Hearing loss is common in the geriatric population. Most hearing loss is associated with presbycusis or age-related hearing loss, impacting one-third of individuals over 65 years and increasing in prevalence with age. Hearing loss impacts quality of life, psychological health, and cognition. Implantable auditory devices are an exceptional option to improve hearing and quality of life. Various implantable auditory devices have been implemented safely with significant improvement in communication and performance on auditory tasks. Counseling is essential to establishing realistic expectations. R

The key targets in the treatment of periprosthetic humeral fractures (PHF) are the preservation of bone, successful bony consolidation and provision of a stable anchoring of the prosthesis with the major goal of restoring the shoulder-arm function. A substantial problem of periprosthetic shoulder fractures is the fact that treatment is determined not only by the fracture itself but also by the implanted prosthesis and its function. Consequently, the exact preoperative shoulder function and, in the case of an implanted anatomical prosthesis, the status and function of the rotator cuff need to b

Transcutaneous osseointegrated prostheses, which anchor titanium alloy implants into the residual limb bone, offer an alternative for amputees suffering from socket-related complications (occurring in up to 75% of cases). The principle is based on the direct titanium-bone interface, which forms a microscopic gap of approximately 20-50 nm that physically blocks bacteria (typically about 500 nm in diameter); combined with the use of high-purity titanium, this makes percutaneous infection risk much lower than that of traditional implants. Mainstream systems include the hollow-screw design (OPRA),

BACKGROUND: A torp (columella-prosthesis) is the typical ossicular reconstruction in cases of a destroyed stapedial arch. Yet, many unsatisfactory hearing results are due to the lack of a stable, reliable anchoring of the base of the prosthesis on the footplate. Some solutions have been postulated, amongst them the perforation of the footplate with a tiny spike at the lower end of the prosthesis, which, however, many otosurgeons regard as too dangerous for the inner ear. Specially designed silicone sheets cannot guarantee a permanent guide of the columella.
METHODS: From our good experience w

CONTEXT AND OBJECTIVE: Osseointegrated implants have acquired an important role in the prosthetic rehabilitation of patients with craniofacial defects. The main indications are lack of local tissue for autogenous reconstruction, previous reconstruction failure and selection of this technique by the patient. This paper presents a clinical case and discusses indications and advantages of the osseointegrated implant technique for retention of auricular prostheses.
TYPE OF STUDY: Case report, Universidade Federal de São Paulo (UNIFESP).
METHODS: A female patient received three auricular implants

UNLABELLED: The bone-anchored hearing aid (BAHA) allows direct bone-conduction through a titanium implant. This avoids skin irritation and seems to improve the sound quality when compared with a conventional bone hearing aid. Aim of this work was to evaluate this new hearing-aid.
PATIENTS AND METHODS: 9 patients have been operated on between May 1994 and April 1997. Two aural atresia and 7 radical mastoidectomy; 6 had prior conventional bone hearing aid and one a bilateral air-conduction hearing aid. Their mean bone conduction threshold was 28 dB (17-38 dB). INTRODUCTION: The new Osia® OSI200 implant incorporates a receiver coil and Piezo Power™ Transducer into one monolithic unit. Appropriate planning and surgical approach is needed for suitable positioning of the device.
METHOD: To optimise the surgical field and provide tension-free wound closure our team have adopted a versatile 'Sheffield-S' post-auricular incision which remains hidden within the hairline.
CONCLUSION: This incision provides adequate exposure for device placement and bone polishing/recessing. The soft tissue approach has resulted in improved operative efficacy particularly i

BACKGROUND: Endo-exo prosthetics (EEP), which belongs to the transcutaneous osseointegrated prosthetic systems (TOPS), provides an alternative bone-anchored rehabilitation method for transfemoral amputees. It led to the question of whether transmitted forces from prosthetic feet are perceptible by osseoperception resulting in proprioceptive feedback of ground conditions.
OBJECTIVES: The following hypotheses emerged for our trial with the null hypothesis: EEP fitting after transfemoral amputation does not influence osseoperception. Alternative hypothesis 1: EEP patients achieve better osseoper

Direct drive bone conduction devices (BCDs) are used to rehabilitate patients with conductive or mixed hearing loss by stimulating the skull bone directly, either with an implanted transducer (active transcutaneous BCDs), or through a skin penetrating abutment rigidly coupled to an external vibrating transducer (percutaneous BCDs). Active transcutaneous BCDs have been under development to overcome limitations of the percutaneous bone anchored hearing aid (BAHA), mainly related to the skin penetration. The attachment of a direct drive BCD to the skull bone can differ significantly between devic

INTRODUCTION: Complex femoral defects with insufficient isthmus (Paprosky III and IV) and Vancouver B3 periprosthetic fractures determines the fixation of the revision stems. The objective is to evaluate the results of the modular revision stems with diaphyseal anchor and distal block (Revitan Zimmer Biomet GmbH, Winterthur, Switzerland). Our hypothesis is that this procedure offers a stable primary fixation in cases of alteration of the femoral isthmus.

OBJECTIVE: Current socket-based methods of prosthetic limb attachment are responsible for many of the dominant problems reported by persons with amputation. In this work, we introduce a new paradigm for attachment via electromagnetic attraction between a bone-anchored ferromagnetic implant and an external electromagnet. Our objective was to develop a design framework for electromagnetic attachment, and to evaluate this framework in the context of transfemoral amputation.
METHODS: We first used inverse dynamics to calculate the forces required to suspend a knee-ankle-foot prosthesis during gai

BACKGROUND: A bone-anchored limb (BAL) is an alternative to a traditional socket-type prosthesis for people with transfemoral amputation. Early laboratory-based evidence suggests improvement in joint and limb loading mechanics during walking with a BAL compared to socket prosthesis use. However, changes in cumulative joint and limb loading measures, which may be predictive of degenerative joint disease progression, remain unknown.
RESEARCH QUESTION: Do cumulative total limb and hip joint loading during walking change using a BAL for people with unilateral transfemoral amputation, compared to

Bone-anchored prostheses (BAP) provide an alternative to traditional socket-suspended prostheses (SSP) for individuals with transfemoral amputation (TFA) by addressing socket-related complications. While most previous studies, primarily single-arm trials, report positive outcomes with BAP, these findings may be overestimated due to lower preoperative baseline function among individuals using SSP prior to BAP implantation. This review aims to offer a more balanced comparison of functional and clinical outcomes between BAP and SSP by focusing exclusively on two-arm comparative studies. A systema
