Blood transfusion is one of the most common interventions in hospitals. Blood tests that support this widespread practice are considered routine, such as 'group and save' (also known as 'type and screen'), yet there has been very limited evaluation of the needs for these tests and the associated costs. The aim of this systematic review is to synthesise international evidence on the costs of group and save and crossmatch tests and their component processes. Seventy-two studies published between 2012 and 2023 were identified and data were extracted. Most cost estimates were derived from local in

The giant panda (Ailuropoda melanoleuca) is a vulnerable species of ursid, endemic to China. With over eight million years of evolution, it is considered a living fossil. Blood transfusion is an important means to treat giant pandas that are critically ill or severely injured, however the blood grouping of giant pandas, as the first element of blood transfusion, has not yet been clarified. Based on immunohematology, the saline-tube test, polybrene test and antigen test were carried out to detect IgM and IgG antibodies in the blood of giant pandas in this study. In parallel, human ABO typing re

Electronic crossmatching (E-XM) is used to detect ABO incompatibility. In developing countries, as many of the donations are from first-time donors, it is difficult to guarantee the accuracy of the ABO/Rh label on these units to use them for E-XM. This problem was overcome with a new software 'hemosoft', using donors' blood group declaration before donation as a tool for E-XM. During registration, donors either declare their blood group or give no comment. For, ABO/Rh grouping, either two results from different donations or only one in concordant with the declaration before donation is needed.

OBJECTIVE: Analyze the reasons for the mismatch between forward and reverse typing of ABO blood types and the mismatch between cross matching.
METHODS: When the forward and reverse typing do not match, use physiological saline method, polyamine method, anti human globulin method, and anti screening positive samples are used for antibody identification.
RESULTS: The factors contributing to discrepancies in blood typing between forward and reverse typing include weakened serum typing, condensation, monoclonal immunoglobulin influence, bone marrow transplantation, and blood type subtypes. The c

The limited value of the antiglobulin phase of the cross match when a careful antibody screening is performed was demonstrated by analyzing 73,407 compatibility tests for 23,857 patients. By the blood grouping and screening of these patients, 178 cases were detected with unexpected blood group antibodies that had not been previously observed. Unexpected antibodies were detected in an additional 13 patients by the saline phase of the cross-match. In addition, the antiglobulin phase disclosed only one patient with a very weak anti-Lea and two patients whose sera gave doubtful reactions, possibly

INTRODUCTION: The transplantation of highly sensitized patients remains a major obstacle. Immunized patients wait longer for a transplant if not prioritized, and if transplanted, their transplant outcome is worse.
CASE PRESENTATION: We report a successful AB0- and HLA-incompatible living donor kidney transplantation in a 35-year-old female patient with systemic lupus erythematosus (SLE) and antiphospholipid syndrome. The patient had a positive T- and B-cell complement-dependent cytotoxicity (CDC) crossmatch and previous graft loss due to renal vein thrombosis. We treated the patient with intr

INTRODUCTION: Discrepancies in gel card immunohematologic testing can result in false-positive reactions, making detecting antibodies and performing crossmatching difficult. Such unexpected reactivity may result from interactions with test system components, including the column matrix, chemicals, or reagents, rather than true antigen-antibody binding. Accurate identification and resolution of these discrepancies are crucial to prevent delays in transfusion and ensure patient safety.
METHODS: This case involved a 24-year-old patient with sickle cell disease who required a blood transfusion, h

In the recent paper entitled "The relics of Jesus and Eucharistic miracles: scientific analysis of shared AB blood type" the issue was raised that assignment of AB blood type using serological methods with such articles may be difficult due to the shared nature of AB antigens between humans and bacteria. Moreover, it was pointed out that examination of unique polymorphic markers would be required to validate a single source of origin. In a responsive commentary, Serafini has argued that because serological AB techniques have proven useful for certain (unrelated) studies in the past, the validi

