Selected other problematic testicular and paratesticular lesions: rete testis neoplasms and pseudotumors, mesothelial lesions and secondary tumors.

The proximity and, in some instances, communication between several structures in the testis and paratestis (rete testis, epididymis, mesothelium, vestigial epithelium and paratesticular soft tissue) result in a plethora of interesting tumors and tumor-like lesions that together pose a formidable diagnostic challenge both because of their morphologic overlap and rarity. The occasional spread of tumors primarily at other sites to this region adds to the potential problem encountered. This review provides an overview of the pathology of nonmesenchymal paratesticular neoplasms and pseudotumors wi

Neither inguinoscrotal hernia nor hydrocele misunderstood giant testicular neoplasm. Case report and literature review.

AIM: This case experience aims to question the current know-how when a masked testicle malignancy occurs, in order to achieve the correct clinical framework and avoid mistakes during surgical procedures. in the evaluation MATERIALS AND METHODS: A 36-year-old male patient was admitted with an incorrect diagnosis of left-sided incarcerated inguinoscrotal hernia, and then discovered a seminomatous testicular neoplasm matched with a hypertensive ipsilateral hydrocele. Therefore, we performed a radical epididymo-orchiectomy and referred the patient to the oncologist for adjuvant chemotherapy after

Myoid gonadal stromal tumor: A rare testicular sex cord stromal neoplasm - a case report and review of the literature.

Myoid gonadal stromal tumor (MGST) is an exceptionally rare benign testicular sex cord-stromal neoplasm with limited published experience. We report a 30-year-old man presenting with a painless right testicular mass and normal serum tumor markers. Ultrasonography and magnetic resonance imaging demonstrated a small enhancing intratesticular lesion suspicious for malignancy. Radical inguinal orchiectomy was performed. Histopathological and immunohistochemical evaluation was consistent with MGST, showing spindle-cell proliferation with diffuse smooth muscle actin, S100, and vimentin positivity an

Bilateral synchronous testicular neoplasms of different lineages: A germ cell tumour and a sex cord-stromal tumour. A case report.

Bilateral testicular tumours are uncommon, and synchronous presentations involving different histogenetic lineages are rarely reported in the literature. Most bilateral tumours are metachronous and of germ cell origin. The coexistence of a malignant germ cell tumour in one testis and a benign sex cord-stromal tumour in the contralateral testis is an unusual finding with diagnostic and therapeutic implications. We present the case of a 45-year-old man who underwent bilateral radical orchiectomy after ultrasonography identified three nodules in the left testis (maximum diameter 24mm) and a 7mm l

Somatic Tumor-Like Neoplasms Arising With Testicular Germ Cell Tumors: Distilling Evolving Understanding, Terminology, and Implications for the Practicing Surgical Pathologist.

Tumors resembling conventional, somatic-type malignancies arise infrequently (<10%) in association with malignant, postpubertal-type testicular germ cell tumors, mainly at the metastatic sites (less frequently at primary), and often after treatment. Historically, the pathogenetic framework for these tumors, indeed their current classification and nomenclature under the WHO 5 th Edition, contemplated their arising from (malignant) postpubertal-type teratomatous components undergoing oncogenic changes homologous to conventional somatic malignancies of the relevant tissue lineages. Recent scholar

Identification of risk factors and prediction models for secondary malignant neoplasms (SMNs)-free survival and SMNs-specific survival in testicular cancer survivors.

OBJECTIVE: This research endeavored to determine the key demographic and pathological factors tied to secondary malignant neoplasms (SMNs) in survivors of testicular cancer and to develop a predictive model. METHOD: A total of 53,309 testicular cancer patients from the SEER national database (1975-2016) were included in our analysis. The primary outcome measured was SMNs-free survival, defined as the duration from testicular cancer diagnosis to the detection of a non-testicular malignancy. The secondary outcome was SMN-specific survival, defined as the period from testicular cancer diagnosis

Inflammatory and Nested Testicular Sex Cord Tumor: A Novel Neoplasm With Aggressive Clinical Behavior and Frequent EWSR1::ATF1 Gene Fusions.

