The influence of dilution on the offline measurement of exhaled nitric oxide.

OBJECTIVE: Measurement of fractional exhaled nitric oxide (FeNO) is used to determine the presence and severity of eosinophilic airway inflammation in asthma and other wheezing illnesses. The gold standard of online measurement during a single prolonged exhalation is not suitable for use in young children. The international guidelines for offline measurements recommend collection of exhaled gas in an appropriate reservoir for later analysis in young children. The apparatus required for gas collection, however, creates dead space within the system, which may result in sample dilution and hence

Cardiorespiratory effects of added dead space in patients with heart failure and central sleep apnea.

BACKGROUND: Inhaled CO(2) has been shown to stabilize the breathing pattern of patients with central sleep apnea (CSA) with and without congestive heart failure (CHF). Added dead space (DS) as a form of supplemental CO(2) was effective in eliminating idiopathic CSA. The efficacy and safety of DS has not yet been evaluated in patients with CHF and CSA. METHODS: We examined the respiratory and cardiovascular effects of added DS in eight patients with CHF and CSA. The DS consisted of a facemask attached to a cylinder of adjustable volume. During wakefulness, the cardiorespiratory response to 200

High-Flow Nasal Cannula Reduces Ventilatory Requirements During Endobronchial Ultrasound.

BACKGROUND: High-flow nasal cannula (HFNC) reduces dead-space ventilation, but this effect is diminished by open-mouth breathing and partial airway obstruction. Consequently, it is uncertain whether HFNC provides respiratory support during endobronchial ultrasound (EBUS) procedures. METHODS: A single-center randomized controlled crossover study was conducted at the Rijnstate Hospital, Arnhem, the Netherlands, from November 2022 to August 2024.Patients with severe COPD (Gold III/IV) were evaluated to determine if HFNC reduces dead space ventilation during an EBUS procedure. Exclusion criteria

Changes in lung mechanics and ventilation-perfusion match: comparison of pulmonary air- and thromboembolism in rats.

BACKGROUND: Pulmonary air embolism (AE) and thromboembolism lead to severe ventilation-perfusion defects. The spatial distribution of pulmonary perfusion dysfunctions differs substantially in the two pulmonary embolism pathologies, and the effects on respiratory mechanics, gas exchange, and ventilation-perfusion match have not been compared within a study. Therefore, we compared changes in indices reflecting airway and respiratory tissue mechanics, gas exchange, and capnography when pulmonary embolism was induced by venous injection of air as a model of gas embolism or by clamping the main pul

Nocturnal desaturation in patients with non-operable chronic thromboembolic pulmonary hypertension.

BACKGROUND: Nocturnal desaturation is occasionally observed in patients with chronic thromboembolic pulmonary hypertension (CTEPH) despite the absence of parenchymal lung disease; however, the underlying mechanism remains unclear. This study aimed to clarify the clinical features of nocturnal desaturation in patients with CTEPH. METHODS: Data of 163 patients with CTEPH who underwent balloon pulmonary angioplasty (BPA) between March 2011 and December 2022 were retrospectively analyzed. We evaluated their hemodynamics using right heart catheterization, arterial blood gas examination, respirator

The effect of a pediatric heat and moisture exchanger on dead space in healthy pediatric anesthesia.

BACKGROUND: Heat and moisture exchangers (HME) are often used to maintain humidity of breathing circuits during anesthesia. It is also known to increase dead space ventilation in respiratory distress syndromes. However, the effect of a pediatric HME in healthy pediatric patients has not yet been clarified. The purpose of this study was to evaluate the effect of a pediatric HME on dead space in healthy pediatric patients during anesthesia. METHODS: 20 ASA physical class I pediatric patients, without respiratory impairment, who underwent elective surgery for inguinal hernia or hydrocele with ge

Short-term effects of humidification devices on respiratory pattern and arterial blood gases during noninvasive ventilation.

BACKGROUND: The impact of humidification devices on ventilatory and arterial blood gases parameters during noninvasive ventilation (NIV) remains controversial. The aim of the study was to compare the short-term impact of heat and moisture exchangers (HMEs) and heated humidifiers (HHs) during NIV for either hypercapnic or hypoxemic acute respiratory failure. METHODS: Consecutive subjects receiving NIV were successively treated with HME and HH in randomized order for 30 min each. At the end of each period, arterial blood gases were measured and ventilatory parameters were recorded. RESULTS: Ei

Effects of inspired CO2, hyperventilation, and time on VA/Q inequality in the dog.

