[Maze procedure for chronic atrial fibrillation associated with mitral valve disease].

From May 1995 to October 1996, 20 patients with mitral valve disease underwent the maze procedure for chronic atrial fibrillation and mitral valve replacement or mitral valvuloplasty. Epicardial mapping data demonstrated that the large macrorecurre flutter circuit was located in left atrium (14/20) and complex fibrillation in right atrium (18/20) of the majority of patients. No early death occurred. 20 patients were followed up for at least 3 months (range 3-20 months) and 14 patients for at least 1 year after operation, sinus rhythm and atrioventricular synchrony were restored (100%), atrial

Long-term results of the Maze procedure in patients with mechanical valve.

BACKGROUND: The advantage of the Maze procedure concomitant with the mechanical valve implantation is unclear. This study evaluated the long-term results of mechanical valve implantation either alone or concomitant with the Maze procedure. METHODS: Between 1990 and 2005, 208 patients underwent mitral valve replacement with a mechanical valve. Maze procedure was concomitantly performed in 77 patients, and 54 of these had restored sinus rhythm (Maze group). The other 131 patients did not perform the Maze procedure (non-Maze group). Survival and late outcomes were studied retrospectively with un

Optimal patient selection for maze procedure in patients undergoing mitral valve disease.

OBJECTIVES: Although risk factors for unsuccessful Maze procedure have been demonstrated, an appropriate patient selection is still controversial. In our institute, Maze procedure is indicated for those whom normal sinus rhythm (NSR) was reestablished by intraoperative direct cardioversion (DC) after ventricular unloading by total cardiopulmonary bypass. The purpose of this study was to evaluate the effectiveness of our indication criteria for Maze procedure in patients with mitral valve disease. METHODS: Between October 2012 and October 2021, MAZE was indicated in 55 patients in whom normal

Intraoperative modified Cox mini-maze procedure for long-standing persistent atrial fibrillation.

BACKGROUND: As the most effective method of curing atrial fibrillation, the classic Maze III procedure eliminates atrial fibrillation in more than 90% of patients. In spite of high efficacy, the complexity and complications associated with this procedure prevents widespread application. In this research, we report our experience with a modified Cox mini-Maze procedure using radiofrequency energy for treating long-standing persistent atrial fibrillation during open-heart surgery. METHODS: A total of 45 patients with long-standing persistent atrial fibrillation who underwent open-heart surgery

The maze III procedure for atrial fibrillation: should the indications be expanded?

BACKGROUND: We favor the maze III procedure over nonsurgical treatments for atrial fibrillation and have advocated addition of the maze in symptomatic patients with other compelling indications for cardiac surgery. METHODS: Characteristics and perioperative outcomes of our 76 cumulative maze recipients between 1993 to 1998 were reviewed. The original maze III technique was employed without modification. RESULTS: Isolated maze III was performed in 19 patients (25%) and combined with other procedures in 57 patients (75%), 49 of these involving one or more valves. Patients having combined proce

Atrial arrhythmia recurrence after the Maze procedure: Insights from catheter-based mapping.

BACKGROUND: The Cox-Maze procedure treats atrial fibrillation (AF) through surgical lesion sets designed to isolate pulmonary veins (PVs) and interrupt atrial reentrant circuits. OBJECTIVE: This study aimed to characterize mechanisms of recurrent atrial arrhythmias after Maze procedures using electroanatomic mapping. METHODS: We retrospectively analyzed 86 patients who underwent Maze procedures from 2008 to 2023 and later required endocardial catheter ablation for recurrent AF or atrial flutter (AFl). Electroanatomic mapping assessed lesion integrity, arrhythmia patterns, and ablation outcom

Haemoptysis and bronchial congestion due to pulmonary vein stenosis after maze procedure.

Pulmonary vein stenosis (PVS) is a rare disease that can cause haemoptysis. Acquired PVS is indicated as a complication associated with cardiac catheter intervention; however, the maze procedure has not been reported to induce PVS. Here, we describe the diagnosis and treatment strategy for the first case of PVS with haemoptysis due to the maze procedure. A 56-year-old man who underwent the maze procedure seven years previously was referred for repeated haemoptysis. Contrast-enhanced computed tomography (CT) revealed complete occlusion of the left superior pulmonary vein. Bronchoscopy revealed

Right-sided maze procedure for right atrial arrhythmias in congenital heart disease.

