Outbreak of Drug-Resistant Mycobacterium tuberculosis Among Homeless People in Atlanta, Georgia, 2008-2015.

OBJECTIVES: Our objective was to describe and determine the factors contributing to a recent drug-resistant tuberculosis (TB) outbreak in Georgia. METHODS: We defined an outbreak case as TB diagnosed from March 2008 through December 2015 in a person residing in Georgia at the time of diagnosis and for whom (1) the genotype of the Mycobacterium tuberculosis isolate was consistent with the outbreak strain or (2) TB was diagnosed clinically without a genotyped isolate available and connections were established to another outbreak-associated patient. To determine factors contributing to transmiss

A qualitative analysis of gabapentin misuse and diversion among people who use drugs in Appalachian Kentucky.

Gabapentin, an anticonvulsant and analgesic for postherpetic neuralgia, has been thought to have no abuse potential despite numerous published reports to the contrary. Gabapentin has been linked with impaired driving and opioid use, highlighting the need to more fully understand its risk profile. Thirty-three individuals reporting recent nonmedical use of gabapentin were recruited from two ongoing longitudinal studies of drug users in Appalachian Kentucky to participate in focus groups. Four sessions were held (two in the community and two in jail settings), during which participants responded

Not an 'iron pipeline', but many capillaries: regulating passive transactions in Los Angeles' secondary, illegal gun market.

OBJECTIVES: California has strict firearm-related laws and is exceptional in its regulation of firearms retailers. Though evidence suggests that these laws can reduce illegal access to guns, high levels of gun violence persist in Los Angeles (LA), California. This research seeks to describe the sources of guns accessed by active offenders in LA, California and reports offenders' motivations for obtaining guns. SETTING: Los Angeles County Jail (LACJ) system (four facilities). METHODS: Random sampling from a screened pool of eligible participants was used to conduct qualitative semistructured

Flow Diversion in Middle Cerebral Artery Aneurysms: Is It Really an All-Purpose Treatment?

BACKGROUND AND OBJECTIVE: The outcome of flow diversion for middle cerebral artery (MCA) aneurysms, one of the most common sites for intracranial aneurysms, has not been defined thoroughly. We assessed our outcomes in MCA aneurysms (MCAAs) treated by flow diversion, i.e., with either dedicated flow diverters or telescopic stents. METHODS: Patients with MCAAs were treated by flow diversion if surgical or other endovascular treatment modalities had failed or were deemed likely to fail. Angiographic and clinical outcome of these patients was assessed retrospectively. Aneurysm location on MCA was

Symptomatic Exposures Among California Inmates 2011-2013.

Prisoners have a high prevalence of substance misuse and abuse, but few studies have examined symptomatic exposures among incarcerated populations. We sought to further characterize the nature of these exposures among this population using the California Poison Control System data. Keyword searches identified inmate cases in 2011-2013 for patients 20+ years old exposed to a single substance and taken to hospital from jail, prison, or police custody. Comparisons were made with non-inmate cases during the same period, using similar limitations. Body stuffers and body packers were analyzed as a s

Prevalence of involvement in the criminal justice system during severe mania and associated symptomatology.

OBJECTIVE: This study sought to determine the prevalence of criminal justice involvement during episodes of mania and to identify whether specific manic symptoms contribute to this risk. METHODS: Data from the 2001-2002 National Epidemiologic Survey on Alcohol and Related Conditions, a nationally representative sample of noninstitutionalized U.S. adults (N=43,093), were analyzed to determine the rate of legal involvement (being arrested, held at the police station, or jailed) of individuals with bipolar I disorder during the most severe lifetime manic episode. RESULTS: Among the 1,044 respon

Court-based participatory research: collaborating with the justice system to enhance sexual health services for vulnerable women in the United States.

BACKGROUND: Although jail screening programs have an important role in the diagnosis and treatment of sexually transmissible infections (STI) and HIV among incarcerated individuals, many arrestees are not screened before release. Justice-involved women are at particularly high risk for these conditions because of individual risk behaviour as well as other network-level risk factors. Court-based programs could provide a critical bridge between these women, STI risk counselling and health services. This formative study explored the features of a program that would encourage STI testing among cou

Open-label pilot study of extended-release naltrexone to reduce drinking and driving among repeat offenders.

OBJECTIVES: A high proportion of persons convicted of driving while impaired repeat the offense. Many continue drinking and driving, even when faced with long jail terms. Hence, they pose a serious public health threat. This preliminary study evaluated extended-release, injectable naltrexone suspension (XR-NTX) and supportive therapy in reducing (1) drinking and (2) attempts to drive after drinking among repeat driving while impaired offenders with an ignition interlock device installed in their vehicles. METHODS: Treatment-seeking volunteers received medical management therapy and 3 monthly

In-stent thrombosis and stenosis after neck-remodeling device-assisted coil embolization of intracranial aneurysms.

BACKGROUND: Intrinsic thrombosis and stenosis are complications associated with the use of neck-remodeling devices in the treatment of intracranial aneurysms. OBJECTIVE: To examine the technical and anatomic factors that predict short- and long-term stent patency. METHODS: We undertook a retrospective review of 161 patients who underwent coil embolization of 168 ruptured and unruptured aneurysms assisted by the use of a neck-remodeling device. One hundred twenty-seven patients had catheter-based angiographic follow-up to evaluate 133 stent-coil constructs (mean, 15.4 months; median, 12.7 mon

Latent TB infection treatment acceptance and completion in the United States and Canada.