A subset of malignant testicular sex cord tumors (TSCTs), heretofore interpreted as Sertoli cell tumors, not otherwise specified, exhibits distinctive morphologic features that partially overlap with those of seminoma. In this study, we evaluated the clinicopathologic and molecular characteristics of 13 such tumors. The patients were 20 to 73 years old (median, 36 y), and all with available data presented with testicular masses (median size, 3 cm), with 2 having synchronous retroperitoneal metastases. All 11 patients with available follow-up developed metastases to retroperitoneal lymph nodes,

Cryptorchidism: aspects of fertility and neoplasms. A study including data of 1,335 consecutive boys who underwent testicular biopsy simultaneously with surgery for cryptorchidism.

PURPOSE: An attempt to make a rational strategy for treatment of cryptorchidism. MATERIALS AND METHODS: 1,335 cryptorchid boys with biopsy at surgery (1,638 specimens). We studied: frequency of no germ cells in biopsies from 698 patients <12 years at surgery; fertility potential of 140 patients who were now adults, and apperance of testicular neoplasia in all biopsies. RESULTS: Lack of germ cells appeared from 18 months. The frequency increased with increasing age. It appeared in 30% (61/202) bilateral, and 18% (88/496) unilateral cases. In men who had undergone bilateral or unilateral orchi

Abdominal disseminated lesions following surgery for a testicular mucinous cystic neoplasm initially diagnosed as mucinous cystadenoma: a case report and literature review.

This report describes a rare testicular mucinous cystic neoplasm in a 78-year-old man who underwent scrotal exploration and right radical orchiectomy. Histopathology of the resected specimen was interpreted as an ovarian-type mucinous cystadenoma with focal epithelial proliferation. Six months after surgery, follow-up imaging demonstrated multiple abdominal and peritoneal lesions suspicious for disseminated disease. Because the abdominal lesions were diagnosed radiologically and were not histologically sampled, metastatic disease could not be pathologically confirmed. In addition, immunohistoc

Neuroglial Differentiation and Neoplasms in Testicular Germ Cell Tumors Lack Immunohistochemical Evidence of Alterations Characteristic of Their CNS Counterparts: A Study of 13 Cases.

Overgrowth of neuroglial tissue is rare in testicular germ cell tumors and mostly reported as isolated cases. We retrospectively reviewed 13 cases of testicular germ cell tumors from 2 institutions from 1995 to 2018. Hematoxylin and eosin slides were collected and reviewed. Immunohistochemistry was performed in all cases with available material. The series included 4 primary tumors and 9 metastases, including 8 retroperitoneal and 1 axillary lymph node (LN). The average age was 34 (range: 19 to 54). Five of the LN dissections were postchemotherapy, with one a recurrence 5 years after the initi

Testicular Rosai-Dorfman disease clonally related to CMML - Case report and literature review.

BACKGROUND: Rosai-Dorfman disease (RDD), a rare form of non-Langerhans cell histiocytosis with heterogenous clinical features, arises from precursor cells that give rise to cells of the histiocytic and monocytic lineages. An association with hematological neoplasms has been reported. Testicular RDD is rarely described, with only 9 reported cases in the literature. Genetic data to assess clonal relationships between RDD and other hematological neoplasms remain scarce. We describe an instance of testicular RDD against a background of chronic myelomonocytic leukemia (CMML), with genetic studies i

Metastatic Testicular Mixed Germ Cell Tumor Presenting as Posterior Scrotal Pain in the Emergency Department: A Case Report.

Testicular neoplasms, or testicular cancer, are not typically seen in the emergency department (ED) since their presentation involves a painless hard mass that emerges slowly over time. Uncommon presentation of testicular neoplasm to the ED with acute onset of scrotal pain may present challenges as an incomplete physical examination without supplemental imaging and laboratory workup may overlook the diagnosis of testicular neoplasm. As a result, a delay in proper treatment may occur. Early recognition of testicular neoplasm can decrease morbidity and mortality and improve overall patient survi

[Realtime sonography of intrascrotal space-occupying lesions].

