INTRODUCTION: This study examined how apathy symptoms and awareness of those symptoms relate to resting-state functional connectivity magnetic resonance imaging (rs-fcMRI) across the Alzheimer's disease (AD) continuum using network segregation metrics.
METHODS: There were eighty-three participants, including cognitively unimpaired (CU; n = 46), cognitively impaired (CI; mild cognitive impairment or AD dementia, n = 21), and mild behaviorally impaired (MBI; n = 16) individuals. Apathy was measured using the Apathy Evaluation Scale (AES)-Self, AES-Informant, and discrepancy scores (informant mi

BackgroundIn Parkinson's disease, abnormalities in reward-effort trade-offs are thought to underlie both apathy and bradykinesia. However, it remains unclear whether effort cost computations contribute to these symptoms independently of reward, and which exact effort costs are involved.ObjectiveTo assess whether effort-based decision-making in the absence of any explicit rewards differs between Parkinson patients and healthy controls and to explore whether putative differences are related to apathy or bradykinesia.MethodsTo address this, we developed a novel decision-making task that isolated

Apathy is a common neuropsychiatric symptom (NPS) in Alzheimer's disease (AD) but can emerge earlier in prodromal and even preclinical stages as part of mild behavioural impairment (MBI-apathy), a syndrome defined by emergent and persistent NPS. In dementia, apathy is associated with higher morbidity, mortality and caregiver distress. However, the significance of MBI-apathy in dementia-free persons, including its associations with AD biomarkers, remains unclear. This study aimed to determine whether MBI-apathy is associated with biomarker evidence of amyloid-beta (Aβ) and tau [phosphorylated (

Gender differences in progressive supranuclear palsy (PSP) may become relevant for clinical trials, treatment decisions and patient counseling. To study gender associated differences we conducted a retrospective data analysis of 191 male and 157 female PSP patients from a large multicenter observational cohort in Germany. While no differences in motor skills, disease severity, daily living abilities, global cognitive status and depressive symptoms were observed between genders, male patients showed significantly higher apathy scores, a finding also noted in other neurological diseases. In this

BACKGROUND: MRI guided laser interstitial thermal therapy (M-LITT) capsulotomy has proven to be efficacious in decreasing refractory obsessive-compulsive disorder (OCD) related symptomatology yet capsulotomy either via radiosurgery or radiofrequency ablation has in some patients led to increased apathy following surgery. The current case series aims to investigate objective patient-reported change in apathy, disinhibition, depression, and executive dysfunction following anterior capsulotomy via M-LITT for OCD.
METHODS: Ten consecutive patients pre- and post-M-LITT completed measures of OCD, a

We investigated the effects of transcranial alternating current stimulation (tACS) targeted to the bilateral medial prefrontal cortex (mPFC) and administered at either delta or alpha frequencies, on brain activity and apathy in people with Huntington's disease (HD) (n = 17). Given the novelty of the protocol, neurotypical controls (n = 20) were also recruited. All participants underwent three 20-min sessions of tACS; one session at alpha frequency (Individualised Alpha Frequency (IAF), or 10 Hz when an IAF was not detected); one session at delta frequency (2 Hz); and a session of sham tACS. Individuals living with human immunodeficiency virus type 1 (HIV-1) are often plagued by debilitating neurocognitive impairments and affective alterations;the pathophysiology underlying these deficits likely includes dopaminergic system dysfunction. The present review utilized four interrelated aims to critically examine the evidence for dopaminergic alterations following HIV-1 viral protein exposure. First, basal dopamine (DA) values are dependent upon both brain region andexperimental approach (i.e., high-performance liquid chromatography, microdialysis or fast-scan cyclic voltammetry). Seco

BACKGROUND: Apathy, a common neuropsychiatric disturbance in Huntington's disease (HD), is subserved by a complex neurobiological network. However, no study has yet employed a whole-brain approach to examine underlying regional vulnerabilities that may precipitate apathy changes over time.
OBJECTIVES: To identify whole-brain gray matter volume (GMV) vulnerabilities that may predict longitudinal apathy development in HD.
METHODS: Forty-five HD individuals (31 female) were scanned and evaluated for apathy and other neuropsychiatric features using the short-Problem Behavior Assessment for a max

Unemployment is common for those with Huntington's disease (HD), a genetic neurodegenerative disorder, and affects patients' quality of life. HD is characterized by motor disturbances, cognitive dysfunction, and psychiatric symptoms. The purpose of this article was to determine which clinical signs of HD are predictive of unemployment. Data for employed (N=114) and unemployed (N=106) HD mutation carriers were used to investigate group differences. Univariate logistic regression analyses, adjusted for age and gender, were performed to determine individual predictors of unemployment. Subsequentl

Neuropsychiatric symptoms are common non-motor symptoms in Parkinson's disease (PD). Apathy and impulse control disorders (ICD) are two opposite motivational expressions of a continuous behavioural spectrum involving hypo- and hyperdopaminergia. Both syndromes share pathological (decreased vs increased) dopamine receptor stimulation states. Apathy belongs to the spectrum of hypodopaminergic symptoms together with anhedonia, anxiety and depression. Apathy is a key symptom of PD which worsens with disease progression. Animal models, imaging and pharmacological studies concur in pointing out dopa

