Using the gravity model to estimate the spatial spread of vector-borne diseases.

The gravity models are commonly used spatial interaction models. They have been widely applied in a large set of domains dealing with interactions amongst spatial entities. The spread of vector-borne diseases is also related to the intensity of interaction between spatial entities, namely, the physical habitat of pathogens’ vectors and/or hosts, and urban areas, thus humans. This study implements the concept behind gravity models in the spatial spread of two vector-borne diseases, nephropathia epidemica and Lyme borreliosis, based on current knowledge on the transmission mechanism of these dis

[Reduced muscular oxygen tension and nerve impulse transmission from antishock hose. Reduction of oxygen tension in the tibial muscle and impulse transmission in the peroneal nerve from pneumatic -1 pressure from antishock hose].

OBJECT OF THE STUDY: The aim of the study was to assess, whether the pneumatic pressure of an antishock-trouser (AST) of 20-40 mm Hg induces a decreased oxygenation of the anterior tibial muscle and attenuates muscular response potential (MRP) of n. peronaeus profundus? METHODS: Among 22 normotensive, healthy volunteers the AST were tested by applying pressure values between 0 and 100 mm Hg and measuring the intracompartmental pressure, the muscular oxygen pressure as well as the MRP by electroneurographic means within a period of 6 hours. RESULTS: The median initial intracompartmental press

G-induced loss of consciousness: retrospective survey results from 2259 military aircrew.

INTRODUCTION: Prevalence of G-induced loss of consciousness (G-LOC) in the United Kingdom Royal Air Force (RAF) was found to be 19.3% in 1987. With the introduction of the Typhoon, a fourth generation aircraft, the prevalence of G-LOC has been re-assessed to determine the effectiveness of current G tolerance training. METHOD: A survey was sent to 4018 RAF aircrew, irrespective of their current role. Information was requested on G-LOC, role and aircraft type, experience, and attitudes toward G-LOC prevention. RESULTS: Responses were received from 2259 (56.2%) individuals, 882 (39%) of whom we

Anterior tibial compartment pressures during intermittent sequential pneumatic compression therapy.

We studied the anterior tibial compartment pressures during the application of a JOBST sequential intermittent pneumatic compression device on 5 healthy human volunteers (10 legs). Intracompartmental pressures were measured using a slit catheter. The measurements of interstitial pressures were highest at maximal calf inflation, and pressures were increased for approximately 120 seconds during each cycle. Pressure measurements in the inflated pressure sleeve varied less than 10% with the measured anterior tibial compartment pressures during intermittent pneumatic compression therapy. This inter

Delayed orthostatic intolerance.

In seven patients who presented with lightheadedness, fatigue, "weakness," and sometimes syncope, blood pressure was found not to fall after standing for 3 to 4 minutes but to fall severely, frequently with syncope or presyncopal symptoms, after 13 to 30 minutes when measured every minute with an automatic device. This delayed orthostatic hypotension could be corrected with inflation of a pressure suit to 45 mm Hg. Its mechanism was further investigated with measurements of plasma catecholamines, plasma cortisol and aldosterone responses to corticotropin, and the effects of norepinephrine infu

Vascular injuries associated with pelvic fractures.

Pelvic fractures, which are most often caused by blunt abdominal trauma in our motor vehicle-oriented society, continue to be associated with significant mortality and morbidity. Hemorrhage is the cause of death in nearly 60 per cent of those patients who die of pelvic fractures. With increasing awareness of the problem and improved methods of management, the mortality rate of acute hemorrhage secondary to pelvic fracture should decrease. Four cases of vascular injuries associated with severe pelvic fractures are discussed. One patient presented with bleeding from a false aneurysm of the super

Venous thrombosis after elective hip replacement--the influence of preventive intermittent calf compression and of surgical technique.

The effect of preventive intermittent calf compression on the incidence, distribution and extent of venous thrombosis after elective hip replacement was examined by randomized trial in 90 patients who were screened for postoperative thrombosis with 125I-fibrinogen leg scanning and impedance plethysmography, followed by routine venography on the seventh postoperative day. Venography showed that leg compression reduced the incidence of calf vein thrombosis from 45 per cent (21/47) in untreated patients to 16 per cent (7/43) (P less than 0.005), but not that of proximal (i.e. popliteal or femoral

Aspects of the management of shock.

Shock is a state of systemic imbalance between supply and demand for oxygenated blood. We discuss here management of shock states not primarily of cardiac origin. Inadequacy or maldistribution of blood volume is important in the pathogenesis of most forms of shock. The Military Anti-Shock Trouser (MAST) suit assists the redistribution of blood from the periphery to the central circulation. Normal saline and Ringer's lactate solution may be used interchangeably for acute volume replacement. Colloid replacement requires smaller volumes but carries a higher risk of pulmonary edema, whereas crysta

No effect of venoconstrictive thigh cuffs on orthostatic hypotension induced by head-down bed rest.

