Equinus deformity is a common cause of foot and ankle pathology. The purpose of our study was to evaluate the role of the plantaris in equinus. Secondary aims were to describe the role of the plantaris in intramuscular gastrocnemius recession and to determine the prevalence of the plantaris in our patient population. We measured ankle dorsiflexion during the steps of a Baumann-type intramuscular gastrocnemius recession. Eighty-nine patients were enrolled in our study. Fourteen of 89 (15.7%) patients did not have a plantaris. A mean dorsiflexion of 9 (interquartile range 6-12)° was obtained aft

The aponeurosis lengthening described in this study was performed on 156 patients (219 procedures) with spastic equinus deformities. There was only one wound complication and no calcaneus deformities from overlengthening. The recurrence rate requiring relengthening was high (48% of the procedures), particularly if the initial operation was performed before the age of 5 years. The length of postoperative immobilization did not affect the recurrence rate. The aponeurosis lengthening technique described is a simple lengthening of the aponeurosis, leaving the soleus largely intact without the use

Adult acquired flatfoot represents a spectrum of deformities affecting the foot and the ankle. The flexible, or nonfixed, deformity must be treated appropriately to decrease the morbidity that accompanies the fixed flatfoot deformity or when deformity occurs in the ankle joint. A comprehensive approach must be taken, including addressing equinus deformity, hindfoot valgus, forefoot supinatus, and medial column instability. A combination of osteotomies, limited arthrodesis, and medial column stabilization procedures are required to completely address the deformity.

BACKGROUND: Equinus is a common deformity in children with bilateral spastic cerebral palsy (BSCP). While dynamic equinus usually is treated by conservative therapy, fixed contractures need surgical correction. To choose the appropriate surgical method, it is important to discriminate between isolated gastrocnemius shortening and combined gastrosoleus complex contracture.
METHODS: In a retrospective study 938 patients with BSCP were studied. Patients underwent gait analysis and clinical examination. 248 patients (496 limbs) met the inclusion criteria. Data from motion analysis and clinical ex

A 47-year-old woman presented with a disabling fixed flexion deformity of the knee and an equinus deformity of the ankle following treatment in a cast for a minor flexion deformity following a tibial plateau fracture. Two years later, the cause was identified as an isolated ischaemic contracture involving the medial head of gastrocnemius. She made a good recovery following extensive corrective surgery and rehabilitation.

Neuromuscular disease commonly affects the rearfoot as equinus, equinovarus, and equinovalgus deformity. Spastic hemiplegia caused by stroke, head injury, and cerebral palsy results in equinovarus deformity of the rearfoot. Spastic diplegia, most frequently caused by cerebral palsy, results in equinovalgus rearfoot deformity. Problems in ambulation, footwear, and bracing, as well as their orthopedic management, in patients with neuromuscular disease are discussed in a case-report format.

Treatment of joint contractures after burn injury is challenging because various factors influence the treatment options including the lack of local tissue, significant fibrotic changes with associated loss of joint pliability, increased prevalence of infection in burn patients, and patient-related medical comorbidities. We present a case of a 60-year-old woman who had severe bilateral postburn ankle equinus deformities after self-immolation in the setting of significant soft tissue injury, fibrosis, and loss of joint movement. This deformity deprived her of the ability to walk, even aided, and

This letter comments on the article "Percutaneous needle tenotomies: indications, procedures, efficacy and safety. A systematic review" by H. Bessaguet et al., published in Annals of Physical and Rehabilitation Medicine (June 2024). We highlight the omission of our study, "Percutaneous Lengthening with an Intramuscular Needle of the Gastrocsoleus Complex Improves Critical Ankle Kinematic Values in Resistant Pediatric Equinus: A Pilot Study," which was published in 2023. We consider that our study is highly relevant, as it presents a minimally invasive needle technique assessed through 3D gait

Gastrocnemius proximal lengthening was first performed to correct spasticity in children, and was adapted for the patient with no neuromuscular condition in the late 1990s. Since then, the proximal gastrocnemius release has become less invasive and has evolved to include only the fascia overlying the medial head of the gastrocnemius muscle. The indications for performing this procedure are a clinically demonstrable gastrocnemius contracture that influences a variety of clinical conditions in the forefoot, hindfoot, and ankle. It is a safe and easy procedure that can be performed bilaterally si

Congenital talipes equinovarus (CTEV), or clubfoot, is the the most common encountered musculoskeletal defect encountered at birth. Most cases present as an isolated deformity, with up to half of them presenting with bilateral deformity. CTEV have also been reported to present as part of syndromic phenomena. Dr Igancio Ponseti proposed a serial casting programme to correct the foot's cavus, forefoot adduction, varus and equinus. Up to 90% of infants will require a tendo-achilles (TA) tenotomy for the persisting equinus deformity. TA tenotomy is deemed a relatively safe procedure, with the most

Medical records of 59 patients with cerebral palsy were reviewed retrospectively to evaluate results of tendoachilles lengthening. Surgical lengthening resulted in highly significant (p < 0.0001) initial average gains in dorsiflexion compared with baseline. These average improvements maintained their statistical significance for seven years postoperatively. The arc of motion was not significantly different postoperatively. The initial ambulatory level was improved in 55% of the patients and maintained in the remainder. However, 14 ankles (11.9%) in eight patients (13.6%) required repeat tendoa

Lower extremity trauma is very often managed by applying an external fixator to stabilize the fracture site. These limbs very frequently require multiple surgeries, which predisposes them to problems like heel pressure sore, equinus deformity and limb edema. These complications prolong the healing time and create obstacles in rehabilitation. Our aim was to modify the usual uniplanar external fixator in such a way that it is able to provide stable limb elevation and also keep the leg and the foot in such a position, so as to avoid the above-mentioned complications. We used our frame in 10 patie

Endoscopic gastrocnemius release (EGR) is a recently developed procedure that is a reliable option for surgical management of ankle equinus contracture. Comfort with endoscopic equipment and surgical anatomy, especially the sural nerve, is of paramount importance for performing the procedure safely, effectively, and efficiently. The primary advantage of the procedure is improved cosmesis and decreased wound complications of the smaller surgical scars. The current body of literature of clinical outcomes for EGR consists of limited level IV case series with broad variations in study rigor, autho

The equinocavovarus deformity that results from tibialis posterior musculotendinous contracture poses a treatment challenge that has few reliable conservative or surgical remedies. In this techniques report, the author describes a tibialis posterior recession procedure that uses the sequential use of topographic anatomical landmarks, which can be useful in correcting contracture of the tibialis posterior musculotendinous unit.

Haemophilic contracture is seen most commonly as an equinus deformity of the ankle, or at the knee or elbow in the form of a flexion deformity. Treatment options are varied, and decision-making is based on the degree of the contracture, its chronicity, the presence of articular subluxation, the patient's ability to participate in treatment, and the available medical facilities. The treatments available fall into four categories: physiotherapy, orthotics, corrective devices, and surgical procedures. Treatment should be primarily by physiotherapy, splintage, and corrective devices. The late or s

Soft-tissue ankle equinus contracture is an important component of numerous foot and ankle deformities. In high-risk patients who have multiple co-morbidities, procedure selection and careful surgical technique are paramount to increase the likelihood of postoperative success. This article discusses the indications for percutaneous Achilles tendon lengthening, open gastrocnemius recession, and endoscopic gastrocnemius recession, and provides a detailed description of each surgical technique with pearls to avoid intra-perative and postoperative complications specific to the high-risk patient.
