Wear debris inhibition of anti-osteoclastogenic signaling by interleukin-6 and interferon-gamma. Mechanistic insights and implications for periprosthetic osteolysis.

BACKGROUND: Wear debris challenge of macrophages provokes the generation of proinflammatory cytokines, which contribute to periprosthetic osteolysis. However, it is not known whether this effect is accompanied by reprogramming of other cytokines present within the periprosthetic tissue that may be involved in anti-osteoclastogenic activities. In the present study, we examined the ability of wear debris particles to inhibit the signaling of two such cytokines, interleukin-6 and interferon-gamma. METHODS: Human osteoclast precursor cells were challenged with particles of titanium or polymethylm

Incidence of subsequent vertebral fracture after kyphoplasty.

STUDY DESIGN: A retrospective review of charts and radiographs of all consecutive patients who underwent kyphoplasty at the authors' center from the initial procedure in September 2000 to the end of the collection period in July 2002. SUMMARY OF BACKGROUND DATA: The best available natural history data would suggest that after experiencing an osteoporotic vertebral compression fracture, patients have a 19% incidence of subsequent fracture in the following year when no surgical intervention is performed. When kyphoplasty is performed, there are conflicting data regarding the incidence of subseq

Does femoral stem choice associate with survivorship and clinical outcomes after conversion total hip arthroplasty? A retrospective analysis and novel treatment algorithm.

BACKGROUND: Conversion total hip arthroplasty (cTHA) after prior proximal femoral fracture fixation can be performed using different femoral stems. This study aimed to (1) determine if initial fracture fixation or pattern associated with stem design in cTHA, (2) evaluate whether stem type was associated with survivorship and outcomes, and (3) propose a radiographic measurement to guide femoral stem selection. METHODS: We retrospectively reviewed 51 patients who underwent cTHA from 2016 to 2020, including 44 patients with previous cephalomedullary nail (CMN; 86.3%) and 9 patients with previous

Fracture load and failure patterns of zirconia and 3D-printed resin endocrowns: an in vitro study.

BACKGROUND: Endodontically treated teeth are more prone to fracture and often require durable restorative solutions. Endocrowns offer a conservative alternative to conventional post-and-core systems. This in vitro study compared the fracture load and failure behavior of milled zirconia and 3D-printed resin endocrowns. MATERIALS AND METHODS: Eighteen extracted human molars were endodontically treated and standardized, then prepared for endocrown restorations using a uniform design (two mm occlusal reduction with a four mm intracoronal extension). Specimens were randomly assigned to receive eit

Biomaterials for vertebral body repair: interior augmentation and total replacement-a literature review of bone fusion outcomes in clinical studies.

BACKGROUND AND OBJECTIVE: Vertebral body repair comprises surgical procedures aimed at restoring height and stability of injured vertebrae, thereby re-establishing spinal structural integrity and load-bearing function. Bone fusion and biomaterial incorporation or integration are key determinants of long-term clinical outcomes, providing biological stability. The ideal biomaterial for vertebral body repair remains undefined. Despite the increasing number of available materials, achieving an optimal balance between mechanical load-bearing capacity and biological potential for progressive bone su

Non-Vascularized Fibular Autograft for Two-Stage Scapulohumeral Arthrodesis in Ballistic Shoulder Injuries: A Case Series and Review.

BACKGROUND: Ballistic trauma to the proximal humerus poses a significant challenge because of extensive bone loss and soft tissue damage, often involving the rotator cuff, deltoid muscle, and axillary nerve. In cases where limb preservation is the priority, scapulohumeral arthrodesis (SHA) remains a viable option, particularly when prosthetic reconstruction is contraindicated because of severe musculotendinous injury. The Masquelet 2-stage induced membrane technique has shown promising results in lower limb reconstruction, yet its application for upper limb arthrodesis in ballistic injuries re

The association between implant design, age, sex and the rate of major reoperation in patients undergoing primary total hip replacement: A retrospective study of UK National Joint Registry and Hospital Episodes Statistics data.

BACKGROUND: Implant revision is an operation with exchange of implants, and is used as a standard outcome after total hip replacement (THR), but may not fully represent the patient experience after a THR. Major reoperation (hereafter referred to as 'reoperation') without revision of implants can also lead to increased patient morbidity and mortality, and most commonly occurs when the femur fractures around an implant (postoperative periprosthetic femoral fractures; POPFF) and is treated with fixation and the implant is left in place. Reliance on revision metrics that do not capture these reope

Suture debris from high-tensile sutures contributes significantly to particle-induced tissue response in shoulder arthroplasty.

HYPOTHESIS AND BACKGROUND: Polyethylene (PE) wear debris has long been known to be important for the development of osteolysis and aseptic loosening in total joint replacements. Evaluation of shoulder hemiarthroplasty (HA) specimens provided a unique opportunity to study the histopathologic response when a PE-bearing surface was absent. We hypothesized that HAs would exhibit no significant inflammatory periprosthetic tissue response due to the absence of significant numbers of wear particles from the articulation. METHODS: We analyzed 13 shoulder HAs. The explants were examined for damage to

Pulp Response to Materials Used in the Management of Deep Carious Lesions Without Pulp Exposure: A Systematic Review and Network Meta-Analysis.

BACKGROUND: Placing a pulp-capping material over the remaining dentine is integral to managing deep carious lesions in permanent teeth without pulp exposure. However, current guidelines do not favour any specific pulp-capping material, and there is no direct clinical evidence that pulp-capping materials maintain pulp vitality better than placing the restoration directly on dentine. OBJECTIVES: To compare the effectiveness of various biomaterials, including pulp-capping materials and restorative materials applied directly over the remaining dentine, against one another in preserving pulp healt

Functional Outcomes of Bipolar Hemiarthroplasty in Unstable Intertrochanteric Femur Fractures in the Elderly: A Prospective Study.

Background Intertrochanteric femur fractures are relatively common injuries among elderly individuals. Treatment options include fixation of intertrochanteric fractures using proximal femoral nails (PFNs), dynamic hip screws (DHSs), and unipolar and bipolar hemiarthroplasty. Unstable types of intertrochanteric fractures (Arbeitsgemeinschaft für Osteosynthesefragen (AO) types 31-A2 and A3) are more common in elderly osteoporotic people. Intertrochanteric femur fractures having a subtrochanteric extension, comminution at the posteromedial cortex, and reverse obliquity are considered to be unstab

The Strength and Fracture Characteristics of One-Part Strain-Hardening Green Alkali-Activated Engineered Composites.

Alkali-activated engineered composites (AAECs) are cement-free composites developed using alkali activation technology, which exhibit strain hardening and multiple micro-cracking like conventional engineered cementitious composites (ECCs). Such AAECs are developed in this study by incorporating 2% v/v polyvinyl alcohol (PVA) fibers into alkali-activated mortars (AAMs) produced using binary/ternary combinations of fly ash class C (FA-C), fly ash class F (FA-F), and ground-granulated blast furnace slag (GGBFS) with powder-form alkaline reagents and silica sand through a one-part mixing method un
