The role of mechanical ventilation in primary graft dysfunction in the postoperative lung transplant recipient: A single center study and literature review.

BACKGROUND: Primary graft dysfunction (PGD) is still a major complication in patients undergoing lung transplantation (LTx). Much is unknown about the effect of postoperative mechanical ventilation on outcomes, with debate on the best approach to ventilation. AIM/PURPOSE: The goal of this study was to generate hypotheses on the association between postoperative mechanical ventilation settings and allograft size matching in PGD development. METHOD: This is a retrospective study of LTx patients between September 2011 and September 2018 (n = 116). PGD was assessed according to the International

De novo autoimmune hepatitis following liver transplantation for primary biliary cirrhosis: an unusual cause of late grafts dysfunction.

De novo autoimmune hepatitis (AIH) is a rare disorder first described in 1998. It occurs in patients who underwent liver transplantation for a different etiology. We present the case of a 56-year-old woman who was diagnosed with primary biliary cirrhosis and had liver transplantation for refractory pruritis. Seven years after transplantation, she presented alterations in the hepatic profile with hypertransaminasemia, elevated alkaline phosphatase and gamma-glutamyl-transferase. Her liver functions test also showed elevated IgG levels. Serum autoantibodies were negative except for antimitochond

Innovative use of EIT-guided prone positioning and inhaled nitric oxide therapy for refractory hypoxemia in primary graft dysfunction: a case report.

Primary graft dysfunction Grade 3 (PGD 3) following lung transplantation significantly increases the incidence of acute and chronic complications. These effects complicate clinical perioperative management and significantly increase mortality. Here, we report a case of PGD 3 and refractory hypoxemia after bilateral lung transplantation at our center. Despite ongoing extracorporeal membrane oxygenation (ECMO) support, the patient's partial pressure of oxygen (PaO₂) remained suboptimal at 71.7 mmHg on postoperative day 4, precluding safe discontinuation of ECMO support. Consequently, EIT-guided

Association of donor single-cell cytokine secretion & circulating cytokine levels with the development & trajectory of severe primary graft dysfunction in lung transplantation.

Whether donor immune biology at procurement is associated with primary graft dysfunction (PGD) trajectory is unknown. Pre-transplant donor T-cell function and circulating plasma proteins were profiled to address this gap. Twenty-one recipients were classified as no PGD (NOGD, n=7), recovery after initial PGD3 (RECV, n=5), or sustained PGD3 (SGD, n=9). The polyfunctionality of T-cells isolated from donor peripheral blood mononuclear cell (PBMC) after stimulation and plasma levels of 92 proteins were assessed. CD8+ polyfunctionality strength index (PSI) was lowest in RECV (p=0.019), driven by ef

Bronchial Culture Growth From the Donor and Recipient as Predictive Factors in the Detection of Primary Graft Dysfunction and Pneumonia After Lung Transplant.

OBJECTIVES: In this study, our aim was to investigate whether bacterial culture growth from donors and recipients is related to early posttransplant complications and to analyze its role in primary graft dysfunction and posttransplant pneumonia in lung transplant recipients. MATERIALS AND METHODS: This retrospective cohort study included patients diagnosed with end-stage lung disease who received a lung transplant for treatment. We examined relationships between donor bronchial lavage, pretransplant recipient sputum, and recipient posttransplant serial bronchial lavage culture results, as wel

Preoperative B-type natriuretic peptide is as independent predictor of ventricular dysfunction and mortality after primary coronary artery bypass grafting.

OBJECTIVE: Elevated B-type natriuretic peptide is associated with increased morbidity and mortality in ambulatory patients with congestive heart failure or acute coronary syndromes. Its utility in predicting adverse cardiac surgical outcomes is less certain. We hypothesized that preoperative plasma B-type natriuretic peptide would independently predict in-hospital postoperative ventricular dysfunction, hospital stay, and up to 5-year mortality after primary coronary artery bypass grafting. METHODS: This is a prospective, longitudinal study of 1023 patients at two institutions undergoing prima

Prolonged Invasive Mechanical Ventilation is Associated With Decreased Survival After Lung Transplantation Among Recipients With Primary Graft Dysfunction: A Lung Transplant Outcomes Group Study.

INTRODUCTION: Prolonged invasive mechanical ventilation (IMV) after lung transplantation is an appealing early prognostic outcome as it can be reproducibly assessed both prospectively and retrospectively. Whether use of IMV at 72 h after lung transplantation is associated with post-transplant graft survival is unknown. METHODS: We performed a retrospective cohort study of 1511 participants in the multi-center Lung Transplant Outcomes Group cohort (2011-2018). Using Cox proportional hazards models and restricted mean survival time, we investigated whether IMV at 72 h was associated with post-t

Impact of initial hematocrit levels during direct procurement and perfusion on the likelihood of primary graft dysfunction in donation after circulatory death heart transplantation.

BACKGROUND: Optimizing perfusion parameters when using ex vivo heart perfusion systems is critical for maximizing organ quality and extending perfusion duration. Hematocrit (HCT) of the perfusate may influence oxygen delivery and perfusion dynamics, yet its clinical significance remains uncertain. METHODS: In this retrospective single-center study, we utilize regression models to explore the relationship between hematocrit (HCT) of the device perfusate during normothermic machine perfusion (using the Transmedics Organ System [OCS]) and recipient outcomes following transplantation. Re-evaluating the role of oxygen delivery and radiographs in the definition of primary graft dysfunction after lung transplantation: A Lung Transplant Outcomes Group cohort study.

