Targeted next‑generation sequencing for research and diagnostics in congenital heart disease, and cleft lip and/or palate.

Congenital heart disease (CHD), and cleft lip and palate (CLP) are currently the most common types of structural malformation in infants. Various methods have been used to identify the disease‑associated genes. However, targeted next‑generation sequencing (NGS) is not yet considered an option for routine use. Thus, the present study aimed to assess the safety and feasibility of using targeted NGS in patients with CHD concomitant with CLP. Between November 2015 and May 2017, a total of 17 patients with CHD concomitant with CLP, who were excluded from a diagnosis of trisomy syndrome, were select

Rare diagnosis of IgG4-related systemic disease by lip biopsy in an international Sjögren syndrome registry.

IgG4-related disease has been recently defined as a distinct clinic-pathologic entity, characterized by dense IgG-4 plasmacytic infiltration of diverse organs, fibrosis, and tumefactive lesions. Salivary and lacrimal glands are a target of this disease and, when affected, may clinically resemble Küttner tumor, Mikulicz disease, or orbital inflammatory pseudotumor. In some patients, the disease is systemic, with metachronous involvement of multiple organs, including the pancreas, aorta, kidneys, and biliary tract. We report a 66-year-old man who presented with salivary gland enlargement and sev

Prevention of severe mucosecretory ear disease and its complications in patients with cleft lip and palate malformations.

Morphological and functional alterations in patients with cleft lip and palate act as predisposing factors in eustachian tube dysfunctions and as a result thereof in the occurrence of mucosecretory ear disease (MSED) and its complications. Development of MSED is the result of tube dysfunction and it consists of: (a) fluctuating conductive hearing loss, (b) swallowing alterations and (c) respiratory functional disorders. Once this syndrome is confirmed in children with clefts, action must be taken through a coordinated multidisciplinary approach with plastic surgery, odontology, and speech ther

Cleft lip repair in trisomy 18: ethical considerations for aesthetic surgery in a life-limiting paediatric disease.

Trisomy 18 (T18) is a disease with severe congenital anomalies that is typically fatal in the perinatal period or early infancy. This case report describes the care of one child with T18 who survived beyond her initially poor prognosis and whose parents requested cosmetic cleft lip repair that would confer significant perioperative risk without direct clinical benefit to the child. After extensive multidisciplinary consultation, shared decision-making via family meeting and a trial waiting period, the patient successfully underwent cleft lip repair with high parental satisfaction and no long-t

The Diagnostic Utility of Lip Biopsy in Paediatric Crohn's Disease: A 10-year Single-centre Retrospective Study.

BACKGROUND: Oral manifestations of paediatric Crohn's disease (CD) are reported in up to 60% of cases. Lip biopsy can be used to histologically diagnose oral CD. We evaluated the utility of lip biopsy in children under initial investigation for potential CD. METHODS: A 10-year retrospective review of electronic patient records at a single tertiary paediatric surgery centre was performed. All patients aged ≤16 years who underwent lip biopsy were included. Clinical features, histology, and diagnostic details were extracted. RESULTS: Forty-two children underwent lip biopsy. Median age at biopsy

Incidence of Gastroesophageal Reflux Disease in Children With Cleft Lip and Palate and an Evaluation of Its Impact on Weight Gain.

Patients with cleft lip and/or palate have higher rates of failure to thrive (FTT), decreased growth, and more often experience feeding difficulties as compared with the general pediatric population (J Child Health Care. 2014;18:72-83). Although insufficient nursing, excessive air intake, and incorrect feeding methods have been established in the literature, the role of gastroesophageal reflux disease (GERD) as a contributing factor in cleft patients has not been thoroughly examined. Presently, there is a paucity of literature analyzing the incidence and effect of GERD on this unique populatio

CAVI-Lowering Effect of Pitavastatin May Be Involved in the Prevention of Cardiovascular Disease: Subgroup Analysis of the TOHO-LIP.

