Distal access and procedural anticoagulation to prevent radial artery occlusion after coronary angiography - the randomised RAPID trial.

BACKGROUND: Distal transradial access (TRA) and procedural anticoagulation (AC) are among the strategies to prevent radial artery occlusion (RAO) that have some gaps in evidence. AIMS: This study assessed the efficacy and safety of different radial access sites and procedural AC in patients undergoing coronary angiography (CAG). METHODS: The RAPID trial is a single-centre, open-label, 2x2 factorial study that randomised patients to procedural AC versus no procedural AC and also to distal versus conventional TRA with further stratification according to pre-existing oral AC. Patients with indi

Five Years After ARCHBR's Global Introduction: A Prospective Validation Study in Egypt.

Five years after the global introduction of the ARCHBR criteria, prospective validation in real-world clinical settings has remained globally lacking. This study aims to prospectively assess the predictive utility of ARCHBR criteria in an Egyptian percutaneous coronary intervention (PCI) population, describe contemporary bleeding patterns, and establish a regional reference for post-PCI major bleeding outcomes. This single-center study included 1,018 PCI patients treated at Alexandria Main University Hospital between March 2022 and August 2023. High bleeding risk (HBR) was defined as ≥1 major

Long-term outcomes following drug-coated balloons versus thin-strut drug-eluting stents for treatment of in-stent restenosis in Chronic Kidney Disease (CKD Dragon-Registry).

We sought to investigated the outcomes of patients with chronic kidney disease (CKD) and drug-eluting stent (DES)-in-stent restenosis (ISR) undergoing percutaneous coronary intervention (PCI) with a drug-coated balloon (DCB) or thin strut drug-eluting stent (thin-DES). Consecutive patients with DES-ISR who underwent PCI with a thin-DES or a paclitaxel-coated DCB for DES-ISR were enrolled. The primary outcome was target lesion revascularization (TLR), while the secondary was target vessel revascularization (TVR) and device-oriented composite endpoint (DOCE). The pooled analysis included 1,317 p

Breaking the Barrier: Unraveling the No-Reflow Phenomenon in Cardiovascular Medicine.

The no-reflow phenomenon is a stubborn and often devastating complication in cardiovascular medicine, where blood flow is restored to an artery, yet the microvasculature remains unresponsive. First identified in 1967, this phenomenon has haunted clinicians and researchers alike, particularly in the context of acute myocardial infarction (AMI). With incidence rates reaching 11.5% in AMI-related percutaneous coronary interventions (PCI), no-reflow is a major contributor to poor cardiac outcomes, including heart failure and increased mortality. At its core, no-reflow stems from microvascular obst

The clinical characteristics, managements, and outcomes of acute myocardial infarction in osteoarthritis patients; a cross-sectional analysis of 6.5 million patients.

OBJECTIVES: The prevalence of osteoarthritis (OA) and cardiovascular disease are increasing and both conditions share similar risk factors. We investigated the association between OA and receipt of invasive managements and clinical outcomes in patients with acute myocardial infarction (AMI). METHODS: Using the National Inpatient Sample, adjusted binary logistic regression determined the association between OA and each outcome variable. RESULTS: Of 6,561,940 AMI hospitalizations, 6.3% had OA. OA patients were older and more likely to be female. OA was associated with a decreased odds of coron

Culprit Lesion Coronary Intervention Before Complete Angiography in ST-Elevation Myocardial Infarction: A Randomized Clinical Trial.

IMPORTANCE: Rapid reperfusion during primary percutaneous coronary intervention (PCI) is associated with improved outcomes among patients with ST-elevation myocardial infarction (STEMI). Although attempts at reducing the time from STEMI diagnosis to arrival at the catheterization laboratory have been widely investigated, intraprocedural strategies aimed at reducing the time to reperfusion are lacking. OBJECTIVE: To evaluate the effect of culprit lesion PCI before complete diagnostic coronary angiography (CAG) vs complete CAG followed by culprit lesion PCI on reperfusion times among patients w

Timing of multivessel revascularization in stable patients with STEMI: a systematic review and network meta-analysis.

