Treatment of a spontaneous intracranial dissecting aneurysm with stent-assisted coil embolization.

Arterial dissection is now recognized as an important cause of stroke. Most reported dissections involve the extracranial portion of the internal carotid artery (ICA) and vertebral arteries. Spontaneous dissecting aneurysms of the intracranial ICA are uncommon. Endovascular treatment for cerebral aneurysms has become widespread; however, the dissecting aneurysm is still difficult to treat if the parent artery has to be preserved. More recently, stenting has been advocated for use with endosaccular coiling, with the coils held in place by the stent. We herein report a spontaneous intracranial I

Embolization of an Intracranial Vertebral Artery Aneurysm via the Deep Cervical Artery.

Treatment of vertebral artery aneurysms can be challenging due to the unusual vascular anatomy or unfeasibility of traditional endovascular techniques. We describe a novel approach for endovascular treatment of a ruptured intracranial vertebral artery aneurysm with bilateral vertebral artery occlusions and hypoplasia of the posterior communicating arteries. Successful coil embolization was performed using a collateral pathway for microcatheterization via anastomosis between the deep cervical artery and the vertebral artery. This case report highlights a novel alternative endovascular treatment

Clinical and Hemodynamic Features of Aneurysm Rupture in Coil Embolization of Intracranial Aneurysms.

Intraprocedural rupture (IPR) during coil embolization (CE) of an intracranial aneurysm is a significant clinical concern that necessitates a comprehensive understanding of its clinical and hemodynamic predictors. Between January 2012 and December 2023, 435 saccular cerebral aneurysms were treated with CE at our institution. The inclusion criterion was extravasation or coil protrusion during CE. Postoperative data were used to confirm rupture points, and computational fluid dynamics (CFD) analysis was performed to assess hemodynamic characteristics, focusing on maximum pressure (Pmax) and wall

New embolization technique for intracranial vascular disease with the assistance of an Apollo microcatheter.

It is difficult to access small and remote intracranial vascular lesions when using routine coil-delivery microcatheters. A thin Apollo microcatheter can access these vascular lesions. The Apollo microcatheter is intended to reduce the risk of catheter entrapment during the deployment of Onyx due to the detachable tip; furthermore, the Apollo microcatheter with a 3 cm detachable tip has 2 markers. X-ray imaging revealed that the marker on the push lever of the coil matched the proximal marker on the Apollo microcatheter. Therefore, it can replace routine coil-delivery microcatheters to deliver

The first decade of flow diversion for intracranial aneurysms with the Pipeline embolization device.

OBJECTIVE: Flow diverter devices have revolutionized the treatment of intracranial aneurysms (IAs) since their approval in 2011 and have continued to evolve. The devices have been widely adopted across institutions and centers over the past decade; however, long-term follow-up after treatment with the Pipeline embolization device (PED) is not well described in the literature. The authors' institution was among the first to begin using PEDs, allowing them to report their series of patients treated with flow diverters ≥ 10 years ago. In this study, the authors aimed to evaluate the long-term ang

Validity of PETRA-MRA for Stent-Assisted Coil Embolization of Intracranial Aneurysms.

OBJECTIVE: Pointwise encoding time reduction with radial acquisition (PETRA) using magnetic resonance angiography (MRA) is a non-enhanced MRA technique employing an ultrashort echo time, and is known to significantly reduce the magnetic susceptibility of coils and stents during post-embolization imaging. We evaluated the quality of PETRA-MRA images for use at the follow-up assessment of stent-assisted coil embolization procedures performed to treat aneurysms. METHODS: A total of six aneurysm patients who were treated by stent-assisted coil embolization were included. All patients underwent PE

Preliminary Study of Tirofiban Infusion in Coil Embolization of Ruptured Intracranial Aneurysms.

BACKGROUND: There have been some reports on the use of intra-arterial tirofiban in ruptured intracranial aneurysms, but few studies have reported on the use of 24 h of intravenous tirofiban infusion in patients with subarachnoid hemorrhage. OBJECTIVE: To present our experience with intravenous tirofiban infusion, in the form of a monotherapy as well as in addition to intra-arterial tirofiban, as a prophylactic, and as a rescue management for thrombus in patients who have undergone embolization with coils for ruptured intracranial aneurysms. METHODS: Between December 2008 and January 2015, we

[Intracranial inflammatory pseudotumor resected after transcatheter embolization: a case report].

A 63-year-old man presented with an exceptionally rare case of intracranial inflammatory pseudotumor manifesting as speech and gait disturbance. Magnetic resonance imaging showed a left convexity mass with significant peritumoral edema. The mass was attached to the convexity dura and mimicked meningioma. Angiography showed tumor staining, so the tumor was embolized using a gelatin sponge by an intravascular technique. The elastic hard tumor was totally resected without much bleeding. Histological examination showed proliferation of spindle cells and capillaries with nonspecific infiltrate of i

The role of transcranial Doppler embolic monitoring in the management of intracranial arterial stenosis.

Intracranial arterial stenosis (IAS) is thought to be responsible for 8% of all ischemic stroke subtypes. The best medical treatment for this condition is still controversial. Transcranial Doppler (TCD) emboli monitoring may help to guide the treatment by measuring the frequency of microembolic signals (MES). We report a case of IAS where TCD emboli monitoring proved useful in establishing the mechanism of stroke by being artery-to-artery emboli and guiding therapy based on the frequent symptoms and number of MES.

Coil embolization of intracranial dissecting vertebral artery aneurysms with subclavian steal.

Subclavian steal phenomenon occurs when cerebral blood flow is diverted to supply the arm. We report 3 patients with asymptomatic subclavian steal phenomenon with retrograde blood flow in the vertebral artery who presented with subarachnoid hemorrhage due to dissecting aneurysms of the involved vertebral artery. The association of subclavian steal phenomenon complicated by subarachnoid hemorrhage due to dissecting vertebral aneurysms has not been previously described.

Emergency endovascular revascularization of tandem occlusions: Internal carotid artery dissection and intracranial large artery embolism.

Internal carotid artery dissection (ICAD) with concomitant occlusive intracranial large artery emboli is an infrequent cause of acute stroke, with poor response to intravenous thrombolysis. Reports on the management of this entity are limited. We present our recent experience in the endovascular management of occlusive ICAD and major intracranial occlusion. Consecutive anterior circulation acute stroke patients meeting Medical Center criteria for endovascular management of ICAD from June 2011 to June 2015 were included. Clinical, imaging, and procedure data were collected retrospectively under

Effect of different anesthesia strategies on intracranial pressure during extubation after intracranial aneurysm embolization: a randomized controlled trial.

Intracranial pressure (ICP) fluctuations during the extubation period pose a risk of severe complications in patients undergoing intracranial procedures. Transorbital sonography was used to measure the ratio of optic nerve sheath diameter (ONSD) to eyeball transverse diameter (ETD), which has emerged as a novel non-invasive method for ICP monitoring. This study aimed to evaluate the effects of different anesthesia strategies on extubation-period ICP, using the ONSD/ETD ratio as a surrogate marker. This prospective randomized controlled trial enrolled 63 patients, who were assigned to three gro
