BACKGROUND: Studies found that patients who underwent coronary artery bypass grafting (CABG) often fail to receive optimal evidence-based secondary prevention medications. We evaluated the effectiveness of a smartphone-based quality improvement effort on improving the prescription of medical therapies.
METHODS: In this cluster-randomized controlled trial, 60 hospitals were randomized to a control arm (n = 30) or to an intervention arm using smartphone-based multifaceted quality improvement interventions (n = 30). The primary outcome was the prescription of statin. The secondary outcomes were

INTRODUCTION: The spaceflight environment presents unique demands on human physiology; among those demands, is sleep. Sleep loss and circadian desynchronization is a major concern for future deep- exploration plans, including long-term crewed missions to the Moon and Mars.
AIMS: Analyze evidence of sleep disruption in crewmembers during low-Earth orbit missions, identify the use of sleep-promoting medication among crewmembers and deepen the comprehension of challenges to sleep physiology for future missions to the Moon and Mars.
RESULTS: Evidence consistently indicates a loss of sleep and ci

Severe traumatic brain injury (TBI) in pediatric patients is a time-dependent condition in which early airway management and rapid access to neurosurgical care are considered important to reduce the risk of secondary brain injury. The potential utility of Helicopter Emergency Medical Services (HEMS) is closely related to the availability of adequate landing sites, particularly during nighttime operations. In Calabria (Southern Italy), nighttime HEMS activity has historically been limited by a marked lack of certified hospital helipads, especially within tertiary (hub) centers. We report the ca

Addressing access to oral health care in many low- to middle-income countries is a complicated issue. Oral and maxillofacial surgeons may help engage with vulnerable populations through carefully planned dentoalveolar mission trips. The process of planning a mission includes selecting a population and identifying their unique needs, designing clinic layouts and workflows, team preparation, collection of supplies, fundraising, and advertising. During the mission, methods for protecting privacy, delivering treatment that is standard of care, and sanitation/sterilization options are reviewed. Eth

BACKGROUND: The logistic peculiarities of an emergency maritime location and the frequent additional threat of accidental hypothermia mean that the treatment of medical emergencies at sea are particularly demanding. This article describes the characteristics of emergency medical missions of the German Maritime Search and Rescue Service (DGzRS) as the main provider of non-helicopter-based medical maritime rescue on the seas off the coasts of Germany.
MATERIAL AND METHODS: A retrospective analysis of all missions by the DGzRS in 2017 and 2018 was carried out. The data and times of the missions

Within the scope of its health products control mission, the French Health Products Safety Agency (Afssaps) collaborating with two expert's sites, has assessed the 16 tests available on the French market in 2002 for rapid diagnosis of the Streptococcus A tonsillitis. The purpose of this study was to verify the reliability and rapidity of these tests and to give some information to the users about their analytical criteria and practicability characteristics. The analytical study has been performed on a same panel of four reference strains of Streptococcus pyogenes dilutions to determine the lim

Female medical missionaries significantly contributed to the advancement of healthcare practices in colonial Kashmir, especially in women's healthcare, nursing, midwifery, and medical outreach. This article analyzes the role of female medical missionaries in the development of institutional healthcare in Kashmir from the late nineteenth to the early twentieth centuries. It inquires how missionary women navigated local social and cultural obstacles, including gender norms and purdah regulations, to enhance access to medical care for Kashmiri women. The study examines the convergence of medicine

Short-term global surgery mission trips are often undertaken with commendable intentions and have undoubtedly benefited many patients and institutions. However, the culture surrounding some outreach programmes may occasionally drift toward performative benevolence, fleeting expertise, disproportionate self-congratulation, and limited engagement with local priorities. Through a deliberately exaggerated "survival guide" format, this satirical commentary playfully examines familiar tendencies within short-term surgical outreach, including documentation exceeding operative contribution, sustainabi

Many healthcare facilities in low-income countries (LICs) lack on-site blood banks, creating unfamiliar barriers to U.S. healthcare workers on mission trips. Abdominal hysterectomies are frequently performed on surgical missions and may require transfusion. In this case study, a patient received warm fresh whole blood (WFWB) from student registered nurse anesthetists and a registered nurse, post-hysterectomy in Guatemala. This case highlights preparedness for critical needs, the value of WFWB, and the educational and personal values gained on international missions. Clinical implications of WF

BACKGROUND: General surgery training programs face declining case volume and diversity. We wanted to determine if resident participation in international surgical missions would increase exposure to cases underrepresented in our program case mix.
METHODS: Accreditation Council for Graduate Medical Education program data from 2008 to 2011 (University of Medicine and Dentistry-New Jersey Medical School, Newark, NJ) were analyzed to identify categories where volume was below national average. This was compared with case logs from 3 missions conducted by International Surgical Health Initiatives

A team of caregivers travelled to the favelas of a Brazilian province. They were subjected to emotionally charged situations, working with people faced with extreme poverty and poor health. They highlight a cultural countertransference requiring an objective approach to the psychological and political repercussions of these situations.

The NASA human system risk board (HSRB) has long focused on trauma and acute medical illness as a key contributor to high level in-flight medical risk. However, ocular issues, trauma, and complaints during spaceflight are poorly characterized. In a retrospective case series, the NASA data from the life sciences data archieve (LSDA) and the lifetime surveillance of astronaught health (LSAH) was queried for eye related complaints and conditions in spaceflight across international space station (ISS) missions and space shuttle (STS) missions. The ISS dataset included missions from the year 2000 t

Long-duration spaceflight missions (including the International Space Station [ISS]) are in one of the most remote and harsh environments humans live and work in. Medical emergencies in space are even more complicated in an already high-risk environment. Despite training, ISS crewmembers face many challenges in diagnosing and managing disease with limited diagnostic capability and equipment on the ISS, restricted medication availability, delayed access to medical professionals; and the latency of space missions. Ocular trauma (e.g., cabin contact with equipment, chemical exposure, and foreign

Pediatric orthopedic surgery in humanitarian aid is conducted mainly in cooperation with emerging countries. Each mission is different, and depends on numerous parameters such as the country, the frequency of such missions, the pathologies encountered, the local structure and team, and the non-governmental organization (NGO) involved. Pathologies vary in etiology (tuberculosis, poliomyelitis) and severity. Each mission requires the presence of an experienced surgeon. Working conditions are often rudimentary. Surgical indications should be restricted to procedures that are going to be effective

To prevent pre-flight and on-orbit infections, NASA crew participate in a comprehensive Health Stabilization Program (HSP), which includes pre-flight quarantine, vaccination and demonstrated immunity, personal protective equipment, and crew and support personnel training. We aggregated historical spaceflight data to assess the incidence of pre-flight infectious exposures in spaceflight, to determine the effectiveness of HSP protocols, and to characterize risk factors associated with pre-flight exposure resulting in on-orbit illness. Medical and operational records on U.S. spaceflight missions
