Long-term studies of metopic synostosis: frequency of cognitive impairment and behavioral disturbances.

Although the occurrence of cognitive impairment and behavioral disturbances in patients with metopic synostosis has been described, the incidence of this dysfunction has not been established. The records of 36 consecutive children with metopic synostosis followed at one craniofacial center from 1978 to 1993 were reviewed and parental questionnaires were completed to establish the frequency of mental retardation, learning disabilities, and behavioral problems associated with this synostosis. Documentation of syndromes, abnormal karyotype, and central nervous system anomalies also was done. The

Developmental expression of MARCKS and protein kinase C in mice in relation to the exencephaly resulting from MARCKS deficiency.

The roles of protein kinase C and its substrates in development are poorly understood. Recently, we disrupted the mouse gene for a major cellular substrate for protein kinase C, the MARCKS protein (Proc. Natl. Acad. Sci. USA, 92, 944-948, 1995). The resulting phenotype consisted of universal perinatal lethality, agenesis of the corpus callosum and other forebrain commissures, and neuronal ectopia and other cortical and retinal lamination disturbances. These mice also had high frequencies of exencephaly (25% overall, 35% in females). In the present study, we have examined the normal expression

Prolonged and extensive IgG immunoreactivity after severe fluid-percussion injury in rat brain.

The relationships between protein extravasation, morphological changes in neurons, and reactive changes in axons were evaluated in rats subjected to right lateral fluid-percussion injury to the brain (4.8-5.6 atm, 20 ms). Serial sections of the brain were immunostained with antibodies to rat immunoglobulin G (IgG) and 68-kDa neurofilament at 1 h to 2 weeks after injury or sham injury. Ischemic changes in neurons were noted in the injured cortex at 6-48 h after injury, and macroscopic hemorrhages were noted in the right corpus callosum and external capsule at 1 h to 1 week after injury. Extrace

Abnormal electroretinogram associated with developmental brain anomalies.

PURPOSE: We have encountered abnormal ERGs associated with optic nerve hypoplasia, macular, optic nerve and chorioretinal colobomata and developmental brain anomalies. Brain anomalies include cortical dysgenesis, lissencephaly, porencephaly, cerebellar and corpus callosum hypoplasia. We describe six exemplar cases. METHODS: Scotopic and photopic ERGs adherent to international standards were performed as well as photopic ERGs to long-duration stimuli. CT or MRI studies were also done. The ERGs were compared to age-matched normal control subjects. RESULTS: ERG changes include reduced amplitude

CNS distribution and overexpression of neurofilament light proteins (NF-L) in mice transgenic for the human NF-L: aberrant accumulation in thalamic perikarya.

Light microscopic immunocytochemistry with monoclonal antibodies recognizing both murine and human light neurofilament proteins (mNF-L and hNF-L) or hNF-L only was used to examine the distribution of NF-L in the CNS of adult mice, normal or transgenic for the human gene. In normal mice, major fiber bundles were immunoreactive to the first antibody, with few exceptions such as the internal capsule, anterior commissure, and corpus callosum. Strong immunoreactivity was also present in the perikarya of motoneurons in the spinal cord and brainstem, as well as in other brainstem nuclei. Faint cell b

The outcome of fetal ventriculomegaly.

Over a 5 year period 38 cases of fetal ventriculomegaly were diagnosed at Queen's Medical Centre, Nottingham. There were 12 cases of spina bifida and all patients opted for a termination of pregnancy. There were 15 cases of isolated ventriculomegaly comprising seven cases of aqueduct stenosis, four abnormalities of the corpus callosum, one cavum septum pellucidum cyst, one case of porencephaly and two cases of mild lateral ventricular dilatation. The fetuses in this group had a relatively good outcome with five babies showing normal development, three with mild development delay and one with m

[Primary intracerebral non-Hodgkin's lymphoma--a clinicopathologic study].

