Perioperative complications associated with fluoroscopy C1 lateral mass screw fixation (Goel technique) versus computed tomography-guided navigation technique: A review of 202 cases from the German Spine Registry (DWG-Register).

BACKGROUND: Overview of the literature - Fractures of the C1 constitute 3%-13% of all cervical spine injuries in adults. Most isolated C1 fractures are stable and can be treated nonoperatively with external immobilization. Traditional surgical options for C1 fracture treatment are occiput-to-C2 fusion or C1 with lateral mass screws (LMSs). Purpose - The aim is to assess the management and perioperative complications of C1 fractures undergoing LMS fusion between fluoroscopy and computed tomography (CT)-guided navigation. METHODS: This was a retrospective multicenter study of data from the DWG-

[Unsupervised data mining for prescription and medication regularity for treatment of cervical vertigo].

Vertebral artery type cervical spondylosis is a common type of cervical spondylosis, also known as cervical vertigo. In recent years, with the increase of people's life pace and the number of office staff, the incidence rate has been rising year by year, with a trend of younger age. Because traditional Chinese medicine has unique advantages in treating cervical vertigo, it is necessary to study the regularity of the Chinese medicine for cervical vertigo. But at present, the research is mainly based on the statistics for frequency and proportion of herbs. In addition, in the process of diagnosi

Prevalence of ponticuli posticus among patients referred for dental examinations by cone-beam CT.

BACKGROUND CONTEXT: Ponticulus posticus (PP) is the bony bridge that can completely or partially embrace the vertebral artery and the suboccipital nerve root at the atlas posterior arch. The PP can be a possible cause of vertigo, vertebrobasilar insufficiency, neck pain, shoulder pain, and cervicogenic headache. Moreover, the vertebral artery injury may happen during atlas lateral mass screw insertion in the presence of PP. PURPOSE: The purpose of this study was to determine the prevalence of PP in a population of patients undergoing dental cone-beam computed tomography (CBCT) and the associa

Blunt cerebrovascular injuries in association with craniocervical distraction injuries: a retrospective review of consecutive cases.

BACKGROUND CONTEXT: Blunt cerebrovascular injuries (BCVIs) have the potential to cause brain, cerebellar, and/or spinal cord ischemia. Certain subtypes of spine fractures, such as vertebral subluxation, fractures through the foramen transversarium, and C1-C3 fractures have been linked to a higher incidence of BCVI. On the other hand, BCVI in association with craniocervical distraction injuries (CCDs) have been only anecdotally reported. PURPOSE: We hypothesized that because CCD is also caused by a high-energy hyperflexion/hyperextension distraction mechanism, it could also be associated with

Lateral mass screw fixation in the cervical spine: a systematic literature review.

BACKGROUND: Lateral mass screw fixation with plates or rods has become the standard method of posterior cervical spine fixation and stabilization for a variety of surgical indications. Despite ubiquitous usage, the safety and efficacy of this technique have not yet been established sufficiently to permit "on-label" U.S. Food and Drug Administration approval for lateral mass screw fixation systems. The purpose of this study was to describe the safety profile and effectiveness of such systems when used in stabilizing the posterior cervical spine. METHODS: A systematic search was conducted in ME

Biomechanical investigation of a novel integrated device for intra-articular stabilization of the C1-C2 (atlantoaxial) joint.

BACKGROUND CONTEXT: The anatomy of the atlantoaxial joint makes stabilization at this level challenging. Current techniques that use transarticular screw fixation (Magerl) or segmental screw fixation (Harms) give dramatically improved stability but risk damage to the vertebral artery. A novel integrated device was designed and developed to obtain intra-articular stabilization via primary interference fixation within the C1-C2 lateral mass articulation. PURPOSE: To assess the atlantoaxial stability achieved with a novel integrated device when compared with the intact, destabilized, and stabili

Freehand determination of the trajectory angle for cervical lateral mass screws: how accurate is it?

