The role of noninitial clusters in the Children's Test of Nonword Repetition: Evidence from children with language impairment and typically developing children.

The Children's Test of Nonword Repetition (CNRep) is one of the most popular tests of nonword repetition. The test is composed of nonwords of different length, and normative data suggest that children experience more difficulties in repeating long nonwords. An analysis of the distribution of phonological clusters in the test shows that noninitial clusters are unequally distributed in the test: They only appear in long nonwords (four and five syllables). For this reason, we hypothesized that the difficulties children encounter with long nonwords may be influenced by the phonological complexity

[The validity of the SET 5-10 (language level test for children aged between five and ten years): first analyses].

INTRODUCTION: No other child developmental domain is as frequently affected by disorders as language acquisition. The SET 5-10 (language level test for children aged between 5 and 10 years), was developed to assess specific language disorders. OBJECTIVE: This study examines the ability of the SET 5-10 to differentiate between the developmental increases of speech competence, present deficits resulting from previous speech deficits and the probability of assessing possible speech deficits of children with an immigrant background. METHODS: Based on data of the norm sample (n=1,052; 51.8% femal

MRI language dominance assessment in epilepsy patients at 1.0 T: region of interest analysis and comparison with intracarotid amytal testing.

The primary goal of this study was to test the reliability of presurgical language lateralization in epilepsy patients with functional magnetic resonance imaging (fMRI) with a 1.0-T MR scanner using a simple word generation paradigm and conventional equipment. In addition, hemispherical fMRI language lateralization analysis and region of interest (ROI) analysis in the frontal and temporo-parietal regions were compared with the intracarotid amytal test (IAT). Twenty epilepsy patients under presurgical evaluation were prospectively examined by both fMRI and IAT. The fMRI experiment consisted of

Exploring the potential of large language models in identifying metabolic dysfunction-associated steatotic liver disease: A comparative study of non-invasive tests and artificial intelligence-generated responses.

BACKGROUND AND AIMS: This study sought to assess the capabilities of large language models (LLMs) in identifying clinically significant metabolic dysfunction-associated steatotic liver disease (MASLD). METHODS: We included individuals from NHANES 2017-2018. The validity and reliability of MASLD diagnosis by GPT-3.5 and GPT-4 were quantitatively examined and compared with those of the Fatty Liver Index (FLI) and United States FLI (USFLI). A receiver operating characteristic curve was conducted to assess the accuracy of MASLD diagnosis via different scoring systems. Additionally, GPT-4V's poten

A Proof-of-Concept Study of Language-Stratified Assessment for Minimal Hepatic Encephalopathy: Integrating the Animal Naming Test and Serum IL-6.

Minimal hepatic encephalopathy (MHE) involves subtle cognitive dysfunction and systemic inflammation and is associated with an increased risk of overt hepatic encephalopathy. The Animal Naming Test (ANT1) is a rapid semantic fluency tool for MHE assessment, but its performance across different primary spoken languages remains unclear. We conducted a prospective proof-of-concept study to evaluate the diagnostic performance of ANT1 and serum interleukin-6 (IL-6) in Mandarin- and Taiwanese Hokkien-speaking cirrhotic patients. A total of 65 cirrhotic patients and 34 healthy controls were enrolled.

Evaluating large language models as clinical laboratory test recommenders in primary and emergency care: a crucial step in clinical decision making.

OBJECTIVES: Large language models (LLMs), such as OpenAI's GPT-4o, have demonstrated considerable promise in transforming clinical decision support systems. In this study, we focused on a single but crucial task of clinical decision-making: laboratory test ordering. METHODS: We evaluated the self-consistency and performance of GPT-4o as a laboratory test recommender for 15 simulated clinical cases of different complexities across primary and emergency care settings. Through two prompting strategies - zero-shot and chain-of-thought - the model's recommendations were evaluated against expert co

Revisiting which language declines more in Spanish-English bilinguals with Alzheimer's disease: Longitudinal decline patterns on the multilingual naming test.

Theories of bilingual language production predict that bilinguals with Alzheimer's disease (AD) should exhibit one of two decline patterns. Either parallel decline of both languages (if decline reflects damage to semantic representations that are accessed by both languages), or asymmetrical decline, with greater decline of the nondominant language (if decline reflects reduced ability to resolve competition from the dominant language with disease progression). Only two previous studies examined decline longitudinally with one showing parallel, and the other asymmetrical, decline. We examined de

Correlation Between Plasma Oligomeric Amyloid-β and Performance on the Language Neutral Visual Cognitive Assessment Test in a Southeast Asian Population.

