"Torsade de pointes" in a patient with variant angina.

Ventricular tachyarrhythmias have been well documented in patients with variant angina. Episodes of torsade de pointes have been described infrequently. We report a case of a 60-year-old male with a previous history of one vessel artery disease and a successful coronary angioplasty with stenting of the left anterior descending artery, who experienced an episode of angina at rest and electrocardiographic evidence of self-terminating torsade de pointes. After a negative coronary angiography and a positive hyperventilation test, the diagnosis of variant angina was considered and beta-blockers dis

Long QT syndromes and torsade de pointes.

In the long QT syndromes (LQTS), malfunction of ion channels impairs ventricular repolarisation and triggers a characteristic ventricular tachyarrhythmia: torsade de pointes. Symptoms in the LQTS (syncope or cardiac arrest) are caused by this arrhythmia. In congenital LQTS, mutations in the genes encoding for ion channels cause this channel malfunction. Six genotypes (LQT1 to LQT6) have been identified, and attempts are being made to correlate different mutations with clinical signs and specific therapy. In acquired LQTS, channel malfunction is caused by metabolic abnormalities or drugs. The l

Torsade de pointe. Report of 18 cases.

Eighteen patients with long QT interval and recurrent ventricular tachycardia, torsade de pointe, were studied. Fourteen patients had an underlying heart disease. The factors involved in the appearance of this arrhythmia were: antiarrhythmic drugs (55.5%), complete heart block (33.5%), Romano-Ward syndrome (5.5%) and hypokalemic periodic paralysis (5.5%). In drug-induced torsade de pointe the QT interval was moderately prolonged. Seven patients (70%) had hypokalemia and 5 patients were bradycardic (50%). In patients with heart block-induced torsade de pointe, the QT interval was markedly prolo

[Torsade de pointes and use of phenothiazine and diuretic].

A 54-year-old woman with recurrent episodes of syncope fainted on admission. An episode of torsade de pointes was registered on electrocardiographic monitoring. We assume that the combined effects of phenothiazine and of hypokalemia (2.6 meq/l) caused by chlorthalidone provoked the condition. Diuretics should be used with caution in patients on phenothiazines.

Torsade de pointes during loading with amiodarone.

Torsade de pointes represents a potential complication of chronic amiodarone therapy. Several reports have emphasized the need for a loading dose in order to achieve therapeutic blood levels rapidly. We report a case of torsade de pointes following a single oral dose of amiodarone (1400 mg or 30 mg kg-1) administered after short intravenous loading for prevention of paroxysmal atrial flutter. Torsades de pointes were preceded and associated with marked QT prolongation and bradycardia. This report suggests that careful monitoring of patients undergoing oral amiodarone loading is necessary.

[Treatment of torsade des pointes by intravenous magnesium].

A 72 year old woman was admitted with decompensation of a hypertensive cardiopathy and treated with diuretics. She developed recurrent syncopal torsades de pointes during the 24th hour which were reduced by a bolus intravenous injection of 3 g of magnesium sulphate (Mg SO4). There was a recurrence 30 minutes later which regressed after a second injection of 3 g of Mg SO4. A continuous intravenous infusion of 18 g/day of Mg SO4 prevented further recurrences of the arrhythmia. Biochemical analysis showed intra and extracellular magnesium deficiency at the time of the torsades de pointes but the

Sotalol, hypokalaemia, syncope, and torsade de pointes.

Thirteen patients developed syncope and a prolonged QTc interval while taking therapeutic doses of sotalol. Polymorphous ventricular tachycardia was observed in 12 patients, and criteria typical of torsade de pointes were present in 10. In 12 patients sotalol had been given with hydrochlorothiazide in a combined preparation, Sotazide, but with inadequate or no potassium supplementation. Serum potassium concentrations were reduced in eight patients. Four patients were taking other drugs known to prolong the QT interval, including disopyramide (three patients) and tricyclic antidepressants (two

Torsades de Pointe Associated with Trazodone Consumption.

INTRODUCTION: Trazodone is a serotonin receptor antagonist and reuptake inhibitor commonly used to treat major depression disorder (MDD), anxiety, and sleep disorders. It is considered safe for the heart due to minimal anticholinergic effects. Prolonged QT intervals can cause polymorphic ventricular tachycardia, known as torsades de pointe (TdP). We present a case of a 67-year-old female with a history of MDD who developed trazodone-induced TdP. Case Presentation. The patient was referred to a tertiary hospital with a ten-hour history of nausea and vomiting. Trazodone (50 mg daily) was started

Torsade de Pointes Due to Hypokalemia and Hypomagnesemia.

AUDIENCE: This scenario was developed to educate emergency medicine (EM) interns but can be used to educate medical students and junior residents. INTRODUCTION: Torsade de Pointes (TdP) is a rare but potentially fatal arrythmia if not quickly diagnosed and properly treated. TdP is defined as a polymorphic ventricular tachycardia (VT) characterized by an oscillatory change in amplitude around an isoelectric line that is associated with a QTc prolongation on the electrocardiogram (ECG).1 It has been well described to predispose to ventricular fibrillation and arrhythmic death. QTc prolongation

Pentavalent antimonial-induced torsade de pointes.

Unexplained sudden deaths following use of pentavalent antimonial drugs have been described, presumably owing to ventricular arrhythmias. A patient is reported with QT interval prolongation and syncopal episodes related to torsade de pointes following treatment of visceral leishmaniasis with meglumine antimoniate. This is the first reported case of documented torsade de pointes following treatment with pentavalent antimonial drugs.

[Iatrogenic torsade de pointes induced by thioridazine].

Torsade de pointes is a form of polymorphous ventricular tachycardia in which the polarity of the QRS complex exhibits phasic alterations in both axis. Traditionally, torsade de pointes has been described in association with a congenital or acquired (including drug and metabolic) causes of QT prolongation. Clinical outcomes range from asymptomatic, self-terminating arrhythmias to ventricular fibrillation resulting in cardiac arrest. For the treatment of torsade de pointes, the conventional antiarrhythmic drugs cannot be relied on, cardiac pacing should be instituted as soon as possible; howeve

Torsade de pointes induced by sotalol and hypokalaemia.

The problem of a sotalol-induced ventricular arrhythmia (torsade de pointes) is presented and the contributory role of hypokalaemia is emphasized. It is concluded that sotalol may not be a suitable beta-blocker to use with a diuretic.

[Torsade de pointes. Apropos of 54 cases].

A retrospective study of 54 torsades de pointe cases in a cardiology department enabled us to specify the main characteristics of this serious arrythmia often observed in intensive care units: --the diagnostic criteria: more than the pattern of tachycardia attack, late ventricular premature beats and particularly QT prolongation are necessary for proper diagnosis. These two criteria allow us to differentiate between torsades de pointe and multiform ventricular tachycardia with similar morphology especially in acute myocardial ischaemia; --their clinical repercussion: the shortness of circulato

Iatrogenic 'torsade de pointes' ventricular tachycardia.

Three patients who developed a distinctive form of ventricular tachycardia with oscillating QRS axis, while they were receiving drugs known to prolong the repolarization time are described. In one of the patients suffering from psychiatric illness and receiving psychotropic drugs the arrhythmia was fatal. It is postulated that the tendency to this arrhythmia was augmented by repeated electric counter shocks and negative bathmotropic drugs. Functional factors may contribute to the pathogenesis of this arrhythmia which seems to constitute an entity.
