Assessment of the Level of Middle Colic Artery Ligation Influence on Survival in Right-Sided Colon Cancer.

Aim: This study aims to investigate whether the addition of ligation of the Middle Colic Artery in its origin to Complete Mesocolic Excision (CME) with Central Vascular Ligation (CVL) affects overall survival and lymph node retrieval. Method: Patients with right-sided colon cancer who underwent curative right or extended right hemicolectomy with CME and CVL were included. In cases with tumors located in the distal part of the ascending colon and the hepatic flexure, the middle colic artery was ligated in its origin. For the cecal and proximal localization of the tumor, the point of ligation wa

Laparoscopic-assisted single-port right colectomy: a standardized top-down technique and tricks to perform a better anastomosis.

Colorectal cancer has become the third most commonly diagnosed cancer, posing a significant public health concern. Advances in surgery, including laparoscopic and robotic techniques, have improved treatment options. A major development in treating right colon cancer is complete mesocolic excision (CME) with central vascular ligation (CVL), introduced by Hohenberger et al. in 2009. This method focuses on precise dissection to preserve the visceral fascia and reduce tumor spread, including regional lymph node dissection and central ligation of colonic arteries. Laparoscopic colectomy with intrac

Cholecystocolonic Fistula Presenting With Gallstone Ileus: A Report of a Rare Surgical Case.

Cholecystocolonic fistula (CCF) is a rare entity resulting from chronic gallstone disease. Here we describe a 60-year-old female who presented with abdominal distension, colicky pain, and constipation for a period of five days. Imaging studies revealed pneumobilia and small bowel obstruction. Exploration via laparotomy revealed a 3.5 cm gallstone lodged in the terminal end of the ileum and a fistulous tract from the gallbladder to the hepatic flexure of the colonic bowel. Surgical intervention consisted of enterolithotomy, cholecystectomy, and colonic resection with primary anastomosis. Fortun

Research progress on surgical approach and intestinal anastomosis methods for laparoscopic right hemicolectomy: A review.

Patients with tumors located in the cecum, ascending colon, and hepatic flexure of the colon often use laparoscopic right hemicolectomy. Although with the help of laparoscope, the surgery is more visual and refined, due to the different shapes of the blood vessels and the different positions of the frequently occurring blood vessels, the surgeons face challenges during surgery. Improper operation may cause unnecessary damage. Therefore, in order to better solve the problem of variation and reduce intraoperative side injuries, we conducted a literature review on the surgical approach and the gu

Stool and symptom testing in ColoREctal Evaluation for Neoplasia in Cystic Fibrosis (SCREEN-CF).

BACKGROUND: People with cystic fibrosis (pwCF) have increased colorectal cancer (CRC) risk. Colonoscopy is recommended, yet CF comorbidities increase complexity and risk. METHODS: We conducted a prospective, observational study of pwCF meeting colonoscopy screening guidelines at an Australian centre (2019 - 2023). Immunochemical faecal occult blood test (iFOBT), faecal calprotectin (FC), and faecal tumour pyruvate kinase isoenzyme type M2 (TuM2-PK) were evaluated for detecting adenomatous polyps and malignant ileocolonic lesions in pwCF. Stools were collected within 3 months of colonoscopy. D

Robot-Assisted Total Proctocolectomy for Familial Adenomatous Polyposis with Multiple Colorectal Cancers Using the Hugo RAS System.

INTRODUCTION: Experience with the Hugo RAS system in robot-assisted colorectal surgery is limited. This is particularly noticeable when focusing on complex procedures, such as total proctocolectomy (TPC). This study aimed to demonstrate the feasibility and safety of using the Hugo RAS system for TPC. CASE PRESENTATION: A 27-year-old woman with multiple colorectal cancers with a background of familial adenomatous polyposis underwent robot-assisted TPC, including lymph node dissection of the entire colorectal region using the Hugo RAS system. The robotic procedure was divided into 3 steps: 1) T

[A Case of Local Recurrence of Ascending Colon Cancer Successfully Resected by Laparoscopic Surgery].

A 93-year-old woman was emergently admitted to our hospital diagnosed with femoral neck fracture. She had a previous history of undergoing laparoscopic right hemicolectomy for ascending colon cancer 2 years and 4 months ago, and blood examination showed severe anemia. Detailed inspection was then performed, and she was diagnosed as having local recurrence of previous ascending colon cancer. To prevent further bleeding, she underwent laparoscopic surgery. The tumor was located nearby hepatic flexure, and direct invasion to the duodenum and the Gerota's fascia was suspected. The region including

Clinical utility of a novel double-band endoscopic band ligation device for consecutive ligation in the management of diverticular bleeding.

BACKGROUND AND AIMS: Endoscopic band ligation is effective for diverticular bleeding; however, conventional devices have drawbacks such as limited visibility, weak suction, and single-use design. Endoligar (Asahi Intecc Co Ltd, Aichi, Japan) addresses these limitations with clear visibility, strong suction, and the ability to perform 2 ligations without removing the endoscope. This report evaluates its clinical utility. METHODS: An 82-year-old man with a history of diverticular bleeding was admitted for hematochezia lasting 3 days. Noncontrast computed tomography and emergency colonoscopy sug

Rare "ileum-ileum-colon type" adult intussusception caused by Meckel's diverticulum inversion: A case report.

RATIONALE: Ileum-ileum-colon intussusception constitutes a small proportion of the classification of intussusception. Both adult intussusception and Meckel's diverticulum are rare clinical entities, with few reports documenting Meckel's diverticulum inversion leading to ileum-ileum-colon intussusception in adults. PATIENT CONCERNS: A 33-year-old Chinese male presented with intermittent abdominal pain persisting for 1 month, that had intensified over the preceding day. DIAGNOSES: Abdominal computed tomography revealed intussusception, suspected to be secondary to a small intestinal lipoma. I

The emperor has no clothes-the need for standardized anatomical segment definitions in the management of colon cancer.

PURPOSE: There is no standardized classification system for defining segments of colon carcinoma within cancer registries. We aimed to conduct a systematic literature review on studies that specifically address colon cancer and provide definitions for each segment. METHODS: Five authors conducted an independent systematic literature search of studies on colon cancer between 01/1992 and 11/2023. Only English-language original articles including cohort studies, case series, previous reviews, meta-analyses, and randomized clinical trials were considered for inclusion. Exclusion criteria were cas

Roux-en-Y Gastric Bypass is Associated With Increased Intestinal Glucose Uptake in Humans.

CONTEXT: In animal models, Roux-en-Y gastric bypass (RYGB) is associated with increased Roux limb intestinal glucose uptake that may contribute to early metabolic benefits, though prospective clinical studies are lacking. OBJECTIVE: The present study aimed to test the hypothesis that Roux limb glucose uptake would increase relative to baseline in a cohort of patients undergoing RYGB. METHODS: RYGB patients underwent preoperative baseline, and 3- and 6-month positron emission tomography/computed tomography postoperative imaging. Maximum and mean standardized uptake values (SUV) were measured

Medullary colon carcinoma in a patient with serrated polyposis: an extremely rare association.

We present the case of a 55-year-old woman with no relevant medical history diagnosed with serrated polyposis following a screening colonoscopy in 2021. In March 2024, a suspicious polypoid lesion was identified in the hepatic flexure. Histological analysis confirmed a poorly differentiated adenocarcinoma. After neoadjuvant chemotherapy, a subtotal colectomy was performed, and the surgical specimen revealed a medullary colon carcinoma, a very rare subtype of adenocarcinoma. The patient is currently disease-free and undergoes colonoscopies every six months. This case highlights the importance o
