The motor unit number index (MUNIX) profile of patients with adult spinal muscular atrophy.

OBJECTIVE: Objective of this study is the comprehensive characterisation of motor unit (MU) loss in type III and IV Spinal Muscular Atrophy (SMA) using motor unit number index (MUNIX), and evaluation of compensatory mechanisms based on MU size indices (MUSIX). METHODS: Nineteen type III and IV SMA patients and 16 gender- and age-matched healthy controls were recruited. Neuromuscular performance was evaluated by muscle strength testing and functional scales. Compound motor action potential (CMAP), MUNIX and MUSIX were studied in the abductor pollicis brevis (APB), abductor digiti minimi (ADM),

Standard and modified statistical MUNE evaluations in spinal-bulbar muscular atrophy.

Motor unit number estimation (MUNE), a technique used in amyotrophic lateral sclerosis (ALS) clinical trials to quantitatively assess motor neuron loss, should also be valuable in assessing progression in spinal bulbar muscular atrophy (SBMA), an x-linked neuronopathy. In ALS, instability of single motor units (SMUP) prompted Shefner et al.6(6) to modify the statistical MUNE method to exclude SMUPs < or = 40 microV. It is unknown if there is similar SMUP instability in the more chronic degenerative disease of SBMA. In this study the standard parameter of excluding SMUP < 10 microV was compared

Modified Campbell diagram to assess respiratory muscle action in speech.

Ten normal and four moderate to severe stutterers participated in the study. Pleural (Ppl) and abdominal (Pab) pressure was studied using oesophageal and gastric balloon catheter systems and VL (VL) was studied using magnetometry. The classical Campbell diagram was modified by plotting Pab versus VL. In a preliminary study we determined whether a surrogate curve could be substituted for the true curve in the Campbell diagram. We obtained true relaxation curves in six subjects. We obtained surrogate chest wall relaxation curves by joining the Pab value at functional residual capacity (FRC) to a

A procedure for the early detection of noise-susceptible individuals.

Currently used threshold audiograms fail to detect the early stages of noise-induced hearing impairment. Threshold shifts may reach 10 to 15 dB before the impairment is detected. Loudness difference limen and critical band phenomena should provide an earlier and more sensitive test for such hearing impairment.

Test-retest measurement properties of the MScanFit method for motor unit number estimation in individuals with Amyotrophic Lateral Sclerosis.

OBJECTIVE: MScanFit is one of the Motor Unit Number Estimation (MUNE) methods considered as a potential biomarker for Amyotrophic Lateral Sclerosis (ALS) diagnosis and clinical trials. We investigated the test-retest measurement properties of MScanFit across multiple muscles in individuals with ALS. METHODS: We recorded MUNE from Abductor Pollicis Brevis (APB), Abductor Digiti Minimi (ADM), and Tibialis Anterior (TA) muscles in seventeen adults with ALS separated by 1-2 weeks. Intraclass correlation and concordance correlation coefficients provided reliability estimates. Overall agreement was

Mechanisms of compensatory plasticity for respiratory motor neuron death.

Respiratory motor neuron death arises from multiple neurodegenerative and traumatic neuromuscular disorders. Despite motor neuron death, compensatory mechanisms minimize its functional impact by harnessing intrinsic mechanisms of compensatory respiratory plasticity. However, the capacity for compensation eventually reaches limits and pathology ensues. Initially, challenges to the system such as increased metabolic demand reveal sub-clinical pathology. With greater motor neuron loss, the eventual result is de-compensation, ventilatory failure, ventilator dependence and then death. In this brief

Facing mixed emotions: Analytic and holistic perception of facial emotion expressions engages separate brain networks.

The ability to decode facial emotions is of primary importance for human social interactions; yet, it is still debated how we analyze faces to determine their expression. Here we compared the processing of emotional face expressions through holistic integration and/or local analysis of visual features, and determined which brain systems mediate these distinct processes. Behavioral, physiological, and brain responses to happy and angry faces were assessed by presenting congruent global configurations of expressions (e.g., happy top+happy bottom), incongruent composite configurations (e.g., angr

Esophageal mechanosensitive mechanisms are impaired in neonates with hypoxic-ischemic encephalopathy.

OBJECTIVES: To test the hypothesis that esophageal mechanodistention in infants with hypoxic-ischemic encephalopathy (HIE) results in altered upper esophageal sphincter (UES), esophageal body, and lower esophageal sphincter (LES) responses, compared with controls. As a secondary aim, we tested the hypothesis that infants with HIE receiving therapeutic hypothermia had different aerodigestive reflex characteristics than infants with HIE who received traditional neonatal care. STUDY DESIGN: Provocative esophageal manometry was performed in 34 neonates (27 with HIE and 7 controls). Mechanodistent

Association between muscle activation and metabolic cost of walking in young and old adults.

BACKGROUND: The net metabolic cost of walking (C(w)) as well as the level of neural activation of agonist and antagonist leg muscles are higher in healthy old compared with young adults. This study examined the association between C(w) and agonist muscle activity and antagonist coactivity in young and old adults. METHODS: Young and old adults walked at 0.98 m/s on a treadmill set at 6% decline, level, and 6% incline, while C(w) and neural activation of leg muscles were measured. RESULTS: C(w) was 7.0% (incline), 19.2% (level), and 47.3% (decline) higher in old adults (overall 18.3%). Old (67

Experimental muscle pain changes feedforward postural responses of the trunk muscles.

