Open reduction and tunneled suspensory fixation for lateral end of clavicle fractures: surgical technique.

Unstable distal end clavicle fractures are associated with significant rates of nonunion and poor functional outcomes. Surgical treatment is paramount for unstable fracture patterns; however, treatment options are various, with each having its advantages and drawbacks. Recently, suture-based coracoclavicular fixation techniques using suture buttons have been implemented with high union rates, satisfactory shoulder function, and low rates of complications. In this report, we demonstrate a modified fixation of unstable lateral clavicle fractures. Our technique entails open reduction and suspenso

Arytenoid adduction: controlling vertical position.

In flaccid laryngeal paralysis, the vocal process (VP) is displaced laterally and superiorly. The arytenoid adduction procedure (AA) moves the VP medially and caudally, closing the glottic gap. However, clinical evidence suggests that the VP is more caudal after AA than in physiological phonation. The neurally intact arytenoid is supported by tonic and phonatory activity of the posterior cricoarytenoid muscle (PCA). We hypothesize that a posterior anchoring suture could replace PCA support, achieving a more natural VP location. Cadaver larynges were scanned with computed tomography at rest and

Stimulation of regeneration of the sciatic nerve by experimentally induced inflammation in rats.

The effects of application of a chromic catgut suture (conditioning lesion) placed close to the sciatic or tibial nerves on regeneration of the sciatic nerve after a crush lesion (test lesion), that had been induced after an appropriate conditioning interval (two or four weeks) were assessed. The catgut suture induced an inflammatory reaction around the nerve during the four weeks after application of the catgut suture (conditioning interval) but no signs of degeneration of nerve fibres were seen. There was a significant increase in length of outgrowth of sensory nerve fibres as measured by th

Prospective, randomized trial of the biofragmentable anastomosis ring. The BAR Investigational Group.

A randomized trial was undertaken to compare the biofragmental anastomotic ring (BAR) with conventional intraperitoneal colorectal anastomotic techniques. Patients were randomized into one of two schemes: BAR versus sutured or BAR versus stapled anastomosis. There were 782 patients entered into the study and 283 patients (36%) had a sutured anastomosis, 104 patients (13%) had a stapled anastomosis, and 395 (51%) had the BAR. Comparison of the BAR with combined suture and stapled controls revealed no significant differences in wound complication, abscess rate, bleeding, anastomotic leaks, ileus

Experimental evaluation of microsurgical techniques in small artery anastomoses.

A number of factors that influence the success of microvascular anastomosis have been studied, including adventitial stripping, choice of suture material and needles, suturing technique, and perfusion of the distal lumen. It is apparent that only minimal stripping of the adventitia is indicated in order to prevent increased necrosis of the vessel ends at the anastomosis site. The use of 10-0 monofilament nylon suture material with needles 75 microns or less in diameter achieves the best results in small vessel anastomoses. Optimal anastomosis of 1 mm. vessels requires interrupted full thicknes

Three-flanged capsular hook for zonular dialysis repair.

To describe a surgical technique using a three-flanged Prolene suture for managing zonular dialysis during cataract surgery in a patient with preoperative zonular deficiency in brown cataract. A patient with zonular deficiency and brown cataract underwent cataract surgery complicated by zonular dialysis between the 4 o'clock and 8 o'clock positions. A three-flanged Prolene suture, fashioned in an iris hook-like configuration, was used. A 26-gauge needle was introduced at the 6 o'clock position to pass a 7-0 Prolene suture through the zonular defect. A capsular tension ring (CTR) was placed to

Surgical strategy for refractory aortitis.

In some instances, we encounter cases suffered from inflammatory aortic diseases (aortitis) in Japan, some of which are at the active stages with systemic inflammation. Most of them are refractory with some technical difficulties of surgical treatment. The aortic wall, particularly, at the active stage, is too fragile to hold the surgical sutures. Consequently, the suture reinforcement with Teflon felt is required. In the late stage after surgery, false aneurysms on the suture line, that is, suture detachment potentially occur. To prevent such sequelae in the early and late phases, continuous

A Novel Hidden Stitching Method for Preventing Complications in Laparoscopic Hernia Repair.

PURPOSE: Barbed suture provides a rapid and effective method for closure in minimally invasive surgery. However, postoperative complications involving barbed suture have been reported in recent years. The aim of this study is to introduce a novel hidden stitching technique for peritoneal closure in laparoscopic hernia repair. METHOD: This study retrospectively analyzed the data of patients with laparoscopic transabdominal preperitoneal patch (TAPP). In the hidden stitch (HS) group, the barbed suture was hidden on the dorsal side of the peritoneum and two stitches were returned in the opposite

Improved anchoring nails: design and analysis of resistance ability : Tensile test and finite element analysis (FEA) of improved anchoring nails used in temporomandibular joint (TMJ) disc anchor.