Primary cerebral lymphomas (PCL) were diagnosed with increasing frequency also in our retrospective study of 44 patients. Clinically these tumors presented with signs of a rapidly growing brain neoplasm. The analysis of CCT data showed that the tumors were of varying density before and showed mostly (60%) homogeneous enhancement after contrast medium application. MR imaging was more sensitive, but could not aid in distinguishing PCL from other brain tumors. The lesions lay mainly (82%) in the supratentorial space and involved the frontal lobe in 42% of cases. Only 16% were located in the periv

[An adult patient with Krabbe's disease--the first case reported in Japan].

A 31-year-old woman, a child of consanguineous parents, who had shown mental retardation and slowness in running ability since childhood, complained of progressive spastic gait for one and a half years prior to admission. On admission, neurological examination revealed severe spastic paraplegia, mental retardation, visual disturbance, cerebellar ataxia, disturbance of deep sensation, sphincter disturbance, pes cavus and pes equinovarus. Protein content in the cerebrospinal fluid was increased to 86.1 mg/dl. Markedly reduced activity of galactosylceramidase in the leukocytes and the cultured fi

Localization of the ras-like rab3A protein in the adult rat brain.

Rab3A is a small GTP-binding synaptic vesicle protein, shown to dissociate from synaptic vesicle membranes upon depolarization-induced exocytosis. Using an antiserum raised against rab3A, we found that the antigen was localized to the neuropil of specific brain regions, but was not present in major fiber tracts or most cell bodies. For example, the neuropil of several thalamic nuclei (i.e., dorsal lateral geniculate nucleus, lateral posterior nucleus, ventroposterior nucleus), cerebral cortex, upper layers of the superior colliculus and matrix zones of the neostriatum, were strongly immunoreac

Histopathological follow-up study of 66 cerebral arteriovenous malformations after therapeutic embolization with polyvinyl alcohol.

Embolization with polyvinyl alcohol (PVA) is an accepted method of rendering complex arteriovenous malformations (AVM's) more amenable to surgery, but its effects on human vascular tissues have not been adequately documented. The authors reviewed the histopathology of 66 intracranial AVM's resected 1 to 76 days after embolization with PVA. The mean age of the patients was 36 years, and their AVM's were located in the cerebral hemispheres (92%), the cerebellum (6%), or the corpus callosum (2%). In 79% of cases, at least one vessel contained PVA particles; in most cases, the vessel was filled wi

The Wolf-Hirschhorn syndrome in fetuses.

Wolf-Hirschhorn syndrome (WHS) with partial deletion of the short arm of chromosome 4 has been exceptionally diagnosed in fetuses. We report prenatal diagnosis of five cases of monosomy 4p. The fetuses were karyotyped for severe intrauterine growth retardation (IUGR) diagnosed on routine ultrasound (US). In addition, cleft-lip and palate and diaphragmatic hernia respectively were found in two cases. The quantity of amniotic fluid was normal in all cases. At autopsy, the fetuses showed the typical craniofacial dysmorphy but without microcephaly. Major renal hypoplasia was the only constant visc

[Transethmoidal encephalomeningocele in neonate--report of two cases and review of literatures].

Transethmoidal meningoencephalocele is caused by protrusion into the nasal cavity of a part of brain and meninges through an ethmoidal defect being a subtype of basal meningoencephalocele. It is extremely rare as only a few cases have been reported so far. It gives characteristic symptoms and signs as follows: (i) mouth breathing and snoring due to intranasal tumor and obstruction from birth, (ii) pulsation of the tumor synchronous with pulse or respiration, (iii) complication of facial deformities such as hypertelorism, cleft lip and palate, and (iv) leakage of cerebrospinal fluid from the no

[Dysphatic development: clinical importance and neurological background].

The timely treatment of children with developmental dysphasia is important in the prevention of subsequent learning and behaviour disorders. The pathophysiology of developmental dysphasia is complex and depends on age. In the preverbal and early verbal phase the severity of the clinical picture is determined by accompanying motor function pathology (simple motor function, dysarthria, general and oral dyspraxia) and by receptive pathology (hearing, and auditory perception). In the verbal period linguistic problems become more prominent (syntax, morphology, semantics and verbal memory), these pr

Fryns syndrome: report on 8 new cases.

