Occlusion of an Intrahepatic Portosystemic Shunt With an Amplatzer Septal Occluder Device in a Dog.

This paper describes the use of an Amplatzer occluder device to attenuate an intrahepatic portosystemic shunt in a dog. To the authors' knowledge, this is the first description of the use of an Amplatzer occluder device for a large intrahepatic shunt. Shunt attenuation in this case was successful with no major complications. The use of such device could be made routine for bigger shunt vessels of suitable configuration instead of accumulating coils or having to proceed with open laparotomy surgery.

Infective endocarditis of transcatheter atrial septal occluder devices: A case report.

UNLABELLED: Infective endocarditis (IE) is caused by bacterial vegetation in valves, but it can also occur in implanted mechanical devices. We report a rare case of IE occurring at the site of percutaneous atrial septal closure devices in a patient in her 50s that had been placed for residual defects on a closure patch in her childhood for an atrial septal defect (ASD). She also had a medical history of distal pancreatectomy for insulinoma in her 40s and had insulin-dependent diabetes mellitus, which means she had been immunocompromised. She visited our hospital with complaints of fever and lum

Choice of device size and results of transcatheter closure of atrial septal defect using the amplatzer septal occluder.

The impact of device size choice on closure results was analyzed in 138 (101 females, 37 males; age 0.5-84.0 years) consecutive patients who underwent transcatheter closure of the secundum atrial septal defect (ASD) using the Amplatzer septal occluder (ASO). The balloon stretched diameter (SD) of ASD was 19.5 +/- 7.2 mm in 123 patients with single defects, and 20.4 +/- 6.6 mm for the largest defects in 15 patients with multiple ASDs. The difference (delta) between ASO size chosen for closure and the stretched diameter of the defect was calculated and divided into groups: A (delta < -2 mm); B (

Feasibility and safety of a new generation of gore septal occluder device in children.

AIM: Transcatheter closure of an ostium secundum atrial septal defect (ASD) is now considered the standard care for most of children with the appropriate anatomy, and is a relatively well-tolerated procedure to reduce the clinical sequelae of ASD, with a low complication rate. METHODS: The present case reports describe our clinical experience of the percutaneous closure of a secundum ASD in 10 children between December 2011 and November 2012, by means of a new generation of device, the GORE Septal Occluder device. RESULTS: The implantation was successful in all except two cases, the device b

Percutaneous atrial septal occluder devices and cardiac erosion: a review of the literature.

OBJECTIVE: To systematically review all cases in the literature of erosion associated with atrial septal defect (ASD) occluder devices. BACKGROUND: Delayed erosion of the aortic or atrial wall is an infrequent but potentially lethal complication of percutaneous septal defect closure using an ASD device. The epidemiology is poorly understood. METHODS: The MEDLINE database was searched for cases of ASD occluder device-associated erosion. RESULTS: Twenty-five articles present erosion events associated with the AMPLATZER™ septal occluder (ASO) device (March 2002-June 2011), documenting 21 indep

Perimembranous Ventricular Septal Defect Device Closure: Choosing Between Amplatzer Duct Occluder I and II.

Transcatheter closure of perimembranous ventricular septal defects (pmVSDs) is a well-established procedure. Recently, Amplatzer duct occluders (ADO) I and II have been reported to close large series of pmVSDs successfully (off-label use). ADOs are economical compared with the standard Amplatzer VSD occluders, a major consideration in developing countries with low-budget programs. We report closure of symptomatic, hemodynamically significant pmVSDs using the ADOI and ADOII devices. Although there are no set criteria for choosing between ADOI and ADOII, the former's price tag includes snare and

Successful retrieval of a Figulla Occlutech septal occluder - embolized device stability and potential solutions.

Go to: We report a case of an atrial septal defect where a Figulla Occlutech device embolized into the right ventricle. As this device has no left atrial hub, we brought the bare device into the inferior vena cava and used a novel two-snare technique to slenderize the device into the sheath. This report highlights additional tips in transcatheter device retrieval.

Unusual presentation of nickel allergy requiring explantation of an Amplatzer atrial septal occluder device.