BACKGROUND: Treatment of latent TB infection (LTBI) is essential for preventing TB in North America, but acceptance and completion of this treatment have not been systematically assessed. METHODS: We performed a retrospective, randomized two-stage cross-sectional survey of treatment and completion of LTBI at public and private clinics in 19 regions of the United States and Canada in 2002. RESULTS: At 32 clinics that both performed tuberculin skin testing and offered treatment, 123 (17.1%; 95% CI, 14.5%-20.0%) of 720 subjects tested and offered treatment declined. Barriers and missed opportunities to HIV testing among injection drug users in two Mexico--US border cities.

INTRODUCTION AND AIMS: Despite increasing HIV prevalence in cities along the Mexico--US border, HIV testing among high-risk populations remains low. We sought to identify barriers associated with HIV testing among injection drug users (IDUs) in Tijuana and Ciudad Juarez, the two largest Mexican border cities located across from San Diego, California and El Paso, Texas, respectively. DESIGN AND METHODS: In 2005, 222 IDUs in Tijuana and 205 IDUs in Ciudad Juarez were recruited by respondent-driven sampling and administered a questionnaire to collect socio-demographic, behavioural and HIV testin

Comparison of an ESAT-6/CFP-10 peptide-based enzyme-linked immunospot assay to a tuberculin skin test for screening of a population at moderate risk of contracting tuberculosis.

Screening for latent tuberculosis infection (LTBI) with the Mantoux tuberculin skin test (TST) has many limitations including false-positive results due to Mycobacterium bovis bacillus Calmette-Guérin (BCG) vaccination. Three hundred ninety adult inmates with normal screening chest radiographs in a county jail were evaluated for LTBI using TST and an ESAT-6/CFP-10 peptide-based enzyme-linked immunospot assay (T-SPOT.TB). LTBI prevalence rates were 19.0% and 8.5% by T-SPOT.TB and TST, respectively. Overall agreement between test results was 82.8% (kappa = 0.29). Positive T-SPOT.TB results were

Widespread skin and soft-tissue infections due to two methicillin-resistant Staphylococcus aureus strains harboring the genes for Panton-Valentine leucocidin.

Infections caused by community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) are emerging as a major public health problem. CA-MRSA has been associated previously with skin and soft-tissue infection (SSTI) and with carriage of staphylococcal cassette chromosome mec (SCCmec) type IV and the Panton-Valentine leucocidin (PVL) virulence factor. To assess the clonal distribution of PVL-carrying strains and the association with SSTI in the San Francisco Bay area, we surveyed six collections of S. aureus isolates-671 isolates in all-collected between 1997 and 2002 originating from in

Role of individual and contextual effects in injury mortality: new evidence from small area analysis.

OBJECTIVE: To analyse the role of individual and contextual variables in injury mortality inequalities from a small area analysis perspective, looking at the data for the city of Barcelona (Spain) for 1992-98. SETTING: Barcelona (Spain). METHODS: All injury deaths in residents older than 19, which occurred in the period 1992-98 were included (n=4393). Age and sex specific mortality rates were calculated for each educational level and each cause of death (traffic injuries, falls, drug overdose, suicide, other injuries). The contextual variables included were the proportion of men unemployed,

Alcohol-exposed pregnancy: characteristics associated with risk.

BACKGROUND: Prenatal alcohol exposure is a leading cause of neurodevelopmental deficits in children. Women who are fertile, drink alcohol, and have unprotected intercourse are at risk for an alcohol-exposed pregnancy, but little is known about this population. METHODS: A survey was administered to 2672 English-speaking women aged 18 to 44 years from six settings, including an urban jail, a drug/alcohol treatment facility, a gynecology clinic, two primary care clinics, and respondents to a media solicitation. Bivariate and multivariate analyses were conducted to explore variables that were cor

Community supervision during Oregon's partial decriminalization Measure 110: Criminal legal system involvement, overdose, and naloxone access.

BACKGROUND: In 2020, the U.S. state of Oregon passed Measure 110 (M110), which aimed to address substance use disorder as a public health issue and reduce disparities in the criminal legal system by decriminalizing personal drug possession and increasing services. The impact of partial drug decriminalization on individuals under community supervision-whose release conditions often prohibit drug use and who M110 excluded-is understudied. METHODS: We used targeted sampling to recruit and survey people who use drugs (PWUD; N = 468) in eight Oregon counties in 2023. We compared PWUD under communi

Liberation Medicine in Authoritarian Contexts: The Brutality of U.S. Mass Incarceration.

This article engages Führer and Vorhölter's (2025) reinvigoration of Nancy Scheper-Hughes's utopian call for liberation medicine in their special issue of Culture, Medicine, and Psychiatry. They offer a practical reconceptualization of liberation medicine as "concrete utopias" in the "medical undercommons". Inspired by Scheper-Hughes's humble call for "good-enough" ethnographic engagement in her documentation of a successful solidarity movement of single-mother agricultural laborers in Northeastern Brazil, I highlight ethnographically a (fragile) concrete utopian initiative: a peer-support pro

Influence of insurance type on healthcare utilization among rural people who use drugs.

BACKGROUND: Insurance possession facilitates healthcare engagement. Though people who use drugs (PWUD) experience substantial health risks, a large proportion are un/underinsured. Medicaid/other insurance eligibility varies by state and criminal legal involvement (CLI), and a better understanding of their association is important to inform policy. METHODS: Participants in the Rural Opioid Initiative were recruited 2018-2020 across 10 states and completed surveys assessing their demographics and insurance status, CLI experiences, substance use-related healthcare utilization, and healthcare bar