Genomics is affecting all areas of medicine. In transfusion medicine, DNA-based genotyping is being used as an alternative to serological antibody-based methods to determine blood groups for matching donor to recipient. Most antigenic polymorphisms are due to single nucleotide polymorphism changes in the respective genes, and DNA arrays that target these changes have been validated by comparison with antibody-based typing. Importantly, the ability to test for antigens for which there are no serologic reagents is a major medical advance to identify antibodies and find compatible donor units, an

BACKGROUND: Despite major advances in serologic testing, extended phenotyping, and blood group genomics, clinically similar transfusion exposures may result in markedly different immune and clinical outcomes. Existing compatibility strategies do not fully explain this biological variability.
OBJECTIVES: To examine transfusion compatibility as an emergent donor-recipient biological state and propose a systems-level conceptual framework that integrates established biological determinants into a testable model for future precision transfusion medicine.
METHODS: This narrative review critically

The slide method for ABO blood group typing is commonly used for mobile blood donation and field applications because the test is easy, cost-effective, can be completed in a few minutes and requires a small volume of reagents. However, the reaction must be observed by an individual with expertise within 2 min; otherwise, drying of the reagent will give a false positive result. Moreover, the blood typing reagents must be stored at 4 °C. The present study aimed to create a paper-based device for ABO and RhD blood typing and combine an optical answer sheet reading concept to read and interpret th

BACKGROUND: Accurate red cell antibody identification is central to immunohematology, yet reagent red cell variability can compromise test reliability. Factors such as manufacturing differences, reagent composition, and cold-chain instability may alter assay sensitivity and obscure result interpretation. This study, for the first time, reports elution of Lewis antigens from reagent red cell panels, highlighting its role in antibody identification errors and diagnostic ambiguity.
MATERIALS AND METHODS: Over one year, five cases with inconclusive antibody identification due to reagent variabili

BACKGROUND: Low-titer O whole blood (LTOWB) is increasingly used to improve trauma outcomes, but maintaining supplies remains challenging, particularly in austere environments. This study evaluated EnzAO1 (FpGalNAcDeAc) and EnzAO2 (FpGalNase) to generate enzyme-converted O type whole blood (ECO-WB) from A type swine whole blood (WB).
STUDY DESIGN AND METHODS: WB from 16 A type and 4 O type swine were collected in Citrate Phosphate Dextrose Adenine-1 (CPDA-1). Type A WB was randomized to enzymatic conversion at either room temperature or 4°C. Each unit was divided into 45 mL aliquots and treat

Canine xenotransfusion may be a temporary emergency option for type-B cats when compatible feline blood is unavailable, but the in vitro compatibility of type-B feline blood with canine blood has not been fully characterized. This study evaluated and compared the slide-agglutination crossmatch compatibility of type-B feline blood with canine blood with different dog erythrocyte antigen (DEA) 1 phenotypes and with type-A feline blood. Blood samples were obtained from 70 type-B cats, 17 type-A cats, 26 DEA 1-positive dogs, and 18 DEA 1-negative dogs. Among 300 major crossmatches, incompatibility

BACKGROUND: ABO blood typing in pre-transfusion testing is a major component of the high workload in blood banks that therefore requires automation. We often experienced discrepant results from an automated system, especially weak serum reactions. We evaluated the discrepant results by the reference manual method to confirm ABO blood typing.
METHODS: In total, 13,113 blood samples were tested with the AutoVue system; all samples were run in parallel with the reference manual method according to the laboratory protocol.
RESULTS: The AutoVue system confirmed ABO blood typing of 12,816 samples

BACKGROUND: Three mistakes in recording blood types in the operation sheets occurred at the operating room of Iwamizawa Municipal General Hospital in 3 months (June-August, 2004).
METHODS: From these accidents, anesthesiologists and the staff improved the blood typing checking system to prevent mismatched transfusion during operation as follows; 1. The blood typing must be done at our out-patient's department, while the blood group compatibility test must be done at our inpatient's department. 2. We must describe the results of blood typing on the anesthesia application form using red colored
