A next generation of the schema therapy model of personality pathology: A cross-cultural and international study protocol.

BACKGROUND: Personality disorders are highly prevalent worldwide imposing substantial personal and social challenges. Schema Therapy is an effective psychotherapeutic approach for personality pathology and other complex characterological problems. New scientific insights prompted a re-evaluation of its theoretical underpinnings leading to a reformulated Schema Therapy theory. Furthermore, the assumed cross-cultural universality of the Schema Therapy model has not been tested. AIMS: This project has two primary aims: (1) To develop revised instruments based on the reformulated theory that are

[Behavior and cognitive therapies focused on The Young Personality Disorders Schemas: a pilot study of 14 cases].

INTRODUCTION: Personality disorders are very frequent in the general population, and especially in psychiatric patients. Various types of psychotherapy are proposed to treat these disorders, directly or not, but some specific cognitive-behavioural therapies (CBT) have been developed for all personality disorders or some of them such as borderline personality disorder. Among the most known models, one can quote Beck, Young and Linehan's approaches. Young therapies are based on the concept of "early maladaptive schemas", and associate cognitive-behavioural techniques and a very specific work on

Integrating personalized experience sampling methodology in schema therapy for late-life depression and comorbid avoidant personality disorder.

OBJECTIVES: Case conceptualization is essential to provide a holistic and person-tailored working theory for psychotherapy. The objective of this study was to illustrate the way how personalized Experience Sampling Method (ESM) can support this process by systematically monitoring daily-life experiences and providing structured feedback during schema therapy in later life. METHODS: A mixed-method case illustration of a patient in her seventies diagnosed with a recurrent depressive disorder and avoidant personality disorder, whose schema therapy had stalled. During eight weeks of schema therap

Embodied meaning in therapeutic discourse during cognitive-behavioral therapy for generalized anxiety disorder: an image schema analysis.

BACKGROUND: Image schemas are simple mental structures that are formed from the body's interaction with the physical environment. One of the leading theories in this area is the one proposed by Mark Johnson. The aim of this study was to identify, classify, and quantify image schemas employed by psychotherapists during cognitive-behavioral therapy sessions with patients diagnosed with Generalized Anxiety Disorder and to explore the conceptual organization of therapeutic discourse through Johnson's image schema framework. METHODS: This study employed a qualitative cognitive discourse analysis a

One-Year Follow-Up: Schema Therapy for Patients With Borderline Personality Disorder and Comorbid Alcohol Use Disorder.

This article describes the 1-year follow-up of a study into the effectiveness of Schema Therapy (ST) for borderline personality disorder (BPD) and comorbid alcohol use disorder (AUD). In the original study, 20 of these patients participated in a multiple baseline case series design study. The results of the original study were promising (a significant decrease of BPD and AUD symptoms). The present study is aimed at examining the longer term benefits of ST for BPD and comorbid AUD. One year after the cessation of the investigational therapy, 17 of the original participants agreed to participate

A Schema Therapy approach to complex dissociative disorder in a cross-cultural setting: a single case study.

Complex dissociative disorders (CDD) include dissociative identity disorder (DID) and the most common other specified dissociative disorder (OSDD, type 1). While consensus-based treatments for CDD are lacking in several international guidelines, patients suffering from CDD show high levels of impairment, treatment utilization and costs. Migrants and refugees often present risk factors for trauma-related and dissociative disorders and need effective and culturally adapted treatments. Schema Therapy (ST) is an integrative psychotherapy that has been recently proposed as a treatment for CDD. This

Do schema modes predict outcome of cognitive behavioural therapy in patients with obsessive-compulsive disorder and anxiety disorders?

OBJECTIVES: Cognitive behavioural therapy (CBT) is the psychological treatment of choice for patients with obsessive-compulsive disorder (OCD) or an anxiety disorder. However, a substantial group (30-40%) does not profit sufficiently. Predictors of outcome inform us about how to adapt interventions for this group. Specifically, schema modes might affect CBT outcome; yet, research into the relation between schema modes and CBT outcome for anxiety disorders and OCD is very limited. In this study, it was investigated whether adaptive and maladaptive schema modes predict CBT outcome controlling fo

A Randomized Controlled Trial Comparing Two Treatments for Vulnerable and Grandiose Narcissism: Unified Protocol vs. Schema-Focused Therapy.

In this study, we compared the efficacy of schema-focused therapy (SFT) versus the unified protocol (UP) in reducing symptom severity of narcissistic personality disorder (NPD) and its common features. We also compared their ability to reduce grandiose and vulnerable symptoms of NPD. One hundred and fourteen individuals were randomly assigned to these groups. Severity of NPD was significantly reduced to an equivalent degree in both treatment groups across a 12-month follow-up period. SFT group showed significantly greater reduction in measures of narcissistic grandiosity, while UP group showed

The interplay between experiential states in first generation schema therapy of Cluster C personality problems: A network approach.

