OBJECTIVE: To describe a rare case of a heterotopic pregnancy with two gestational sacs in the cervix and one in the uterine cavity.
DESIGN: Case report.
SETTING: Tertiary university hospital.
PATIENT(S): A 45-year-old woman was diagnosed with a triplet gestation 7 weeks following IVF treatment for primary infertility of 5 years' duration. Transvaginal ultrasound scan revealed three gestational sacs: one sac inside the uterine cavity containing a live fetus, and two sacs in the uterine cervix, one containing a live fetus and a second empty sac.

A 34-year-old woman was diagnosed by ultrasound with a six-week ectopic pregnancy with implantation of the gestational sac in the bladder wall. She was initially treated with methotrexate 50 mg/m(2) i.m. Although a 22% reduction in the levels of beta-hCG was observed at first, there was a rupture of gestational sac. The patient underwent an emergency laparotomy with a resulting partial cystectomy, and made an uneventful recovery.

The correlation of transvaginal ultrasound (TVU) findings with serial hCG determinations was evaluated during early pregnancy in 60 patients. In these patients, the time of ovulation was accurately determined by ultrasonic monitoring of follicular rupture. We report three cases of first-trimester Cesarean scar pregnancy (CSP) treated with evacuation therapy. In one patient with a gestational age of 7 weeks and with deep sac implantation, there was massive uterine bleeding and uterine perforation following evacuation. BACKGROUND: Intra-uterine pregnancy coexisting with cervical pregnancy (heterotopic pregnancy) is a rare condition and its management is challenging because of the massive bleeding associated with cervical pregnancy. Uterine artery embolization followed by hysteroscopic removal of cervical and intra-uterine products of conception can theoretically prevent massive bleeding and provide a direct view during the removal.
