Colonoscopy in asymptomatic individuals with a family history of colorectal cancer.

BACKGROUND: This study was performed to evaluate the use of total colonoscopy as the optimal screening test in asymptomatic individuals with a family history of colorectal cancer (CRC). METHODS: Colonoscopy was performed in 249 asymptomatic individuals who had one or two first-degree relatives (FDRs) with CRC; individuals with three or more FDRs with CRC were excluded. RESULTS: Eighty-six colonic lesions were found in 51 individuals (51 of 249; 20.5%). Among these 51 subjects, 27 had neoplastic polyps (n = 38) and 29 had metaplastic polyps (n = 44). Although no invasive cancer was detected,

Atherothrombotic risk factors in subjects with a family history of stroke.

BACKGROUND: Subjects with stroke in a first-degree relative are at increased risk of atherothrombotic vascular disease. We aimed to examine atherothrombotic risk factors in such a population. METHODS: 145 subjects with a first-degree relative with stroke and 143 controls were recruited. Measurements were made of features of the insulin resistance syndrome, haemostatic and fibrinolytic risk factors, and glucose tolerance tests. RESULTS: The relatives had higher systolic blood pressure, HDL, 2-hour glucose, factor VII:C, fibrinogen, vWF and more had smoked. Differences remained in factor VII:C

Family history study of posttraumatic stress disorder with secondary psychotic symptoms.

OBJECTIVE: A family history approach was used to determine if posttraumatic stress disorder (PTSD) with secondary psychotic symptoms was associated with a familial vulnerability to schizophrenia and other psychoses. METHOD: Family history methods were used to compare rates of familial psychopathology in the first-degree relatives of three proband groups: 1) patients with DSM-IV PTSD with secondary psychotic symptoms, 2) patients with DSM-IV PTSD without psychotic symptoms, and 3) healthy matched comparison subjects. RESULTS: PTSD with secondary psychotic symptoms was not associated with an e

Hereditary cancer: family history, diagnosis, molecular genetics, ecogenetics, and management strategies.

The translation of knowledge about hereditary breast cancer and its improved control, as well as prevention through prophylactic surgery, has been significantly accelerated through the veritable explosive discoveries in molecular genetics inclusive of BRCA1 and BRCA2 germline mutations. Needed however, among the physician community, medical geneticists, and genetic counselors, is a raised level of knowledge about hereditary breast cancer syndromes. Particular attention needs to be given to their extant genotypic and phenotypic heterogeneity, their natural history, and foremost, the requirement

Family history of cancer and incidence of acute leukemia in adults.

Family history of cancer may represent shared genetic and environmental risk factors for leukemia. The authors examined associations of first-degree family history of cancer with adult acute leukemia incidence by using data on 811 patients (or their proxies) identified at diagnosis and 637 population-based controls in the United States and Canada during 1986-1990. For proxy-interviewed patients, relative risks were elevated for family history of any cancer (relative risk = 1.7, 95% confidence interval (CI): 1.3, 2.4), hematopoietic cancer (relative risk = 1.8, 95% CI: 1.1, 3.0), leukemia (rela

Breast and cervical cancer screening activities among family physicians in Nigeria.

Breast and cervical cancers are the two commonest malignancies among Nigerian women; where they constitute about 50% of female cancers. A total of 63 out of 82 family physicians from 4 out of the 6 health zones in Nigeria and the Federal Capital Territory, Abuja, Nigeria completed a self-administered questionnaire to determine their knowledge and screening practices for breast and cervical cancers. Forty nine percent (49%) of family physicians perceived cancer as one of the serious health problem facing women in Nigeria. Although 93.7% of responding physicians have heard of breast cancer scree

TM4SF2 gene involvement reconsidered in an XLMR family after neuropsychological assessment.

The TM4SF2 gene (localized at Xp11.4 between the loci DXS564 and DXS556) has been found to be mutated in one MRX family. In order to define the corresponding behavioral phenotype, global IQ and specific cognitive skills were assessed in seven males and three females of this family, independent of subject status. Mental retardation (MR) was mild in three patients and moderate in three others. Despite the broad variability of severity of MR, a cognitive profile specific to the MR in this family was documented. It was characterized by language disorder that was more marked in the articulatory com

Linkage analysis of a large inbred family with congenital megaloblastic anemia.