BACKGROUND: Atrial fibrillation and flutter, commonly associated with congenital heart anomalies that cause right atrial dilatation, may cause significant morbidity and reduction of quality of life, even after surgical repair of the anomalies. METHODS: In an effort to reduce the incidence of atrial tachyarrhythmias after repair of right-sided congenital heart disease, we performed a concomitant right-sided maze procedure. RESULTS: Eighteen patients with paroxysmal atrial fibrillation or flutter (n = 12) or chronic atrial fibrillation or flutter (n = 6) aged 10.9 to 68.4 years (mean 34.9 year

Cox maze procedure for chronic atrial fibrillation associated with mitral valve disease.

Between April 1992 and October 1993, we combined a modified maze procedure with mitral valve repair (n = 26) or replacement (n = 36) in 62 patients with atrial fibrillation, including 16 patients undergoing reoperation. Associated procedures included aortic valve operation (n = 22), tricuspid annuloplasty (n = 28), atrial plication (n = 10), and others (n = 3). Duration of atrial fibrillation varied from 0.1 to 23 years (average 8.3 +/- 6.4 years), the f-wave voltage ranged from 0 to 0.45 mV (0.16 +/- 0.09 mV), and cardiothoracic ratio varied from 46% to 85% (64% +/- 9%). We modified the maze

Cox/maze procedure for atrial septal defect with atrial fibrillation: management strategies.

Atrial fibrillation is found at late follow-up in approximately half of all adults who have had correction of atrial septal defect, even if it was not present preoperatively. These patients are thus exposed to the risks of stroke and chronic drug therapy even after a successful operation. Simultaneous surgical correction of atrial septal defect and atrial fibrillation was accomplished in a 52-year-old man by means of the Cox/maze procedure. The small added risk and the substantial benefit of eliminating atrial fibrillation suggest that this approach is warranted in selected adults with atrial

Higher F-wave frequency associates with poor procedural success rate after Maze procedure.

OBJECTIVES: Persistent atrial fibrillation (AF) causes atrial remodeling, which causes myocardial fibrosis and micro-reentry. Fibrosis may reduce wave voltage and micro-reentry may enhance the dominant frequency (DF) of the F-wave. We investigated whether the DF predicts procedural success by the Maze procedure. METHODS: In 138 consecutive patients who underwent mitral valve surgery and a modified Cox-Maze III procedure for persistent AF in Nagoya University in 2002-2018, 96 (70%) were successfully cardioverted (group S); 42 had persistent or relapsed AF after surgery (group F). An off-pump biatrial mini-maze procedure for long-standing persistent atrial fibrillation.

OBJECTIVES: The goal of the present study was to determine the safety and efficacy of a modified off-pump biatrial mini-maze procedure to treat long-standing persistent atrial fibrillation (AF). METHODS: Patients with long-standing persistent AF underwent our modified mini-maze procedure using bipolar radiofrequency ablation. Those patients first underwent a mini-maze procedure using the Dallas lesion set protocol. Second, a purse-string suture was performed on the right atrium, and then 4 ablation lesions were made to the superior vena cava, the inferior vena cava, the appendix of the right

Simultaneous hybrid maze procedure for long-standing persistent atrial fibrillation with dilated atrium.

BACKGROUND: The efficacy of catheter-based or thoracoscopic ablation for treating long-standing persistent atrial fibrillation (AF) with a dilated atrium remains suboptimal. This study aimed to assess the feasibility and initial results of simultaneous hybrid ablation with a new biatrial lesion set in these patients. METHODS: Twenty-seven consecutive patients with long-standing persistent AF and dilated atrium underwent simultaneous hybrid ablation with a new biatrial lesion set consistent with the principle of the Cox maze procedure at our institution. Patients were followed up at 3, 6, and

Sinus node function after cardiac surgery: is impairment specific for the maze procedure?

BACKGROUND: Maze surgery is a final solution for intractable atrial fibrillation (AF), but an adverse effect on postoperative sinus node function has been reported. Whether this also applies to other types of cardiac surgery is unclear. METHODS: We assessed postoperative rhythm by means of repeated exercise tolerance testing, ambulatory electrocardiography, and non-invasive testing of autonomic function between 1 and 12 months after four types of cardiac surgery. Fourteen patients without structural cardiac disease and medically refractory AF underwent the maze III procedure, 11 patients with

Initial experience with a modified left atrial maze procedure concomitant to cardiac surgery.

BACKGROUND: The excellent results of the Cox-Maze-III operation showed that a surgical treatment of atrial fibrillation is possible. The maze operation is surgically demanding and time-consuming. This fact led to the development of simplified modifications. Due to the good results of an exclusive left atrial maze procedure in combination with mitral valve surgery and new technologies of ablation, it seems reasonable to perform these techniques concomitant to cardiac surgery on a large number of patients.¶ METHODS: From November 1999 until June 2000 a modified maze procedure concomitant to a c
