Pediatric Parotid Chronic Sclerosing Sialadenitis in an African-American Female: A Rare Case and Review of the Literature.

Chronic sclerosing sialadenitis (CSS) or Küttner tumor is an under-recognized, benign fibroinflammatory disease most commonly seen in the submandibular gland of older adult males. Sialolithiasis or bacterial infection was first suspected as an etiology, but CSS is now considered an immunoglobulin G4-related disease (IgG4-RD). IgG4-RD can affect almost every organ in the body, characterized by organ fibrosis with IgG4-positive plasmacytes. Numerous autoimmune-related diseases have been unified under IgG4-RD, including Mikulicz disease (MD), autoimmune pancreatitis, Reidel's thyroiditis, and oth

Bessel's disease-the first report of an IgG4-related disorder.

Many aspects of IgG4-related diseases were initially described during the late 19th and early 20th century. A variety of clinical presentations caused by this common pathology have been named after the researchers who first described the disorders, such as Mikulicz, Küttner, Riedel or Ormond. However, the initial description of retroperitoneal fibrosis dates back to even 50 years earlier, when in 1846, the Prussian private practitioner Raphael Jakob Kosch described a hitherto unknown constellation of symptoms and pathological findings in a famous patient. This celebrity was the mathematician a

Rare diagnosis of IgG4-related systemic disease by lip biopsy in an international Sjögren syndrome registry.

IgG4-related disease has been recently defined as a distinct clinic-pathologic entity, characterized by dense IgG-4 plasmacytic infiltration of diverse organs, fibrosis, and tumefactive lesions. Salivary and lacrimal glands are a target of this disease and, when affected, may clinically resemble Küttner tumor, Mikulicz disease, or orbital inflammatory pseudotumor. In some patients, the disease is systemic, with metachronous involvement of multiple organs, including the pancreas, aorta, kidneys, and biliary tract. We report a 66-year-old man who presented with salivary gland enlargement and sev

Head and neck manifestations of IgG4-related disease: current understanding.

IgG4-related disease (IgG4-RD) is a systemic fibroinflammatory condition characterized by elevated serum IgG4 levels and dense infiltration of IgG4-positive plasma cells with fibrosis in affected organs. Since its initial recognition, clinical awareness of IgG4-RD has expanded, and head and neck involvement has become increasingly well described. Among these, IgG4-related dacryoadenitis and sialadenitis (IgG4-DS), historically referred to as Mikulicz’s disease, represents the most common manifestation. Patients typically present with persistent, painless swelling of the major salivary and lacr

Immunoglobulin G4 (IgG4)-Related Sialadenitis and Dacryoadenitis With Chronic Rhinosinusitis.

Immunoglobulin G4-related disease (IgG4-RD) is a new disease entity of rare and complex immune-mediated fibroinflammatory conditions that can affect any organ. The concomitance of IgG4 sclerosing sialadenitis and dacryoadenitis with rhinosinusitis is extremely rare. We report a case of IgG4 sclerosing sialadenitis and dacryoadenitis (Mikulicz's disease) diagnosed in a middle-aged African American man with a long-standing history of chronic rhinosinusitis who presented with progressively worsening bilateral salivary and lacrimal glands swelling. Imaging revealed pansinusitis, symmetric enlargem

Mechanisms and assessment of IgG4-related disease: lessons for the rheumatologist.

Recognition of IgG4-related disease as an independent chronic inflammatory disorder is a relatively new concept; previously, the condition was thought to represent a subtype of Sjögren's syndrome. IgG4-related disease is characterized by elevated serum levels of IgG4 and inflammation of various organs, with abundant infiltration of IgG4-bearing plasma cells, storiform fibrosis and obliterative phlebitis representing the major histopathological features of the swollen organs. The aetiology and pathogenesis of this disorder remain unclear, but inflammation and subsequent fibrosis occur due to ex

PRSS1 isoforms promote type 2 immune responses in IgG4-related disease.

Trypsin is commonly used as an adjunctive therapeutic agent for chronic inflammatory and autoimmune diseases. This study investigated the impact of trypsinogen (PRSS1) variants on type 2 immune responses mediated by M2 macrophage polarization in immunoglobulin G4-related disease (IgG4-RD). Whole-exome sequencing of affected tissue samples from patients with IgG4-RD, including two cases of IgG4-related autoimmune pancreatitis and one case of Mikulicz disease, identified novel PRSS1 variants, namely p.Lys70Asn and p.Phe73Leu. Activation of M2 macrophages was observed in these affected tissues. B

[IGg4 related autoimmune hepatitis].

A 50-year-old man was investigated for painless jaundice. The histologic pattern on the liver biopsy study met the criteria of autoimmune hepatitis. Further clinical and laboratory investigation revealed multi-organ involvement, including Mikulicz's disease of the salivary glands and pancreatic insufficiency. The diagnosis of IgG4-related disease was suggested by a finding of elevated blood levels of IgG4. IgG4-related disease is an inflammatory fibrosing condition characterized by T-cell infiltration of affected organs, presence of IgG4-positive plasma cells, and elevated levels of IgG4 in se

Mantle cell lymphoma presenting as Mikulicz syndrome.

A middle-aged woman presented with insidious onset swelling of bilateral lacrimal and parotid glands. Mikulicz syndrome is a symptom complex caused by a variety of systemic disorders like lymphoma, sarcoidosis, amyloidosis, human immunodeficiency virus infection, tuberculosis, etc. Biopsy from her lacrimal glands revealed mantle cell lymphoma. Although involvement of salivary and lacrimal glands as a part of generalized lymphomatous involvement is not uncommon, Mikulicz syndrome as a presenting manifestation of a lymphoma is very rare.