Many studies have identified changes in trunk muscle recruitment in clinical low back pain (LBP). However, due to the heterogeneity of the LBP population these changes have been variable and it has been impossible to identify a cause-effect relationship. Several studies have identified a consistent change in the feedforward postural response of transversus abdominis (TrA), the deepest abdominal muscle, in association with arm movements in chronic LBP. This study aimed to determine whether the feedforward recruitment of the trunk muscles in a postural task could be altered by acute experimental

Selective stimulation of cat sciatic nerve using an array of varying-length microelectrodes.

Restoration of motor function to individuals who have had spinal cord injuries or stroke has been hampered by the lack of an interface to the peripheral nervous system. A suitable interface should provide selective stimulation of a large number of individual muscle groups with graded recruitment of force. We have developed a new neural interface, the Utah Slanted Electrode Array (USEA), that was designed to be implanted into peripheral nerves. Its goal is to provide such an interface that could be useful in rehabilitation as well as neuroscience applications. In this study, the stimulation cap

Association between motor unit remodeling and interindividual variability in muscle strength of older adults.

Marked interindividual variability in muscle strength exists among older adults, which cannot be fully explained by differences in muscle mass alone. Although motor unit remodeling is a characteristic feature of neuromuscular aging, whether variability in motor unit remodeling contributes to interindividual differences in muscle strength remains unclear. The purpose of this study was to examine the association between motor unit remodeling and muscle strength in older adults. Forty-nine older adults (25 men and 24 women) participated in this study. Motor unit parameters of the abductor pollici

Inhibition of muscle sympathetic action potential firing and recruitment patterns following cannabis inhalation in humans.

Cannabis is among the most widely used recreational substances, but the physiological consequences of acute and chronic use remain poorly studied. We recently demonstrated that acute cannabis use reduces muscle sympathetic nerve activity (MSNA). However, MSNA bursts comprise action potentials (APs) of varying size, which exhibit distinct firing and recruitment patterns that represent the communication between the sympathetic nervous system and the circulation. Applying a continuous wavelet transform to the microneurographic MSNA signal, we investigated the hypothesis that sympathetic AP firing

Impact of β-range-induced oscillatory activity on human input-output relationship of the corticospinal pathway.

Objective: The aim of the study was to show that short-lasting (90 s) transcranial alternating current stimulation (tACS) at 20 Hz delivered over the left primary motor cortex (M1) is able to change the shape of recruitment curve of the corticospinal pathway. Methods: The corticospinal pathway was studied during tACS by means of the relationship between the intensity of transcranial magnetic stimulation (TMS) delivered over the left M1 and corresponding motor evoked potentials (MEPs) recorded from the right first dorsal interosseus muscle (FDI), in nine healthy subjects. In order to extract cha

An examination of motor unit firing rates during steady torque of maximal efforts with either an explosive or slower rate of torque development.

NEW FINDINGS: What is the central question of this study? The aim was to explore agonist and antagonist motor unit firing rates during maximal efforts performed with either an explosive or a slower rate of torque development. What is the main finding and its importance? The antagonist muscle presented a motor unit firing rate relationship similar to the agonist muscle. Additionally, the motor units of both muscles exhibited higher firing rates during explosive maximal contractions than during maximal contractions performed at a slower rate of torque development. These results could prove usefu

Is the 'reverse onion skin' phenomenon more prevalent than we thought during intramuscular myoelectric recordings from low to maximal force outputs?

There are isolated instances in the literature that suggest the 'onion skin' phenomenon is not always present. That is, newly recruited high threshold motor units (MU) have higher discharge rates than previously recruited low threshold MUs. Therefore, the purpose of this paper was to investigate the presence of the 'onion skin' phenomenon in a large sample of intramuscular myoelectric recordings from low to maximal force outputs. Forty-eight participants performed rapid isometric dorsiflexion contractions at 20, 40, 60, 80 and 100 % MVC while intramuscular electrical activity was recorded. A b

[Electromyographic assessment of the temporalis muscle prior to a lengthening myoplasty in patients with Moebius syndrome].

BACKGROUND/OBJECTIVES: Temporalis muscle lengthening myoplasty improves tightening of the lips and rehabilitates smile for patients with congenital facial palsies. Because Moebius syndrome is heterogeneous, a careful evaluation is mandatory before deciding to perform myoplasty. This series shows the role of electromyography for investigating temporalis muscle and trigeminal nerve motor functions. METHODS: We conducted a retrospective study of 18 patients with no upward movements of the labial commissure and absent or unsightly smile. Electromyography was used to study the temporalis muscle bi

How does pain affect jaw muscle activity? The Integrated Pain Adaptation Model.

Pain and limitation of movement are two cardinal symptoms of temporomandibular disorders but it is unclear how one influences the other. The relationship between pain and movement is clinically significant but controversial with two major theories having been proposed: the Vicious Cycle Theory and the Pain Adaptation Model. The Vicious Cycle Theory proposes a vicious cycle between pain and muscle activity. This theory has little scientific basis but underpins many management strategies. The Pain Adaptation Model is more evidence-based and proposes that pain causes changes in muscle activity to