Oligomeric amyloid-β (OAβ), an upstream driver of Alzheimer's disease (AD) neuropathology, correlates with poor cognitive performance and brain volume reduction. Its effect on cognitive performance measured by the language neutral Visual Cognitive Assessment Test (VCAT) remains to be evaluated. We studied the correlation of plasma OAβ with VCAT scores and grey matter volume (GMV) in a Southeast Asian cohort with mild cognitive impairment. Higher plasma OAβ significantly correlated with lower; cognitive scores (VCAT, Mini-Mental State Examination) and GMV/intracranial volume ratio. Such finding

Auditory-motor mapping training: Testing an intonation-based spoken language treatment for minimally verbal children with autism spectrum disorder.

We tested an intonation-based speech treatment for minimally verbal children with autism (auditory-motor mapping training, AMMT) against a nonintonation-based control treatment (speech repetition therapy, SRT). AMMT involves singing, rather than speaking, two-syllable words or phrases. In time with each sung syllable, therapist and child tap together on electronic drums tuned to the same pitches, thus coactivating shared auditory and motor neural representations of manual and vocal actions, and mimicking the "babbling and banging" stage of typical development. Fourteen children (three females)

What do diagnostic test data tell us about differences in the profiles of children diagnosed with reading disability or language impairments?

UNLABELLED: A detailed evaluation of a child's strengths and weaknesses allows for a valid and reliable diagnosis of reading disability for the selection of optimal intervention practices. Using archival data from a clinic that specialized in the diagnosis of reading disabilities, we compared the performance of 60 children diagnosed with specific reading disability (SRD) and 31 children with language impairments and reading disability (LIRD) on measures of cognitive reasoning, verbal ability, phonological manipulation, rapid serial naming, and phonological memory. While the SRD and LIRD groups

Hemispheric asymmetry of the arcuate fasciculus: a preliminary diffusion tensor tractography study in patients with unilateral language dominance defined by Wada test.

OBJECTIVE: Lateralization of language function is a prominent feature of human brain function, and its underlying structural asymmetry has been recently reported in normal right-handed subjects. By means of diffusion tensor tractography (DTT), we investigated the asymmetry of the language network, namely, the arcuate fasciculus in patients in whom the unilateral language dominance was defined by Wada test. METHODS: DTT was performed in 24 patients with a focal lesion or an epileptic focus outside the C-shaped segment of the arcuate fasciculus. The arcuate fasciculus was reconstructed by placi

Beyond the Wada: An updated approach to pre-surgical language and memory testing: An updated review of available evaluation techniques and recommended workflow to limit Wada test use to essential clinical cases.

The Intracarotid amobarbital test (IAT), also called Wada test, is considered the "gold standard" for lateralizing language dominance in the pre-surgical evaluation of patients with epilepsy. In addition, it has been further modified to assess the postoperative risk of amnesia in patients undergoing temporal lobectomy. Since then it has been utilized to lateralize language and assess pre-surgical memory function. Over the years, its popularity has declined due to several limitations and availability of alternative procedures like fMRI and MEG. A survey of its use in the pre-surgical evaluation

Adaptation and pilot testing of the Early Learning Outcome Measure (ELOM) (4&5) Years Assessment tool in South African Sign Language.

Early Childhood Development is a key national priority in South Africa which has developed the Early Learning Outcome Measure (ELOM 4&5) specifically designed to measure the progress of 4- and 5-year-old children across 5 domains of early childhood development. This age-validated, population-standardised instrument has been shown to have measurement equivalence and lack of bias across South Africa's 11 official spoken languages. In 2023, South African Sign Language was formally recognised as 12th official language of South Africa, but no ELOM (4&5) exists in SASL despite over 6,000 deaf childr

Performance of Retrieval-Augmented Generation Large Language Models in Guideline-Concordant Prostate-Specific Antigen Testing: Comparative Study With Junior Clinicians.

BACKGROUND: Prostate-specific antigen (PSA) testing remains the cornerstone of early prostate cancer detection. Society guidelines for prostate cancer screening via PSA testing serve to standardize patient care and are often used by trainees, junior staff, or generalist medical practitioners to guide medical decision-making. However, adherence to guidelines is a time-consuming and challenging task, and rates of inappropriate PSA testing are high. Retrieval-augmented generation (RAG) is a method to enhance the reliability of large language models (LLMs) by grounding responses in trusted externa
