A Type-Entry-Malperfusion-Based Propensity Score Matched Analysis Depending on Surgical Expertise in Patients Without Malperfusion Undergoing Surgery for Acute Type A Aortic Dissection.

OBJECTIVES: This study investigates differences in short- and mid-term outcomes in patients without malperfusion undergoing surgery for acute type A aortic dissection between specialized aortic surgeons and non-aortic surgeons. METHODS: Patients who underwent surgery for acute type A aortic dissection between 2013 and 2023 defined as M0 (no malperfusion) according to the type-entry-malperfusion classification were included and divided into 2 groups according to the surgeon's expertise: aortic surgeon vs non-aortic surgeon group, whereas an aortic surgeon was defined by expertise in extensive

Clinical characteristics and outcomes of type A intramural haematoma and aortic dissection: a systematic review and meta-analysis of 6457 patients.

BACKGROUND: Type A intramural haematoma (TAIMH) and type A aortic dissection (TAAD) are both managed surgically as acute aortic syndromes, but whether their clinical profiles and outcomes differ remains unclear. We performed the first comprehensive meta-analysis to compare patient characteristics, operative findings and complications between TAIMH and TAAD. METHODS: Systematic searches of six databases identified studies comparing TAIMH and TAAD, with data pooled using random-effects models and Hartung-Knapp-Sidik-Jonkman corrections for small samples. Subgroup and meta-regression analyses as

Surgical Outcomes of Stanford Type A Aortic Dissection in Jehovah's Witness Patients.

PURPOSE: Surgery for Jehovah's Witness patients with Stanford type A aortic dissection (TAAD) carries a high surgical risk, and few reports have examined outcomes in this patient population. This study evaluated perioperative outcomes of surgery for TAAD in Jehovah's Witness patients. METHODS: Eight Jehovah's Witness patients who underwent surgery for TAAD at our institution between February 2016 and January 2025 were retrospectively reviewed. No patients were receiving antiplatelet or anticoagulant therapy at the time of surgery. Preoperative characteristics, operative data, and postoperativ

Arterial variation relevant to transfemoral transcatheter aortic valve replacement (TF-TAVR): a cadaveric and literature based analysis.

PURPOSE: Transfemoral transcatheter aortic valve replacement (TF-TAVR) is the preferred vascular access route for aortic valve implantation, but procedural success is highly dependable on iliofemoral vascular anatomy. This study aimed to directly assess anatomic risk factors for TF-TAVR using cadaveric dissection and highlight their relevance in the context of imaging-based literature. METHODS: Thirty-two formalin-embalmed cadavers (64 limbs) were dissected to assess luminal diameters of the common femoral (CFA), external iliac (EIA), and common iliac arteries (CIA). Anatomical configurations

The GERAADA Risk Score for Early Mortality After Surgery for Acute Type A Aortic Dissection: An External Validation in the Dutch Setting.

OBJECTIVES: Acute type A aortic dissection (ATAAD) carries high mortality, with emergency surgery being the cornerstone of treatment. The German Registry for Acute Type A Aortic Dissection (GERAADA)-score is advocated in guidelines to predict 30-day mortality after ATAAD surgery. This study investigates its performance in a Dutch cohort, with an emphasis on malperfusion definitions, age groups, and sex. METHODS: Adults undergoing emergency surgery for ATAAD at 5 Dutch centres (2007-2024) were included in a multicentre database. External validation of the GERAADA-score was performed with these

Ascending Thoracic Aortic Aneurysms in a Veterans Affairs Health System: Longitudinal Outcomes and Risk Factors.

BACKGROUND: The decision to perform ascending thoracic aortic aneurysm (ATAA) repair is primarily guided by diameter thresholds, but the optimal timing remains debated. We aimed to analyze ATAA outcomes in a large cohort of veterans. METHODS: Retrospective cohort study of patients with ATAA with diameter≥4.0 cm under surveillance between 1998 and 2024. Outcomes included surgical repair, all-cause mortality, and aortic events. Fine-Gray competing risks regression evaluated the association of baseline diameter with all-cause mortality, adjusting for age, hypertension, smoking, heart failure, an

Predictive value of machine learning for mortality risk in aortic dissection: a systematic review and meta-analysis.

BACKGROUND: Aortic dissection (AD) is a critical cardiovascular disorder with substantial risks of short-term mortality. Some researchers have endeavored to utilize machine learning (ML) approaches to develop predictive models for the risk of mortality in AD. However, systematic evidence about the accuracy of these models remains scarce, which poses challenges to the development and enhancement of risk assessment tools. Therefore, this study seeks to systematically review the reliability of ML in forecasting the risk of mortality in AD. METHODS: A search was implemented through PubMed, Cochra

Activating PRKG1 Variant Enhances Smooth Muscle Cell Deformability to Cause Aortopathy.

Aortic dissection can strike without warning. Whereas the condition is typically linked to aging and chronic hypertension, rare genetic variants emerge as silent culprits. One variant, V219I in PRKG1, has been found in patients with aortic aneurysms despite near-normal aortic diameters. Vascular smooth muscle cells expressing the V219I variant were larger, more deformable, and showed aberrant actin cytoskeleton dynamics. They exhibited altered extracellular matrix signaling and weakened structural integrity, highlighting a shift toward increased tissue elasticity as the causal molecular pathom

Multimodal imaging of acquired aortic diseases: clinical efficacy, comparative analysis, and future perspectives.

Aortic diseases, particularly acute aortic syndromes (AAS) and aortic aneurysms (AA), represent critical cardiovascular conditions with high mortality rates requiring precise imaging for diagnosis and management. This review provides a comprehensive analysis of current imaging diagnostic techniques, focusing specifically on acquired thoracic and abdominal aortic pathologies. We first evaluate the comparative efficacy of Computed Tomography Angiography (CTA) and Magnetic Resonance Imaging (MRI) in the diagnosis of AAS (including aortic dissection, intramural hematoma, and penetrating atheroscle

Cysto-Hernio-Aortic Dissection.

Hepatic cyst, left external inguinal hernia, and type-A aortic dissection in an apparently healthy 51-year-old man with untreated hypertension and a history of surgery for right inguinal hernia in preschool age are herein reported. When multiple structural lesions coexist, a possible shared connective-tissue vulnerability should be considered.

Outcomes of total arch replacement and frozen elephant trunk in acute aortic syndrome.

OBJECTIVES: There are several studies from all over the world reporting on frozen elephant trunk implantation and total arch replacement in acute aortic syndrome demonstrating mostly favourable outcomes. Most of these studies present younger study populations carrying a rather low perioperative risk for adverse outcomes. Herein, we present our single centre experience with the frozen elephant trunk procedure in patients with acute aortic syndrome. The patients in this cohort carried a rather high perioperative risk. A considerable number of patients had undergone resuscitation, presented with

Adaptive geometric-attention multi-task framework with knowledge distillation for aortic dissection detection in non-contrast CT.

Aortic dissection (AD) is a life-threatening cardiovascular emergency. Non-contrast-enhanced computed tomography (NCE-CT) could provide timely AD screening with fewer contraindications compared to CE-CT imaging. However, NCE-CT examinations lack distinctive imaging characteristics of AD, leading to high rates of missed diagnoses and misdiagnoses, and increased radiologist workload. In this paper, we propose a novel end-to-end multi-task framework for automated aortic segmentation and AD detection using NCE-CT images. The framework comprises three main components: a deformable feature extractor