BACKGROUND: Apathy is an enduring and debilitating feature related to poor outcome in patients with first-episode psychosis (FEP). The biological underpinnings of apathy are unknown. We tested if FEP patients with persistent apathy (PA) differed from FEP patients without persistent apathy (NPA) in specific brain structure measures in the early phase of illness.
METHODS: A total of 70 Norwegian FEP patients were recruited within 1 year of first adequate treatment. They were defined as having PA (N=18) or NPA (N=52) based on Apathy Evaluation Scale score at baseline and 1 year later. MRI measur

BACKGROUND AND PURPOSE: This study aimed to reveal the structural basis of post-ischaemic stroke apathy, especially in relation to disruptions in structural connectivity.
METHODS: Eighty-eight participants were included. The Apathy Evaluation Scale, clinician version, was used to characterize the severity of apathy. Diffusion tensor imaging tractography was used to examine white matter integrity and to reconstruct white matter networks using 90 nodes based on the automated anatomical labeling atlas. The degree for each node was extracted to determine the relationship to the severity of apathy

INTRODUCTION: Overlaps in symptom presentation limits the capacity to predict functional impairment and future care needs in younger-onset dementia syndromes.
METHODS: A general additive model (GAM) was applied to cross-sectional retrospective data from 375 participants with younger-onset dementia; 152 behavioral-variant frontotemporal dementia (bvFTD), 118 Alzheimer's disease (AD), 66 semantic dementia, and 39 progressive nonfluent aphasia (PNFA). This GAM aimed to explore the dynamic interrelationships between established measures of global cognition, apathy, and functional impairment.

OBJECTIVES: Self-perceived cognitive difficulties are a potential early marker of Alzheimer's disease (AD), frequently co-occur with depressive symptoms. This has hindered consideration of these subjective cognitive difficulties as a risk factor for AD. However, accumulating evidence points to dissociations between specific depressive symptoms (i.e., apathy, affective or mood symptoms, and others), with some suggestion that apathy is particularly predictive of and more proximate to AD and may drive the relationship between depressive symptomatology and AD. This study examines whether apathy dr

OBJECTIVE: Apathy, defined as diminished self-initiated purposeful behavior, is common in Parkinson's disease (PD) and affects health-related quality of life (HRQoL). Although apathy is now recognized as a multidimensional syndrome, the contributions of total apathy and its dimensions to HRQoL, when mood and cognition are accounted for, remain unclear. The authors examined these relationships for individuals with PD.
METHODS: Twenty-eight adults with PD without dementia completed the Dimensional Apathy Scale (DAS), General Anxiety Disorder-7 (GAD-7), Beck Depression Inventory-II (BDI-II), Par

Apathy, or loss of motivation, is a prominent syndrome accompanying both Alzheimer's and Parkinson's diseases, in addition to other disorders. One approach to understanding motivational loss is to examine the processes underlying goal-directed behaviour. Weighing up rewarding outcomes against the effort costs required to obtain them (effort-based decision-making) is a core computation when deciding to act for outcomes. Although a growing body of evidence points to disruption of this computation in people with apathy, which underlying decision parameters drive this disruption, their neural asso

BACKGROUND: Apathy and depression are the most prevalent neuropsychiatric symptoms in amyotrophic lateral sclerosis (ALS). Although insufficiently investigated, their distinction holds important clinical relevance for accurate diagnosis of ALS with behavioural impairment and for patients' prognosis and management. In the present study, we aimed to assess both apathy and depressive symptoms in patients with ALS and whether they have similar or different functional neural correlates.
METHODS: Using graph analysis and connectomics, global and lobar nodal properties and regional functional brain

INTRODUCTION: The role of apathy in predicting dementia risk, especially with coexisting depression, remains unclear in community-dwelling older adults.
METHODS: We analyzed data from 12,646 older adults without baseline dementia across ten population-based cohorts from six continents (Cohort Studies of Memory in an International Consortium study). Apathy, depression, and dementia outcomes were assessed using harmonization protocols.
RESULTS: Over 5.7 years of follow-up, 1160 developed dementia and 552 Alzheimer's disease (AD). Apathy prevalence was 34.1% and more than three times higher in

Background and Objectives: Apathy and sleep problems are frequently observed among non-motor symptoms (NMSs) in Parkinson's disease (PD), but the relationship between the two has not been well investigated. This study aimed to determine the extent to which apathy and sleep disturbances are present in people with early PD and whether apathy affects sleep quality. Materials and Methods: Patients diagnosed with early PD, defined as modified Hoehn and Yahr (mHY) stages 1-3 and a disease duration of no more than 5 years, were included in the study. Demographic characteristics were collected, and mo

INTRODUCTION: Emerging evidences support the contribution of cholinergic deficiency to apathy in Parkinson's disease (PD). We aimed to ascertain the role of structural alterations of cholinergic basal forebrain (BF) and its innervated cortical regions in the pathogenesis of apathy in PD.
METHODS: Twenty-one PD patients with apathy (PD-A), 28 without apathy (PD-NA), and 20 healthy controls (HCs) were recruited in this study. Changes in subregional volumes of the BF were compared. Cortical thickness and local gyrification index (LGI) analysis were adopted to reveal the concomitant cortical alte
