Reviewing smokeless tobacco epidemiology, carcinogenesis, and cessation strategy for otolaryngologists.

OBJECTIVES/OBJECTIVES: We aimed to provide an otolaryngologist-targeted summary regarding the epidemiology, carcinogenesis, and cessation st … on strategies. CONCLUSIONS: Smokeless tobacco use continues to be prevalent among patients seen by otolaryngologists, particularly of various Indian and Southeast Asian descent. The data presented in this article will aid in the identification of at risk patients. The provided recommended cessation strategies will tool otolaryngologists for patient counseling and …

Survey of current perspectives on laryngopharyngeal reflux among Canadian otolaryngologists.

PURPOSE: Laryngopharyngeal reflux (LPR) -- gastroesophageal reflux above the upper esophageal sphincter -- is a common problem encountered by otolaryngologists. Despite consensus guidelines, the presentation, diagnosis, and treatment remain controversial. We surveyed Canadian otolaryngologists to assess current perspectives. METHODS: Web-based questionnaires were e-mailed to 135 otolaryngologists. Respondents were categorized by subspecialty as head and neck (H&N) or non-H&N (rhinology, otology, laryngology, facial plastics, general and pediatric otolaryngology). Data were analyzed to determi…

Surgical Considerations in Endoscopic Pituitary Approaches for the Otolaryngologist.

Advances in endoscopic surgical technique have ushered in a new era of pituitary surgery with improved rates of resection and minimized operative morbidity and burden. Anatomically, endoscopic transnasal transsphenoidal pituitary surgery is split into nasal, sphenoidal, and sellar stages, each with unique considerations. Recent developments in knowledge and technology seek to build on the success of the endoscope in pituitary surgery, while expanding its capabilities.

The Role of the Otolaryngologist in the Evaluation and Management of "Sinus Headache".

Patients will continue to present to the otolaryngologist's office with "sinus headaches" as their primary complaint. Otolaryngologists should take particular care in establishing a precise diagnosis. A thorough clinical history, comprehensive head and neck examination, well-performed nasal endoscopy, and imaging as necessary are essential components for effective diagnosis and treatment plan implementation. It is fundamental to acknowledge the criteria for diagnosing the various headache disorders that may disguise themselves as sinonasal complaints. Moreover, this patient population accurate…

Otolaryngologists are uniquely poised to facilitate early diagnosis and initiation of therapy for these children, potentially reducing long-term sequelae. LEVEL OF EVIDENCE: 4. Vagal nerve stimulator implantation: the otolaryngologist's perspective.

Vagal nerve stimulation (VNS) is a recognised and effective measure in treating refractory epilepsy and depression. VNS implantation is a widely accepted surgical procedure, most commonly performed by neurosurgeons. Otolaryngologists, in particular those with an interest in head and neck surgery, are very familiar with the surgical anatomy and dissection of the vagus nerve in the carotid sheath. We present a retrospective analysis of the first 12 patients to be implanted in our department. Our series suggests that otolaryngologists can safely and effectively perform VNS implantation. Otolaryng…

Medical Malpractice Trends in Thyroidectomies among General Surgeons and Otolaryngologists.

OBJECTIVES: This study aims to examine litigation trends with thyroidectomies in the United States from 1984 to 2018. METHODS: We used the Westlaw legal database to collect data on the defendant, plaintiff, case demographics, alleged reasons for malpractice, additional complications, and case outcomes. RESULTS: The most common reason for litigation was vocal cord paralysis (51%, n = 28), with the majority ruling in favor of the defendant (64%, P = .042). Of those, 43% of cases (n = 12) were due to unilateral recurrent laryngeal nerve (RLN) injury, and 39% (n = 11) were due to bilateral RLN i

Contemporary decongestant practices of Canadian otolaryngologists for endoscopic sinus surgery.

BACKGROUND: Cocaine has traditionally been the topical decongestant most frequently used for visualization of the surgical field in Endoscopic Sinus Surgery (ESS). Alternatives include xylometazoline, oxymetazoline, and epinephrine. The understanding of the safety profile of each agent is changing, as are the practices of Otolaryngologists-Head & Neck Surgeons. The objective of this study is to determine decongestant use practices in ESS across Canada, which has not previously been studied. METHODS: A cross-sectional survey design using a 24-item electronic questionnaire was distributed to ac…

OBJECTIVES/HYPOTHESIS: To examine expert testimony offered by otolaryngologists in defense of the tobacco industry and to assess whether opinions rendered were congruent with evidence in the scientific literature. METHODS: Data sources include publically available expert witness depositions and trial testimony of board-certified otolaryngologists employed by the tobacco industry in defense of lawsuits brought by smokers suffering from head and neck cancer. The challenge of diagnosing intracranial pressure elevations as an otolaryngologist.

PURPOSE: This article addresses the complex clinical scenario where patients present to otolaryngologists with symptoms typically ascribed to chronic rhinosinusitis (CRS) or migraines which may in fact stem from elevations in intracranial pressure. We aim to clarify the diagnostic challenges and emphasize the importance of considering elevated intracranial pressure (eICP) as its symptoms overlap with both CRS and migraines. METHODS: This narrative review synthesizes clinical experiences and literature to discuss the differential diagnoses involving CRS, facial pain/pressure, migraines, and eI…

Eosinophilic Esophagitis: What the Otolaryngologist Needs to Know.

Eosinophilic esophagitis is a male-predominant disease with presentations ranging from nonspecific feeding issues to dysphagia and food impaction. The currently proposed pathophysiology is a combination of genetics, allergens, and epithelial barrier impairment. Diagnosis is reliant on history, endoscopic examination, and biopsy. Recent guidelines recognize the role of concurrent gastroesophageal reflux disease. Treatment is based on 3 paradigms: diet, drugs, and dilation. Drug therapy has historically focused on topical corticosteroids; as of 2022, dupilumab was approved for targeted biologic

Otolaryngologist's role in the diagnosis of amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is a progressive and late-onset fatal neurodegenerative disease characterised by selective death of moto … itted to an electromyography that confirmed the ALS diagnosis. This case highlights the key role of otolaryngologists in the diagnosis of ALS, in a way that many patients with a bulbar ALS form are initially studied by an otolaryngologist.

Lumbar drains: Practical understanding and application for the otolaryngologist.

INTRODUCTION: Lumbar drains are frequently used in patients with otolaryngologic concerns. These can be used therapeutically or prophylactically with the primary purpose being to modulate CSF pressure. Within otolaryngology, lumbar drains are most frequently used for cerebrospinal fluid leaks - either due to cerebrospinal fluid fistulas or in skull base surgery as these allow for potential healing of the defect. While not typically placed by otolaryngologists, a basic understanding of lumbar drains is beneficial in the context of patient management. MANAGEMENT: A lumbar drain is inserted into…