INTRODUCTION AND OBJECTIVES: Multivessel percutaneous coronary intervention (MV-PCI) is recommended in patients with ST-segment elevation myocardial infarction (STEMI) and multivessel coronary artery disease (CAD) without cardiogenic shock. The present network meta-analysis investigated the optimal timing of MV-PCI in this context. METHODS: We pooled the aggregated data from randomized trials investigating stable STEMI patients with multivessel CAD treated with a strategy of either MV-PCI or culprit vessel-only PCI. The primary outcome was all-cause death. The main secondary outcomes were car

A pilot study of perfusion balloon predilatation in conjunction with intracoronary nicorandil administration for acute coronary syndrome.

Slow-flow or no-reflow phenomenon is a common procedural complication during percutaneous coronary intervention (PCI). Given the presence of fragile plaque or thrombotic materials, we hypothesized that long-time predilatation using a perfusion balloon in conjunction with intracoronary nicorandil administration reduces the risk of slow-flow or no-reflow in patients presenting with acute coronary syndrome (ACS). Subjects were patients presenting with ACS who underwent PCI between April 2020 and April 2022. We retrospectively investigated the incidence of slow-flow or no-reflow during the procedu

Very short-term tissue coverage of the CD34 antibody-covered sirolimus-eluting stent: an optical coherence tomography study.

It is well known that patients with acute myocardial infarction (AMI) have delayed vessel healing despite accelerated endothelial progenitor cells (EPC) mobilization. The COMBO stent is a unique biodegradable-polymer sirolimus-eluting stent with an anti-CD34 antibody coating which captures EPC and potentially promotes vessel healing. However, there are limited data about strut tissue coverage at the very short-term period after COMBO stent implantation. This was a prospective study to investigate strut tissue coverage within 1 month after COMBO stent implantation using optical coherence tomogr

Real-world evidence of switching P2Y12 receptor-inhibiting therapies to prasugrel after PCI in patients with ACS: results from EFF-K registry.

BACKGROUND: Potent P2Y12 inhibitors are recommended for up to 12 months after percutaneous coronary intervention (PCI) in patients diagnosed with acute coronary syndrome (ACS). However, the prescription pattern is diverse in real world practice, which includes various switching between antiplatelet regimens. In this study, we analyzed the prescription patterns of prasugrel, and assessed the safety and effectiveness of P2Y12 inhibitors switching patterns in a real world registry of patients subjected to PCI after ACS. METHODS: The EFF-K study included 3077 ACS patients receiving prasugrel-base

Safety and efficacy of a novel calcified plaque modification technique - Shockwave Intravascular Lithotripsy - in patients with coronary artery disease: Mid-term outcomes.

BACKGROUND: Coronary interventions in calcified lesions are associated with a higher rate of adverse clinical events. Initial aggressive plaque modification along with post-implantation optimization is pivotal for achieving a favorable outcome of percutaneous coronary intervention (PCI). Recently, the Shockwave C2 Intravascular Lithotripsy (S-IVL) System, a novel acoustic wave-based device designed to modify calcified plaque, has been introduced into clinical practice. AIMS: We evaluated the mid-term safety and efficiency of S-IVL in a cohort of 131 consecutive patients with severely calcifie

Nine-month angiographic and 2-year clinical outcomes of the RECOVERY trial: A randomized study of the biodegradable polymer sirolimus-eluting COMBO dual-therapy stent versus a polymer-free sirolimus-eluting stent in Chinese patients.

OBJECTIVES: We evaluated the safety and efficacy of the novel dual-therapy sirolimus-eluting and endothelial progenitor cell (EPC) capture COMBO stent. BACKGROUND: (Very) late stent thrombosis (ST) and neo-atherosclerosis limit the performance of drug-eluting stents. The capture of EPCs accelerates stent re-endothelialization, thereby potentially decreasing the risk of restenosis and ST. METHODS: In total, 440 patients with de novo lesions in native coronary arteries were randomized (1:1) to either receive the COMBO stent (n = 220) or Nano polymer-free sirolimus-eluting stent (n = 220).
