Opioid use disorder is a treatable chronic disease, yet access to medications for opioid use disorder remains uneven, particularly in rural and underserved communities where prescriber shortages, travel burdens, and restrictive scope-of-practice laws delay care. Community pharmacists are highly accessible medication experts who already monitor therapy, counsel patients, identify safety concerns, and coordinate care. States should authorize pharmacists to initiate or prescribe medications for opioid use disorder, consistent with federal authority and established standards of care. Pharmacy-base

African nations still struggle with several healthcare challenges such as high prevalence of communicable and non-communicable diseases, underdeveloped healthcare systems, lack of healthcare personnel, and uneven distribution of quality healthcare services. Resource constraints and a fragmented healthcare system make the aforementioned challenges even more difficult to solve. However, digital health technology and artificial intelligence (AI) can be viewed as revolutionary instruments that may help to overcome the mentioned challenges and enhance healthcare practices in Africa. The need for individualized dosages has led to the exploration of three-dimensional (3D) printing technology for producing personalized medicines. This technology allows for dosage, size, shape, and taste adjustments based on individual patient characteristics, yet its translation from research into routine pharmacy compounding remains limited. Practical guidance on implementation, validation, and integration into existing quality systems is lacking. This work provides a complete, implementation pathway for semi-solid extrusion (SSE) pharma printing in a (hospital) pharmacy or compounding faci

Encouraging routine COVID-19 vaccinations is likely to be a crucial policy challenge for decades to come. To avert hundreds of thousands of unnecessary hospitalizations and deaths, adoption will need to be higher than it was in the autumn of 2022 or 2023, when less than one-fifth of Americans received booster vaccines1,2. One approach to encouraging vaccination is to eliminate the friction of transportation hurdles. Previous research has shown that friction can hinder follow-through3 and that individuals who live farther from COVID-19 vaccination sites are less likely to get vaccinated4. Howev

Pharmacy benefit managers (PBMs) administer prescription drug benefits for insurers, employers, and other payers through formulary design, rebate negotiation, pharmacy network management, and utilization controls. In diabetes care, cost-containment strategies such as formulary-driven medication switching, step therapy, and formulary exclusivity have increasingly restricted access to essential medications and technologies. These rebate-driven practices often prioritize financial incentives over clinical appropriateness, leading to forced nonmedical switching, higher out-of-pocket costs, reduced

BACKGROUND: Traditionally anaesthetic drugs for obstetrics are prepared as a contingency and stored until they are required for emergency use or have expired. Expiry is based on presumed reduction in sterility and efficacy although evidence for this is inconsistent. Preparation in advance introduces the risk of error and potential for tampering by a third party. Discarding and re-preparing drugs daily represents significant wastage with associated cost implications. We predicted that practice of drug preparation would differ widely across the UK, so conducted a national survey.

BACKGROUND: A revised consensus guideline on therapeutic drug monitoring (TDM) of vancomycin for serious methicillin-resistant Staphylococcus aureus (MRSA) infections was recently published with endorsement of numerous American pharmacy and medical societies. Changing practice from trough TDM to area-under-the-curve-(AUC)-guided dosing was suggested.
METHODS: Recent literature was critically appraised to determine whether AUC TDM is appropriate for Canadian hospital practice.
RESULTS: Previous 2009 vancomycin consensus guidelines recommended trough levels of 15-20 mg/L for serious MRSA infec

PURPOSE: Pharmacists are critical clinical partners for patients managing complex medication regimens, particularly those with chronic disease, polypharmacy, or heightened vulnerability. This commentary aims to highlight how these same principles such as careful dose titration, vigilant monitoring, and patient-centered counseling are indispensable in medical cannabis care and to illustrate the impact of pharmacist integration using New York State's tightly regulated medical cannabis program as a primary example.
METHODS: Drawing on nearly a decade of practice in a specialty pharmacy and medic

INTRODUCTION: Creation of an inclusive learning environment and provision of individual adjustments are duties of higher education providers. However, laboratory practical teaching is not always inclusive beyond general university requirements. Lab classes can present many barriers to disabled learners. Proactive adjustments embedded into the design and preparation of laboratory classes can make lab-based teaching as inclusive and accessible as possible.
PERSPECTIVE: The main challenges, difficulties, and barriers experienced by students with disabilities during laboratory classes were identi

Vaccine therapeutics and the practice of immunization provision are ever-changing. As pharmacy-based immunization services continue to flourish in the United States, more and more patients are requesting both routine and travel vaccines in community pharmacies. However, vaccine recommendations from the Centers for Disease Control and Prevention/Advisory Committee on Immunization Practices (CDC/ACIP) can sometimes differ from product-specific US Food and Drug Administration (FDA)-indicated uses. In addition, changes in vaccine schedules, product availability, and disease outbreaks can present i

BACKGROUND AND PURPOSE: Health equity attainment requires dismantling implicit bias and structural racism. Mitigating bias in clinical interventions and implementing structural interventions to impact where people live, work, play, and eat fosters optimal patient outcomes. Consequently, pharmacy students need exposure to these concepts. The objective of this project was to evaluate an elective course focused on exposing students to the root causes of health disparities, contemporary factors that perpetuate disparities, and evidence-based policies to reduce health disparities.

Sepsis is a leading cause of death in pregnancy and results in significant perinatal mortality. These deaths occur despite the younger age of pregnant patients, the low rate of comorbid conditions and the potential for effective interventions that should result in rapid resolution of illness. To date, no "evidence-based" recommendations are specific to the pregnant patient who is critically ill or septic. Optimal care for the septic patient requires a multidisciplinary team with expertise in obstetrics, maternal-fetal medicine, critical care, infectious disease, anesthesia, and pharmacy. Coord

Expedited partner therapy (EPT) is an evidence-based practice in which partners of patients with chlamydia are given antibiotics or a prescription for antibiotics without prior medical evaluation. This practice facilitates partner treatment and prevents reinfection of the patient. EPT remains underutilized due to multilevel implementation barriers. Barriers may exist at the level of legislation, policy, health care system, health care provider (HCP), pharmacist, patient, or partner. Qualitative interviews were performed with 11 EPT experts across the United States to uncover barriers in implem

BACKGROUND: Human papillomavirus (HPV) vaccine coverage among US adolescents remains below national targets. Community pharmacies are accessible vaccination sites, but proactive HPV vaccination is hindered by fragmented pharmacy management and immunization information system (IIS) workflows. Clinical decision support systems (CDSS) may reduce these workflow barriers, yet evidence for their utility in pharmacy-based HPV vaccination is limited.
OBJECTIVE: To pilot-test a CDSS package designed to support proactive adolescent HPV vaccination in community pharmacies and to assess early-phase imple
