Personalized precision irradiation of patients with left-sided breast cancer is possible by examining the setup errors of 3- and 4-mm gated window widths for those treated with deep inspiration breath-hold (DIBH) treatment. An observational study was performed via a retrospective analysis of 250 cone-beam computed tomography (CBCT) images of 60 left-breast cancer patients who underwent whole-breast radiotherapy with the DIBH technique between January 2021 and 2022 at our hospital. Among them, 30 patients had a gated window width of 3 mm, while the remaining 30 had a gated window width of 4 mm;

BACKGROUND AND PURPOSE: Reducing motion is vital when radiotherapy is used to treat patients with hepatobiliary (HPB) and pancreatic malignancies. Abdominal compression (AC) and breath-hold (BH) techniques aim to minimise respiratory motion, yet their adoption remains limited, and practices vary. This review examines the impact of AC and BH on motion, set-up errors, and patient tolerability in HPB and pancreatic patients.
MATERIALS AND METHODS: This systematic review, conducted using PRISMA and PICOS criteria, includes publications from January 2015 to February 2023. Eligible studies focused

Objective.Adaptive radiotherapy aimed to account for inter- and intra-fractional anatomical changes to improve target coverage and spare normal tissues. Commercial softwares could quantify anatomical and setup differences between planning computed tomography (pCT) and image-guidance cone-beam computed tomography (CBCT) using density gamma passing rate (dGPR). This study evaluated the utility of dGPR as an automated, site-specific trigger for offline adaptive workflows, using statistical process control (SPC) to define tolerance thresholds and correlating dGPR with setup errors, planning target

The goal of the study was to evaluate the inter- and intrafractional patient setup accuracy of target volumes located in the head, thoracic, abdominal, and pelvic regions when using SGRT, by comparing it with that of laser alignment using patient skin marks, and to calculate the corresponding setup margins. A total of 2303 radiotherapy fractions of 183 patients were analyzed. All patients received daily kilovoltage cone-beam computed tomography scans (kV-CBCT) for online verification. From November 2019 until September 2020, patient setup was performed using laser alignment with patient skin m

AIMS: Clinical implementation of image-guided intensity-modulated radiotherapy is rapidly evolving. Helical tomotherapy treatment delivery involves daily imaging before intensity-modulated radiotherapy delivery. This can be a time consuming resource-intensive process, which may not be essential in head and neck radiotherapy, where effective immobilisation is possible. This study aimed to evaluate whether an offline protocol implementing the shifts derived from the first few fractions can be an acceptable alternative to daily imaging for helical tomotherapy.
MATERIALS AND METHODS: We retrospec

BACKGROUND: Geometrical uncertainties in patients can severely affect the quality of radiotherapy.
PURPOSE: We evaluated the dosimetric efficacy of robust optimization for helical intensity-modulated radiotherapy (IMRT) planning in the presence of patient setup uncertainty and anatomical changes.
METHODS: Two helical IMRT plans for 10 patients with localized prostate cancer were created using either minimax robust optimization (robust plan) or a conventional planning target volume (PTV) margin approach (PTV plan). Plan robustness was evaluated by creating perturbed dose plans with setup unce

PURPOSE: The single isocenter for multiple targets (SIMT) technique has become a popular treatment approach for multiple brain metastases. However, the rotational error that is introduced is usually not considered in planning target volume (PTV) expansion. We have developed a statistical model that takes into account both translational and rotational uncertainties. In this study, we incorporated the rotational error into PTV margin expansion for the clinical use of the SIMT technique.
METHODS AND MATERIALS: In the statistical model, both translational and rotational errors are assumed to foll

PURPOSE: Respiratory motion and patient setup error both contribute to the dosimetric uncertainty in radiotherapy of lung tumors. Managing these uncertainties for free-breathing treatments is usually done by margin-based approaches or robust optimization. However, breathing motion can be irregular and concerns have been raised for the robustness of the treatment plans. We have previously reported the dosimetric effects of the respiratory motion, without setup uncertainties, in lung tumor photon radiotherapy using free-breathing images. In this study, we include setup uncertainty.

AIM: The occurrence of setup errors in external beam radiation is an inevitable aspect. As a result of technological progress in image verification, it became feasible to decrease its occurrence; however, complete eradication remained unattainable. Various methodologies have been implemented in clinical settings to measure and evaluate the extent of setup mistakes. Further analysis is required to gain a deeper understanding of the effects of setup mistakes on dose-volume histograms (DVHs), which will inform decision-making in the context of setup-related issues. This study aimed to quantify th
