Cellular and Tissue Mechanisms of Laser-Assisted Regeneration of Tympanic Membrane Perforation.

Chronic tympanic membrane (TM) perforations remain a significant clinical problem due to limited spontaneous healing and the drawbacks of surgical interventions. The present study aimed to evaluate the effects of low-intensity infrared laser irradiation on cellular mechanisms of regeneration and to assess its therapeutic potential in TM repair. Primary fibroblast cultures isolated from chinchilla tissue were exposed to laser radiation at wavelengths 1560 nm and 1450 nm. Cellular responses were analyzed using MTT assay, scratch test, Ki-67 immunostaining, and ELISA for collagen type II. In addi

Optical Coherence Tomography for Quantitative Point-of-Care Ear Imaging.

OBJECTIVE: To determine whether an optical coherence tomography (OCT) otoscope can provide quantitative morphological measurements of the tympanic membrane (TM) that distinguish abnormal ears from normal controls. METHODS: Patients with ear disease and normal volunteers were imaged using an OCT otoscope. Integrated video otoscopy with simultaneous OCT imaging guided probe positioning. OCT volumes (< 5 s each) were acquired and fused to generate wide-field volumetric images of the TM and middle ear (ME). Datasets were analyzed using semi-automated segmentation to quantify TM thickness and surf

Unsuccessful Healing of Tympanic Membrane Perforations Following Myringoplasty-A Nationwide Retrospective Study.

INTRODUCTION: Risk factors for unsuccessful healing of a tympanic membrane perforation following myringoplasty lack consensus. Large cohort studies are missing. METHODS: Data were collected for myringoplasties (with/without concomitant ossiculoplasty) performed the years 2014-2019 with a follow-up visit using the Swedish Quality Register for Ear Surgery (SwedEar). Risk factors for unsuccessful surgical outcome, defined as non-healed tympanic membrane at follow-up, were calculated using logistic regression. RESULTS: The study population included 3622 cases and 12.8% had a non-healed tympanic

Tympanostomy Tube Placement: The Effect of Location on Tube Retention and Risk of Perforation.

OBJECTIVE: The retrospective cohort study compares tube location in the anterior-inferior quadrant versus the posterior-inferior quadrant and time of tube retention along with the risk of early extrusion and perforation. The study hypothesis holds that both anterior-inferior placement and posterior-inferior placement are equally effective and safe locations for tube placement with equal tube retention and risk for tympanic membrane perforation. METHODS: An IRB-approved retrospective chart review was completed at a major Academic Center's Children's Hospital. The study included data from child

Candida lusitaniae as an Unusual Cause of Otomycosis: A Case Report and Literature Review.

Otomycosis is a fungal infection of the external auditory canal that may extend to the tympanic membrane. Although commonly caused by Aspergillus and Candida species, infections due to uncommon fungal pathogens may present diagnostic and therapeutic challenges. We report the case of a 46-year-old woman who presented to the Department of Otorhinolaryngology (ENT) with intense pruritus of the right external auditory canal, associated with a whitish discharge and tympanic perforation on otoscopic examination. Mycological analysis of the ear discharge identified Candida lusitaniae. To our knowledg

Biomaterials and Biological Therapies in Tympanic Membrane Regeneration: Systematic Review and Perspective.

OBJECTIVE: Chronic tympanic membrane perforations generally require surgical repair, most often via tympanoplasty. While effective, these procedures involve anesthesia, carry significant costs, and may not always be accessible. The objective of this review is to explore biomaterials and biological therapies that could promote tympanic membrane regeneration and potentially reduce the need for surgery. DATA SOURCES: PubMed and Google Scholar between September 1958 and December 2025. REVIEW METHODS: A PRISMA-guided systematic review of clinical studies about biomaterials or biological therapies

The Swansea Technique: a novel method of endoscopic underwater excision of external auditory canal exostoses using a piezoelectric saw.

BACKGROUND: External auditory exostosis (surfer's ear) is a progressive condition of benign hyperostotic overgrowths of the external auditory canal, secondary to repetitive cold-water exposure, in which symptoms correlate to degree of exostosis. Surgical intervention is the only available treatment, most commonly using the osteotome or microdrill, but these carry a risk of sensorineural hearing loss, post-operative stenosis, temporomandibular dysfunction and tympanic membrane perforation. METHODS: We describe in detail the Swansea technique, an innovative method of canaloplasty with endoscopi

Clinical application of a modified cartilage ring reposition technique in tympanoplasty.

BACKGROUND: Extensive cartilage-harvest during tympanoplasty can result in tragus atrophy. It can bring inconvenience to life by impairing headphone or hearing aid use. This study proposed a modified technique to preserve tragus shape and function by creating a large cartilaginous-perichondrial complex and repositioning the cartilage ring into the tragus. METHODS: A retrospective analysis of 53 patients undergoing type I tympanoplasty compared outcomes between an experimental group (n = 27 ) and a control group (n = 26). Postoperative outcomes—including tragal cartilage function scores, graft s

Technical note: A graft modification in butterfly cartilage myringoplasty using a triangular excision.

BACKGROUND: Butterfly myringoplasty is a simple and effective technique for small-to-moderate, non-marginal tympanic membrane perforations. Various modifications of the graft have been described. However, when the residual tympanic membrane is thin, it may tear during graft placement. METHODS: Inspired by the Paparella type 2 ventilation tube, we applied a minor modification to the cartilage graft in butterfly myringoplasty. A small triangular excision was made on the medial wing of the graft, allowing controlled rotational insertion through the perforation while preserving the integrity of t

[ENT emergencies in general practice (part 1)].

ENT complaints are common in primary care, yet their assessment can be challenging. Their clinical evaluation requires a knowledge of anatomy, pathology, and differential diagnoses. The boundaries between benign conditions and those requiring specific treatment or specialist referral is sometimes unclear. A structured clinical reasoning process, combined with a few key references, helps avoid the most common pitfalls. This illustrated practical guide is designed to support primary care physicians in providing safe, pragmatic, and guideline-based management. This first part deals with infectiou

Fibrous Biomaterial Scaffold for Tympanic Membrane Repair: Microarchitectural Engineering and Structure Function Performance.

Tympanic membrane (TM) perforations, arising from infections, injuries, or chronic otitis media, remain a frequent clinical finding and can lead to hearing problems when the tissue does not regenerate adequately. Although autologous grafts are still the standard option for repairing persistent defects, they come with well-known limitations. Beyond the need for additional harvesting procedures, these grafts rarely reproduce the intricate, fibrous layering of the native TM, which can compromise sound transmission after healing. In search of alternatives, fibre-based scaffolds have attracted cons