Orthostatic intolerance (OI) is the most serious symptom of cardiovascular deconditioning induced by head-down bed rest or weightlessness. Wearing venoconstrictive thigh cuffs is an empirical countermeasure used by Russian cosmonauts to limit the shift of fluid from the lower part of the body to the cardio-cephalic region. Our aim was to determine whether or not thigh cuffs help to prevent orthostatic hypotension induced by head-down bed rest. We studied the effect of thigh cuffs on eight healthy men. The cuffs were worn during the day for 7 days of head-down bed rest. We measured: orthostatic

Cost-effective prevention of pulmonary embolus in high-risk trauma patients.

OBJECTIVE: To define the cost-effectiveness of screening ultrasound (US) and prophylactic inferior vena cava filters (VCF), approaches aimed at reducing the incidence of pulmonary embolus (PE) in high-risk trauma patients. DESIGN: Cost-effective analysis. MATERIALS AND METHODS: We constructed a decision tree with three approaches for PE prevention: no intervention, US, and VCF. Probabilities in each subtree were taken from published data. Sensitivity analyses evaluated all assumptions, probabilities, and outcomes for effects on baseline conclusions. RESULTS: US is more cost-effective than V

Venous thromboembolism in the high-risk trauma patient: do risks justify aggressive screening and prophylaxis?

OBJECTIVE: Deep venous thrombosis (DVT) and pulmonary embolism (PE) are considered to be a major source of morbidity and mortality among trauma patients. Several reports have identified high-risk patients with recommendations for management ranging from frequent duplex scanning to placement of prophylactic inferior vena cava (IVC) filters. We reviewed our experience with a large trauma population to determine whether such approaches are justified. METHODS: We analyzed 2,868 consecutive trauma admissions over 22 months and identified 280 patients (10%) in high-risk groups who survived > or = 4

Effects of intermittent pneumatic calf compression in normal and postphlebitic legs.

The changes in common femoral vein flow produced by three different intermittent pneumatic compression devices were recorded with a Doppler velocity detector in 20 normal legs and 20 with postphlebitic syndrome. Mean and peak velocity increases were measured and expressed as a percent of resting baseline values. There was no significant difference in the peak velocity increase produced by the three devices in normals and abnormals; however, there were differences in the mean velocity increases. The devices worked as well on postphlebitic legs as on normal ones. Correct cuff application was mor

Management of orthostatic hypotension in the elderly.

Orthostatic hypotension (OH) is a common problem in old age that can seriously affect the quality of life, leading to a progressive decline in health. In the elderly, the literature suggests that multiple causes predominate. This article proposes a stepped-care approach to the management of OH. A stepped-care therapy applied appropriately allows maximum benefit with the least number of drugs, thereby minimizing side effects.

Echocardiographic evaluation of left ventricular function in critically ill patients: dynamic loading challenge using medical antishock trousers.

STUDY OBJECTIVE: We hypothesized that a dynamic left ventricular (LV) evaluation during a loading challenge might enhance diagnostic capabilities of routine transesophageal echocardiography in critically ill patients and selection of therapeutic options against circulatory failure, particularly the choice between volume expansion and vasoactive agent infusion. DESIGN: Prospective clinical study in a group of 26 patients requiring hemodynamic support by vasoactive infusion because of low systemic arterial pressure (< 90 mm Hg by invasive monitoring) during mechanical ventilation. Hemodynamic alterations in venous blood flow produced by external pneumatic compression.

Venous stasis associated with prolonged bed rest can enhance the risk of deep venous thrombosis (DVT). Pneumatic compression of the lower extremities can reduce this risk by preventing venous stasis. When selecting a method of leg compression for their patients, physicians must chose between two distinctly different types of compression devices. One device applies pressure with a single-chambered sleeve to the below knee region while the other applies pressure in a sequential gradient fashion from the ankle to the thigh. The current prospective study was designed to evaluated the ability of tw

[Man in the condition of weightlessness (author's transl)].

In the space flights by the USA and USSR to date adequate experience has been obtained on the adaptation of man to living conditions in space--with the technical assistance of an artificial protective atmosphere in the space ship or space suit--and to the singular factor of "weightlessness" (microgravity, low gravity) and how he re-adjusts again after returning to a gravitational field. There is no ultimate or absolute physiological barrier for long term space flight--with one exception: decalcification of the skeleton. For short space flights, however, space sickness is a serious disorder.
