The use of mechanical bowel preparation in laparoscopic gynecologic surgery: a decision analysis.

OBJECTIVE: The use of mechanical bowel preparation prior to laparoscopy is common in gynecology, but its use may affect the rates of perioperative events and complications. Our objective was to compare different mechanical bowel preparations using decision analysis techniques to determine the optimal preparation prior to laparoscopic gynecological surgery. STUDY DESIGN: A decision analysis was constructed modeling perioperative outcomes with the following mechanical bowel preparations: magnesium citrate, sodium phosphate, polyethylene glycol, enema, and no bowel preparation. Comparisons were

Medical decision analysis for personalized oncology at the patient bedside.

Medical decision analysis is a method to make rational clinical decisions under uncertainty, enabling a mathematical combination of probabilities and utilities (i.e. values assigned to outcomes under risk). Decision analysis is commonly used in health economics, but it is underexploited in the clinic. With a view to fostering the use of medical decision analysis at the cancer patient bedside, this paper provides basic templates for some typical clinical decisions in cancer treatment, namely affecting: the quantity/quality of life trade-offs in curable cancer; adjuvant/neoadjuvant treatments; c

Integrating Shared Decision-Making Into Breast Cancer Postmastectomy Reconstruction: A Systematic Review of Evidence, Barriers, and Real-World Implementation.

This systematic review synthesizes empirical evidence on the development, effectiveness, patient experience, and implementation of decision aids (DAs) for postmastectomy breast reconstruction (PMBR) and to evaluate the role of pre-consultation educational interventions in supporting shared decision-making (SDM). Following PRISMA guidelines and a preregistered protocol (OSF: https://osf.io/mu9hv), six electronic databases and gray literature sources were searched for studies on PMBR DAs through November 2025. Eligible studies were categorized using a five-domain framework aligned with IPDAS and

Digital Patient Decision Aids for Endometriosis Management: A Scoping Review.

BACKGROUND: Endometriosis treatment requires women to navigate complex, preference-sensitive decisions. Patient Decision Aids (PtDAs) help patients make value-aligned choices. However, the scope and quality of digital PtDAs for endometriosis, and the cap acity of conversational AI platforms to act as PtDAs, remain unclear. OBJECTIVES: Systematically map digital PtDAs for women of reproductive age with endometriosis, describe their content, development, and evaluation, and assess quality and replicability. SEARCH STRATEGY: Electronic databases, Google Scholar, grey literature, and web searche

Empowering patient choice: a systematic review of decision aids for benign prostatic hyperplasia.

BACKGROUND: Benign prostatic hyperplasia (BPH) is a complex condition that affects ~3.2 million men in the UK. As men often face multiple treatment options, discussion and consideration of their priorities and preferences is necessary; however, research indicates this is not always adopted in practice. OBJECTIVES: To evaluate decisional interventions currently available for men with symptomatic BPH, distinct from those designed for prostate cancer. METHODS: Eight databases (PubMed, Web of Science, EBSCO, Science Direct, Scopus, Google, Cochrane Library, Centre for Reviews and Dissemination)

Fostering Shared Decision-Making Between Patients and Health Care Professionals in Clinical Practice Guidelines: Protocol for a Project to Develop and Test a Tool for Guideline Developers.

BACKGROUND: Clinical practice guidelines (CPGs) are designed to assist health care professionals in medical decision-making, but they often lack effective integration of shared decision-making (SDM) principles to reflect patient values and preferences, particularly in the context of preference-sensitive CPG recommendations. To address this shortcoming and foster SDM through CPGs, the integration of patient decision aids (PDAs) into CPGs has been proposed as an important strategy. However, methods for systematically identifying and prioritizing CPG recommendations relevant to SDM and related de

Physicians' perspectives on using a patient decision aid in female stress urinary incontinence.

INTRODUCTION AND HYPOTHESIS: A treatment choice for female stress urinary incontinence (SUI) is preference sensitive for both patients and physicians. Multiple treatment options are available, with none being superior to any other. The decision-making process can be supported by a patient decision aid (PDA). We aimed to assess physicians' perceptions concerning the use of a PDA. METHODS: In a mixed methods study, urologists, gynecologists and general practitioners in the Netherlands were asked to fill out a web-based questionnaire. Questions were based on the Tailored Implementation for Chron

Theory-based approach to developing an implementation plan to support the adoption of a patient decision aid for Down syndrome prenatal screening.

