High Neutrophil Alkaline Phosphatase Activity in Adult-Onset Still's Disease.

INTRODUCTION: Adult-onset Still's disease (AOSD) is an autoinflammatory disorder characterized by reactive neutrophilia and dysregulated cytokine release. Mature neutrophils exhibit increased alkaline phosphatase enzyme activity within cytoplasmic granules, particularly in response to inflammation or acute infection. However, whether neutrophil alkaline phosphatase (NAP) activity is elevated in active AOSD, a hyperinflammatory state, remains unclear. METHODS: We enrolled 114 patients diagnosed with AOSD, 47 patients with diffuse large B-cell lymphoma (DLBCL), 25 patients with antiphospholipid

[Siltuximab as a salvage therapy in multi-refractory adult-onset Still's disease: A case series].

INTRODUCTION: Adult-onset Still's disease (AOSD) is a systemic auto-inflammatory disorder characterized by a cytokine storm. Four major cytokines can be specifically targeted in severe, refractory or corticosteroid-dependent forms: interleukin 1 (IL-1), interleukin 6 (IL-6), interleukin 18 (IL-18), and Tumor necrosis factor alpha (TNFa). According to the French national protocol for adult-onset Still's disease, anti-IL-6 agents are the second-line of treatment after corticosteroids. We report here on the efficacy and safety of siltuximab prescribed off-label in patients with multi-refractory f

IL-6R blockade combined with immunosuppressants alleviates adult-onset Still's disease through immune remodeling: a mass cytometry study.

BACKGROUND: Early administration of IL-6R blockade is effective in adult-onset Still's disease (AOSD), but the underlying immune alterations during combined immunotherapy remain unclear. METHODS: We employed high-dimensional single-cell mass cytometry and an unbiased bioinformatics pipeline to characterize the immune landscape in treatment-naïve AOSD patients, stable AOSD patients after 24 weeks of IL-6R blockade plus methotrexate and prednisone, and matched healthy controls. Cytokine profiling was conducted using a high-throughput cytometric bead array, and potential regulatory networks were

Targeting neutrophil-driven inflammation in adult-onset still's disease: molecular insights from gene expression profiles.

BACKGROUND: The rarity and heterogeneity of adult-onset Still's disease (AOSD) pose significant challenges in understanding its precise pathogenic mechanisms, developing effective treatment options, and establishing therapeutic strategies. A comprehensive analysis of gene expression profiles could help to bridge the knowledge gaps in those areas. METHODS: A blood transcriptomic dataset comprising 31 patients with AOSD and 22 healthy controls was fetched. Cellular and molecular features were identified by analyzing differentially expressed genes (DEGs) and functional enrichment. Optimal molecu

Secondary Pulmonary Alveolar Proteinosis Complicated by Hemophagocytic Syndrome in a Patient with Adult-onset Still's disease: A Case-based Review.

Treatment of secondary pulmonary alveolar proteinosis (SPAP) focuses on managing the underlying disease, typically myelodysplastic syndrome. However, immunosuppressive therapy may worsen the condition in autoimmune disease-associated SPAP. We report the case of a 74-year-old woman with hemophagocytic syndrome and adult-onset Still's disease, who developed ground-glass opacities and consolidations during immunosuppressive treatment. SPAP was confirmed by a biopsy and negative for anti-granulocyte-macrophage colony-stimulating factor antibodies. Although whole-lung lavage was considered if SPAP

nCD64 index as a new early predictive biomarker for macrophage activation syndrome in adult-onset still's disease.

Macrophage activation syndrome (MAS) represents a severe and potentially life-threatening complication of adult-onset Still's disease (AOSD), necessitating the identification of sensitive and specific biomarkers for early diagnosis. Our study found significantly elevated CD64 mRNA expression in neutrophils of AOSD patients compared to healthy controls (p = 0.029). The neutrophil CD64 index (nCD64 index) positively correlated with key clinical manifestations, including splenomegaly, sore throat, pulmonary infiltrates, and pericarditis. Effective treatments led to a rapid and significant decreas

Atypical adult-onset Still's disease that fails to meet Yamaguchi and Fautrel criteria.

Adult-onset Still's disease is a rare systemic autoinflammatory condition of unknown aetiology. It is a diagnosis of exclusion based primarily on clinical features and non-specific laboratory findings using either the Yamaguchi or Fautrel criteria. Here, we present a case of atypical adult-onset Still's disease in a young man with twice-daily fevers, night sweats and evanescent rash on his face but not enough features to meet the Yamaguchi or Fautrel criteria after extensive (but unrevealing) infectious, autoimmune and malignancy workups. He responded well to corticosteroid therapy with period

Efficacy and safety of ruxolitinib in adult patients with refractory rheumatic disease-associated macrophage activation syndrome.

OBJECTIVE: Rheumatic disease-associated macrophage activation syndrome (RD-MAS) is a rare and life-threatening complication of rheumatic diseases, with approximately 30% of cases being refractory to conventional therapeutic protocols. Ruxolitinib, a Janus kinase 1/2 inhibitor, has emerged as a potential therapy for refractory RD-MAS. This study aimed to evaluate its efficacy and safety in patients with refractory RD-MAS. METHODS: A meticulous chart review was conducted on 20 refractory RD-MAS patients treated with ruxolitinib. Data from no ruxolitinib treatment RD-MAS patients served as histo

Fever, joint pain, and systemic inflammation in a young adult: recognising adult-onset Still's disease.

A young man presented with recurrent high-spiking fevers, diffuse arthralgia, hepatomegaly and a transient salmon-coloured rash. Laboratory evaluation revealed leukocytosis, markedly elevated inflammatory markers and extreme hyperferritinaemia. Extensive infectious and autoimmune workups were negative. The patient met Yamaguchi criteria for adult-onset Still's disease (AOSD) and responded well to corticosteroids and biologic therapy with canakinumab. This case highlights the diagnostic complexity of AOSD and the importance of early consideration in patients with systemic inflammation and prolo

Specialized pro-resolving mediators and autoimmunity: Recent insights and future perspectives.

Inflammation is a response to injuries involving multiple cellular and molecular mechanisms. Different stimuli, such as trauma or microbial invasion, trigger an acute inflammatory response consisting, at least in principle, of two phases: initiation and resolution. Although the acute phase of inflammatory response represents a protective and usually self-limited mechanism, it can sometimes persist and evolve into chronic inflammation, a key driver in the development of many rheumatic and autoimmune diseases. The biosynthesis of specialized pro-resolving mediators (SPMs) orchestrates the resolu

Adult-onset Still's disease complicated by treatment resistant pure red cell aplasia requiring cyclosporine: A case report and review of cases.

Pure red cell aplasia (PRCA) is a very rarely seen complication of adult-onset Still's disease (AOSD). As there are only a select few documented cases of this complication in the medical literature, its typical course and response to treatment are yet to be fully understood. We report a case of a 36-year-old woman with adult-onset Still's disease complicated by severe pure red cell aplasia. Treatment of the underlying Still's disease with glucocorticoids, IL-1 inhibitors, and tocilizumab improved the clinical features of the patient's disease; however, the patient remained with severe transfus