In 848 examinations of the testes in 317 patients, including 253 follow-up and/or multiple follow-up examinations and 36 unilateral testicle examinations (e.g., orchidectomy, aplasia or nondescent) it was shown that sonography is a diagnostic method which is on the one hand capable of clearly differentiating cystic from solid space-occupying processes, and on the other hand also offers certain possibilities for evaluating the status of solid findings. Histologic identification of malignant tumors was only achieved with teratomas. The differential diagnostic problem was in differentiating scrot

Testicular manifestation of a transformed mycosis fungoides.

Testicular neoplasms occur in more than 90% of cases, due to primary testicular germ cell tumors. Other entities are non germ cell tumors of the testis, testicular manifestation of lymphomas or metastases. International and interdisciplinary co-operation has led to the development of urological guidelines and to good therapeutic success for testicular neoplasms. The gold standard for treatment of a testicular neoplasm is the radical orchiectomy. However, for individual cases with suspected lymphoma, a treatment decision differing from the guidelines may be reasonable. We present the case of a

Testicular Germ Cell Tumor Presenting as Torsion in a Young Patient.

Testicular torsion is a relatively common urological emergency, which involves the twisting of the spermatic cord and its contents leading to ischemia to the testes, which usually presents as sudden, severe scrotal pain. In comparison, testicular neoplasms are far less commonly encountered in the emergency department as they often present as painless hard masses that grow slowly over longer periods of time. Extremely rare cases of testicular neoplasms present as sudden scrotal pain that causes a challenging task in the emergency department as physical examinations and ultrasound findings could

Paratesticular localization of burned out non-seminomatous germ cell tumor--NSGCT: a case report.

Testicular cancer is broadly divided into seminoma and non-seminoma types for treatment planning because seminomas are more sensitive to radiation therapy. Non-seminomatous germ-cell testis tumors represent a majority of all testicular neoplasms and include yolk sac tumor, embryonal carcinoma, choriocarcinoma, teratoma and undifferentiated tumors. Malignant neoplasms account for approximately 25% of neoplasms of the paratesticular tissues and most of them are sarcomas. We report a case of completely undifferentiated germ cell tumor of spermatic cord and discuss its diagnosis.

Systematic Review: An Update on the Spectrum of Urological Malignancies in Lynch Syndrome.

BACKGROUND: Lynch syndrome is an autosomal dominant disorder that predisposes individuals affected to certain malignancies. Colon and endometrial cancers are the malignancies most highly associated with Lynch syndrome. However, growing body of evidence links Lynch syndrome to urological cancers. OBJECTIVE: This review aims to clarify the type of urological malignancies that fall under the Lynch-associated cancer spectrum. METHODS: Using PRISMA guidelines, a systematic search between January 1990 to February 2018, was conducted using the MEDLINE database with the application of the following

Early prenatal detection of an intra-abdominal cryptorchid testicular teratoma.

Intra-abdominal prenatally detected testicular neoplasms are rare; however, increased use of prenatal ultrasonography has led to the discovery of these uncommon neoplasms. We report the fifth case of a prenatally detected intra-abdominal testicular teratoma, which, in this instance, was detected early in pregnancy as a cystic mass within the fetal abdomen that subsequently underwent torsion later in pregnancy before delivery.

Invasion of the inferior vena cava by testicular seminoma: demonstration by computed tomography and venography.

Involvement of the inferior vena cava (IVC) by testicular neoplasms is unusual and often clinically unsuspected. We present a patient with testicular seminoma that involved the IVC secondarily and caused multiple pulmonary emboli requiring the placement of a caval filter. The radiographic features of this phenomenon are presented with special emphasis on the computed tomographic (CT) appearance of caval thrombosis and pulmonary embolus. The characteristically low density lymphadenopathy of seminoma may obscure the diagnosis of caval thrombosis on CT. Although the findings are nonspecific, tumo