AIM: In the TOHO Lipid Intervention Trial Using Pitavastatin (TOHO-LIP), a multicenter randomized controlled trial, pitavastatin significantly reduced cardiovascular (CV) events compared to atorvastatin in patients with hypercholesterolemia. To investigate the mechanism by which pitavastatin preferentially prevents CV events, we investigated the relationship between CV events and cardio-ankle vascular index (CAVI) using the TOHO-LIP database. METHODS: For the subgroup analysis, we selected patients from a single center, Toho University Sakura Medical Center. After excluding those who had CV e

The 22q11.2 Microdeletion in Pediatric Patients with Cleft Lip, Palate, or Both and Congenital Heart Disease: A Systematic Review.

The 22q11.2 deletion syndrome (22q11.2DS) is present in approximately 5 to 8% of patients with cleft lip, palate, or both (CL/P) and 75 to 80% of patients with congenital heart disease (CHD). In a literature review, we consider this association of 22q11.2DS in pediatric patients with CL/P and CHD. Early diagnosis of 22q11.2DS in pediatric patients with CL/P and CHD helps to optimize a multidisciplinary treatment approach for 22q11DS. Early diagnosis, thereby, can improve quality of life for these patients and awareness of other potential clinical implications that may require attention through

Lip mucosal purpuralike lesions as an uncommon oral indicator of Sjögren disease: A case report and literature review.

BACKGROUND: Lip mucosal purpura is rare in Sjögren disease (SjD) and is easily overlooked without classic sicca symptoms, posing a diagnostic challenge. CASE DESCRIPTION: A patient had persistent oral red macules for 7 years. Serology revealed antinuclear antibody, anti-SSA/Ro, and anticentromere antibody positivity, meeting SjD criteria. This prolonged clinical course before diagnosis underscores the challenge in identifying SjD with atypical manifestations. Treatment included hydroxychloroquine immunomodulation and supportive oral care. This case underscores that isolated oral lesions can b

Synchronous Analysis of Speech Production and Lips Movement to Detect Parkinson's Disease Using Deep Learning Methods.

BACKGROUND/OBJECTIVES: Parkinson's disease (PD) affects more than 6 million people worldwide. Its accurate diagnosis and monitoring are key factors to reduce its economic burden. Typical approaches consider either speech signals or video recordings of the face to automatically model abnormal patterns in PD patients. METHODS: This paper introduces, for the first time, a new methodology that performs the synchronous fusion of information extracted from speech recordings and their corresponding videos of lip movement, namely the bimodal approach. RESULTS: Our results indicate that the introduce

IgG4-related disease involving vital organs diagnosed with lip biopsy: A case report and literature review.

Immunoglobulin G4-related disease (IgG4-RD) is a recently recognized new disease entity characterized by elevated serum IgG4 and infiltration of IgG4 plasma cells in affected tissues. Histological examination is essential for definitive diagnosis, as other pathological conditions can also present with serum IgG4 elevation. However, IgG4-RD frequently involves vital or internal organs that are difficult to perform biopsies. We herein report a unique case of IgG4-RD involving vital organs that could be successfully diagnosed by alternative lip biopsy, an accessible, little invasive procedure, de

Cleft lip and palate, hypertelorism, brachycephaly, flat facial profile, and congenital heart disease in three brothers.

A "new" syndrome was identified by McPherson and Clemens [1996: Am J Med Genet 62:58-60] in a brother and sister with bilateral cleft lip and palate, hypertelorism, flat facial profile, flat occiput, and complex heart defects. The brother also had a bilobed tongue and the sister had malrotation of the intestine and bifid thumbs. We describe three brothers with similar anomalies apart from the bilobed tongue, malrotation of the intestine, and bifid thumbs. McPherson and Clemens [1996: Am J Med Genet 62:58-60] suggested autosomal recessive inheritance. Our observation of three affected brothers

Bilateral Lower Lip Swelling as an Early Manifestation of Crohn's Disease in a 16-Year-Old Patient: A Case Report.

Crohn's disease and ulcerative colitis are chronic inflammatory bowel diseases that are often diagnosed with a significant delay. Although Crohn's disease can affect the entire gastrointestinal tract-from the mouth to the anus-ulcerative colitis is limited to the colon and rectum. This report describes the case of a 16-year-old male patient who suffered from a painless, bilateral swelling of the lower lip persisting for 4 months. Elevated fecal calprotectin (510 mg/kg) and subsequent endoscopic examination confirmed the diagnosis of Crohn's disease. Immunosuppressive therapy led to full resolu