BACKGROUND: Primary graft dysfunction (PGD) is severe lung injury following lung transplantation. Variable radiograph interpretation and changes in mechanical ventilation and extracorporeal membrane oxygenation (ECMO) support for hypoxemia may limit the current PGD definition's validity. METHODS: We assessed whether changes in the PGD definition accounting for oxygen delivery method (intubated, non-intubated, ECMO) were associated with differential graft survival. Severe PGD was grade 3 at 48 or 72 hours after transplant. We estimated crude and standardized time from transplantation to death

Ten-minute asystolic warm ischemic time (AWIT) predicts mortality and severe primary graft dysfunction in donation after circulatory death hearts recovered with thoracoabdominal normothermic regional perfusion.

BACKGROUND: Donor warm ischemic time (WIT) during donation after circulatory death (DCD) has been linked to adverse outcomes in noncardiac transplants, but its impact on heart transplant recipients remains unclear. This study evaluated the association between prolonged WIT and outcomes in adult recipients of thoracoabdominal normothermic regional perfusion (NRP)-recovered cardiac allografts. METHODS: We retrospectively reviewed adult heart transplant recipients who received NRP-recovered DCD allografts at a single center between October 2020 and February 2025. WIT was defined as (1) withdrawa

Molecular adsorbent recirculating system (Mars) in patients with primary nonfunction and other causes of graft dysfunction after liver transplantation in the era of extended criteria donor organs.

Liver dysfunction is an important cause of morbidity and mortality after orthotopic liver transplantation (OLT). The Molecular Adsorbent Recirculating System (MARS) is an albumin-based dialysis system designed to enhance the excretory function of a failing liver. MARS has been successfully used in patients affected by advanced liver disease and presenting with severe cholestasis. The aim of this study was to evaluate the safety and clinical efficacy of MARS in patients with liver dysfunction after OLT. Seven patients (primary nonfunction, 2 patients; graft dysfunction, 5 patients) fulfilled th

Innate immunity in lung transplantation.

Innate immune pathways early after pulmonary transplantation have been shown to cause primary graft dysfunction (PGD) and also predispose to late graft failure. Recent studies in animal models have elucidated critical mechanisms governing such innate immune responses. Here, we discuss pathways of inflammatory cell death, triggers for sterile and infectious inflammation, and signaling cascades that mediate lung injury early after transplantation. These studies highlight potential avenues for lung-specific therapies early following lung transplantation to dampen innate immune responses and impro

The Impact of Steatosis on the Outcome of Liver Transplantation: A Meta-Analysis.

BACKGROUND AND AIMS: Liver transplantation is one of the most effective treatments for end-stage liver disease as well as for cases of acute liver failure. Facing organ donor shortage, liver transplant teams had to use marginal organs. Thus, increasing availability is a key concern of donor liver grafts including steatotic livers. However, the use of steatotic liver is still controversial. The aim of this systematic review and meta-analysis was to analyze the impact of steatosis on the outcome of liver transplantation. METHODS: We searched PubMed, Cochrane Library, Embase, Web of knowledge, a

Extracellular Vesicles in Lung Transplantation: Biomarkers, Pathophysiological Players, and Therapeutic Weapons?

In the field of lung transplantation (LTx), the survival of lung transplant recipients (LTRs) is limited by events such as primary graft dysfunction (PGD), infections, and acute rejection (AR), which promote the development of chronic lung allograft dysfunction (CLAD). Extracellular vesicles (EVs), including exosomes and microvesicles, have emerged as key players in LTx because of their roles in immune regulation, inflammation, and antigen presentation. EVs carry immunologically active molecules such as MHC class I/II proteins, cytokines, and lung self-antigens (SAgs), suggesting their involve

Circulating growth differentiation factor-15 as a novel biomarker in heart transplant.

AIMS: This study aimed to examine (i) whether circulating growth differentiation factor-15 (GDF-15) is associated with acute cellular cardiac allograft rejection (ACR); (ii) a longitudinal trend of GDF-15 after heart transplantation; and (iii) the prognostic value of GDF-15 in predicting a composite outcome of severe primary graft dysfunction (PGD) and 30 day mortality post-transplant. METHODS AND RESULTS: Serum samples were collected before heart transplantation and at every endomyocardial biopsy (EMB) post-heart transplantation in de novo transplant patients. [Lymphatics and lung transplantation. A review].

The role of lymph circulation in lung transplantation (LTx) has not really been studied since the first animal models, which allowed the development of solid organ transplantations. However, the oedema observed in the grafts immediately after LTx often remains unpredictable and unexplained. Although it is an integral part of the entity called "primary graft failure". Despite its multifactor aspects making the interpretation difficult, the possibility of a change in the lymph circulation is proposed to explain an oedema occurrence. The animal models focusing on this point were mainly developed

The search for peripheral tolerance in lung transplantation.

PURPOSE OF REVIEW: Median survival after lung transplantation is 5.7 years, which lags behind other solid organ transplants, such as heart, liver, and kidney. The major barrier to long-term survival in lung transplant recipients is chronic lung allograft dysfunction (CLAD). This review discusses the challenge of CLAD as a barrier to tolerance and identifies key areas in the field that require further development. RECENT FINDINGS: CLAD is a heterogenous disease in its kinetics of onset and severity and remains a clinical diagnosis of exclusion, based on a decline in allograft function. While a

Early Graft Dysfunction Following Heart Transplant: Prevention and Management.

Heart transplant can be considered as the "gold standard" treatment for end-stage heart failure, with nearly 5.7 million adults in the United States carrying a diagnosis of heart failure. According to the International Society for Heart and Lung Transplantation registry, nearly 3300 orthotopic heart transplants were performed in 2016 in North America. In spite of significant improvements in overall perioperative care of heart transplant recipients for the past few decades, the risk of 30-day mortality remains 5% to 10%, primarily related to early failure of the allograft. Early graft dysfuncti
