Barber-Say Syndrome: report of a new case.

We present a girl with lax, redundant skin, ectropion, bulbous nose, macrostomia, and absence of mammary glands. To our knowledge, she represents the fourth described case of Barber-Say Syndrome (BSS). BSS and ablepharon macrostomia syndrome (AMS) share common and distinctive clinical manifestations that involve the same structure of the skin and adnexa. We hypothesize that they may derive from a defective regulation of the same gene.

Bilateral Transverse Facial Cleft as an Isolated and Asyndromic Deformity.

Congenital macrostomia or transverse facial cleft is a rare congenital craniofacial anomaly, which affects the esthetics and functions of oral cavity. It is usually associated with deformities of other structures developed from the first and second branchial arches. Bilateral transverse cleft, occurring alone is uncommon. Since the deformity is rare, its treatment has not been commonly described in the literature. We report a case of congenital bilateral macrostomia as an isolated, asyndromic deformity to add one more case in the literature and surgical technique has been discussed here.

Barber Say Syndrome (A New Case Report).

Barber Say syndrome (BSS) is a rare ectodermal dysplasia with neonatal onset characterized by congenital generalized hypertrichosis, atrophic skin, ectropion and macrostomia. A literature review showed less than 20 previously reported cases of Barber Say syndrome. This presentation reports a one day old female with syndrome face, low hairline, coarse face, macrostomia, thin upper lip, bilateral ectropion and hypertelorism, hypertrichosis, senile skin appearance, hypoplastic nipples and one area of mild skin atrophy. These findings are consistent with BSS.

Surgical Strategies Before Auricular Reconstruction: A Treatment Algorithm for Craniofacial Anomalies in Goldenhar Syndrome.

Goldenhar syndrome (GS) presents complex craniofacial malformations and challenging timing for auricular reconstruction. This study characterizes the craniofacial phenotype and staged surgical management preceding ear reconstruction. Single-center retrospective cohort of 30 patients (2019-2024). Craniofacial anomalies and all procedures performed before auricular reconstruction were recorded. Categorical data were summarized as n (%), and ages as median. Auricular anomalies were universal; microtia occurred in 27/30 (90.0%) and external auditory canal atresia in 43.3%. Hemifacial microsomia wi

Unusual Bilateral Oblique Craniofacial Cleft with Atypical Cleft of Palate: A Case Report with 3 Years of Follow-Up.

Oblique facial clefts are a rare entity and can be associated with palatal anomalies like synechiae caused by amniotic bands. Their treatment is individualized following sound reconstructive surgical principles. This is a case report of a 15-month-old child with cleft palate in which the right palatal shelf was represented by a tongue-like projection, associated with macrostomia and bilateral medial and lateral oro-ocular clefts. The palatal cleft was closed in two stages and the oblique clefts in the first stage. At 4 years of age, the velopharyngeal incompetence was addressed by a superiorly

The Presentation Patterns of Tessier 7 Clefts.

Given its rarity, there are few large-number studies regarding Tessier 7 clefts. A review and classification of 81 Tessier 7 patients who presented to our craniofacial center was performed. The most common Tessier 7 clefts were middle positioned, followed by agenetic, superiorly rotated, and inferiorly rotated clefts, respectively. A total of 77/81 patients had simple macrostomia and 4/81 patients had macrostomia with muscle diastasis. The most common associated anomalies were craniofacial microsomia, skin tags, and ear anomalies occurring in 80%, 71%, and 37% of patients, respectively. In thi

Surgical correction for Tessier number 7 craniofacial cleft using a medially overcorrected design.

BACKGROUND: Various surgical techniques have been used to correct Tessier number 7 craniofacial cleft, which involves macrostomia, ear deformity, and hemifacial microsomia. To achieve symmetrical and satisfactory results in patients with macrostomia, the authors performed a 1-mm medial overcorrection on the cleft side and evaluated the results of this procedure. METHODS: A retrospective medical record review of patients diagnosed with Tessier number 7 craniofacial cleft from March 1999 to February 2017 was performed. Using clinical photographs, outpatient clinic records, and operative records

A rare case of accessory maxilla and bilateral Tessier no. 7 clefts, a 10-year follow-up.

A Tessier no. 7 cleft is a lateral facial cleft which originates from the oral cavity and extends towards the tragus, involving both soft-tissue and skeletal components. A male patient presenting with both maxillary jaw duplication and bilateral Tessier no. 7 clefts, which has been reported only twice in the literature, is described. Bilateral facial clefts, macrostomia and chondro-cutaneous remnants were noted, which were repaired and resected. With further growth, facial asymmetry and asymmetric facial nerve dysfunction became apparent. Radiographic examination showed an accessory maxillary

Ablepharon and craniosynostosis in a patient with a localized TWIST1 basic domain substitution.

The TWIST family is a group of highly conserved basic helix-loop-helix transcription factors. In humans, TWIST1 haploinsufficiency causes Saethre-Chotzen syndrome, which is characterized by craniosynostosis. Heterozygous localized TWIST1 and TWIST2 basic domain substitutions exert antimorphic effects to cause Sweeney-Cox syndrome, Barber-Say syndrome, and ablepharon-macrostomia syndrome, respectively. Sweeney-Cox syndrome, Barber-Say syndrome, and ablepharon-macrostomia syndrome share the facial features of ablepharon, hypertelorism, underdevelopment of the eyelids, and cheek pads adjacent to

Tessier no. 7 cleft: a new subclassification and management protocol.

BACKGROUND: Tessier described rare craniofacial clefts anatomically. The no. 7 cleft is a lateral facial cleft consisting of macrostomia, lateral facial muscular diastasis, and bony abnormalities of the maxilla and zygoma. Early computed tomographic imaging provided preliminary insight into the bony abnormality. This article reviews this patient group, defining the clinical and radiological features, to advise optimal protocol management. METHODS: A retrospective case-note review of 15 patients with Tessier no. 7 clefts managed by the Australian Craniofacial Unit over the past 25 years was pe

Partial midfacial duplication.

A case of craniofacial duplication is presented. Details on this rare form are described, and its treatment is discussed with brief review of the pertinent literature. We excised the duplicate maxilla and also discovered bilateral macrostomia. A year later, a mass appeared again. By CT scan, we found that there was a mass in the skull base extruding to the superior wall of cavitas pharyngis. The mass below the sphenoid bone and the ethmoid bone connected with the skull base. CT scan also showed malformation of the first cervical vertebra and odontoid process had bifurcated. Once again, we exci

A rare bilateral Tessier no. 6 and 7 clefts.

The authors describe a patient with bilateral Tessier no. 6 and no. 7 clefts. The case consists of macrostomia combined with maxillary, zygomatico-orbito-temporal deformities, dental anomalies and mandibular retrusion. In addition, the cleft is located bilaterally in the maxillary arch with a double deciduous posterior dentition. This is rare. The clinical and radiological features are reported and the treatment plans are discussed.

Tessier number 7 cleft with oblique clefts of bilateral soft palates and rare symmetric structure of zygomatic arch.

Craniofacial clefts are very rare congenital deformities. Tessier's numbering classification is a simple system universally accepted. Excluding cleft lip and palate, the transverse or lateral cleft (Tessier number 7) is the most common type of craniofacial cleft, manifesting macrostomia without skeletal abnormalities. We present a case of Tessier number 7 cleft with oblique clefting of the soft palate bilaterally and rare symmetric structures of the zygomatic arch.