[A tuberculous etiology in Mikulicz's syndrome].

A patient 20 years old presents bilateral hypertrophy of the lacrimal glands with discreet exophthalmos and conjunctival injection at the upper supero-external fornix level. The ocular affection appeared 6 weeks before as a painless tumefaction initially at the right eye and after that at the left eye also, with palpebral fold reduction. From childhood the patient has a ganglionary tuberculosis confirmed by the biopsy of a lateral cervical ganglion. Under the rim of the orbit on palpate the lacrimal gland as a depressible, painless mass, which is continued in the orbit. A lacrimal gland biopsy

Membranous nephropathy associated with IgG4-related disease.

Immunoglobulin G4 (IgG4)-related systemic disease is a rare condition characterized by high levels of circulating IgG4 and IgG4-positive plasma cell infiltrates in various organs, including the pancreas, salivary glands, biliary tract, liver, lung, and kidney. We describe a case of a 54-year-old man with IgG4-related systemic disease presenting with autoimmune pancreatitis and Mikulicz disease. Steroid therapy decreased circulating IgG4 levels and promoted regression of clinical signs. Thereafter, an increase in serum IgG4 values was followed by the occurrence of nephrotic-range proteinuria. K

[Diverse manifestations of orbital pseudotumor in a six-year-old child (case study)].

Nonspecific, or idiopathic, orbital inflammation (IOI) is a group of diseases characterized primarily by inflammation. Another term-pseudotumor-reflects its trait of mimicking oncological processes. Its clinical manifestations are highly polymorphic. There is limited literature on the clinical presentation of IOI in children. This clinical case presents idiopathic inflammation in a six-year-old child that manifested in various forms: initially as unilateral dacryoadenitis, then bilateral, followed by bilateral mass lesions under both orbital roofs. Several specific conditions such as lacrimal

Long-term clinical course of IgG4-related systemic disease accompanied by hypophysitis.

A 70-year old man with a 14 year history of Sjögren syndrome, interstitial pneumonia, and autoimmune hepatitis (AIH) was admitted to our hospital due to hyponatremia with a one month history of fatigue, thirst, and nausea. Laboratory tests on admission revealed that this patient had a central adrenal insufficiency. Pituitary magnetic resonance imaging (MRI) further showed swelling of the stalk and posterior lobe of his pituitary, suggesting infundibulo-hypophysitis. Based on his past history of autoimmune disease, his serum IgG4 levels were measured and found to be remarkably high (924 mg/ dL)

Gallium SPECT/CT in evaluation of IgG4-related disease: A case report and literature review.

BACKGROUND: The clinical picture of IgG4-related sclerosing disease (IgG4-RSD) may mimic lymphoma, and should be in the differential diagnosis of patients with this clinical picture. CASE SUMMARY: A 32-year-old female had recurrent swelling of both eyelids for more than 15 years. Examination revealed elastic, firm, swollen lacrimal glands about 2-3 cm in diameter that was not painful. Head and orbits magnetic resonance imaging (MRI) showed mass lesions over the bilateral lacrimal glands, submandibular glands, and left foramen of ovale. The differential diagnosis included lymphoid tissue, infl

Chemotherapy of progressive hairy-cell leukaemia.

5 patients with hairy-cell leukaemia were treated with chemotherapy. 3 of them received multiple courses of CHOP (cyclophosphamide-doxorubicin-vincristine-prednisolone); 1 patient received cyclophosphamide monotherapy for 6 months. The 5th patient developed a Mikulicz's syndrome after splenectomy and was initially treated with cyclophosphamide monotherapy, then received 1 course of CHOP, and finally 9 courses of intermediate-dose methotrexate. 1 complete remission (45 months+) and 3 partial remissions (75+, 68, 32+ months) were observed, while 1 patient had progressive disease. Our results ind

Autoantibody-associated congenital heart block: the clinical perspective.

Congenital heart block (CHB) can occur in association with structural heart disease, such as atrioventricular septal defects, left atrial isomerism, and abnormalities of the great arteries, with tumors, such as mesotheliomas, or as an isolated defect. In 1928, Aylward reported the occurrence of CHB in two children whose mother "suffered from Mikulicz's disease." This curious clinical observation was further solidified by the 1970s, with reports of CHB in children whose mothers had autoimmune diseases and that the maternal sera contained antibodies to Ro ribonucleoproteins. It was subsequently

[A contribution to diagnosis, specificity and treatment of rhinoscleroma (author's transl)].

As a disease with a peculiar geographical distribution rhinoscleroma is a kind of chronic inflammation which may cause some diagnostic difficulties in non-endemic regions. In general, the diagnosis based on histological features and the microbiological identification of the causative agent. In case where a definite microbiological identification could not be achieved the diagnosis should be expressed in the following terms: "rhinosclerotic granuloma with features of specificity". The diagnosis is facilitated by the aid of cytological methods which are easy to perform and do not hurt the patien

Comprehensive diagnostic criteria for IgG4-related disease (IgG4-RD), 2011.

BACKGROUND: IgG4-related disease (IgG4-RD) is a novel clinical disease entity characterized by elevated serum IgG4 concentration and tumefaction or tissue infiltration by IgG4+ plasma cells. Although IgG4-RD is not rare and is clinically important, its clinical diagnostic criteria have not been established. Comprehensive diagnostic criteria for IgG4-RD, including the involvement of various organs, are intended for the practical use of general physicians and nonspecialists. METHODS: Two IgG4-RD study groups, the Umehara and Okazaki teams, were organized by the Ministry of Health, Labor and Wel