We tested a theory-derived network of within-person causal relations between experiential states in schema therapy of Cluster C personality problems. The sample consisted of 35 patients with panic disorder and/or agoraphobia and DSM-IV Cluster C personality traits or disorders who participated in an inpatient programme. In a 6-week schema therapy phase, the patients had nine or 10 individual sessions and completed measures of experiential states (anger, sadness, depression, anxiety, optimism, and schema belief) before and after each session. The impact of these six states on each other and on

Differential effectiveness of dialectical behavioural therapy and schema therapy in patients with borderline personality disorder: a secondary analysis of a randomised clinical trial.

BACKGROUND: Borderline personality disorder (BPD) can be treated successfully with specific psychological treatments, but there is no clear evidence of superiority of one specific treatment at the group level. Due to high heterogeneity in BPD, individual patients might benefit differently from specific treatments. METHODS: Based on a randomised trial comparing 18 months of dialectical behaviour therapy (DBT) and schema therapy (ST) for BPD, differential effectiveness was examined using causal forest analyses. Baseline variables on BPD criteria, general psychopathology, traumatic childhood exp

Feasibility of Schema Therapy for Recurrent Depression in a Disaster Relief Worker with Prior Post-Traumatic Stress Disorder Treatment Using Prolonged Exposure Therapy.

This report presents the follow-up treatment course of a previously published case that demonstrated the effectiveness of prolonged exposure (PE) therapy for a disaster relief worker. The patient, a municipal employee in Fukushima Prefecture, developed post-traumatic stress disorder (PTSD) and mood disorders after the 2011 Great East Japan Earthquake and subsequent disasters. This follow-up focuses on the period from 2021 to early 2024, during which the patient experienced symptom recurrence after his father's death. This event revealed psychological patterns similar to his disaster-related re

Group schema therapy versus group cognitive behavioral therapy for patients with social anxiety disorder and comorbid avoidant personality disorder: A randomized controlled trial.

BACKGROUND: Patients with social anxiety (SAD) and comorbid avoidant personality disorder (AVPD) are severely impaired. Group cognitive behavioral therapy (GCBT) is considered an effective treatment for SAD. More knowledge on treatment of SAD with comorbid AVPD is needed. Schema therapy, developed for personality and chronic mental disorders, may be a promising treatment. METHODS: We conducted a randomized controlled trial in an outpatient population (n = 154) with both SAD and AVPD. Group Schema Therapy (GST) and GCBT were compared on SAD symptoms (Liebowitz Social Anxiety Scale) and manifes

Mechanisms of Change in Day Treatment Group Schema Therapy for Severe Personality Disorders: A Multiple Baseline Single-Case Study.

Although schema therapy (ST) is an effective intervention for individuals with personality disorders (PDs), its mechanisms of change are not yet well delineated. Understanding these mechanisms is essential for determining the key processes that optimize treatment efficacy. This study investigated mechanisms of change in day treatment group ST (DT-GST) for patients with severe PDs. PD severity was the primary outcome in a multiple-baseline single-case design, assessed before and after DT-GST, alongside idiosyncratic negative core beliefs measured weekly over 30 weeks. Secondary outcomes were ea

"When Standard Treatments Are Not Enough" - Showcasing Schema Therapy for the Treatment of Anorexia Nervosa and Borderline Personality Disorder.

Whilst standard cognitive and behavioural treatments lead to clinical improvement for approximately half of the people with eating disorders, preliminary evidence indicates that complex comorbidity, including personality difficulties, insecure attachment patterns and Posttraumatic stress disorder, may hinder treatment engagement and outcomes. Such comorbidities tend to be associated with increased emotional dysregulation, rigidity of beliefs, and dissociation. Schema Therapy is a transdiagnostic approach that interweaves the treatment of current eating disorder symptomatology and comorbid pres

Group schema therapy versus group cognitive behavioral therapy for social anxiety disorder with comorbid avoidant personality disorder: study protocol for a randomized controlled trial.

BACKGROUND: Social anxiety disorder (SAD) with comorbid avoidant personality disorder (APD) has a high prevalence and is associated with serious psychosocial problems and high societal costs. When patients suffer from both SAD and APD, the Dutch multidisciplinary guidelines for personality disorders advise offering prolonged cognitive behavioral therapy (CBT). Recently there is increasing evidence for the effectiveness of schema therapy (ST) for personality disorders such as borderline personality disorder and cluster C personality disorders. Since ST addresses underlying personality character