BACKGROUND: Our team has developed a decision aid to help pregnant women and their partners make informed decisions about Down syndrome prenatal screening. However, the decision aid is not yet widely available in Quebec's prenatal care pathways. OBJECTIVE: We sought to identify knowledge translation strategies and develop an implementation plan to promote the use of the decision aid in prenatal care services in Quebec, Canada. METHODS: Guided by the Knowledge-to-Action Framework and the Theoretical Domains Framework, we performed a synthesis of our research (11 publications) on prenatal scre

To biopsy or not to biopsy in suspected herpes simplex encephalitis: a quantitative analysis.

The authors performed a decision analysis to determine whether a patient suspected to have herpes simplex encephalitis (HSE) should undergo a brain biopsy or be treated empirically with medical therapy. In most cases, empiric treatment with acyclovir would be slightly favored; brain biopsy was not essential in management. However, brain biopsy was found useful for patients who had low CSF glucose at the time of initial lumbar puncture; such patients may have a very high risk to have other treatable conditions such as tuberculosis, brain abscess, toxoplasmosis, or cryptococcosis. The results of

Computer-aided decision system for the clubfeet deformities.

A computer-aided decision system (CADS) based on ontology in pediatric orthopedics was developed to assess, without assumptions performed, the abnormalities of the musculoskeletal system of lower limbs. The CADS consists of four components. The first component is a diagnosis-based ontology, called Ontologie du Système Musculosquelettique des Membres Inférieurs (OSMMI). The second component is a database for collecting clinical observations, e.g., the birth classification of the clubfeet deformities. The third component uses statistical methods (principal component analysis and decision tree) f

Pilot implementation of an electronic diagnostic support tool (AiD-DST) designed to identify the cause(s) of delirium.

OBJECTIVE(S): The identification of cause(s) of delirium remains a clinical challenge within medicine. Our group have previously successfully developed and tested the Aetiology in Delirium-Decision Support Tool (AiD-DST). The AiD-DST is designed to help medical professionals close the gap on the detection of cause(s) of delirium. Here, we report on use of AiD-DST in the real-world setting. METHODS: A real-world implementation study of the AiD-DST within a general medical ward of a metropolitan hospital was conducted over a 10-week period. Analysis of obstetricians' decision making on CTG recordings.

Interpretation of cardiotocogram (CTG) is a difficult task since its evaluation is complicated by a great inter- and intra-individual variability. Previous studies have predominantly analyzed clinicians' agreement on CTG evaluation based on quantitative measures (e.g. kappa coefficient) that do not offer any insight into clinical decision making. In this paper we aim to examine the agreement on evaluation in detail and provide data-driven analysis of clinical evaluation. For this study, nine obstetricians provided clinical evaluation of 634 CTG recordings (each ca. 60min long). We studied the

Beyond pre-travel risk assessment: machine learning for clinical diagnosis in returned travellers.

Machine learning (ML) applications in post-travel clinical care remain limited. ML-based models show good diagnostic performance for malaria when clinical, laboratory and travel itinerary data are incorporated, but only modest performance for gastrointestinal illness. Further work is needed to develop practical, clinically integrated ML-based decision-support tools for post-travel clinical care.

User perceptions of machine learning models as decision support for colorectal cancer multidisciplinary team conferences (AID-SIM-2): a qualitative simulation study.

BACKGROUND: Multidisciplinary team (MDT) conferences are considered a cornerstone of decision-making in cancer diagnostics and care. However, the current literature has not demonstrated improved patient outcomes based on the decisions of the MDT conferences. AIM: We aimed to evaluate how four different decision-support modalities impacted the decision-making process and the internal discussions in the MDTs in a multicenter simulation study, with a focus on user perceptions. METHODS: Four colorectal cancer centers with MDTs participated. We performed four simulations in each center. Each simu

Decision analysis of histamine H2-receptor antagonist maintenance therapy versus Helicobacter pylori eradication therapy: a randomised controlled trial in patients with continuing pain after duodenal ulcer.

BACKGROUND: Much has been published on the efficacy and cost effectiveness of Helicobacter pylori eradication treatment as an alternative to histamine H2-receptor antagonist maintenance treatment in peptic ulcer disease. However, most studies have analysed and emphasised H. pylori eradication rates rather than management/control of symptoms and the associated cost savings. Although H. pylori eradication therapy is very successful in clearing the infection, dyspeptic symptoms may persist and management of these can be expensive. OBJECTIVE: The aim of this study was to assess the cost implicati