Systemic allergic reaction to a percutaneous patent foramen ovale (PFO) occlusion device is a rare event with only scattered reports in the literature. Serious allergic reactions to these devices have a poorly defined incidence, presentation, and natural history. We present a woman with a previously unknown nickel allergy who developed severe chest pain beginning the morning after percutaneous device closure of the PFO. Despite multiple visits to her cardiologists and primary care physicians, the cause of her chest pain remained unclear. After seeking a second opinion at our medical center, sk

Early-to-mid onset bacterial endocarditis following implantation of Gore Cardioform Septal Occluder device.

We present the unusual case of a 37-year-old female who was found to have bacterial endocarditis related to a 30 mm Gore Cardioform Septal Occluder that was placed following multiple paradoxical strokes, due to a patent PFO. Cardiothoracic Surgery was consulted and patient underwent surgical explantation with pericardial patch repair. Patient was discharged on postoperative (POD) 10 with 6 weeks of IV Cefazolin. Endocarditis following PFO closure-device implantations remains an exceedingly rare complication. In those that do develop endocarditis, the majority occur in early postop period. The

Surgical removal of an atrial septal occluder device embolized to the main pulmonary artery.

Percutaneous closure of atrial septal defects in adults has emerged as an alternative to surgery. We report a sequela of such closure in a 16-year-old boy: embolization of the atrial septal defect occluder into the main pulmonary artery when the patient experienced an episode of intense coughing immediately after device deployment. We removed the device surgically and closed the atrial septal defect in a standard manner, with an autologous pericardial patch.

Late infective endocarditis of an atrial septal occluder device presenting as a cystic mass.

We report an atypical echocardiographic presentation of a vegetation in a patient with late infective endocarditis of an atrial septal defect (ASD) occluder device. Transesophageal echocardiography demonstrated a penduculated mass attached to the left atrial side of the occluder device. This mass presented as an oscillating echo free area surrounded by a membrane attached to the device by a thin stalk. At time of surgical excision, the lesion did not present as a spherical cyst. It was assumed that the content of the echo free mass had already emptied into the left atrium. Histopathology diagn

A constricting band: an unusual cause of incomplete expansion of Amplatzer septal occluder device.

Various problems and complications have been reported during transcatheter closure of ostium secundum atrial septal defect with Amplatzer septal occluder. We report an unusual problem that was responsible for incomplete expansion of the waist of the device, not yet reported in world literature. A polyester band in continuation with the polyester mesh was found constricting the waist of the device, which after being cut led to successful redeployment of the device.

Severe mitral valve insufficiency after transcatheter atrial septal defect closure with the amplatzer septal occluder: a device-related complication.

Atrial septal defect (ASD) is the one of the most common congenital cardiac diseases. Percutaneous device closure of the ASD has developed as an alternative to traditional surgical closure with a similar outcome, decreased surgical trauma, and shorter hospital stay. However, several complications have been reported. We now report a rare complication of late severe mitral insufficiency (MI) after a successful closure of a secundum ASD with an Amplatzer device. The potential mechanisms may be the continual traction of the oversized mismatched device on the root of the mitral annulus and the insu

Real time three-dimensional transthoracic echocardiography for guiding Amplatzer septal occluder device deployment in patients with atrial septal defect.

BACKGROUND: Transcatheter Amplatzer septal occluder (ASO) device closure of atrial septal defects (ASDs) has traditionally been guided by two-dimensional transesophageal echocardiography (2D-TEE) and intracardiac echocardiography (ICE) modalities. Real time three-dimensional transthoracic echocardiography (RT3D-TTE) provides rotating images to define ASD and adjacent structures with potential as an alternative to 2D-TEE or ICE for guiding the device closure of ASD. Our aim was to assess the feasibility and effectiveness of RT3D-TTE in parasternal four-chamber views to guide ASO device closure

Delayed recurrent pleuropericardial effusions following atrial septal defect closure with Amplatzer septal occluder device: a case of postpericardiotomy syndrome?

The authors present the case of a 41-year-old woman, who presented with moderate pericardial effusion and bilateral pleural effusion 11 months following a secundum atrial septal defect closure with an Amplatzer septal occluder (ASO) device. The acute presentation responded well to diuretics and a short course of corticosteroid therapy. The patient, however, continued to experience pleuritic chest pain and a recurrence of pericardial effusion a month later. The patient showed significant symptomatic improvement with a 2-month course of down-titrating prednisolone. Six months later, the echocard