In a recent study by Tsukimoto et al. (J. Appl. Physiol. 68: 2488-2493, 1990), CO2 inhalation appeared to reduce the size of the high ventilation-perfusion ratio (VA/Q) mode commonly observed in anesthetized mechanically air-ventilated dogs. In that study, large tidal volumes (VT) were used during CO2 inhalation to preserve normocapnia. To separate the influences of CO2 and high VT on the VA/Q distribution in the present study, we examined the effect of inspired CO2 on the high VA/Q mode using eight mechanically ventilated dogs (4 given CO2, 4 controls). The VA/Q distribution was measured firs

Metabolic acidosis and respiratory acidosis impair gastro-pyloric motility in anesthetized pigs.

UNLABELLED: Acidosis impairs smooth muscle function in various organs. However, the effects of acidosis on the gastroduodenal tract are unknown while its dysfunction has potential perioperative harmful consequences. We investigated the effects of metabolic (MA) and respiratory acidosis (RA) on upper gut motility in tracheally ventilated pigs whose anesthesia was induced with halothane and maintained with alpha-chloralose-urethane administration (IV). Increased dead space and perfusion of hydrochloric acid 1 N (150 mL over 30 min) were used to induce RA and MA, respectively. Measurements of fun

Asbestos-induced pleural fibrosis and impaired exercise physiology.

To further assess the clinical significance of asbestos-induced pleural fibrosis, we performed cardiopulmonary exercise testing in 90 subjects who were exposed to asbestos. Of the 82 subjects without an abnormal resperate exercise, 35 had normal pleura, 33 had circumscribed pleural plaques, and 14 had diffuse pleural thickening. Interstitial fibrosis (International Labor Organization [ILO]. > or = 1/10) was present in 14 of 35 subjects with normal pleura, 13 of 33 subjects with circumscribed pleural plaques, and 2 of 14 subjects with diffuse pleural thickening. Although pleural fibrosis did no

[Standardization of the values of the concentration of exogenous substances in the urine and expired air].

Workplace pollutants, inhaled by exposed people, can be subsequently excreted by the kidney as biotransformed or by the lung as parent substances. In both cases the concentration values need to be corrected to a given hydration level (the urinary metabolites) or to a value adjusting the effects of respiratory manoeuvres and those of the mixing between alveolar and dead space air (the volatile substances eliminated through the respiratory tract). The classical methods of standardization of urinary values lie in expressing the data as a value adjusted to a constant specific gravity of urine (1.0

A comparison of respiratory function in divers breathing with a mouthpiece or a full face mask.

A comparison was made of respiratory function in submersed divers breathing with either a mouthpiece or a full face mask while exposed to varying depths (15 and 190 fsw), exercise loads (0-175 W), and static lung loads (0 and -20 cmH2O). The two types of breathing equipment were designed to be identical in terms of functional dead space volume and resistance to gas flow. When compared with data from experiments utilizing a full face mask, use of a mouthpiece caused a modest fall in expired minute volume at both depths. The majority of this decline may have been the consequence of a decrease in

A simple method for isocapnic hyperventilation evaluated in a lung model.

BACKGROUND: Isocapnic hyperventilation (IHV) has the potential to increase the elimination rate of anaesthetic gases and has been shown to shorten time to wake-up and post-operative recovery time after inhalation anaesthesia. In this bench test, we describe a technique to achieve isocapnia during hyperventilation (HV) by adding carbon dioxide (CO2) directly to the breathing circuit of a standard anaesthesia apparatus with standard monitoring equipment. METHODS: Into a mechanical lung model, carbon dioxide was added to simulate a CO2 production (V(CO2)) of 175, 200 and 225 ml/min. Dead space (

Cardiogenic mixing: mechanisms and experimental evidence in dogs.

Cardiogenic mixing was studied in seven anaesthetized closed-chest dogs undergoing mechanical ventilation by comparing single-breath washout of two poorly soluble inert gases of widely differing diffusivities (helium (He) and sulphur hexafluoride (SF6)) in normal conditions with the heart beating (control), and during reversible temporary myocardial arrest (heart arrest). Cardiac arrest of approximately 20 s duration was induced repeatedly (8-15 times) by intracoronary injection of acetylcholine (approximately 35 mg) facilitated by a non-occluding 7-French gauge angiographic catheter maintaine

Lung-protective ventilation strategies and adjunctive treatments for the emergency medicine patient with acute respiratory failure.

Respiratory failure is a frequent disease process encountered in the emergency department. There is significant need for improvement in the care of patients on mechanical ventilation. If not contraindicated, lung-protective ventilation strategies should be used. It is important to consider pathophysiology (shunting, dead space ventilation, and low venous admixture) when formulating treatment strategies in patients who are difficult to oxygenate or ventilate or when Pao2, Paco2, and pH can only be maintained at unsafe ventilator settings.
