Analysis of Emergency Department Length of Stay for Mental Health Patients at Ten Massachusetts Emergency Departments.

STUDY OBJECTIVE: Prolonged boarding times in the emergency department (ED) disproportionately affect mental health patients, resulting in patient and provider dissatisfaction and increased patient morbidity and mortality. Our objective is to quantify the burden of mental health boarding and to elucidate the effect of insurance together with demographic, social, and comorbid factors on length of stay. METHODS: We conducted a cross-sectional observational study of 871 consecutive patients requiring an ED mental health evaluation at one of 10 unaffiliated Massachusetts hospitals. Demographics; i

Mapping the Use of Ketamine in Treatment-Resistant Depression and Other Psychiatric Disorders: A Scoping Review of Practice Patterns, Efficacy, and Patient Demographic Trends.

BACKGROUND: Ketamine has emerged as a novel treatment for psychiatric disorders, particularly treatment-resistant depression (TRD). Although intravenous (IV) ketamine is not approved by the Food and Drug Administration (FDA) for TRD, esketamine, an FDA-approved therapeutic, has contributed to the widespread clinical use of off-label IV ketamine across the United States. This scoping review highlights significant shifts in ketamine therapy, particularly after FDA approval of esketamine, the impact of COVID-19 on treatment accessibility, and increased regulatory scrutiny from the Drug Enforcemen

Racial and Ethnic Disparities in Mental Health Care for Children and Young Adults: A National Study.

Psychiatric and behavior problems are common among children and young adults, and many go without care or only receive treatment in carceral settings. We examined racial and ethnic disparities in children's and young adults' receipt of mental health and substance abuse care using nationally representative data from the 2006-2012 Medical Expenditure Panel Surveys. Blacks' and Hispanics' visit rates (and per capita expenditures) were about half those of non-Hispanic whites for all types and definitions of outpatient mental health services. Disparities were generally larger for young adults than

Characterization of Young Children Presenting to the Emergency Department for Mental Health Complaints.

OBJECTIVES: From 2011 to 2015, psychiatric emergency department visits among youth in the United States increased 28% and psychiatric visits made up 10% of all pediatric emergency department (ED) visits. Previous research has focused on adolescent mental health, with little characterization of children 10 years of age and younger with mental health complaints. The primary objective of this study was to describe these children who presented to a pediatric ED for mental health complaints in terms of demographics and psychosocial factors. METHODS: One researcher reviewed medical records of child

Optimization of hospitalization duration and cost structure for chronic schizophrenia patients in psychiatric hospitals.

OBJECTIVE: Chronic schizophrenia patients in psychiatric hospitals often have prolonged stays, high insurance resource consumption, and low efficiency. Studies quantifying the joint optimization of payment and operational efficiency are lacking, complicating the trade-off between cost control and relapse prevention. This study aimed to identify factors associated with hospitalization duration and costs using real-world data and machine learning. METHODS: A retrospective study was conducted on inpatients with chronic schizophrenia admitted to Huai'an No.3 People's Hospital from 2022 to 2024. D

Paternal age and 13 psychiatric disorders in the offspring: a population-based cohort study of 7 million children in Taiwan.

Although paternal age has been linked to certain psychiatric disorders, the nature of any causal relationship remains elusive. Here, we aimed to comprehensively assess the magnitude of a wide range of offspring's psychiatric risk conferred by paternal age, leveraging a pedigree inferred from covered-insurance relationship (accuracy >98%) in Taiwan's single-payer compulsory insurance program. We also examined whether there is an independent role of paternal age and explored the potential effect of parental age difference. A total cohort of 7,264,788 individuals born between 1980 and 2018 were i

Psychiatric Morbidity Following Intestinal Infectious Diseases: A Nationwide Cohort Study in South Korea.

Intestinal infectious diseases (IIDs), typically considered self-limiting, may exert lasting effects on mental health. This nationwide retrospective cohort study investigated the association between recurrent IID and the subsequent development of psychiatric disorders in South Korea. Using data from the National Health Insurance Service-National Sample Cohort (2002-2013), adults with three or more IID diagnoses were matched to controls without IID by age, sex, and health screening year. Eight psychiatric outcomes were examined: depressive disorder, bipolar disorder, anxiety disorder, obsessive

Psychiatric medication use among Manhattan residents following the World Trade Center disaster.

To assess medication use in New York after the September 11th attacks, a telephone survey was conducted in October 2001 (N = 1,008). The prevalence of psychiatric medication use 30 days before the disaster was 8.9 and 11.6% 30 days after, a small but significant increase. The most important factor predicting postdisaster use was predisaster use--92% of those who used medications postdisaster used them predisaster. In addition, 3.3% used psychiatric medications 30 days postdisaster, but not 30 days before. Those who had panic attacks, posttraumatic stress disorder (PTSD), and insurance coverage

Psychiatric disorders and the burden of chronic gastrointestinal diseases: Evidence from a nationwide cross-sectional study.

OBJECTIVES: Psychiatric disorders are leading contributors to global morbidity. Chronic gastrointestinal (GI) diseases are also highly prevalent. Despite the gut-brain axis theory, large-scale evidence comparing the burden of functional versus organic GI diseases across diverse psychiatric conditions is scarce. We aimed to evaluate the prevalence odds of chronic GI disorders-categorized into functional, organic, and hepatobiliary-pancreatic (HBP) diseases-among adults with psychiatric disorders compared with matched controls. METHODS: We performed a nationwide cross-sectional study using Sout

Temporal Trends in Autologous Breast Reconstruction Among Patients With Psychiatric Diagnoses: From Disparity to Equity.

BACKGROUND: During the last decades, advances in microsurgical techniques and equipment, alongside improvements in the perioperative care, have contributed to a steady rise in the use of autologous breast reconstruction (ABR). However, access to advanced reconstruction procedures has not been uniform across the patient population, exhibiting significant disparities related to race, ethnicity, comorbidity burden, insurance status, and socioeconomic status. Prior studies examining the relationship between psychiatric diagnoses and breast reconstruction have largely focused on short-term outcomes

[Effectiveness and limitation of newly approved drugs for Alzheimer's disease].

Galantamine, rivastigmine, and memantine, for the treatment of Alzheimer-type dementia, became covered by national health insurance last year in Japan. Galantamine and rivastigmine are choline esterase inhibitors, and memantine is a glutamate NMDA receptor antagonist. Galantamine also acts on the nicotinic acetylcholine receptor, in addition to ChEI, and it has an APL action and modulates the receptor function by inducing a structural change of the receptor, as its characteristics. Rivastigmine is used only in patch form, and exhibits an inhibitory effect on butyryl choline esterase. Memantine
