Noninvasive ventilation through a helmet in postextubation hypoxemic patients: physiologic comparison between neurally adjusted ventilatory assist and pressure support ventilation.

PURPOSE: Neurally adjusted ventilatory assist (NAVA) has been shown to improve patient-ventilator interaction and reduce asynchronies in intubated patients, as opposed to pressure support ventilation (PSV). This is a short-term head-to-head physiologic comparison between PSV and NAVA in delivering noninvasive ventilation through a helmet (h-NIV), in patients with postextubation hypoxemic acute respiratory failure. METHODS: Ten patients underwent three 20-min trials of h-NIV in PSV, NAVA, and PSV again. Arterial blood gases (ABGs) were assessed at the end of each trial. Diaphragm electrical ac

Physiological changes and compensatory mechanisms by the action of respiratory muscles in a porcine model of phrenic nerve injury.

Phrenic nerve damage may occur as a complication of specific surgical procedures, prolonged mechanical ventilation, or physical trauma. The consequent diaphragmatic paralysis or dysfunction can lead to major complications. The purpose of this study was to elucidate the role of the nondiaphragmatic respiratory muscles during partial or complete diaphragm paralysis induced by unilateral and bilateral phrenic nerve damage at different levels of ventilatory pressure support in an animal model. Ten pigs were instrumented, the phrenic nerve was exposed from the neck, and spontaneous respiration was

Short-term effects of synchronized vs. non-synchronized NIPPV in preterm infants: study protocol for an unmasked randomized crossover trial.

BACKGROUND: Non-invasive ventilation (NIV) has been recommended as the best respiratory support for preterm infants with respiratory distress syndrome (RDS). However, the best NIV technique to be used as first intention in RDS management has not yet been established. Nasal intermittent positive pressure ventilation (NIPPV) may be synchronized (SNIPPV) or non-synchronized to the infant's breathing efforts. The aim of the study is to evaluate the short-term effects of SNIPPV vs. NIPPV on the cardiorespiratory events, trying to identify the best ventilation modality for preterm infants at their f

The application of esophageal pressure measurement in patients with respiratory failure.

This report summarizes current physiological and technical knowledge on esophageal pressure (Pes) measurements in patients receiving mechanical ventilation. The respiratory changes in Pes are representative of changes in pleural pressure. The difference between airway pressure (Paw) and Pes is a valid estimate of transpulmonary pressure. Pes helps determine what fraction of Paw is applied to overcome lung and chest wall elastance. Pes is usually measured via a catheter with an air-filled thin-walled latex balloon inserted nasally or orally. To validate Pes measurement, a dynamic occlusion test

Advanced Point-of-care Bedside Monitoring for Acute Respiratory Failure.

Advanced respiratory monitoring involves several mini- or noninvasive tools, applicable at bedside, focused on assessing lung aeration and morphology, lung recruitment and overdistention, ventilation-perfusion distribution, inspiratory effort, respiratory drive, respiratory muscle contraction, and patient-ventilator asynchrony, in dealing with acute respiratory failure. Compared to a conventional approach, advanced respiratory monitoring has the potential to provide more insights into the pathologic modifications of lung aeration induced by the underlying disease, follow the response to therap

Breathing pattern and thoracoabdominal motion during exercise in chronic obstructive pulmonary disease.

Subjects with chronic obstructive pulmonary disease (COPD) present breathing pattern and thoracoabdominal motion abnormalities that may contribute to exercise limitation. Twenty-two men with stable COPD (FEV1 = 42.6 +/- 13.5% predicted; age 68 +/- 8 years; mean +/- SD) on usual medication and with at least 5 years of diagnosis were evaluated at rest and during an incremental cycle exercise test (10 watts/2 min). Changes in respiratory frequency, tidal volume, rib cage and abdominal motion contribution to tidal volume and the phase angle that measures the asynchrony were analyzed by inductive r

Leakage estimation using Kalman filtering in noninvasive mechanical ventilation.

Noninvasive mechanical ventilation is today often used to assist patient with chronic respiratory failure. One of the main reasons evoked to explain asynchrony events, discomfort, unwillingness to be treated, etc., is the occurrence of nonintentional leaks in the ventilation circuit, which are difficult to account for because they are not measured. This paper describes a solution to the problem of variable leakage estimation based on a Kalman filter driven by airflow and the pressure signals, both of which are available in the ventilation circuit. The filter was validated by showing that based

Noninvasive ventilation and the upper airway: should we pay more attention?

In an effort to reduce the complications related to invasive ventilation, the use of noninvasive ventilation (NIV) has increased over the last years in patients with acute respiratory failure. However, failure rates for NIV remain high in specific patient categories. Several studies have identified factors that contribute to NIV failure, including low experience of the medical team and patient-ventilator asynchrony. An important difference between invasive ventilation and NIV is the role of the upper airway. During invasive ventilation the endotracheal tube bypasses the upper airway, but durin

Reverse Triggering With and Without Breath Stacking During Pressure-Controlled Ventilation: Exposure Burden and Associations With Ventilation Outcomes.

