KL-6 as a novel serum marker for interstitial pneumonia associated with collagen diseases.

OBJECTIVE: To investigate the diagnostic value of the serum concentrations of KL-6, a mucinous glycoprotein expressed on type II pneumocytes, for interstitial pneumonia (IP) in various collagen diseases. METHODS: Serum KL-6 levels were measured by ELISA. RESULTS: The mean values and the positive rates of serum KL-6 levels for patients with rheumatoid arthritis, systemic sclerosis, or polymyositis/dermatomyositis with IP were significantly higher than those without IP. Sensitivity, specificity, positive and negative predictive values of serum KL-6 level for IP associated with collagen disease

Distal Allograft Pancreatectomy After Pancreas Transplantation: A Case Report and Review of Literature.

Paraneoplastic lesions such as intraductal papillary mucinous neoplasms (IPMNs) are uncommonly observed in the graft of recipients of pancreas transplants, despite the well-established association between immunosuppression and malignancy development. Typically, allograft pancreatectomy is performed due to complications such as acute rejection or vascular thrombosis. We report a rare case of a patient who developed histologically confirmed IPMN occurring 8 years after pancreas transplantation. As the detection of paraneoplastic lesions increases with advancing imaging techniques and pancreas al

Endoscopic retrograde cholangiopancreatography: A comprehensive review as a single diagnostic tool.

Endoscopic retrograde cholangiopancreatography (ERCP) was initially introduced in clinical practice as diagnostic tool. However, the presence of adverse events and the development of non-invasive techniques, such as magnetic resonance cholangiopancreatography (MRCP) and endoscopic ultrasound (EUS), limited its role as a stand-alone diagnostic choice, modifying its role to the leader of therapeutic pancreatobiliary endoscopy. Despite technological advances, there are still conditions where non-invasive diagnostic modalities are inconclusive, such as indeterminate biliary and pancreatic duct str

Review of Various Perianal Lesions on CT and MRI.

The perianal area consists of complex and diverse anatomical structures. Various conditions can affect this area, including inflammatory diseases, such as perianal abscesses, fistulas, and condyloma acuminata, and malignant tumors, such as mucinous adenocarcinoma, squamous cell carcinoma of the rectum, and squamous cell carcinoma of the anus. Due to the small size of the perianal area, lesions may be easily overlooked. However, with advancements in imaging technology, perianal lesions can now be identified using CT and MRI, aiding in the planning of treatment strategies. This pictorial essay a

Signet-Ring Cell Type Adenocarcinoma of the Urachus: A Rare Case.

Urachal adenocarcinoma is a rare subset of bladder cancer originating from the urachus. We present a case of a 62-year-old woman with a 2-year history of persistent dysuria, urinary frequency, and urgency. On physical examination, a firm, non-tender suprapubic mass was detected. Cystoscopy was performed and a bladder mass was detected. After obtaining the biopsy, pathology confirmed a mucinous signet-ring cell type adenocarcinoma of urachal origin with negative margins and no vascular or perineural invasion. Following negative tumor markers, colonoscopy and CT scans for metastasis, the patient

Update of newly-recognized salivary gland neoplasms: molecular and immunohistochemical findings and clinical importance.

With the advancement of molecular testing and the routine use of immunohistochemical stains, salivary gland tumours previously categorized as adenoma or adenocarcinoma, not otherwise specified, are being reclassified with distinct diagnoses. Newly recognized benign entities include: sclerosing polycystic adenoma, keratocystoma, intercalated duct hyperplasia and adenoma, and striated duct adenoma. Newly recognized malignant salivary gland tumours include: microsecretory adenocarcinoma, sclerosing microcytic adenocarcinoma, and mucinous adenocarcinoma. Additionally, rare subtypes of mucoepidermo

Pancreatoscopy-Guided Endotherapies for Pancreatic Diseases.

Per-oral pancreatoscopy (POP) is a pancreas-preserving modality that allows for targeted pancreatic duct interventions, particularly in cases where standard techniques fail. POP specifically has an emerging role in the diagnosis, risk stratification, and disease extent determination of main duct intraductal papillary mucinous neoplasms (IPMNs). It has also been successfully used for laser ablation of IPMNs in poor surgical candidates, lithotripsy for complex stone disease, and laser stricturoplasty. As experience with POP increases beyond select referral center practices, further studies valid

Renal Neoplasia: Rare Subtypes and Uncommon Clinical Presentations.

Herein, the authors have discussed a series of uncommon familial kidney cancer syndromes (including hyperparathyroidism-jaw tumor syndrome and PTEN hamartoma tumor syndrome), sporadically occurring tumors (BRAF and MTOR pathway-mutated tumors, and juxtaglomerular cell tumors), and uncommon patterns of well-established subtypes of kidney cancer (mucinous tubular spindle cell carcinoma, fumarate hydratase-deficient, and TFE3-rearranged renal cell carcinoma). The rarity of these tumors often leads to diagnostic odysseys for pathologists and patients. Appropriate classification of these rare tumor

[Differential diagnosis of sclerosing skin diseases: scleredema adultorum of Buschke].