Different methods of lateral mass screw placement in the cervical spine have been described with separate trajectories for each technique in the sagittal and parasagittal planes. In the latter, plane 30° has been recommended in the modified Magerl's technique as the optimum angle to avoid injury to the vertebral artery and nerve root. The estimation of this angle remains arbitrary and very much operator dependant. The aim of this study was to assess how accurately the lateral trajectory angle of 30° is achieved by visual estimation amongst experienced surgeons in a tertiary spinal unit and to

Cervical pedicle screws: conventional versus computer-assisted placement of cannulated screws.

STUDY DESIGN: Prospective clinical study with postoperative radiologic control of pedicle screw placement in the cervical spine. OBJECTIVES: To evaluate whether cervical pedicle screws can be placed safely in a conventional technique when using cannulated screws and separate stab incisions. Also, to evaluate if accuracy and safety of pedicle screw placement can be improved using a computer-assisted surgery (CAS) system (VectorVision; BrainLAB AG, Heimstetten, Germany). SUMMARY OF BACKGROUND DATA: Pedicle screws are rarely used in the cervical spine compared to the use in lumbar and thoracic

Effects of Photobiomodulation on Changes in Cognitive Function and Regional Cerebral Blood Flow in Patients with Mild Cognitive Impairment: A Pilot Uncontrolled Trial.

BACKGROUND: Photobiomodulation (PBM) affects local blood flow regulation through nitric oxide generation, and various studies have reported on its effect on improving cognitive function in neurodegenerative diseases. However, the effect of PBM in the areas of the vertebral arteries (VA) and internal carotid arteries (ICA), which are the major blood-supplying arteries to the brain, has not been previously investigated. OBJECTIVE: We aimed to determine whether irradiating PBM in the areas of the VA and ICA, which are the major blood-supplying arteries to the brain, improved regional cerebral bl

Traumatic basal subarachnoid hemorrhage. Report of six cases and review of the literature.

Basal subarachnoid hemorrhage due to rupture of normal extra- and intracranial arteries, in association with minor trauma to the face and neck and alcohol intoxication, has been well described but often goes unrecognized at autopsy. This results in the incorrect classification of the manner of death as natural. Six cases of subarachnoid hemorrhage due to mild-to-moderate blows to the head or neck are presented. All were men in the age range 28-61 years (mean, 38.8 years). Four had blood alcohol levels of 0.09-0.28 g % at autopsy, and five of six were comatose or dead within 30 min of the initi

The Role of Cerl2 in the Establishment of Left-Right Asymmetries during Axis Formation and Heart Development.

The formation of the asymmetric left-right (LR) body axis is one of the fundamental aspects of vertebrate embryonic development, and one still raising passionate discussions among scientists. Although the conserved role of nodal is unquestionable in this process, several of the details around this signaling cascade are still unanswered. To further understand this mechanism, we have been studying Cerberus-like 2 (Cerl2), an inhibitor of Nodal, and its role in the generation of asymmetries in the early vertebrate embryo. The absence of Cerl2 results in a wide spectrum of malformations commonly k

"Tardus and parvus" phenomenon in upper limb arteries for identifying subclavian arterial stenosis.

BACKGROUND: The detection of subclavian artery (SCA) stenosis or occlusion is important, and an easy and accurate noninvasive screening method is needed. METHODS: Pulsed Doppler spectra of bilateral SCA, vertebral arteries, axillary arteries, and brachial arteries were recorded and analyzed both in patients with SCA stenosis and healthy subjects. RESULTS: Thirty-five patients showed SCA stenosis less than 80% (mean: 72 +/- 14%; Vmax: 450.1 +/- 101.5 cm/sec) and seven patients demonstrated one-side SCA occlusion. The Vmax, systolic acceleration (SA) and resistance index (RI) were much lower (

[Quality criteria in using percutaneous transluminal angioplasty of supra-aortic arteries].

OBJECTIVE AND METHOD: On the basis of 252 personally performed angioplasties and almost 2000 published results of percutaneous transluminal angioplasties (PTA) in supra-aortic arteries quality criteria for PTA in supra-aortic arteries are to be elaborated with special reference to the carotid artery. RESULTS: To keep the complication rates low the indication must be strictly observed and particular attention paid to the morphological criteria. Experience has shown that PTA should be applied in brachiocephalic arteries only in the presence of symptomatic, high-grade, circular or short stenosis
