conversion to methadone and tapering to prevent withdrawal in critically ill children using physiology based pharmacokinetic modeling and simulation.

Long-term opioid use is common in critically ill children and can cause withdrawal symptoms at discontinuation. To avoid withdrawal, short-acting opioids are usually converted into a long-acting drug, such as methadone, which is then carefully tapered. However, the exact conversion ratio and the optimal tapering protocol are unknown. This study aimed to design optimized conversion and tapering schedules of morphine and fentanyl to methadone for pediatric intensive care unit patients using physiologically-based pharmacokinetic (PBPK) modeling and expert panel decision-making. PBPK models of fen

Patients' perspectives towards biologic dose reduction in psoriasis: a qualitative study.

Dose reduction of biologics for psoriasis could contribute to more efficient use of these expensive medicines. Evidence on opinions of patients with psoriasis regarding dose reduction is sparse. The objective of this study was therefore to explore patients' perspectives towards dose reduction of biologics for psoriasis. A qualitative study was conducted, comprising semi-structured interviews with 15 patients with psoriasis with different characteristics and treatment experiences. Interviews were analyzed by inductive thematic analysis. Perceived benefits of biologic dose reduction according to