The name Fryns syndrome was given to a new variable multiple congenital anomaly syndrome, almost always lethal, described in 1978, and now known to be autosomal recessive. Since that date, 20 patients have been reported in the literature. We describe 8 new cases, 6 of which were diagnosed in a series of 112,276 consecutive births (livebirths and perinatal deaths). The prevalence of this syndrome can be estimated to be around 0.7 per 10,000 births. These new cases confirm that the most frequent anomalies are diaphragmatic defects, lung hypoplasia, cleft lip and palate (often bilateral), cardiac

New disorders of neurometabolism: when and how to investigate them.

It is often difficult to know how far to investigate children with delayed development, and the paediatrician may feel frustrated by the low yield of the usual investigations. Yet the benefits of diagnosis are sufficiently great that few would quibble with the need for some investigations. It would be best if clinical signs directed the testing, but in practice most retarded children provide no clues to the direction in which investigation should go. This may be true, for example, of the young child with X-linked mental retardation with fragile X chromosome--Sanfilippo syndrome--one of the rar

Real-time ultrasonography in the neonate: a systematic study of a high-risk infants population.

During a 12-month period a prospective and systematic study was carried out by means of portable real-time ultrasound (US) scanner in order to detect cerebral lesions in a population of high-risk neonates. Newborn infants were allocated into two groups: group A: all premature infants (n = 83) of less than or equal to 34 weeks' gestation or less and group B: neonates (n = 36) of more than 34 weeks' gestation presenting with abnormal neurological signs. Group A: the overall incidence of periventricular haemorrhage (PVH) was 47% (15 grade I, 16 grade II, 3 grade III, 1 Plexus choroid haemorrhage,

Neurite orientation dispersion and density imaging for preoperative evaluation of cerebral microstructure and neurodevelopment in children with congenital heart disease.

BACKGROUND: Congenital heart disease (CHD) is a type of heart structure defect caused by the abnormal development of the heart and major blood vessels during the embryonic stage. With the continuous innovation of surgical treatment techniques and the improvement of perioperative management, the survival rate of children with CHD has significantly improved. However, accompanying central nervous system development problems have become increasingly prominent. This study investigated the clinical utility of neurite orientation dispersion and density imaging (NODDI) in evaluating preoperative cereb

Neuroprotection is improved by watertightness of fetal spina bifida repair in the sheep model.

OBJECTIVES: A contributing factor to unsuccessful prenatal spina bifida aperta (SBA) repair via an open approach may be incomplete neurosurgical repair causing persistent in-utero leakage of cerebrospinal fluid (CSF) and exposure of the fetal spinal cord to amniotic fluid. We aimed to investigate the neurostructural and neurofunctional efficacy of watertight prenatal SBA repair in a validated SBA fetal lamb model. METHODS: A well-powered superiority study was conducted in the validated SBA fetal lamb model (n = 7 per group). The outcomes of lambs which underwent watertight or non-watertight m

Prognosis of feeding and swallowing function in children and adolescents/young adults after corpus callosotomy.

BACKGROUND: Acute disconnection syndromes frequently occur after corpus callosotomy (CC), with poor oral intake being among the symptoms. Most patients achieve normal oral intake by 2 weeks postoperatively. However, in some cases, feeding and swallowing functions fail to recover, and risk factors for delayed improvement in oral intake remain unclear. This study aimed to investigate the frequency of delayed improvement in oral intake at 2 weeks after CC, to descriptively examine feeding and swallowing function outcomes at 6 months postoperatively, and to identify associated risk factors. Alexithymia mediates white-matter microstructure and substance use escalation in trauma-exposed individuals.

INTRODUCTION: Alexithymia has been separately associated with differences in white-matter and with substance use (SU). Childhood trauma has also been associated with white-matter differences, alexithymia, and SU. This study explored the moderating effect of childhood trauma on the indirect effect of white-matter microstructure on SU escalation through alexithymia. METHODS: Participants were 44 young adults from a larger longitudinal cohort, and data on childhood trauma, alexithymia, white-matter were acquired initially. SU escalation was calculated from the quantity and frequency of SU initia