BACKGROUND: Anchorage is one of the most important treatments for severe temporomandibular joint disorder (TMD). Anchoring nails have shown great success in clinical trials; however, they can break under pressure and are difficult to remove. In this study, we aimed to evaluate an improved anchoring nail and its mechanical stability. METHODS: The experiment consisted of two parts: a tensile test and finite element analysis (FEA). First, traditional and improved anchoring nails were implanted into the condylar cortical bone and their tensile strength was measured using a tension meter. Second,

Association Between Opioid Intake and Disability After Surgical Management of Ankle Fractures.

BACKGROUND: Opioid-centric pain management strategies have created an epidemic of prescription opioid abuse. This study assesses whether opioid intake is associated with disability, satisfaction with treatment, and pain at the time of suture removal and at 5 to 8 months after suture removal following open reduction and internal fixation of ankle fractures. METHODS: We enrolled 102 adult patients in the study at the time of suture removal, 59 of whom were available for follow-up at 5 to 8 months. At the time of suture removal, we recorded opioid use; trauma-related factors; and scores on measu

A new technique of fascial closure for laparoscopic incisions.

The objective was to develop a technique and instrumentation for closure of 10-mm and larger laparoscopic incisions with a single instrument under direct laparoscopic vision, ensuring complete fascial and peritoneal closure. The Carter-Thomason Needle-Point Suture Passer functions as both a needle and a grasper, allowing for performance of laparoscopic directed fascial and peritoneal closure. It uses a 2.7-mm diameter grasping tool with a single-action jaw. The device introduces the suture through the muscle, fascia, and peritoneal layers under direct laparoscopic vision, drops the suture, pic

Dentofacial changes produced by extraoral forward force in the Macaca irus.

Eleven Macaca irus monkeys with a mixed or a permanent dentition were used in a study of the possible effects of extraoral forward force on the growth of the dentofacial skeleton. An intermittent forward force of 300 Gm. per side was applied, and the effects were analyzed by means of model casts, cephalometric radiographs, tetracycline bone marking, microradiographs, and histologic preparations. The results obtained were as follows: 1. An extraoral forward force caused significant changes in the circum-maxillary sutures and the maxillary tuberosity. 2. It was found that intermittent force of 3

Evaluating the feasibility of a novel device for enhanced control of chordal length assessment in mitral valve surgery: A porcine model study.

OBJECTIVE: Determining the appropriate expanded polytetrafluoroethylene (ePTFE) chord length remains a critical step in mitral valve repair. Freehand ePTFE suturing and premeasured loops are widely used but present technical challenges. A novel chordal holder device was developed to temporarily secure the ePTFE suture during pressurized saline infusion testing, enabling precise subsequent adjustment of chordal length. This feasibility study evaluated this technology in an ex vivo porcine heart model. METHODS: In 20 ex vivo porcine hearts, Carpentier type II mitral regurgitation was induced by

Comparison of Staple, Anchor, and Tenodesis Screw for Posterior Tibialis Tendon Fixation: A Biomechanical Analysis.

Various posterior tibialis tendon fixation techniques are described in literature. Suture anchor, staple and tenodesis screws are widely used for posterior tibialis tendon transfer, but their stiffness and the maximal ultimate failure load were not tested before. We aimed to compare the initial ultimate failure load and stiffnesses of suture anchor, staple and tenodesis screws on bovine tendon fixation to bovine metaphyseal bone. Thirty-five fresh bovine ankle joints and hooves were obtained from a local abattoir. Metatarsals bones with long extensor tendons were harvested. Staple group had 15

Recombinant human bone morphogenetic protein-2-induced craniosynostosis and growth restriction in the immature skeleton.

BACKGROUND: Recombinant human bone morphogenetic protein-2 (rhBMP-2) delivered on an absorbable collagen sponge is a U.S. Food and Drug Administration-approved therapy effective at generating bone formation. In pediatric patients for whom other therapeutic options have been exhausted, rhBMP-2 is used off-label to address problematic bony defects. In the skeletally immature patient, the safety of rhBMP-2 therapy remains uncertain. Experiments are needed that investigate the effect of rhBMP-2 on growth and development in clinically relevant models. METHODS: Ten juvenile rabbits underwent creati

3-D computational analysis of the stress distribution on the leaflets after edge-to-edge repair of mitral regurgitation.

BACKGROUND AND AIM OF THE STUDY: Edge-to-edge repair is an effective, recently introduced method to correct mitral insufficiency by suturing the leaflets at the site of regurgitation, though durability of the method has not been proven. To overcome the limitations of the clinical approach, simulations may be used to predict clinical outcome. In this study, the mechanical stress acting on leaflets imposed by the edge-to-edge suture was evaluated as a means of assessing the clinical risk of late fibrosis or tissue degeneration. METHODS: A 3-D finite element simulated the stress pattern followin