Scleredema adultorum of Buschke is an important differential diagnosis in sclerosing diseases. Diagnosis is based on the typical histology with mucin deposits and grossly increased dermal width and a clinical presentation of diffuse non-pitting induration of the skin starting at the nape of the neck and interscapular region extending to shoulders and upper thorax, causing dysmobility due to dermal stiffness. Even though the pathogenesis remains unclear, three subtypes can be distinguished: association with infections, paraproteins, or most frequently with diabetes mellitus. Management of the d

Urachal Adenocarcinoma: A Case Report of a Rare Tumor.

Urachal adenocarcinoma is a rare and aggressive bladder cancer involving the urachus, an embryological fibrous remnant of the allantois extending from the bladder to the umbilicus. Usually discovered in the advanced stages, this cancer can commonly present with a poor prognosis. We report a case of a 34-year-old male patient with an unremarkable medical history who presented to the emergency department with severe, sudden onset, sharp abdominal pain. Follow-up imaging revealed a tubular structure with mildly thickened and enhancing margins emanating from the anterior wall of the urinary bladde

Synchronous Acute Appendicitis and Acute Cholecystitis.

The occurrence of synchronous acute cholecystitis and appendicitis is rare. There are few cases reported and small series in the literature. We report the case of a 77-year-old male who presented to the emergency department with right-sided abdominal pain. He was diagnosed preoperatively with acute calculous cholecystitis. During the initial laparoscopy, a complicated appendiceal phlegmon was found and was treated with a one-step laparoscopic approach and subsequent antibiotics. The patient had an uneventful recovery and was discharged on postoperative day (POD) 2. The pathology report confirm

[A case of blindness caused by mucocele of frontal sinus].

A case of blindness caused by frontal sinus mucocele was reported. The patient presented with pain and swelling in his left eye and decreased visual acuity with only light sensation for 3 days. Orbital imaging examination suggested a mass of communication in the left frontal sinus and the upper quadrant of left orbit, which was considered as a mucinous cyst involving the left superior orbital wall, surrounding the left lacrimal gland, pushing down the left superior rectus muscle, squeezing the left eyeball and protruding forward and downward. The patient was diagnosed with a left frontal sinus

Histological type and typing of breast carcinomas and the WHO classification changes over time.

The World Health Organization's new classification of breast tumors has just been published. This review aims to examine the morphological categorization of breast carcinomas which is still principally based on histological features and follows the traditions of histological typing. It gives a subjective and critical view on the WHO classifications and their changes over time, and describes the changes related to some of the most common or challenging breast carcinomas: in situ carcinomas, invasive breast carcinomas of no special type, lobular, cribriform, tubular, mucinous, papillary, metapla

A rare case of recurrent isolated eyelid myxoma: Case report and review of literature.

Myxoma, a benign mesenchymal tumour, can rarely present as an isolated eyelid swelling. A 33-year-old male presented with a progressively increasing swelling in the temporal part of the right upper eyelid of 5-month duration. The patient was misdiagnosed as chalazion and underwent incision and curettage locally. Subsequently, the patient presented with recurrence of the swelling and was treated with excision of the lesion. Histopathology showed the presence of spindle- to stellate-shaped cells in a myxoid matrix. Alcian blue stain was positive for mucin. The systemic evaluation did not show an

Primary vaginal adenocarcinoma of intestinal-type: case report of a rare gynaecological tumour.

Vaginal cancer is rare and accounts for only 1 to 2% of all gynaecological malignancies. They arise as primary squamous cell cancers or are the result of extension from the cervix or vulva. Primary mucinous vaginal adenocarcinoma of intestinal-type is an extremely rare malignancy of unknown histogenesis with a diagnostic dilemma for the clinician and histopathologist. We presented the case of a 40-year-old Para 0+2 woman with the complaint of a mass in the vagina and recurrent vaginal bleeding who was evaluated and worked-up for examination under anaesthesia and biopsy of the vaginal mass. Scleromyxedematous Changes in a Patient With Long-Standing Mycosis Fungoides Who Progressed to Sézary Syndrome.

Mycosis fungoides (MF) variants with different clinicopathologic and immunohistochemical features have been well-delineated. We report a case of scleromyxedematous changes arising in a patient with long-standing MF who progressed to Sézary syndrome (SS) shortly afterward. Total-skin electron-beam radiation therapy resulted in an excellent response, controlling both the MF/SS and the scleromyxedematous lesions; however, the patient died few months later. Although mucin deposition has been described in association with MF/SS (mainly follicular mucinosis in folliculotropic MF), there are limited

[Imaging of chronic pancreatitis].

Imaging of chronic pancreatitis. Chronic pancreatitis is a combination of imaging pattern: duct abnormalities, with irregularity, enlargement alternating with stenosis, of main pancreatic duct (beading) and enlargement of branch duct (more than 3 thin branch ducts visible); intra ductal or parenchymal calcifications; parenchymal atrophy, global or focal. When CT is typical, no more imaging is needed. On the contrary, MRI with MRCP (2 D one) is necessary to explore doubtful chronic pancreatitis, particularly to find out discrete ductal irregularities. Then pancreas ductal carcinoma could be dep