OBJECTIVE: Megaloblastic anemia during infancy and early childhood often reflects a hereditary disorder of cobalamin's absorption, transport, or intracellular metabolism. There are 3 well defined autosomal recessive syndromes manifesting with megaloblastic anemia due to defects in cobalamin absorption or transport, namely congenital pernicious anemia, Imerslund-Grasbeck syndrome and Transcobalamin II deficiency. The genes responsible for the 3 disorders are gene intrinsic factor (GIF), MGA1 and TCN2, as well as the gene for Transcobalamin I, TCN1 are mapped or cloned, or both. METHODS: We des

Charcot-Marie-Tooth disease and sleep apnoea syndrome: a family study.

BACKGROUND: Charcot-Marie-Tooth (CMT) disease is a genetically heterogeneous group of hereditary motor and sensory polyneuropathies in which sleep apnoea has rarely been reported and no causal relation shown. We looked for an association between the most common subtype of CMT disease (CMT1A) and sleep apnoea syndrome. METHODS: Having diagnosed sleep apnoea and CMT in one family member (index case), we prospectively investigated 13 further members not previously suspected of having neuropathy or apnoeas. All had a neurological examination, electroneuromyography, polysomnography, and genetic te

Medical students' experience with psychiatric diagnoses in a family medicine clerkship.

BACKGROUND: Many medical schools require a family medicine clerkship, yet little is known about the quantity and diversity of the diagnoses encountered by the students. PURPOSE: This study examines patients encountered with psychiatric diagnoses using quantitative data collected by students in a family practice clerkship. METHODS: Over a 2-year period, 445 students completed 3,320 patient encounter forms for patients with a psychiatric diagnosis, noting their comfort level and responsibilities. RESULTS: The patients' diagnoses reflect those seen in a typical family practice. Of the 71,869 p

Family mealtime while growing up: associations with symptoms of bulimia nervosa.

The purpose of this study was to examine associations between bulimia nervosa and family meals. Female college students ( N = 560) completed surveys assessing eating disorder pathology, family meal frequency, and family environment. Results indicate that dinner was the meal most frequently shared with family members. The frequency of eating dinner together as a family was inversely associated with scores on two measures of bulimia, and these associations remained statistically significant after controlling for other familial factors. Familial variables associated with bulimic symptoms included

[Albright hereditary osteodystrophy: identification of a novel mutation in a family].

Studies to detect mutations in the GNAS1 gene were performed in a male patient with features of Albright hereditary osteodystrophy and resistance of target tissues to parathyroid hormone (Pseudohypoparathyroidism Ia). The same investigations were carried out in the patient's mother who showed somatic features of Albright's hereditary osteodystrophy and brachymetacarpia without resistance to parathyroid hormone (Pseudopseudohypoparathyroidism). A point mutation designated c.794GA (R265H) in exon 10 of GNAS1 was identified in DNA from the patient and his mother. This novel mutation in exon 10 of

The well-being of youngsters coming from six different family types.

Families, or rather parents are in a state of flux: sometimes they row a lot, they divorce, they go and live together again and they die. This is why more and more children grow up in family forms other than the nuclear families with their biological fathers and mothers. What, in the long run, is the effect of this on the well-being of adolescents and young adults between 12-30? We have examined intact families that function well, mediocre or badly, one-parent families and stepfamilies after a divorce and one-parent families after being widowed. The data derive from the Dutch longitudinal pane

Characterization of the gene family encoding alternative oxidase from Candida albicans.

Candida albicans possesses a cyanide-resistant respiratory pathway mediated by alternative oxidase (AOX), which seems to be encoded by a gene family with two members. Cloning and expression of AOX1a, one of the genes encoding alternative oxidase from C. albicans, has previously been reported [Huh and Kang (1999) J. Bacteriol. 181, 4098-4102]. In the present study we report the isolation of another gene coding for alternative oxidase, designated AOX1b. AOX1b contains a continuous open reading frame that encodes a polypeptide consisting of 365 amino acids. Interestingly, AOX1a and AOX1b were fou