BACKGROUND: Reverse triggering is a complex form of asynchrony with different phenotypes. The exposure burden and clinical outcomes of reverse triggering during pressure-controlled ventilation (PCV) are underexplored. We aimed at determining the incidence of reverse triggering and characterizing the impact of breath-stacking. METHODS: This was a prospective, observational analysis. Adult subjects with PCV under long-term monitoring were included. The primary outcome was duration of invasive mechanical ventilation. Secondary outcomes included changes in mechanics and tidal volumes associated w

Work of Breathing by the Campbell Diagram: Physiology and Practice.

The work of breathing (WOB) quantifies the energy cost of ventilation. This tutorial explores the fundamental physiology of WOB, its measurement via the Campbell diagram, and its bedside clinical application. The diagram visually partitions WOB into elastic (lung and chest wall) and resistive components, offering a physiological phenotype of respiratory failure. We describe how pathologies like ARDS and COPD alter the load-capacity balance: reduced compliance drastically increases elastic work (flattening the pressure-volume loop), while air flow obstruction increases resistive work (widening

Assessing mechanical ventilation asynchrony through iterative airway pressure reconstruction.

BACKGROUND AND OBJECTIVE: Respiratory mechanics estimation can be used to guide mechanical ventilation (MV) but is severely compromised when asynchronous breathing occurs. In addition, asynchrony during MV is often not monitored and little is known about the impact or magnitude of asynchronous breathing towards recovery. Thus, it is important to monitor and quantify asynchronous breathing over every breath in an automated fashion, enabling the ability to overcome the limitations of model-based respiratory mechanics estimation during asynchronous breathing ventilation. METHODS: An iterative ai

Interest of monitoring diaphragmatic electrical activity in the pediatric intensive care unit.

The monitoring of electrical activity of the diaphragm (EAdi) is a new minimally invasive bedside technology that was developed for the neurally adjusted ventilatory assist (NAVA) mode of ventilation. In addition to its role in NAVA ventilation, this technology provides the clinician with previously unavailable and essential information on diaphragm activity. In this paper, we review the clinical interests of EAdi in the pediatric intensive care setting. Firstly, the monitoring of EAdi allows the clinician to tailor the ventilatory settings on an individual basis, avoiding frequent overassista

Basic concepts of mechanical ventilation: Normal breathing pattern.

Mechanical ventilation is a cornerstone in the management of critically ill pediatric patients, ensuring adequate gas exchange and reducing the work of breathing. Its appropriate use requires a solid understanding of respiratory physiology, lung mechanics, and patient-ventilator interaction. This article reviews the basic concepts of mechanical ventilation, including respiratory system anatomy and physiology, as well as key mechanical properties (compliance, resistance, and time constant) that determine respiratory dynamics. Ventilatory modes are classified according to control variable, breat

Artificial Intelligence Algorithm to Monitor Inspiratory Muscle Effort and Patient-Ventilator Dyssynchrony During Mechanical Ventilation.

OBJECTIVE: Current methods for estimating inspiratory muscle pressure ( Pmus ) during mechanical ventilation are either invasive or dependent on occlusion maneuvers. A noninvasive artificial intelligence (AI) algorithm estimating in real-time the amplitude and timing of Pmus , enabling continuous monitoring of patient effort, driving pressure, and synchrony with the ventilator was designed, and its performance was evaluated against the gold standard obtained with esophageal manometry ( Pmus,es ). DESIGN: A prospective diagnostic accuracy study. SETTING: Two ICUs from the University of São Pa

Complementary home mechanical ventilation techniques. SEPAR Year 2014.

This is a review of the different complementary techniques that are useful for optimizing home mechanical ventilation (HMV). Airway clearance is very important in patients with HMV and many patients, particularly those with reduced peak cough flow, require airway clearance (manual or assisted) or assisted cough techniques (manual or mechanical) and suctioning procedures, in addition to ventilation. In the case of invasive HMV, good tracheostomy cannula management is essential for success. HMV patients may have sleep disturbances that must be taken into account. Sleep studies including complete

Prolonged neural expiratory time induced by mechanical ventilation in infants.

Mechanical ventilation may interfere with the spontaneous breathing pattern in infants because they have strong reflexes that play a large role in the control of breathing. This study aimed to answer the following questions: does a ventilator-assisted breath 1) reduce neural inspiratory time, 2) reduce the amplitude of the diaphragm electrical activity, and 3) prolong neural expiration, within the delivered breath? In 14 infants recovering from acute respiratory failure (mean age and weight were 2.3 +/- 1.3 mo and 3.95 +/- 0.82 kg, respectively), we measured 1) the electrical activity of the d
